  • BIOMECHANICAL STUDY ON EFFECT OF POLYMETHYLMETHACRYLATE AUGMENTATION ON CERVICAL STABILITY AFTER ANTERIOR CERVICAL INTERBODY FUSION

    Objective To evaluate the effect of polymethylmethacrylate (PMMA) augmentation on cervical stabil ity after anterior cervical interbody fusion (ACIF) before and after fatigue. Methods Twelve porcine cervical spines (C3-7) were subjected to testing angular displacement parameters, including the range of motion (ROM), neutral zone (NZ), and elastic zone (EZ), in nondestructive flexion and extension, right/left lateral bending, and left/right rotation on Motion Analysis motion capture system and MTS-858 servo-hydraul ic testing machine. Intact cervical spines served as control group (group A); oneleveldiscectomy and fusion was performed with anterior plate fixation based on group A as group B; flexion and extension,left/right lateral bending (5 000 cycles) fatigue testing based on group B as group C; the augmentation screw channel was used based on group C as group D; and flexion and extension, left/right lateral bending fatigue testing were performed based on group D as group E. Results The ROM, NZ, and EZ in group A were significantly different from those in other groups (P lt; 0.05) at flexion/extension, left/right bending, and left/right rotation. The ROM, NZ, and EZ in group B were significantly smaller than those in group C (P lt; 0.05) in flexion/extension, left/right bending, and left/right rotation, but there was no significant difference when compared with group D (P gt; 0.05). The ROM and NZ in flexion/extension and the EZ in flexion in group B were significant smaller than those in group E (P lt; 0.05), but there was no significant difference in the other indexes (P gt; 0.05). The ROM, NZ, and EZ in group C in flexion and extension, left/right lateral bending, and left/right rotation were significantly higher than those in groups D and E (P lt; 0.05). The ROM and NZ in flexion and extension and left/right lateral bending, and the ROM in left/right rotation, and the EZ in flexion and extension, right bending, and left/right rotation in group D were significantly smaller than those in group E (P lt; 0.05), but there was no significant difference in the other indexes (P gt; 0.05). Conclusion PMMA augmentation can significantly increase the instant cervical stabil ity and provide a biomechanics basis in cervical anterior plate fixation.

  • Fatigue and Its Related Factors in Patients with Maintenance Hemodialysis

    ObjectiveTo examine the prevalence of fatigue and analyze its related factors in patients undertaking maintenance hemodialysis. MethodsA total of 235 patients undertaking maintenance hemodialysis in West China Hospital were investigated from January to February 2013. The measures used in the study included Piper Fatigue Scale Revised and socio-demographic questionnaire. ResultsThe prevalence of fatigue in hemodialysis patients was 83.4%. The rates of severe fatigue, moderate fatigue and low fatigue were 10.2%, 43.0% and 30.2%, respectively. The findings of univariate analysis indicated that patients with different age and sleep situation, with or without complications, with or without fatigue after dialysis reported different total fatigue scores. ConclusionThe prevalence of fatigue is high in hemodialysis patients. Aged 60 years and above, with complications, poor sleep quality were significant factors related to fatigue in patients with hemodialysis.

  • Analysis of Fatigue and Life Quality of Patients with Diabetic Foot and Research on the Influencing Factors

    ObjectiveTo understand the status of life quality and fatigue level in patients with diabetic foot and to explore its influencing factors. MethodsRandom sampling method was used in collecting the general data of 80 diabetic foot patients from January 2013 to January 2014. The questionnaire of Fatigue Scale-14 and Diabetes Specific Quality of Life Scale were analyzed, and the influencing factors of fatigue and life quality were investigated. ResultsThe total score mean value of fatigue was 8.63±3.39 and the total score mean value of life quality was 71.00±19.84. The life quality and fatigue in patients with diabetic foot were positively correlated (P < 0.01) . ConclusionsThe life quality of patients needs to be improved. We should especially pay attention to their mental health and try to meet their psychological needs, in order to ease the patients’ fatigue level and improve their life quality. The key for diabetic foot nursing is to discover and assess the fatigue symptoms of patients with diabetic foot diseases.

