  • Administration of Medical Materials for the Daytime-sickroom During Wenchuan Earthquake

    Objective To summarize the experience of preparation and administration of medical materials for the daytime-sickroom during the treatment of patients who were injured during Wenchuan earthquake in order to supply management experience for future emergency situations. Methods Data concerning the preparation and administration of medical materials and medical treatment for the daytime-sickroom were collected systematically. Results The high level of activity and order during the rescue provided quality care to patients injured in the earthquake. All 224 patients were treated effectively from May 12-29.Conclusion The daytime-sickroom acts as a combination sickroom, which allowed flexibility. The daytime-sickroom can manage medical materials and succesfully insure the treatement of patients when emergent events happen.

  • Correlation between urinary disorders and imaging changes of cerebral small vessel diseases in community-dwelling populations

    ObjectiveTo explore the correlation between urinary disorders and imaging changes of cerebral small vessel diseases (CSVDs) in community-dwelling populations.MethodsA cross-sectional analysis was conducted on participants enrolled in the Shunyi study from June 2013 to April 2016. Eligible participants were community-dwelling populations aged ≥35 years with interpretable magnetic resonance imaging scans and no history of stroke or urinary system diseases. Data on demographic characteristics, vascular risk factors, cognitive functions, and urinary disorders (including any form of urinary disorders, incontinence, daytime urination frequency, and nocturnal urination frequency) were collected. Imaging changes including white matter hyperintensities (WMHs), lacunes, cerebral microbleeds (CMBs), perivascular spaces (PVSs), and brain volume were measured using 3 T magnetic resonance imaging. Logistic regression model analysis was performed to identify the potential correlations between urinary disorders and imaging markers of CSVD.ResultsA total of 916 participants (with a mean age of 57.4 years; 36.2% were males) were finally enrolled in this study based on the enrollment criteria. CSVD imaging changes of WMHs, lacunes, CMBs, PVSs or brain volume were not associated with any form of urinary disorders in multivariable models (P>0.05). CSVD imaging changes were not associated with presence of urinary incontinence (P>0.05). In terms of urinary frequency, the CSVD imaging changes were not related to nocturnal urinary frequency (P>0.05). However, lower brain volume was correlated with daytime urination frequency [3-5 vs. <3 times per day: odds ratio (OR)=2.520, 95% confidence interval (CI) (1.278, 4.972), P=0.008; >5 vs. <3 times per day: OR=3.115, 95%CI (1.317, 7.372), P=0.010].ConclusionBrain atrophy may affect daytime urination frequency in community-dwelling populations.

  • Analysis of the factors influencing the same-day discharge of surgical patients based on the day pattern

    Objective To analyze the influencing factors of single-center day surgery patients who are discharged from the hospital on the same-day, in order to provide reference and basis for the clinical practice of follow-up day surgery. Methods The electronic medical records of patients who underwent day surgery in the Day Surgery Center of the First Affiliated Hospital of the Air Force Military Medical University between February and October 2021 were analyzed retrospectively. The patients were divided into the same-day discharge group and non-same-day discharge group. The baseline data and perioperative indicators of the patients were analyzed. Results A total of 857 patients were included, including 264 patients (30.81%) in the same-day discharge group and 593 patients (69.19%) in the non-same-day discharge group. Univariate analysis showed that there were significant differences between the two groups in gender, age, body mass index, whether the first one, disease classification, anesthesia method, and intraoperative blood loss (P<0.05). Logistic regression analysis showed that gender, whether the first one, disease classification, surgical grade, anesthesia method, and intraoperative blood loss were independent factors affecting the delayed discharge of patients undergoing daytime surgery (P<0.05). Conclusions There are many factors that affect day surgery patients’ discharge. It is suggested that more rigorously screen patients for day surgery, improve medical technology, strengthen out-of-hospital continued care, and optimize management procedures, so as to shorten the time of patients in hospital and provide more information for patients, and provide more efficient and convenient medical services for patients.

  • Exploration of intelligent daytime surgery full process management based on interactive design information system

    The full process information management of daytime surgery can help medical staff complete centralized patient management, improve the closed-loop quality of daytime surgery, and maximize the efficiency and management level of hospital daytime surgery operation. Since 2021, the First Hospital of Lanzhou University has integrated internal information exchange resources, big data, and artificial intelligence, created a full process management platform for daytime surgery, and explored the intelligent management of daytime surgery processes. This article shares the experience of building an intelligent daytime surgery full process management model based on interactive design information system from the aspects of platform interaction design, intelligent management mode, application effectiveness, in order to provide a reference for optimizing intelligent closed-loop management of daytime surgery.

