  • STUDY ON MECHANISM AND ABILITY OF BLOOD SUPPLY COMPENSATION IN THE INVOLVED AREA OF LIVER FOLLOWING LIGATION OF THE THIRD GRADE BRANCHES OF DUCTS ENCLOSED IN THE GLISSON’S CAPSULE

Objective To observe the blood circulation compensation in the involved area of the liver following ligation of the third grade branches of hepatic artery and portal vein and bile duct enclosed in Glisson’s capsule. Methods Ligation of the third grade branches of these ducts was carried out in 7 pigs. Uptake of 99mTc-EHIDA in the liver was scanned with SPECT pre-and post-operatively. Liver angiography of hepatic artery and portal vein were taken at regular interval. Corrosion casts of these ducts were made with ABS following extirpation of the liver at the end of experiment. The histological specimens were examined with electronic microscope. Results Compensatory circulation occurred between involved and noninvolved part of the liver through the sinusoids in 30-60 minutes after ligation. In the 6 weeks following the procedure, there was also blood supply in the affected region of liver, and collateral developed through hepatic aterioles and capillaries. Conclusion Liver has an ability to establish compensatory blood supply on the condition of ischema in a local region of liver.

  • Analysis of the Causes of Cardiopulmonary Complications in Elderly Patients after Thoracotomy

Objective To investigate the causes and prevention of cardiopulmonary complications in elderly patients after thoracotomy. Methods Respiratory and circulatory status were monitored and postoperative complications were documented in 58 elderly patients either aged over 65 years, with major organ dysfunction or underwent highly invasive procedures (experimental group) during July 2001 to Dec. 2003. The results were compared with those from 56 young patients(〈65 years) receiving thoracotomy in the same period (control group). Results Patients in experimental group had significantly more preoperative cardiopulmonary co-morbidities and poorer spirometry than those in control group (P〈0.05). Four patients died after operation in experimental group. There were significantly more postoperative complications in experimental group than those in control group [58. 6% (34/58) vs. 17. 9% (10/56), P=0. 000], especially functional complications [51.7% (30/58) vs. 12.5% (7/56), P = 0. 000]. The rate of respiratory complications was also significantly higher in the experimental group . Multivariance logistic regression showed that preoperative pulmonary morbidity (OR=5.4) and obesity (OR=4. 9) were independent risk factors for pulmonary complications after thoracotomy in elderly patients. Conclusions Cardiopulmonary co-morbidities commonly seen are responsible for surgical morbidities, especially the functional complications in elderly patients underwent thoracotomy. Respiratory complications are the major causes of death in the elderly after thoracotomy. Pulmonary co-morbidity and obesity are independent risk factors for respiratory complications. Supraventricular tachycardia is the major type of cardiovascular complications after thoracotomy and is predicted by preoperative cardiovascular morbidity. Close monitoring of cardiopulmonary status of the elderly may identify patients at risk in developing functional complications and help improve surgical outcome.

  • STUDY ON MECHNISM OF THE CIRCULATION RECONSTRUCTING OF SKIN FLAP AFTER EARLY-REPEATED SHORT ISCHEMIA TRAINING

Objective To investigate the effects of repeated shortischemia training on flap survival area, vascular endothelial growth factor and the microvascularsel density. Methods Seventy-two rabbits were divided into:the experimental group(n=64), the skin flaps were constructed in two sides of back, one side flap were given ischemia training for 15 minutes and 8 times one day at the pedicles from the 1st to 8th day after operation (group A), the other side flap was served as a control (group B), the corresponding site was only marked as a blank control group (group C).Then, 8 pedicles of group A and group Bwere isolated every day. The surviving area of all skin flaps were calculated on the5th day after isolating operation. The vascular endothelial growth factor(VEGF)and microvessel density(MVD) of the 3 groups were checked with immunohistologochemical staining. Results After the operation, all animalswere survival with normal vitality. The survival flap area of group A were significant more than that of group B after 3 days(Plt;0.05).The expressions of VEGF and MVD of group A and group B were higher than group C. The expression of VEGF of group A was significantly higher than that of group B(Plt;0.01). The counting of MVD of group A was also significantly higher than that of group B(Plt;0.05). There was positive correlation between flap survival area and MVD in group A. The relation of time point was n and n 2 respectively, correlation coefficient was 0.850. As well as MVD and VEGF were positive correlation, correlation coefficient was 0.801. Conclusion Early repeated shortischemia training can increase flap survival area, the mechanism maybe involve the increased expression of VEGF, which can increased skin flap microvascular density.

  • TREATMENT OF ISCHEMIA IN LOWER EXTREMITIES BY PRIMARY ARTERIZATION IN SITU OF VANA SAPHENA MAGNA

In order to summarize the experience in the treatment of ischemic necrosis of lower extremities resulted from thrombotic occluded angittis, 15 cases were reported, which were treated by primary arterization in situ of V. Saphena magna. With a period of follow-up, 4-26 months on the average, it was found that symptoms in 14 cases were much allayed obviously, except 1 case with little relief. It suggested that primary arterization in situ of V. saphena magna could improve the circulation of the ischemic extremity rapidly without any influence of venous reflux.

  • Effects of CO2 Pneumoperitoneum and Posture on Circulation Function for Elderly Patients with Rectal Cancer During Laparoscopic Operation

ObjectiveTo explore the effects of CO2 pneumoperitoneum and posture on circulation function for elderly patients with rectal cancer during laparoscopic operation. MethodsRetrospective analysis of clinical data of 48 elderly patients with rectal cancer was performed, and the data included the heart rate (HR), systolic blood pressure (SBP), diastolic blood pressure (DBP), mean arterial pressure (MAP), cardiac output (CO), cardiac index (CI), central venous pressure (CVP), pH, partial pressure of oxygen (PaO2), and partial pressure of carbon dioxide (PaCO2) at before anesthesia (T1), after anesthesia (T2), after the establishment of CO2 pneumoperitoneum (T3), placement in trendelenburg position after operation (T4), 30 minutes after operation (T5), 60 minutes after operation (T6), 90 minutes after operation (T7), and 120 minutes after operation (T8). ResultsThe levels of HR, SBP, DBP, MAP, CO, CI, pH, PaO2, and PaCO2 at 8 time points didn't statistically differed with each other (P>0.05), but the levels of CVP at T7 and T8 time point were significantly higher than those of T1 and T2 time point (P<0.05). ConclusionsDuring laparoscopic operation period for the elderly patients with rectal cancer, the CVP will gradually increase. If the preoperative assessment and administration are done well during peri-operation period, laparoscopic operation is safe for the elderly patients with rectal cancer.
