  • Analysis of randomized controlled trials on otorhinolaryngologic diseases in China

    Objective To evaluate the quality of randomized controlled trials in otorhinolaryngology in China and provide to comprehend the possibility of its contribution in providing reliable, evidence in clinical practice; thus providing evidence to elevate the clinical treatment level. Methods Five Chinese clinical otorhinolaryngology journals were searched and randomized controlled trials were identified and analyzed according to the standards of evidence-based medicine. Results Two hundred and eighty seven issues were referred to and eighty-one randomized controlled trials were identified and analyzed. Of these randomized controlled trials, 34.57% (28/81) had definite diagnostic standards, 38.27% (31/81) had inclusion standards and 33.33% (27/81) had exclusion standards; only 1.23% (1/81) got the approval of the participants; 40.74% (33/81) had moderate sample size; 3.70% (3/81) had large sample size and no one mentioned sample size estimation; 81.48% (66/81) didn’t report the method of randomization and 38.27% (31/81) had baseline comparison; 18.52% (15/81) didn’t define the control interventions and 8.64% (7/81) even didn’t explicate the experimental interventions; 32.10% (26/81) used blank comparison; 86.42% (70/81) didn’t use blindness; 37.04% (30/81) didn’t mention the adverse effects; 23.46% (19/81) used accredited standards to evaluate the outcomes; l l.11% (9/81) mentioned the loss of participants and only 1.23% (1/81) treated the loss with statistics methods. Conclusions The quantity and quality of the otorbinolaryngologic randomized controlled trials in present review can not meet the clinical need. Higher quality of randomized controlled trials are required to improve the level of prevention and the treatment of otorhinolaryngologic diseases.

  • A Survey of Participants’ Satisfaction of the New-style Rural Cooperative Medical Care in Wenjiang District of Chengdu

    Objective To investigate the satisfaction of participants in Wenjiang District of Chengdu regarding the New-style Rural Cooperative Medical Care (NRCMC), and to analyse its factors in order to help the decision makers optimize and improve the scheme in the future.Methods Three towns of Wenjiang District were selected for study by computer simple random sampling. Five percent of NRCMC participants were randomly selected in Wenjiang for a face-to-face interview using a questionnaire. Data entry and statistical analysis were completed by Epidata 3.0 and SPSS 11.5 respectively. Results A total of 2500 questionnaires were conducted for face-to-face interviews, and 2438 questionnaires were returned (response rate 97.6%); 58% participants were satisfied with NRCMC, and the results of stepwise multiple logistic regression analysis indicated that their desire to participation (partial regression coefficient B=-3.54, P=0.014) and the satisfaction with the present compensation proportion (partial regression coefficient B=-4.62, P=0.018) were the most important factors that influenced the total satisfaction with NRCMC.Conclusion It is of great significance to strengthen the promotion of NRCMC; reasonably establish the compensation mode and proportion; encourge voluntary participation for the sake of their benefits to improve the quality and performance of NRCMC.

  • Evidenced-Based Therapy of Lupus Nephritis with Mycophenolate Mofetil

    Objective We searched for evidence on mycophenolate mofetil(MMF) as a treatment for patients with diffuse proliferative lupus nephritis. Methods We attempted to find the current best evidence by searching The Cochrane Library(Issue 4, 2005), MEDLINE(1990 to June 2007), CMB(1980 to December 2006), CNKI(1979 to October 2007). We critically appraised the available evidence. Results Four systematic reviews and 6 randomized controlled trials of high quality were available. MMF and prednisolone were found to be an effective continuous induction-maintenance treatment for diffuse proliferative lupus nephritis. MMF was associated with less drug toxicity. Conclusion Given the current evidence and our clinical experience, and considering the patient and the values and preferences of his family,MMF was given at 1 g daily in combination with steroids at the beginning. No obvious adverse effects occurred during 3 months of follow-up.

  • Characteristics of Pathogens from Lung Cancer Patients with Infection after Surgery

    Abstract: Objective To analyze the characteristics of pathogens isolated from patients with lung cancer after surgery, and provide instructions for the prevention and treatment of postoperative infection. Methods A retrospective investigation of the pathogenic microbiology data of 159 patients(142 males and 17 females with average age at 61.8±9.6 years) with lung cancer after surgery in West China Hospital from January 2009 to December 2011 was performed.We got the specimen from the patients by routine methods, identified the pathogens, and conducted drug susceptibility tests in vitro. Results The pathogens were mainly isolated from sputum of the patients (90.6%, 155/171). Canidia albicans(25.7%, 44/171), Klebsiella pneumonia(15.2%, 26/171), Acinetobacter calcoaceticus-A. baumannii(13.4%, 23/171)and Pseudomonas aeruginosa(8.2%, 14/171) were the predominant species. Among A. calcoaceticus-A. baumannii isolates, 39.1%(9/23) of them were multidrug resistant strains. And 23.1%(6/26)of K. pneumonia isolates produced extended spectrum beta-lactamases. P. aeruginosa isolates showed high in vitro susceptibility to the antibacterial agents. Conclusion Pathogens are mainly isolated from sputum of patients after surgery for lung cancer. Canidia albicans and gram-negative bacilli are predominant isolates and should be the major targets for the prevention and treatment of postoperative infection.

