  • Clinical Features and Common Etiologies of Recurrent Acute Pancreatitis

    Objective To analyze the clinical features, common etiologies, prevention measurements of recurrent acute pancreatitis (AP). Methods The clinical characteristics and imaging examination data of 43 patients with recurrent AP were analyzed retrospectively, which was compared with the results of 258 patients with primary AP. The recurrence etiologies were analyzed. Results There were no significant differences on the fever, jaundice, abdominal pain relief time, pancreatic local complications, and ratio of severe AP between two groups (P>0.05). Comparion of etiologies between recurrent AP and primary AP, cholecystitis and diet factor (alcoholic) had priority in patients with primary AP (P=0.038, P=0.006, respectively), but the hyperlipidemic, duodenal nipple disease, and small stone in the common bile duct were the major etiologies in patients with recurrent AP (P=0.007, P=0.008, respectively). No relapse was found within the follow up for 3 months to 2 years (the average time was 14.2 months). Conclusion Find out the exact etiology and performe correct therapy are the key to the treatment and prevention of recurrent AP.

  • Effect of B Ultrasound-Guided Percutaneous Catheter Drainage Combined with Veno-Venous Hemofiltration at Different Time Points and Multi-Site in Treatment of Hyperlipidemic Severe Acute Pancreatitis

    ObjectiveTo investigate the therapeutic effect of B ultrasound-guided percutaneous catheter drainage combined with veno-venous hemofiltration at different time points and multi-site in treatment of hyperlipidemic severe acute pancreatitis (HL-SAP). MethodsThe clinical data of 34 patients with HL-SAP initially underwent B ultrasound-guided percutaneous catheter drainage combined with veno-venous hemofiltration at different time points and multi-site from January 2010 to June 2014 were retrospectively analyzed. According to the different of the onset to treatment time, 34 cases were divided into the≤24 h group and > 24 h group. The serum platelet activating factor (PAF) and triglyceride (TG) at the time of admission and after admission 1, 3, 5, 7, and 10 days were detected, and the hospitalization time, mortality, and the rate of conversion to open surgery were observed. ResultsThe levels of PAF and TG in the both groups tended to gradually decrease with different degrees, were significantly lower than that the before treatment (P < 0.05), and the decline of PAF and TG in≤24 h group were more significant than > 24 h group. The hospitalization time, mortality, and the rate of conversion to open surgery in the≤24 h group were significantly lower than those of the > 24 h group (P < 0.05). ConclusionThe early using of B ultrasound-guided percutaneous catheter drainage combined with veno-venous hemofiltration at different time points and multi-site would have a beneficial impact on the management of HL-SAP and complications.

  • Continuous Veno-venous Hemofiltration for Patients with Hyperlipidemic Pancreatitis: A Systematic Review

    ObjectiveTo systematically review the effect and safety of continuous veno-venous hemofiltration (CVVH) on patients with hyperlipidemic pancreatitis. MethodsDatabases including the Cochrane Library (Issue 2, 2014), PubMed, EMbase, CBM, CNKI and WanFang Data were electronically searched for randomized controlled trials (RCTs) about CVVH on patients with hyperlipidemic pancreatitis till Feb. 12, 2014. According to the inclusion and exclusion criteria, literature was screened, data were extracted, and the methodological quality of included studies was also assessed. Then, meta-analysis was performed using RevMan 5.2 software. ResultsA total of 7 trials met eligibility criteria, involving 360 cases, including 183 cases of CVVH group and 177 cases of control group. The results of meta-analysis showed that compared with only routine medical treatment, CVVH significantly reduced the levels of lipid (WMD=-4.63, 95%CI-5.98 to-3.27, P < 0.000 01), levels of IL-6 (WMD=-29.59, 95%CI-34.30 to-24.89, P < 0.000 01), overall mortality (RR=0.39, 95%CI 0.18 to 0.84, P=0.02), and APACHE II score (WMD=-3.34, 95%CI-5.12 to-1.56, P=0.000 2) after treatment. ConclusionCVVH is more effective for hyperlipidemic pancreatitis than only routine medical treatment. Due to the limited quantity and quality of the included studies, further high-quality, multicenter, large-scale RCTs are required to verify the above conclusion.
