  • The Clinical Investigation of the Dense Silver Needle Acupuncture Therapy Combined with Brachial Plexus Nerve Block for Old Patients with Shoulder Periarthritis

    【摘要】 目的 观察密集型银质针松解术结合臂丛神经阻滞麻醉治疗老年肩周炎的疗效。 方法 2007年4月-2008年6月,将120例老年肩周炎患者(gt;60岁)随机分为治疗组、对照组各60例;治疗组采用密集型银质针松解术结合臂丛神经阻滞麻醉治疗,对照组则用单纯密集型银质针松解术治疗。 结果 治疗组痊愈43例、显效15例、好转2例;对照组痊愈41例、显效16例、好转3例。总有效率均为100%。两组疗效差异无统计学意义(Pgt;0.05),治疗组心率血压变化值及疼痛评分均低于对照组,差异有统计学意义(Plt;0.05)。 结论 密集型银质针松解术结合臂丛神经阻滞为治疗老年肩周炎的安全、有效方法。【Abstract】 Objective To investigate the effect of dense silver needle acupuncture therapy combined with brachial plexus nerve block for old patients with shoulder periarthritis.  Methods A total of 120 patients with shoulder periarthritis diagnosed in our hospital between April 2007 and June 2008 were randomly divided into treatment group and control group with 60 in each group. Patients in the treatment group were treated with dense silver needle acupuncture therapy combined with brachial plexus nerve block, while patients in the control group were only treated with dense silver needle acupuncture therapy. Results In the treatment group, 43 patients were cured, the treatment produced effect in 15 patients, and improvement could be found in two patients. For the control group, those three numbers were respectively 41, 16 and 3. The effectice rate was 100% in both groups. The difference of effect between the two groups was not statistically significant (Pgt;0.05), while the variation of blood pressure and heart rate and the visual analogue scales in the treatment group were significant lower than these in the control gvroup (Plt;0.05) Conclusion The dense silver needle acupuncture therapy combined with brachial plexus nerve block is safe and effective in treating old patients with shoulder periarthritis.

  • The Clinical Investigation of the DeepPressed Pain Points Injection on the Refractory Cancer Pain

    摘要:目的: 探讨深部痛点阻滞在治疗顽固性癌痛患者中的作用。 方法 : 将156例顽固性癌痛患者,随机分为深部痛点阻滞组及对照组,每组78例。深部痛点阻滞组根据疼痛部位,应用软组织外科学理论找出相应压痛点,给予痛点深部阻滞治疗;对照组找出压痛点但仅按三阶梯用药原则给予口服药物治疗。 结果 : 深部痛点阻滞组及对照组两组患者经治疗后疼痛控制均较理想(深部痛点阻滞组VAS13, 对照组VAS17),但是深部痛点阻滞组吗啡用量明显少于对照组(吗啡日平用量深部痛点阻滞组为335mg, 对照组为15287 mg)。便秘的发生率深部痛点阻滞组为5384%,对照组为7692%。 结论 : 深部痛点阻滞能较好的控制顽固性癌痛,同时可明显减少吗啡的用量,副作用相对发生要少。Abstract: Objective: To investigate the effect of the deeppressed pain points injection on the refractory cancer pain and provide reference for relieving the cancer pain. Methods : One hundred and fiftysix patients with refractory cancer pain were diagnosed by the soft tissue surgery,finding the pain point by pressing deeply,the patients with deeppressed pain points were divided into deeppressed pain points injection group and control group randomly, The patients in deeppressed pain points injection group were treated with deeppressed pain points injection; the patients in control group were treated with drugs according with the WHO analgesic ladder. Results : The patients in deeppressed pain points injection group got the same pain relief as those in control group (VAS 13 in deeppressed pain points injection group, 17 in control group ),but the daily dose of morphine in deeppressed pain points injection group was less than that incontrol group (morphine 335mg/day in deeppressed pain points injection group,15287mg/day in control group ) significantly. There was 5384% patients with constipation in deeppressed pain points injection group, 7692% in control group. Conclusion : The refractory cancer pain can be controlled effectively by the deeppressed pain point injection and the daily dose of morphine to control the cancer pain is decreased significantly. The side effect in deeppressed pain points injection group was less than that in control group.

  • Addiction medicine education on massive open online course improves the stigma of substance use disorder patients: a randomized controlled trial

    Objective To explore the relation between stigma and substance use of patients with substance use disorder and explore the impact of learning addiction medicine massive open online course (MOOC) on patients’ stigma, depression and anxiety. Methods Patients with substance use disorder in Meishan Detoxification Center from January 11 to 18, 2020 were selected. The patients were randomly divided into control group and teaching group. Both groups participated in regular labor reform while the teaching group attended extra MOOC course. The general demographic data and the duration and amount of substance use were collected before intervention. Before and after intervention, all the patients completed scales including Chinese Version of Substance Use Stigma Mechanisms Scale (SUSMS-C), Zung Self-Rating Depression Scale (SDS) and Zung Self-Rating Anxiety Scale (SAS). The correlation between stigma and depression, anxiety, substance use characteristics were analyzed, as well as the changes of stigma, depression and anxiety before and after MOOC intervention.Results A total of 78 patients were included, including 41 in the control group and 37 in the teaching group. There was no correlation between the scores of SUSMS-C and the duration or amount of substances use (P>0.05). SAS score and SDS score were positively correlated with the total score of stigma, internalized stigma, enacted stigma (family) and enacted stigma (healthcare providers) (P<0.05). SDS scores and SAS scores had no correlation with the anticipated stigma (P>0.05). After intervention, the total score of stigma, anticipated stigma (family) and anticipated stigma (healthcare providers) in the teaching group decreased (P<0.05). In the control group, only anticipated stigma (family) decreased (P=0.047). There was no significant difference in other SUSMS-C items, SAS scores, or SDS scores between the two groups before and after intervention (P>0.05). Conclusions Stigma may not be able to protect patients with substance use disorder from using substances. Instead, stigma may potentially cause negative emotional consequences like depression and anxiety. The addiction medicine MOOC can be used as an intervention to reduce the stigma of patients with substance use disorder.