  • THE EFFECT OF ANTI-EXUDATE OF WOUND WITH PERMANENT MAGNETIC FLAT DRESSINGS

    An experimental study was designed on the treatment of 8 superficial burn wounds and 5 wounds after removing the split-thickness skin grafts with 200GS permanent magnetic flat dressings, and some of the wounds were chosen at random which did not received any treatment and would be served as control. It was noted that the volume of exudate in the treatment group was 11.9 50.7% less than that of the control group, and the total protein, the albumin and globulin contents in the treatment cases were far less in exudate than those of the control cases. It was concluded that the magnetic therapy not only decresed the exudate, but also diminished the amount of total protein, albumin and globulin in the exudating fluid and it was of benefit to wound recovery.

  • Investigation on the Awareness of and Training Needs for the National Essential Medicine (2009 version) in Key-workers or Persons Who are in Charge of Village Town Hospitals/Community Medical Service Centers in Chengdu City

    Objective To provide basic data for further application of national essential medicine list (EML) in Chengdu, through a survey of key-workers or persons, who attended the lecture of EML, in charge of village town hospitals or community medical service centers, and to find out their awareness of and attitudes to the essential medicine and requirements and suggestions for EML training. Methods Based on principles and methods of modern cognitive psychology, we designed the questionnaire and carried out the survey face to face. Two people input the data independently and verified them. Proportions were used to analyze the data through the Excel software. Results (1) The proportions ofcorrect answers to the questions of essential medicine conception, use range of EML and the relationship between EML and essential insurance medicines list were 89%, 91% and 87%, respectively. (2) 46% of the persons investigated (117 persons) chose “always consider preferentially using the essential medicine when prescribing”, 31% (81 persons) “consider using the essential medicine in most cases”. (3) 41% of the persons investigated (103 persons) chose “comparatively welcome” for the EML. (4) Problems of implementing EML in village town hospitals or community medical service centers, in the order of importance decreasing, were: “income and compensation”, “awareness of the essential medicine and prescribing habit of the doctor”, “patient’s cognition of essential medicine and preference to drug use”, “safety and effectiveness of the essential medicine”, “the applicability of essential medicine ” and “the purchase and delivery of essential medicine”, respectively. (5) The ways of getting information of essential medicine were 19.6% (187 person-time) through documents, 18.6% (177 persontime) through conferences and 15.3% (146 person-time) through lectures. (6) 60% of the persons investigated (154 persons) attended training of the essential medicine once or twice. (7) 77% of the persons investigated (196 persons) were trained for less than one week for essential medicine. (8) Requirements and suggestions on the training of essential medicine: 42% (71 person-time) enlarging the targeted people, 22% (38 person-time) increasing the training time,16% (27 person-time) improving practicability and specificity of training contents, 11% (18 person-time) diversifying training ways, 9% (15 persontime) requiring “training according to needs ” which addresses specific problems after investigation. Conclusion Training of essential medicine should be strengthened for medical workers in the village town hospitals or community medical service centers, and researches should be carried out on evaluation of EML applicability, performance of EML implementation, financial aiding, income of medical worker, procurement and reimburse, etc.

  • Xingnaojing Injection for Intracerebral Hemorrhage: A Systematic Review

    Objective To determine whether Xingnaojing injection can improve functional outcome without causing harm in patients with intracerebral hemorrhage. Methods We searched the electronic bibl iographic databases: Cochrane Central Register of Controlled Trials(CENTRAL, Issue 3, 2007), MEDLINE (1996 to November 2007),EMBASE (1984 to November 2007) and China Biological Medicine Database (1978 to November 2007). We also did handsearching to identify other publ ished and unpubl ished data. Data were extracted and evaluated by two reviewers independently with a designed extraction form. The Cochrane Collaboration’s RevMan 4.2.10 was used for data analysis. Results Thirteen randomized trials involving 1 035 patients were included. The number of people who had died or were dependent at the end of long-term follow-up (at least 3 months) were reported in three trials. Eleven trials measured neurological deficit at the end of treatment. No severe adverse events were reported. The results of meta-analyses indicated that Xingnaojing injection might decrease the number of death or dependency compared to the control group, but the difference was not statistically significant (Peto-OR 0.57, 95%CI 0.32 to 1.01); and Xingnaojing injection were associated with a significant improvement in neurological deficit (Peto-OR 3.70, 95%CI 2.67 to 5.13). Conclusion The evidence currently available showed that Xingnaojing injection may decrease the risk of death or dependency, and can also reduce neurological deficit in patients with intracerebral hemorrhage. However, more high-qual ity trials are needed.