  • A comparative study of day operation in West China Hospital of Sichuan University and the nationally recommended catalogue for day surgery

    Objective To analyze the current status of day surgery in West China Hospital of Sichuan University and compare it with the Recommended Catalogue of Day Surgery (2022 Edition), in order to provide reference for the update and expansion of day surgery procedures in hospitals. Methods The day surgical procedures carried out in West China Hospital of Sichuan University from October 2009 to July 2023 were collected. The day surgical procedures were classified based on surgical specialties, types of procedures, methods of operation, and surgical grading, etc. The day surgical procedures in West China Hospital of Sichuan University were compared with those in the Recommended Catalogue of Day Surgery (2022 Edition). Results A total of 576 types of daytime surgeries were carried out in West China Hospital of Sichuan University, involving 15 specialties. A total of 662 types of day surgeries were recommend in the nationally recommended catalogue for daytime surgery, involving 11 specialties. Among the nationally recommended surgical procedures, West China Hospital of Sichuan University had carried out 233 types of surgical procedures, and 26 additional types of surgical procedures were carried out as outpatient surgeries. According to the classification of surgical difficulty, the Level Ⅱ and Ⅲ surgeries were mainly carried out in West China Hospital of Sichuan University, and the Level Ⅱ surgeries were mainly recommend in the nationally recommended catalogue for day surgery. Conclusions The high-quality development of day surgery in China should not be limited to the supplement of surgical procedures. It is necessary to strive to increase the proportion of minimally invasive and fourth-level surgeries while ensuring medical quality and safety, and improve the overall medical level of day surgery.

  • Feasibility exploration of microscopic spermatic vein ligation for unilateral varicocele under daytime surgical mode

    Objective To explore the feasibility of microscopic spermatic vein ligation for unilateral varicocele under daytime surgical mode. Methods The clinical data of patients with unilateral varicocele who underwent daytime surgical treatment at the General Hospital of Northen Theater Command between January and December 2022 were retrospectively analyzed. The relevant indicators of patients were collected, including age, surgical time, intraoperative blood loss, total hospitalization expenses, and postoperative condition. Results A total of 159 patients were included, aged 18-41 years, with an average age of (27.79±4.80) years, an average surgical time of (49.23±5.57) minutes, and an average intraoperative blood loss of (8.94±3.58) mL. One patient had fever, nausea and acid regurgitation on the 2nd day after discharge, and one patient had urinary retention on the day after operation. Five patients had a pain score of ≥3 in the surgical area on the 1st day after surgery. All patients had no postoperative incisions with bleeding or hematoma. Follow up showed that all patients returned to normal life on the 3rd day after surgery and returned to work on the 10th day after surgery. At 3-6 months after surgery, the sperm quality of 94 patients improved. One patient experienced disease recurrence 6 months after surgery. Conclusion Microscopic spermatic vein ligation for unilateral varicocele under daytime surgical mode is safe and feasible, and is worthy of promotion.

  • Investigation and analysis of the current situation of daytime treatment services under the centralized treatment mode in a large-scale military hospital

    Objective To analyze the current situation of the centralized treatment mode for daytime treatment (excluding daytime surgery) services in a large-scale military hospital, and provide practical support for the management and operation of the centralized treatment mode for daytime treatment. Methods Data on patients undergoing centralized daytime treatment in the Department of Ambulatory Medicine, First Medical Center of PLA General Hospital were retrospectively collected between February 7, 2021 and December 31, 2023. The patients’ condition, daytime treatment operation indicators, and quality control indicators were statistically analyzed. Results A total of 14903 patients underwent daytime treatment, including 8548 females and 6355 males. The patients’ age ranged from 18 to 74 years old. There were 9757 patients (65.47%) who had been hospitalized ≥ 2 times. The main payment method for patients was medical insurance (83.16%). Breast cancer accounted for the highest proportion (26.45%). The main treatment methods for patients were targeted therapy and immunotherapy (68.03%). The patients experienced a total of 1146 drug-related adverse reactions, mainly involving anti-tumor drugs. Among them, 15 cases were drug-related adverse reactions treated on the day of treatment, and the other 1131 cases were drug-related adverse reactions that occurred outside the hospital. After optimizing the entire process of daytime hospitalization (timed based treatment) in January 2023, compared to 2022, the average waiting time for patients after admission was reduced by 26.89%, the bed utilization rate increased by 9.09%, and patient satisfaction increased to 98.98%. In 2023, the default rate of daytime treatment was 1.15%, the cancellation rate of daytime treatment on the day of treatment was 4.39%, and the discharge follow-up rate reached 100%. Conclusions The daytime treatment under the centralized treatment mode in the the Department of Ambulatory Medicine, First Medical Center of PLA General Hospital focuses on breast cancer patients, and the main treatment methods are targeted therapy and immunotherapy. Time based treatment is beneficial for shortening the average waiting time of patients after admission, improving bed utilization and patient satisfaction.

  • Association between gastroesophageal reflux disease and obstructive sleep apnea: a Mendelian randomization study

    ObjectiveTo investigate the causal relationship between gastroesophageal reflux disease (GERD) and obstructive sleep apnea (OSA) with its typical symptoms (snoring and daytime sleepiness) by using Mendelian randomization (MR). MethodsThe inverse-variance weighted method was used as the main analysis method to assess the causal effect. Sensitivity and pleiotropy analyses were carried out using leave-one-out and MR-Egger analysis, and then heterogeneity tests were conducted. ResultsIn the MR analysis, genetically predicted GERD was associated with a greater risk of OSA (IVW: OR=1.528, 95%CI 1.374 to 1.699, P=5.315E‒15). Additional MR results were consistent with the IVW results, and no pleiotropy or heterogeneity was found. We also discovered a significant causal relationship between GRED and snoring (IVW: OR=0.959, 95%CI 0.949 to 0.969, P=1.507E‒15), and daytime sleepiness (IVW: OR=1.024, 95%CI 1.021 to 1.036, P=4.580E‒5), with no evidence of pleiotropy. ConclusionThe MR study supports a causal effect between GERD and OSA with its typical symptoms (daytime sleepiness and snoring).