  • Effect of Spiritual Care on Improving the Psychology Stress Levels of Relatives of Patients with Terminal Cancer

    ObjectiveTo explore the effect of spiritual care on improving the psychological stress levels of relatives of patients with terminal cancer. MethodsDuring January 2013 and January 2014, 220 relatives of patients with terminal cancer were selected. Convenience sampling method was adopted to select 100 relatives out of 190 who were agreed to be participated in the investigation, who were divided into the trial group and the control group with 50 in each according to the random alphabet method. The control group was given routine care and psychological counseling, and the trial group was given spiritual care intervention additionally. Before intervention, all of the individuals in both of the two groups should conduct the questionnaire of general demographic data, caregiver stress scale, fatigue rating scale, quality of life scale (QLS), social support scale (SSS), and relatives stress scale (RSS). ResultsAfter one month's intervention, caregiver stress scale score (52.14±4.75), fatigue rating score (76.75±8.69), RSS score (15.71±3.97), SSS score (22.59±2.22), the QLS score (66.9±7.5) in the trial group were significant better than those in the control group (P < 0.05). After intervention, all the scores in the trial group were significant better than whose before the intervention (P < 0.05). ConclusionFor the relatives of the patients with terminal cancer, spiritual care can reduce the occurrence rate of stress and fatigue, relieve the psychological stress level, and improve the social support and quality of life.

  • Relationship between Fatigue and Quality of Life in Patients with Obstructive Sleep Apnea

    ObjectiveTo assess the fatigue in patients with obstructive sleep apnea hypopnea syndrome (OSAHS), and analyze the factors caused fatigue and the relationship between quality of life (QOL) and fatigue. MethodsOne hundred and sixty-nine patients with OSAHS and 78 subjects without OSAHS diagnosed by polysomnography (PSG) between December 2010 and March 2011 in West China Hospital were recruited in the study. Fatigue was assessed by using multidimensional fatigue inventory (MFI), excessive daytime sleepiness by Epworth sleepiness scale(ESS), QOL by functional outcomes of sleep questionnaire (FOSQ). ResultsFatigue in the patients with OSAHS was more severe than that of the controls (51.06±13.39 vs. 44.82±9.81, P < 0.001), but no difference was revealed in the patients with different degree of OSAHS. Fatigue was positively correlated with ESS score(r=0.210), total sleep time intervals(r=0.156), and the ratio of time of SpO2 below 90% in total sleep time(r=0.153)(P < 0.05), and was negatively correlated with the average oxygen saturation(r=-0.171, P < 0.05) and all subscales of FOSQ(P < 0.01). ConclusionsFatigue in patients with OSAHS is more severe than that of controls. Fatigue can significantly reduce QOL, and the impact is greater than that of excessive daytime sleepiness.

  • Effect of prolonged oxygen inhalation after total hip arthroplasty on early postoperative rehabilitation in elderly patients

    ObjectiveTo assess the effect of prolonged oxygen inhalation after total hip arthroplasty (THA) on early postoperative rehabilitation in elderly patients.MethodsPatients who underwent primary unilateral THA from June to December 2018 were divided into a conventional oxygen inhalation group (control group) and a prolonged oxygen inhalation group (trial group) according to a random number table. The control group received oxygen inhalation via a single nasal catheter from their return to the ward after surgery until 08:00 a.m. on the first day postoperatively. In addition to routine oxygen inhalation, the trial group received oxygen inhalation during the rehabilitation exercise in bed and before half an hour of floor activities on the first and second day after operation. Oxygen saturation, heart rate, respiratory rate, blood pressure, pain, fatigue score, hip mobility and Pittsburgh Sleep Quality Index (PSQI) scores were recorded and analyzed at 24 and 48 hours after surgery.ResultsA total of 60 patients were included finally, 30 patients in each group. At 24 and 48 h after surgery, the differences were statistically significant in the respiratory rate [(20.30±1.20) vs. (21.40±1.04) breaths per minute, (18.87±1.14) vs. (22.03±0.85) breaths per minute], oxygen saturation [(94.70±2.34)% vs. (90.60±0.73)%, (96.40±1.81)% vs. (91.20±0.85)%], abduction angles of hip joint [(33.50±5.44) vs. (29.31±5.30)°, (38.67±2.60) vs. (33.00±4.84)°], pain scores (3.03±1.27 vs. 5.07±1.14, 1.43±0.97 vs. 3.60±1.13) and fatigue scores (4.17±1.34 vs. 8.20±0.61, 3.23±1.38 vs. 7.43±1.19) between the trial group and control group (P<0.05). PSQI scores (2.20±0.76 vs. 3.97±0.76) on the first day after surgery, and hip flexion [(105.17±4.82) vs. (99.50±2.40)°] , heart rate [(72.86± 6.38) vs. (79.40±3.97) beats per minute], diastolic blood pressure [(71.43±10.24)vs. (77.57±4.59) mm Hg (1 mm Hg=0.133 kPa)] at 48 h after surgery in the trial group were better than the those in control group (P<0.05).ConclusionCompared with conventional oxygen inhalation, prolonged oxygen inhalation after THA in elderly patients can improve postoperative pain, fatigue, sleep and hip function.

