  • Integrated Treatment in Elderly Patients with Respiratory Failure due to COPD

    Objective To evaluate the clinical efficacy of integrated treatment ( sequential noninvasive following invasive mechanical ventilation, bronchoscope suction, combined inhalation) in patients with acute respiratory failure induced by chronic obstructive pulmonary disease ( COPD) . Methods 59 elderly patients with COPD and acute respiratory failure in ICU fromJuly 2006 to July 2009 were enrolled in the study. The patients were randomized into three groups, ie. a non-invasive mechanical ventilation group ( NIV group) , a sequential non-invasive following invasive mechanical ventilation group ( SV group) , a integrated treatment group ( IT group) . APACHEⅡ score, clinical pulmonary infection score ( CPIS) ,arterial blood gas analysis, respiratory rate ( RR) , heart rate ( HR) , and mean artery blood pressure ( MAP)at 0 h, 3 h, 3 d, 12 d after treatment were recorded. Results With the extension of treatment time,APACHEⅡ score, CPIS score, RR, HR, PaCO2 , and white blood cells gradually reduced, while pH and PaO2 gradually increased in the three groups ( P lt;0. 05) . The differences in RR, HR, PaCO2 , and PaO2 at the time of 3 d and 12 d were significant between the three groups( P lt;0. 05) . The occurrence of pulmonary infection control ( PIC) window and ventilation associated pneumonia ( VAP) had no difference among the three groups( P gt;0. 05) . The duration of total mechanical ventilation, durations of ICU stay and hospital stay were shorter in SV group than those in NIV group( P lt;0. 05) . The duration of total mechanical ventilation,duration of invasive mechanical ventilation, durations of ICU stay and hospital stay were shorter in IT group than those in SV group( P lt;0. 05) . The incidence of VAP was higher in SV group than NIV group, but lower in IT group than SV group( P lt;0. 05) . Hospital mortality was lower in SV group than NIV group, and higher in IT group than SV group( P lt; 0. 05) . Conclusion In elderly COPD patients with acute respiratory failure, integrated treatment given early can reduce the duration of mechanical ventilation, the length of ICU and hospital stay, and decrease the morbidity of VAP and mortality.

  • Effect of Nursing Interventions on Medical Behavior of Diabetic Patients in China: A Meta-analysis

    Objective To evaluate the effect of nursing interventions on the medical behavior of diabetic patients. Methods We searched CMB (1978 to September 2007), CNKI (1994 to September 2007), VIP (1989 to September 2007) and Papers on Academic Conference of China (1989 to September 2007), and hand-searched relevant journals. We identified randomized controlled trials (RCTs) and quasi-RCTs of nursing interventions plus routine therapy versus simple routine therapy for diabetic patients. The methodological quality of included studies was assessed and data was extracted and analyzed by using The Cochrane Collaboration’s RevMan 4.2.10 software. Results In total, 10 RCTs and 2 quasi-RCTs were included. Two of these studies described blinding. The quality of the included trials was low. Meta-analyses showed that nursing interventions significantly improved medication compliance (RR 1.33, 95%CI 1.21 to 1.45), medical behaviors of physical therapy (RR 1.48, 95%CI 1.30 to 1.69), diet control (RR 1.54, 95%CI 1.39 to 1.71), blood glucose monitoring (RR 1.56, 95%CI 1.41 to 1.72) and regular inspection (RR 1.66, 95%CI 1.41 to 1.96) in patients with diabetes. Conclusion Nursing interventions can improve the medical behavior of diabetic patients, increase patient-controlled capacity and enhance awareness of diabetes. It is also beneficial for blood glucose control and stability.

  • Medical Rescue and Treatment during Two Weeks after Min-Zhang Earthquake in Gansu Province

    Objective To retrospectively analyze data of 4 377 victims during 2 weeks after Min-Zhang earthquake (Richter scale: 6.6), to provide references for decision-making of earthquake medical rescue. Methods We collected data of the wounded during two weeks after the earthquake, analyzed injuries and general rescue in hospital, input the data into Excel, and analyzed the data using SPSS 11.0. Descriptive analysis was conducted. Results As of 14 days after the earthquake, a total of 4 377 victims had been treated, including 858 hospitalized and 194 seriously-injured. The victims were from Min county, Zhang county, Dangchang county, and Li county. 145 injured people had been transferred to 10 municipal hospitals in Gansu province. Mental health centre of Gansu province had conducted psychological intervention among more than thousands of person-times. Disease control and prevention institutions in Dingxi city started severe natural disaster contingency planning 2 hours after earthquake, stipulated and improved the post-quake disease control technology plan, and dispatched quickly anti-epidemic emergency teams to complete tasks that included disposal of carcasses, monitoring water quality and epidemics, disinfecting environmental ruins, epidemic control in resettled areas, and large-scale health education. Conclusion The headquarter, in charge of medical relief and anti-epidemic work in the stricken areas of Min county and Zhang county, integrated resources, made overall command and quick response, and accomplished the phased tasks including anti-quake work, medical relief and anti-epidemic work, which replenished valuable data and experience.

