  • RELATIONSHIP BETWEEN EXPOSURE TIME AND CRYOPRESERVATION OF ARTICULAR CARTILAGE

    OBJECTIVE To search an optimal method for improving viability of cryopreserved articular cartilage. METHODS Articular cartilage which was sampled from the rabbits were randomly divided into 5 groups. Fresh cartilage was group I, other groups were frozen. Before frozen, other cartilage was exposured in 10% DMSO at 4 degrees C for 30 minutes(group II), 1 hour(group III), 2 hours (group IV), 4 hours(group V), then were stored in liquid nitrogen for 1 week. Viabilities of the chondrocytes were detected by Typan-blue staining, electron transmission microscope, and determination of incorporation 3H-TdR after the temperature returned to normal. RESULTS 1. The cells were injuried at different extent after the cartilage was frozen. In group I, survival rate of cells was 96% and incorporation of 3H-TdR was (4,953.13 +/- 583.27)%, statistic difference was significant between group I and other groups(P lt; 0.01). The microstructure of group I was normal while other groups all had damage of the organella, 2. Structures and functions of cells in group IV were best among frozen groups. Organella were less damaged than group II, III, V, survival rate of cells was 56% and incorporation of 3H-TdR was (1,139.88 +/- 146.39)%, statistic difference was significant between group IV and group II, III, V(P lt; 0.01). CONCLUSION If cartilage are exposured in 10% DMSO at 4 degrees C for 2 hours before frozen, optimal cryopreservation can be achieved.

  • Definition and Application of Population Impact Measures

    This paper is aimed to introduce the definition and application of Population Impact Measures (PIMs). The PIMs use Number Need to Treat (NNT) for reference and generalize the variables of clinical research to interest population, which then can be used to measure the effectiveness of interventions and the harmfulness of risk factors, so as to provide evidence for making public health policy. Of the PIMs, the variables used for measuring the effectiveness of interventions include Disease Impact Number (DIN), Population Impact Number (PIN) and Number of Events Prevented in your Population (NEPP); The variables used for measuring the harmfulness of risk factors include Exposure Impact Number (EIN), Exposed Cases Impact Number (ECIN), Population Impact Number (PIN), Case Impact Number (CIN) and Population Impact Number of Eliminating a Risk factor (PIN-ER-t).

  • Clinical Analysis of Thyroid Lobectomy by Meticulous Capsular Dissection and Exposure of Recurrent Laryngeal Nerve in 452 Cases

    Objective To explore the clinical significance on protection of parathyroid and recurrent laryngeal nerve (RLN) by meticulous capsular dissection and exposure of RLN in thyroid lobectomy. Methods Clinical data of 452 patients who underwent thyroid lobectomy by meticulous capsular dissection and exposure of RLN in our hospital from Jan. 2010 to Dec. 2012 were retrospectively analyzed. Results All of the 452 cases underwent thyroid lebectomy successfully without death, and the operative time was 45-110min (average 60 min), the blood loss was 5-100mL (average 20mL). The pathological results showed that there were thyroid adenoma in 193 cases, nodular goiter in 175 cases, Hashimoto thyroiditis in 38 cases, thyroid cancer in 46 cases. After operation, 4 cases suffered RLN injury, 1 of the 4 cases recovered after removal of drainage tube, and other 3 cases recovered during 0.5 to 3.0 months. In addition, 2 cases suffered laryngeal nerve injury whose symptoms disappearred within 1 week, 5 cases suffered parathyroid founctional damage without permanent hypocalcemia whose symptom had kept 1-5 days after treatment. Three cases were reoperated because of bleeding, including branch of anterior venous bleeding in 1 case, thyroid side arterial tube bleeding in 1 case, and thyroid stump bleeding in 1 case. Twenty one cases suffered hypothyroidism in 1 month after operation, and no recurrence happened during the followed up period. Conclusions Meticulous capsular dissection can effectively protect function of parathyroid and reduce the injury probability of RLN. Exposure of RLN is safe and feasible, which plays an important role in avoiding serious RLN injury.

  • Investigations of 451 Coal Worker's Pneumoconiosis complicated with COPD in Western District of Beijing

    ObjectiveTo understand the prevalence,risk factors,clinical features and the medication of coal worker's pneumoconiosis(CWP) complicated with chronic obstructive pulmonary disease (COPD). MethodsPulmonary function testing results,clinical symptoms,medication of 451 patients with CWP were collected. Then, the risk factors relevant to the incidence of COPD in CWP were analyzed with Cox multivariate regression. ResultsThe prevalence of COPD in CWP was 44.6%,and the incidence was rising with the increasing of CWP phases, exposure duration,smoking index,with the odds ratio of 3.20,1.09,and 1.01,respectively.The patients with CWP and COPD were suffered more symtoms with a CAT score of 25.5 but received less regular medications. ConclusionsThe incidence of COPD in CWP is obviously higher than that in common populaton.Exposure duration, smoking index and phases of CWP are the high risk factors for COPD in the population of CWP. The patients with CWP and COPD are suffered more symtoms but receive irregular medications.

    ObjectiveTo explore various exposure technology of gastroesophageal region and their advantages and disadvantages by laparoscopic surgery in upper abdominal. MethodThe related literatures on various exposure technology of gastroesophageal region by laparoscopic surgery in upper abdominal at home and abroad in recent years were collected and reviewed. ResultsNow, there were main three exposure technologies of gastroesophageal region by laparoscopic surgery in upper abdominal, including traditional retractor liver retraction, suspension liver retraction, and adhesion liver retraction. Each method had its own advantages and disadvantages. Conclusionthe most suitable exposure method of gastroesophageal region by laparoscopic surgery in upper abdominal should be selected according to situations of surgery and patient.