  • Mediating effect of depression on fatigue and quality of life in asthma patients

    ObjectiveTo explore the mediating effect of depression between fatigue and quality of life in patients with bronchial asthma, and to provide a clinical basis for alleviating fatigue and improving the quality of life in asthma patients.MethodsBronchial asthma patiens were recruited with convenience sampling method to conducta questionnaire survey in outpatients department of respiratory of a tertiary hospital in Guangxi from November 2018 to March 2019. The general data questionnaire, the Chinese version of Checklist Individual Strength-Fatigue, the Self-rating Depression Scale, the Questionnaire for Asthma Quality of Life in Adult, and the Asthma Control Test were used. We collected data to analyze the mediating effects of depression between fatigue and quality of life in patients with bronchial asthma.ResultsFinally, 120 patients were included. There were statistically significant differences in quality of life among patients with different ages, education levels, residences, time of high incidence of symptoms, degrees of lung function impairment, asthma control conditions, and degrees of depression, and between patients with fatigue and the ones without fatigue (P<0.05). The quality of life score was negatively correlated with depression score and fatigue score (r=−0.749, −0.770; P<0.001). The depression score was positively correlated with fatigue score (r=0.769, P<0.001). The fatigue score had a negative predictive effect on quality of life score [standardized partial regression coefficient (β’)=−0.587, P<0.001], and a positive predictive effect on depression scores (β’=0.657, P<0.001). After adding depression score, the effect of fatigue score on quality of life score decreased and the β’ changed from −0.587 to −0.319, suggesting that depression played a partial mediating role in the relationship between fatigue and quality of life. Mediation tests showed significant mediation effects.ConclusionsRelieving or eliminating fatigue can improve the quality of life in asthma patients directly. At the same time, it can indirectly improve the quality of life in asthma patients through relieving depression.

  • Analysis of the occurrence and influencing factors of fatigue in asthma patients

    ObjectiveTo investigate the fatigue of asthma patients, and to analyze its influencing factors, and provide a reference for clinical intervention.MethodsThe convenience sampling method was adopted to select asthma patients who were in clinic of the First Affiliated Hospital of Guangxi Medical University from November 2018 to March 2019. The patients’ lung function were measured. And questionnaires were conducted, including general data questionnaire, Chinese version of Checklist Individual Strength-Fatigue, Asthma Control Test, Chinese version of Self-rating Depression Scale. Relevant data were collected for multiple stepwise linear regression analysis.ResultsFinally, 120 patients were enrolled. The results of multiple stepwise linear regression analysis showed that age, education level, place of residence, time period of frequent asthma symptoms, degree of small airway obstruction, Asthma Control Test score and degree of depression were the influencing factors of fatigue in asthma patients (P≤0.05). Multivariate linear stepwise regression analysis showed that degree of small airway obstruction, degree of depression and time period of frequent asthma symptoms were the main influencing factors of fatigue in asthma patients, which could explain 51.8% of the variance of fatigue (ΔR2=0.518).ConclusionsThe incidence of fatigue in asthma patients is at a relatively high level. Medical staff should pay attention to the symptoms of fatigue in asthma patients. For asthma patients, it is recommended to strengthen standardized diagnosis and treatment, reduce the onset of symptoms at night and eliminate small airway obstruction. Psychological intervention methods are needed to improve patients’ depression, reduce fatigue symptoms, and improve quality of life.

  • Human muscle fatigue monitoring method and its application for exoskeleton interactive control

    Aiming at the human-computer interaction problem during the movement of the rehabilitation exoskeleton robot, this paper proposes an adaptive human-computer interaction control method based on real-time monitoring of human muscle state. Considering the efficiency of patient health monitoring and rehabilitation training, a new fatigue assessment algorithm was proposed. The method fully combined the human neuromuscular model, and used the relationship between the model parameter changes and the muscle state to achieve the classification of muscle fatigue state on the premise of ensuring the accuracy of the fatigue trend. In order to ensure the safety of human-computer interaction, a variable impedance control algorithm with this algorithm as the supervision link was proposed. On the basis of not adding redundant sensors, the evaluation algorithm was used as the perceptual decision-making link of the control system to monitor the muscle state in real time and carry out the robot control of fault-tolerant mechanism decision-making, so as to achieve the purpose of improving wearing comfort and improving the efficiency of rehabilitation training. Experiments show that the proposed human-computer interaction control method is effective and universal, and has broad application prospects.