  • Efficacy and safety of docetaxel chemotherapy combine with 3-dimensional conformal radiation therapy in treatment of esophageal cancer: a systematic review

    ObjectiveTo systematically review efficacy and safety of docetaxel chemotherapy combine with 3-dimensional conformal radiation therapy (3D-CRT) in treatment of esophageal cancer.MethodsDatabases including PubMed, EMbase, The Cochrane Library, Web of Science, CNKI and WanFang Data were searched from inception to February 2017 to collect randomized controlled trials (RCTs) and quasi-randomized control trials (qRCTs) about docetaxel chemotherapy combine with 3D-CRT in treatment of esophageal cancer. Two reviewers independently screened literature, extracted data and assessed the risk of bias of included studies. Then meta-analysis was performed by using Stata 12.0 software.ResultsA total of 17 RCTs involving 1 353 patients were included. The results of meta-analysis showed that, compared with the radiotherapy alone, the docetaxel chemotherapy combine with 3D-CRT could improve effective rate (OR=1.25, 95%CI 1.07 to 1.47, P=0.003), 3-year survival rate (OR=1.91, 95%CI 1.19 to 3.06, P=0.006), but there were no significant differences in 1-year survival rate (OR=1.28, 95%CI 0.95 to 1.72, P=0.105), hematologic toxicities (OR=1.13, 95%CI 0.85 to 1.49, P=0.389) and gastrointestinal reactions (OR=1.19, 95%CI 0.90 to 1.57, P=0.181).ConclusionsCompared with radiotherapy alone, docetaxel chemotherapy combine with 3D-CRT can improve the effective rate, 3-year survival rate, but not increase the incidences of adverse effects. Due to limited quantity and quality of the included studies, more high quality studies are needed to verify above conclusion.

  • Application effect of fast track surgery for patients with esophageal cancer: A systematic review and meta-analysis

    Objective To evaluate the effect of fast track surgery (FTS) after esophageal cancer surgery. Methods The randomized controlled trial (RCT) and observational studies about FTS for esophageal cancer in PubMed、EMbase、The Cochrane Library、Web of Science、CBM、CNKI and WanFang databases were searched up to May 2017. Then the studies were screened according to the inclusion and exclusion criteria by two researchers. Data were analyzed by Stata12.0 software. Results Totally 13 RCTs and 5 observational studies with 2 447 patients were eligible for analysis. Compared with the control group, incidence of postoperative complications (OR=0.53, 95%CI 0.40 to 0.71, P<0.05) significantly reduced in the FTS group, but there was no significant difference between the two groups in readmission rate (OR=1.21, 95%CI 0.83 to 1.76, P=0.313) and 30 d mortality rate (OR=0.72, 95%CI 0.43 to 1.20, P=0.207). Conclusion FTS can safely and effectively accelerate the recovery of patients with esophageal cancer and it owns important clinical values.

  • Efficacy and safety of transapical transcatheter aortic valve replacement in the treatment of aortic regurgitation in patients with large aortic annulus

    ObjectiveTo investigate the feasibility and safety of transcatheter aortic valve replacement (TAVR) through apical approach for aortic regurgitation of large annulus.MethodsFrom November 2019 to May 2020, 10 male patients aged 64.50±4.20 years with aortic valve insufficiency (AI) underwent TAVR in the Department of Cardiovascular Surgery, Xijing Hospital. The surgical instruments were 29# J-valveTM modified and the patients underwent TAVR under angiography. The preoperative and postoperative cardiac function, valve regurgitation, complications and left ventricular remodeling were summarized by ultrasound and CT before and after TAVR.ResultsA total of 10 valves were implanted in 10 patients. Among them, 1 patient was transferred to the aortic arch during the operation and was transferred to surgical aortic valve replacement; the other 9 patients were successfully implanted with J-valve, with 6 patients of cardiac function (NYHA) class Ⅱ, 4 patients of grade Ⅲ. And there was a significant difference between preoperation and postoperation in left ventricular ejection fraction (44.70%±8.78% vs. 39.80%±8.48%, P<0.05) or aortic regurgitation (1.75±0.72 mL vs. 16.51±8.71 mL, P<0.05). After 3 months, the patients' cardiac function was good.ConclusionTAVR is safe and effective in the treatment of severe valvular disease with AI using J-valve.