  • Prevalence and Risk Factors of Hypertension in Urban and Rural Residents in Chengdu: A Cross-Sectional Survey

    ObjectiveTo investigate the prevalence and risk factors of hypertension in urban and rural residents in Chengdu. MethodsA cluster random sampling survey was carried out from February to October in 2010. A total of 2 011 residents aged 35-70 years in four communities of Chengdu (two communities in urban area, two in rural area) were investigated for the incidence of hypertension. Analysis was performed using SPSS 18.0 software. ResultsThe standardized prevalence of hypertension was 31.30% among residents aged 35-70 years, which was much higher in urban than that in rural:39.03% vs. 24.90% (P < 0.05). The results of multivariate logistic regression analysis showed that, age, waist, heart rate, fasting blood glucose and education level were the major risk factors of hypertension for male; while age, waist, heart rate, fasting blood glucose, triglyceride and uric acid were the major risk factors for female. ConclusionThe prevalence of hypertension in urban is higher than that in rural in Chengdu. Prevention and intervention should be performed more for whom have increased waist circumfluence, abnormal levels of fasting glucose, and increased heart rate.

  • Urban and Rural Residents' Depression and Its Influencing Factors in Chengdu City: A Status-quo Survey

    ObjectiveTo investigate the status-quo of urban and rural residents' depression in the communities of Chengdu city and its influencing factors. MethodsA multi-stage stratified cluster sampling survey was carried out from February to October 2010. Residents aged from 35 to 70 years in 4 urban and rural communities of Chengdu were investigated in the incidence of depression. The health survey questionnaire and PHQ-2 Depression Screening Scale were used for the household investigation. Data was analyzed using SPSS 18.0 software, and the risk factors were also analyzed using multivariate logistic methods. ResultsA total of 2 027 residents were interviewed, including 1 015 urban (616 women and 399 men, with mean age of 58.90 ±9.48 years) and 1 012 rural residents (582 women and 430 were men, with mean age of 54.94 ±9.64 years). The results showed that the overall depression prevalence rate of residents aged from 35 to70 years old in Chengdu was 2.91% (59/2 027); and the prevalence was 4.33% (44/1 015) in urban and 1.48% (15/1 012) in rural. The prevalence was significantly higher in urban areas than that in rural areas (χ2=13.296, P < 0.001). The results of multivariate analysis showed that a significant difference was found in the prevalence of depression among the different marital status groups, and more divorced people suffered from depression. No statistically significant correlation was found between the factors (including occupation, education level and family income status) and depression. ConclusionThe prevalence of depression among residents in urban and rural communities of Chengdu was high, and the divorced population should be paid more attention to in the prevention and treatment of depression.

  • Investigation of the status of human resources in primary health care institutions in the three circles of Chengdu

    Objective To investigate the situation of human resources of primary health care institutions in Chengdu and provide references for rational allocation of health personnel in primary health care. Methods From October to November 2016, self-administered questionnaire was used to investigate the situation of the human resources of health workers in a total of 390 primary health care institutions in the three circles of Chengdu (including the subsidiary counties, towns, and districts) during 2015. The Health Resources Density Index (HRDI) was calculated and its equilibrium was analyzed. Frequency and constituent ratio were used for descriptive analysis. Results Three hundred and seventy valid questionnaires were recovered. The number of clinical doctors, general practitioners, and registered nurses per 10 000 residents in Chengdu was 5.32, 1.38, and 4.32, respectively. Doctors’ and nurses’ HRDI was 0.52 and 0.42 respectively. The number of intermediate and junior professional titles accounted for 15.91% and 52.13% respectively. The ratio of doctors to nurses was 1∶0.81, and the ratio of general practitioners to nurses was 0.32∶1. Structure rationality of distribution density and professional titles of healthcare personnel showed a decreasing trend from the first circle to the third circle. Conclusions The personnel troop of primary health care service in Chengdu has been formed. However, the professional title structure and the distribution of professional categories still need to be improved. The situation of primary health workers in the third circle is relatively backward compared with other circles in Chengdu. It is suggested that the number of nurses and general practitioners should increase properly, and rational intervation should be carryed out in the distribution of primary health workers, so as to balance and develop the grass-root health personnel in the three circles of Chengdu.
