Objective To explore the treatment effects of one-way valve lung volume reduction surgery for heterogeneous emphysema. Methods Seven heterogeneous emphysema Beagle dogs were each implanted in right lung2-4 pieces ofbronchial one-way valve. The animals were sacrificed two months later.Macro- and microscopic pathology changes of the lungs were observed. Results Of 21 valves implanted,2 were expectorated, and 19 were in normal position. The dogs did not have obvious adverse reactions.Generally specimens of the occluded lobes had different levels of lung volume decrease, such as texture stiffened, focal scar formation, atelectasis, pulmonary fibrosis.Pathological biopsy showed moderate alveolitis and pulmonary interstitial fibrosis. Conclusion One-way valve lung volume reduction is effective for heterogeneous emphysema
Compared with traditional head to head meta-analysis, network meta-analysis has more confounding factors and difficulties to handle. Due to the mutual transitivity of evidence in network meta-analysis, heterogeneity may be brought into indirect meta-analysis. Hence, effective differentiation and correct handling of heterogeneity are being current focus. In order to ensure the reliability of the results of network meta-analysis, the concept of homogeneity is proposed and a series of methods are developed for differentiation and handling of homogeneity. Based on the extension of Bucher methods, current methods for differentiation and handling of homogeneity has extended to ten quantitative measures (eg., node analysis method, hypothesis tests, and two-step method). However, because of the differences and the focus of fundamental methodological theories as well as the limitation of statistics power, no highly-effective method has been worked out. Therefore, the exploration of highly-effective, simple and high-resolved methods are still needed.
ObjectiveTo introduce sensitivity and homogeneity tests in network meta-analysis and its implementation in R software. MethodsUsing an example, we performed sensitivity analysis by comparing the random effect model with the fixed effect model. Homogeneity analysis was performed using metaphor package and combinat package in R software. ResultsThe results of the two models were similar, and the data was steady. The results of homogeneity analysis showed that the confidential intervals in all loops were crossed over with blank value; and direct and indirect estimates of the effects in network meta-analysis were not significantly different, with good homogeneity. ConclusionNetwork meta-analysis is a kind of indirect comparison analysis method, and its sensitivity is especially important. The introduction of homogeneity makes network meta-analysis more accurate. Using R software for sensitivity and homogeneity analysis in network meta-analysis is a feasible method.
Beijing Tongren Hospital, Capital Medical University is a multi-zone hospital, adopting a mixed mood of centralized and decentralized management of ambulatory surgery. In order to guarantee the medical quality and safety, and improve the quality of ambulatory surgery, during the past three years, through homogeneity management in three dimensions, including quality control, efficiency connotation, and service safeguard, the number of ambulatory surgery disease types carried out in the hospital increased from 121 to 251; the working efficiency of the day operating room was improved, and the rate of doctors’ temporary cancellation of operations was reduced from 3.04% to 1.68%; the medical quality index was improved, the rate of unplanned reoperation decreased from 0.173‰ to 0.078‰, and the patient satisfaction increased from 94% to 99%. It shows that the homogenization management is an effective management method to effectively guarantee the quality and safety of medical treatment and improve the quality of ambulatory surgery in multi-zone and mixed management mode.
ObjectivesTo analyze the resident achievement assessment data of West China Hospital of Sichuan University in 2017, so as to provide experience for further realization of homogeneity in residency training.MethodsWe extracted the 381 residents' achievement assessment data of West China Hospital of Sichuan University in 2017, and compared data in the theory, skills and the overall passing scores of residents with different status types and different academic levels through χ2 test.ResultsThe results of the theory test (χ2=1.384 9, P=0.709 1), skills test (χ2=3.363 1, P=0.339 0), overall assessment (χ2=4.274 7, P=0.233 3) showed no statistically significant difference in residents with different status. The result of the theory test (χ2=0.417 5, P=0.811 6), skills test (χ2=0.461 4, P=0.794 0) and the overall assessment (χ2=0.680 0, P=0.711 8) showed no statistically significant differences in residents with different academic levels.ConclusionsIt can be concluded that the residency training in West China Hospital of Sichuan University has achieved homogeneity.