  • Diagnostic value of exhaled volatile organic compounds in pulmonary cystic fibrosis: A systematic review

    ObjectiveTo explore the diagnostic value of exhaled volatile organic compounds (VOCs) for cystic fibrosis (CF). MethodsA systematic search was conducted in PubMed, EMbase, Web of Science, Cochrane Library, CNKI, Wanfang, VIP, and SinoMed databases up to August 7, 2024. Studies that met the inclusion criteria were selected for data extraction and quality assessment. The quality of included studies was assessed by the Newcastle-Ottawa Scale (NOS), and the risk of bias and applicability of included prediction model studies were assessed by the prediction model risk of bias assessment tool (PROBAST).ResultsA total of 10 studies were included, among which 5 studies only identified specific exhaled VOCs in CF patients, and another 5 developed 7 CF risk prediction models based on the identification of VOCs in CF. The included studies reported a total of 75 exhaled VOCs, most of which belonged to the categories of acylcarnitines, aldehydes, acids, and esters. Most models (n=6, 85.7%) only included exhaled VOCs as predictive factors, and only one model included factors other than VOCs, including forced expiratory flow at 75% of forced vital capacity (FEF75) and modified Medical Research Council scale for the assessment of dyspnea (mMRC). The accuracy of the models ranged from 77% to 100%, and the area under the receiver operating characteristic curve ranged from 0.771 to 0.988. None of the included studies provided information on the calibration of the models. The results of the Prediction Model Risk of Bias Assessment Tool (PROBAST) showed that the overall bias risk of all predictive model studies was high bias risk, and the overall applicability was unclear. ConclusionThe exhaled VOCs reported in the included studies showed significant heterogeneity, and more research is needed to explore specific compounds for CF. In addition, risk prediction models based on exhaled VOCs have certain value in the diagnosis of CF, but the overall bias risk is relatively high and needs further optimization from aspects such as model construction and validation.

  • Report of Medical Rescue during Two Weeks after Min-Zhang Earthquake in Gansu Province

    Objective To retrospectively analyze medical rescue during Min-Zhang earthquake. Methods Medical rescue data reported from cities and counties in Gansu province were summarized in the earthquake relief and medical rescue headquarter of Gansu province. They were input into a database using Excel software. Descriptive analysis was conducted using SPSS 17.0. Results After the earthquake, Gansu Heath System quickly responded to the situations and started the contingency plan for earthquake, while patches of medical rescue teams were sent to the stricken areas. As of 14 days after the earthquake, 4 377 victims were treated in medical institutions of each levels. Hospital infection was comprehensively in control and psychological intervention was conducted among the target population. Conclusion The emergency response of Gansu Heath System was quick, effective, and well organized. Facing extraordinary disaster, Gansu province smoothly accomplished the tasks of earthquake relief and medical rescue at the initial stage.

  • Effect of prone position ventilation on patients with acute respiratory distress syndrome

    ObjectivesTo investigate the effect of prone position ventilation (PPV) on patients with acute respiratory distress syndrome (ARDS).MethodsPatients with ARDS who received PPV treatment in the this hospital were enrolled from January 1, 2017 to December 31, 2017. The changes in heart rate, respiratory mechanics and blood gas index before and after PPV in patients, the inhaled oxygen concentration (FiO2), oxygenation index (PaO2/FiO2), pressure sore and other related complications were observed and compared in patients before and after PPV.ResultsA total of 28 patients with ARDS were registered, including 21 males and 7 females. Fourteen patients were complicated with chronic obstructive pulmonary disease (COPD) and 20 were dead in 28 days. After PPV, the peak pressure and plateau pressure decreased significantly, PaO2 and SaO2 increased significantly, system compliance improved considerably but PaCO2 did not change. There was no significant difference in the changes of heart reat, respiratory rate, minute volume, tidal volume and positive end-expiratory pressure between before and after PPV. FiO2 decreased significantly, PaO2/FiO2 increased significantly, and pressure sore increased significantly on day 1 post-PPV in comparison to pre-PPV and on day 7 post-PPV in comparison to day 1 post-PPV. A total of 13 unplanned extubation occurred during the entire PPV procedure, 9 of them were gastric tube slipping, 2 were urethral catheter slipping, 1 was tracheal tube slipping, and 1 was deep venous catheter slipping. There were 17 cases of artificial airway obstruction, 7 cases of hypotension, 3 cases of arrhythmia, and 4 cases of keratitis. In the subgroup analysis, the age of the patients complicated with COPD was significantly higher, but there was no difference in additional baseline data and the survival rate.ConclusionPPV can significantly improve the patient's respiratory status, especially oxygenation and respiratory mechanics, but PPV can increase the incidence of complications such as pressure sore, and PPV does not improve the prognosis.
