Abstract: Objective To introduce the new procedure of endobronchial ultrasoundguided transbronchial needle aspiration (EBUSTBNA) for staging lung cancer and diagnosing thoracic diseases, in order to determine its value in the evaluation of thoracic diseases. Methods We retrospectively reviewed the data of all patients examined with EBUSTBNA our institution between September 2009 and May 2010. Among the patients, there were 75 males and 31 females with an average age of 62.3 years old. Based on their primary indication, we divided all the 106 patients into three categories. (1) There were 76 patients with known or bly suspected lung cancer. Enlarged mediastinal lymph nodes on radiographic examination of the chest (≥1.0 cm) were detected in all the patients. (2) There were 22 patients with enlarged mediastinal lymph nodes or mediastinal masses of unknown origin. (3) There were 8 patients with pulmonary mass located close to the central airways. Results (1) 76 patients underwent EBUSTBNA for known or bly suspected lung cancer. Among them, 58 patients were confirmed to have mediastinal lymph nodes metastasis on EBUSTBNA. Sixteen in the 18 patients with negative EBUSTBNA underwent thoracoscopy or thoracotomy for pulmonary resection and mediastinal lymph node dissection. Postoperative pathology confirmed that 12 patients did not have metastatic nodes, 2 patients had metastatic nodes and 2 other patients had benign lesions within the lung. The diagnostic sensitivity, specificity and accuracy of EBUSTBNA for the mediastinal staging of lung cancer were 96.66%(58/60), 100.00%(12/12) and 97.22%(70/72), respectively. (2) 22 patients underwent EBUSTBNA for the evaluation of mediastinal adenopathy or mass in the absence of any identifiable pulmonary lesion. Among them, 7 had malignancy, 13 had benign diseases on EBUSTBNA and the sensitivity of EBUSTBNA in distinguishing malignant mediastinal diseases was 87.50% (7/8). (3) 8 patients with pulmonary mass located close to the central airways were accessed by EBUSTBNA. Definite diagnosis was achieved in 7 patients, and lung cancer was detected in 6 patients. The sensitivity and the diagnostic accuracy of EBUSTBNA for the diagnosis of unknown pulmonary mass was 85.71%(6/7) and 87.50%(7/8), respectively. All the procedures were uneventful, and there were no complications. Conclusion EBUSTBNA is a highly effective and safe procedure. We believe that EBUSTBNA should be used routinely in the diagnosis and staging of thoracic diseases.
Endobronchial ultrasound guided tranbronchial needle aspiration (EBUS-TBNA) is a new technology developed in the past 10 years. In the USA, EBUS-TBNA has been recommended as an important means for preoperative lymph node staging of lung cancer, and becomes a new standard for mediastinal staging of lung cancer. A large number of clinical data shows that EBUS-TBNA is a novel approach which owes the advantages of accuracy in diagnosis and safety in operating. What's more, its value in the diagnosis of some disease in chest has been widely recognized. The aim of this article is to review the current application of EBUS-TBNA in the diagnosis of early lung cancer, diagnosis and tumor staging of advanced lung cancer, the relationship between EBUS-TBNA and traditional inspection including CT, positron emission tomography/computed tomography (PET) and mediastinoscopy, and the application value of EBUS-TBNA for superior vena cava syndrome and some non-neoplastic diseases.
ObjectiveTo investigate whether liquid-based cytology (LBC) can improve diagnostic value of cytological assessment of endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA). MethodsA cohort of 600 cases who underwent EBUS-TBNA from June 2012 to September 2013 was enrolled in this prospective study in West China Hospital. EBUS-TBNA was carried out under local anesthesia and moderate sedation. The procedure was performed with echobronchoscopes. Histological tissues were stained with hematoxylin and eosin for further study. Additional immunohistological analysis was performed for establishing a reliable diagnosis as necessary. Aspirates were smeared on glass slides and separate aspirates were processed by the monolayer SurePath method. ResultsIn total, 480 cases of malignant tumors and 120 cases of benign lesions were confirmed by histological examination. The sensitivity of SurePath liquid-based preparations and conventional smears was 82.1% and 56.0%, and the specificity was 87.5% and 82.5%, respectively. The combined specificity was 100.0%. The positive predictive value of two methods combined was 96.3% and 92.8%, whereas the negative predictive value was 54.9% and 31.9%, respectively. The difference between the two methods was significant (P < 0.05). ConclusionsLBC preparation can improve cytological assessment of EBUS-TBNA. Histological study is necessary when the cytological diagnosis is obscure.
ObjectiveTo evaluate the diagnostic value of endobronchial ultrasound technology in combination with LungPoint virtual navigation system for pulmonary peripheral nodules. MethodsRetrospective analysis of 317 patients with peripheral pulmonary nodules who underwent endobronchial ultrasound at the endoscopy center of Shanghai Pulmonary Hospital from January 2021 to March 2022 was used as the study population. They were divided into the endobronchial ultrasound group (EBUS-GS group) and the virtual navigation combined with endobronchial ultrasound group (VBN+EBUS-GS group) according to whether the path was planned with the LungPoint virtual navigation system preoperatively or not. The diagnostic rate, bronchoscopic arrival rate, arrival time, operation time and complications were compared between the EBUS-GS group and the VBN+EBUS-GS group, and the factors associated with the diagnostic rate of endobronchial ultrasound were analyzed. ResultsThere were 101 malignant nodules and 216 benign nodules. The mean size of lung nodules was (1.9±0.7) cm and (1.8±0.6) cm in the EBUS-GS and VBN+EUBS-GS groups, respectively (P>0.05); The time to reach the lesions was 7 (5 - 9) and 4 (3 - 5) min, and the total operation time was 18 (16 - 20) and 16 (14 - 18) min, respectively (P<0.05). The arrival rates of endobronchial ultrasound in the two groups was 82.6% and 98.1% (P<0.05), respectively. The overall diagnostic rate, malignant nodule diagnostic rate and benign nodule diagnostic rate of the two groups were 61.3% vs. 64.8%, 67.9% vs. 68.6% and 57.6% vs. 63.1% respectively (P>0.05). There was one pneumothorax in the EBUS-GS group after examination (0.6%, 1/155). No complications such as hemoptysis or infection occurred in all patients. ConclusionsLungPoint virtual navigation can significantly improve the arrival rate of lesions under endobronchial ultrasound, significantly reduce the arrival time of endobronchial ultrasound to the lesions and the total operation time, which is beneficial to improve the efficiency of clinical examination.
