  • Analysis of the efficacy of femtosecond laser-assisted sub-Bowman keratomileusis on the correction of myopia and astigmatism with six dimensional eye tracking system

    ObjectiveTo evaluate the effectiveness of using a six-dimensional eye-tracking system during femtosecond laser-assisted sub-Bowman keratomileusis (FS-SBK) surgery to correct myopia and astigmatism. MethodsA total of 23 patients (36 eyes) with ametropia undergoing FS-SBK were retrospectively analyzed and divided into the static cyclotorsion control (SCC) group (11 patients, 20 eyes) and the non-SCC group (12 patients, 16 eyes). According to the static eyeball rotation degrees, the SCC group was further divided into three subgroups: within ±2° group (5 patients, 9 eyes), ±2°-±5° group (4 patients, 7 eyes), and above ±5° group (2 patients, 4 eyes). The preoprative and postoperative one-month uncorrected visual acuity, best corrected visual acuity, diopter of spherical power, diopter of cylindrical power, corneal curvature, and the rotating degree in SCC were observed; the root-mean-square values of the total higher-order aberrations, spherical aberration, coma, Strehl ratio, etc. when the pupil diameter was 6.5 mm were extracted; and the therapeutic efficacy was observed. ResultsNo severe intraoperative or postoperative complications occurred in any subject. The effectiveness index of the SCC group was 0.947±0.145, and that of the non-SCC group was 1.005±0.141, with no statistically significant difference (P>0.05). Compared with the preoperative levels, in the two groups, the postoperative uncorrected visual acuity was significantly elevated; the postoperative diopter of spherical power, diopter of cylindrical power, and corneal curvature difference were reduced; the postoperative total higher-order aberrations, spherical aberration, and coma increased; all the differences mentioned above were statistically significant (P<0.05). In the SCC group, the difference between the preoperative and the postoperative Strehl ratio was statistically significant (P<0.05). There was no significant difference in postoperative indicators between the SCC group and the non-SCC group (P>0.05). The difference in postoperative increment of coma between the SCC group and the non-SCC group was statistically significant (P<0.05). In the SCC group, no statistically significant difference was found in postoperative increment of any indicator among the three subgroups (P<0.05). ConclusionsFS-SBK of six-dimensional eye-tracking system is effective in correcting myopia and astigmatism. FS-SBK can reduce lower-order aberrations while introducing higher-order aberrations, and whether SCC is performed intraoperatively is meaningless.

  • Effects of SMILE with different residual stromal thicknesses on corneal biomechanical properties of rabbits in vivo

    Femtosecond laser small incision lenticule extraction (SMILE) with different residual stromal thicknesses (RST) is set to investigate its effect on corneal biomechanical properties of rabbits in vivo. In this study, 24 healthy adult Japanese rabbits were randomly divided into group A and B. The RST of group A was set 30% of the corneal central thickness (CCT), and the RST of group B was 50% of the CCT. The thickness of the corneal cap in both groups was set one third of CCT. Corneal visualization Scheimpflug technology (Corvis ST) and Pentacam three-dimensional anterior segment analyzer were used to determine corneal biomechanical and morphological parameters before surgery, and 1 week, 1 month and 3 months after surgery. Pearson correlation analysis was used to analyze factors affecting corneal biomechanical parameters after SMILE. The results showed that the corneal stiffness of group A was significantly higher than that of group B at 1 week and 1 month after surgery, and most biomechanical parameters returned to preoperative levels at 3 months postoperatively. The results of correlation analysis showed that postoperative CCT and RST were the main factors affecting corneal biomechanical parameters after SMILE. There was no significant difference in corneal posterior surface height (PE) between 3 months after surgery and before surgery in both two groups. It indicates that although the ability to resist deformation of cornea decreases in SMILE with thicker corneal cap and less RST, there is no tendency to keratoconus, which may be related to the preservation of more anterior stromal layer.

  • Establishment and validation of nomogram model for intraocular hypertension after femtosecond laser in situ keratomileusis for high myopia

    ObjectiveTo investigate the risk factors of high intraocular pressure (IOP) after femtosecond laser in situ keratomileusis (FS-LASIK) in patients with high myopia, and construct and verify nomogram model. MethodsA retrospective clinical study. From January 2019 to January 2021, 327 patients (654 eyes) with high myopia treated with FS-LASIK in the Department of Ophthalmology of the 910th Hospital of the People's Liberation Army Coalition Security Force were included in the study. The patients were categorized into high IOP group and non-high IOP group according to whether high IOP occurred after surgery, which were 60 cases and 120 eyes (18.35%, 60/327) and 267 cases and 534 eyes (81.65%, 267/327), respectively. The clinical data of patients in the two groups were analyzed and observed, and the indicators with differences were subjected to one-way and multifactorial logistic regression analyses, and the results of the regression analyses were visualized to obtain the column line graphs using R3.5.3 software, and the accuracy of the column line graphs was verified by the consistency index (C-index), the calibration curves, and the subject's work characteristic curves (ROC curves). ResultsComparison of the number of cases of affected corneal thickness (χ2=7.424), corneal curvature (χ2=9.849), glucocorticoid treatment (χ2=7.222), intraoperative IOP fluctuation (χ2=11.475), corneal hysteresis (χ2=6.368), and the incidence of intraoperative complications (χ2=6.673) in the hypertensive IOP group and the nonvisualized IOP group were statistically significant (P<0.05). Binary logistic regression analysis showed that corneal thickness >450 μm, corneal curvature≤38 D, glucocorticoid treatment, intraoperative IOP fluctuation, corneal hysteresis ≤8.0 mm Hg (1 mm Hg=0.133 kPa), and intraoperative complications were the risk factors for the occurrence of high IOP after FS-LASIK surgery in patients with high myopia (P<0.05). The C-index of the column-line graph prediction model based on this was 0.722 (95% confidence interval 0.684-0.760), the calibration curve and the ideal curve were basically the same, and the area under the ROC curve was 0.709. ConclusionsCorneal thickness>450 μm, keratometric curvature ≤38 D, glucocorticoid treatment, intraoperative fluctuation of intraocular pressure, and corneal hysteresis ≤8.0 mm Hg are the risk factors for the development of hyperopic IOP in highly risk factors for the development of high IOP after FS-LASIK surgery in myopic patients. The column-line diagram model constructed on the basis of the risk factors hava good accuracy.
