  • The framework of the innovation of clinical practice: from PARIHS to i-PARIHS

    Evidence-based nursing practice has received wide attention by scholars both domestically and abroad since it was proposed, and evidence utilization forms the key link of evidence-based practice activities. Promoting action on research implementation in health services framework (PARIHS) has been widely accepted and applied in the innovation of clinical practice. The research team has improved it to i-PARIHS based on years of continuous testing and improvement, as well as feedback from users. This article aims to compare PARIHS and i-PARIHS, and elaborates on basic connotations and core elements of i-PARIHS, so as to provide basis for the innovation of clinical nursing practice in China.

  • Observation of the disinfection effect of two alcohol-containing hand disinfectants after heating in operation rooms

    Objective To study the disinfection effect of hand disinfectants containing alcohol after heating, and the comfortable feeling of using warm disinfectants among the medical staff in operation rooms. Methods Between November 2016 and January 2017, we analyzed the disinfection effect of two alcohol-containing hand disinfectants after heating in operation rooms through on-the-spot disinfection and inspection. Questionnaire was used to investigate the comfortable feeling of using warm hand disinfectants. Results After standing for 7 days at a temperature of 37℃, the major content of the two disinfectants decreased by less than 10%. Three minutes of disinfection by using the two kinds of disinfectants had an average natural-bacteria killing logarithm higher than 1.00, and they both had a qualified disinfection effect. Moreover, 96.7% of the medical personnel believed that warm disinfection products were more comfortable. Conclusion After heating, the two kinds of alcohol-containing hand disinfectants are effective in hand disinfection and more comfortable to use, which can improve hand hygiene compliance in surgical staff.

  • The development and evolution of theoretical frameworks for behavior change in the implementation science: a scoping review

    ObjectiveTo analyze the development and evolution of the theoretical domains framework (TDF), behavior change wheel (BCW), and behavior change technique (BCT). MethodsThe PubMed, EMbase, EBSCO, Web of Science, CNKI, WanFang Data, VIP and CBM databases were electronically searched to collect related studies on the establishment and development of theoretical frameworks for behavior change from the inception to September 4, 2022. Two researchers independently screened the literature, extracted the data, and conducted qualitative analysis and summary. ResultsA total of 11 studies were included, and the establishment and development of TDF, BCW and BCT taxonomy was analyzed, integrated and presented in this article. ConclusionTDF, BCW and BCT taxonomy are the fundamental theoretical framework of behavior change, which can be used independently and can be mapped and integrated with each other, providing theoretical guidance for the analysis of obstacles and promoting factors in the implementation of research and the design of intervention measures, with good promotion value. However, there are few domestic applications, so expanding the application in the local environment is necessary.

  • Investigation on the Factors Related to Premature Ovarian Failure and Diminished Ovarian Reserve

    Objective To compare the outpatient in gynecology department with health screening women in aspects of general condition, bearing, livelihood, economics and family, and to analyze the factors that relate to premature ovarian failure (POF) and diminished ovarian reserve (DOR). Method To survey 178 outpatients of gynecology department and health screening women visited from January to December of 2007, and to investigate the possible related factors through a questionnaire including the condition of basic information (e.g. age, marital status, occupation and education), menstruation, pregnancy and delivery, contraception, health, livelihood, economics, psychology, society, family and so on. The data were analyzed with SPSS 13.0 software. Results A total of 45 POF patients, 83 DOR patients and 50 healthy women were included. The results of questionnaire showed that the physiological factor was the main reason for the occurrence of POF and DOR; the menstrual disorders, autoimmune diseases (such as systemic lupus erythematosus, idiopathic thrombocytopenic purpura, glomerulonephritis, rheumatoid arthritis, etc.), malignancy cancer, pelvic surgery, smoking and drug abuse, and lack of sleep were closely related to POF and DOR; the social and psychological factors were other main reasons to cause POF and DOR. Conclusion The onset of both POF and DOR is caused by many factors, such as physical, social and psychological factors. The effective prevention and treatment for these diseases should be conducted positively in accordance with these three aspects.

  • The framework of evidence-based continued quality improvement

    Objective This study was to develop a framework of evidence-based continuous quality improvement and provide a framework for nursing professionals to promote evidence transfer and clinical nursing quality improvement. Methods Guided by the principles of PDCA, evidence-based nursing and work process, a framework of evidence-based quality continuous improvement was established through a citation analysis of evidence implementation related projects and literatures. Results The framework of evidence-based continuous quality improvement consisted of four stages and twelve steps, including evidence searching, practice audit, evidence implementation and outcome evaluation, and then it would turn to next cycle. Conclusion The framework of evidence-based continuous quality improvement provides nursing professionals a concept and method of promoting evidence into practice and promoting clinical nursing quality improvement.

This paper introduces the background and process of StaRI development, and interprets the core content of StaRI. It is expected that StaRI will provide support for domestic researchers to carry out implementation studies and writing implementation research reports.

  • Study of correlation and status of compliance with ketogenic diet in children with refractory epilepsy

    Objective To investigate the compliance of ketogenic diet in children with refractory epilepsy and its correlation with the curative effect, and to provide support and basis for the implementation and improvement of the long-term management of ketogenic diet in this patient population. MethodsA total of 106 children and their families who were followed up on ketogenic diet in the Department of Pediatrics of Fudan University from March 2019 to January 2022 in the Department of Ketogenic Multidisciplinary Treatment (MDT) were selected by convenience sampling method. General data questionnaire and ketogenic compliance questionnaire were used for investigation and follow-up. ResultsThe mean compliance of ketogenic diet in refractory epilepsy children was (13.27±3.68). The compliance scores of ketogenic children with different therapeutic effects and cognitive functions were significantly different. The compliance score was significantly correlated with the therapeutic effect and cognitive level, that is, the higher the therapeutic effect of ketogenic diet in children with higher compliance score, the better the cognitive improvement. ConclusionThe compliance of ketogenic diet in children with refractory epilepsy needs to be further improved. Improving the compliance of ketogenic diet is of great significance for the efficacy of ketogenic diet. Medical staff should actively develop the corresponding intervention program and follow-up management mode to further improve the treatment compliance of children's families, improve the treatment effect and improve the quality of life of children's families.

  • Interpretation on guideline implementation with decision support checklist (GUIDES)

    Guideline implementation with decision support checklist (GUIDES) aims to assist the self-reflection of evidence-based clinical decision support system (CDSS) related professionals to enhance the process monitor and continuous improvement of evidence-based CDSS. This paper interpreted the development process, target user, and assessment method of GUIDES, analyzed the practical value of GUIDES through a typical example, and then reflected on the GUIDES and current studies on evidence-based CDSS in China. It is expected to provide references for future studies.
