  • Exploration and practice of day surgery in Department of Gastroenterology of West China Hospital of Sichuan University

    As one of the crucial measures in the action plan for improving health services, day surgery is committed to efficiently integrating medical resources and improving the quality of medical services. Nowadays, public hospitals in China are vigorously promoting the day surgery cases. The normalization of day surgery is also one of the major goals for the Department of Gastroenterology to meet the current medical needs and explore sub-professional development in line with the discipline’s advantages. Taking the current status of day surgery in the Department of Gastroenterology of West China Hospital of Sichuan University as an example, this paper explores and constructs the “Huaxi Model of Day Surgery in the Department of Gastroenterology”, focusing on the management mode, operation process, operation implementation, quality and safety assurance of day surgery, so as to facilitate the exchange of experiences.

  • Research status and progress on precise scheduling of day surgery

    Day surgery is flourishing in public hospitals in China with the advantages of strong planning, short stay and high efficiency. Under the background of “diversification of surgeons, diversification of disease structure and precision of scheduling needs”, higher requirements are put forward for refined scheduling strategies of day surgery. The research of scientific and precise surgical scheduling strategy is of great significance to realize efficient coordination and optimal allocation of day surgical resources. This article reviews the necessity of precise scheduling of day surgery, the current situation of scheduling of day surgery, the key dimensions affecting the scheduling of day surgery, and the evaluation system for precise scheduling of day surgery.

  • A comparative study of day operation in West China Hospital of Sichuan University and the nationally recommended catalogue for day surgery

    Objective To analyze the current status of day surgery in West China Hospital of Sichuan University and compare it with the Recommended Catalogue of Day Surgery (2022 Edition), in order to provide reference for the update and expansion of day surgery procedures in hospitals. Methods The day surgical procedures carried out in West China Hospital of Sichuan University from October 2009 to July 2023 were collected. The day surgical procedures were classified based on surgical specialties, types of procedures, methods of operation, and surgical grading, etc. The day surgical procedures in West China Hospital of Sichuan University were compared with those in the Recommended Catalogue of Day Surgery (2022 Edition). Results A total of 576 types of daytime surgeries were carried out in West China Hospital of Sichuan University, involving 15 specialties. A total of 662 types of day surgeries were recommend in the nationally recommended catalogue for daytime surgery, involving 11 specialties. Among the nationally recommended surgical procedures, West China Hospital of Sichuan University had carried out 233 types of surgical procedures, and 26 additional types of surgical procedures were carried out as outpatient surgeries. According to the classification of surgical difficulty, the Level Ⅱ and Ⅲ surgeries were mainly carried out in West China Hospital of Sichuan University, and the Level Ⅱ surgeries were mainly recommend in the nationally recommended catalogue for day surgery. Conclusions The high-quality development of day surgery in China should not be limited to the supplement of surgical procedures. It is necessary to strive to increase the proportion of minimally invasive and fourth-level surgeries while ensuring medical quality and safety, and improve the overall medical level of day surgery.

  • Analysis of the application effect of same-day surgery mode in adult patients with inguinal hernia repair under enhanced recovery after surgery mode

    Objective To explore the application effect of same-day surgery mode in adult patients with inguinal hernia repair under enhanced recovery after surgery mode. Methods The perioperative data of adults undergoing inguinal hernia repair in the Day Surgery Center of West China Hospital, Sichuan University between August 2020 and March 2022 were analyzed retrospectively. The adult patients with inguinal hernia repair who received routine daytime surgery were taken as the control group (routine group), and the adult patients with inguinal hernia repair who received same-day surgery were selected as the trial group (same-day group). The differences in safety, cost and patient experience between the two groups were compared and analyzed. Results A total of 319 patients were included, including 152 in the routine group and 167 in the same-day group. There was no significant difference in gender, education level, occupation and hernia ring diameter between the two groups (P>0.05). The age of the patients in the same-day group was older than that in the routine group [(49.49±12.88) vs. (46.41±14.12) years, P<0.05]. The hernia position of the two groups was mostly on the right side, but there was a difference in the hernia position (P<0.05). In terms of safety indicators, the majority of patients in the two groups used local anesthesia. The proportion of local anesthesia (98.2% vs. 76.3%), the amount of intraoperative bleeding [2.8 (2.0, 5.0) vs. 1.3 (0.0, 5.0) mL] in the same-day group were higher than those in the routine group, and the operation time [25.2 (20.0, 33.0) vs. 32.3 (26.0, 40.7) min] in the same-day group was shorter than that in the routine group (P<0.05). There was no significant difference between the two groups in the time of getting out of bed and the complications rate on the 3rd and 28th days after operation (P>0.05). There were no intraoperative complications in both groups. In terms of cost indicators, there was no significant difference between the two groups in the hospitalization cost (P>0.05). The surgery cost of the same-day group was higher than that of the routine group [1472.0 (1438.1, 1614.6) vs. 1450.3 (1428.1, 1438.1) yuan, P<0.05]. The drug cost [109.2 (81.3, 138.7) vs. 255.8 (127.0, 261.6) yuan] and the total medical cost [8418.5 (8207.4, 9129.9) vs. 8912.1 (8325.9, 9177.9) yuan] in the same-day group were lower than those in the routine group (P<0.05). In terms of patient experience indicators, the postoperative pain score [0.3 (0.0, 1.0) vs. 0.2 (0.0, 0.0)] and satisfaction score [3.3 (3.0, 4.0) vs. 3.0 (3.0, 3.0)] of the same-day group were higher than those of the routine group (P<0.05). Conclusion Both the same-day surgery mode and the routine surgery mode of adult patients with inguinal hernia repair have high safety, but the same-day surgery mode is more economical and patient satisfaction is higher than the routine surgery mode, which suggest that the same-day surgery mode of adult patients with inguinal hernia repair under enhanced recovery after surgery mode is feasible, safe and economic, and further optimizes and improves the content and quality of daytime surgical medical services.

  • Safety and feasibility of gluteal muscle contracture in day surgery mode

    Objective To explore the safety and feasibility of day surgery program of surgical treatment of mild to moderate gluteal muscle contracture. Methods We retrospectively analyzed the clinical data of the patients who received invasive surgery to release gluteal muscle contracture in West China Hospital of Sichuan University between July 2021 and July 2022. Postoperative telephone follow-up was conducted on the 2nd, 3rd, and 30th day after surgery. The basic conditions, surgery duration, intraoperative blood loss, anesthesia duration, pain score 6 h after surgery, hospitalization expenses, postoperative complications and patient satisfaction were observed. Results A total of 44 patients, 17 males and 27 females, aged 18-51 years with a median age of 30.5 years, diagnosed as bilateral mild to moderate gluteal muscle contracture, were included in this study. All patients except one with unilateral surgery were successfully finished the bilateral surgeries and discharged as planned. The mean surgery duration was (71±13) min, intraoperative blood loss was (20±17) mL, anesthesia duration was (118±21) min. All patients complained mild pain or no pain (Numerical Rating Scale score≤3) except one patient received pain rescue (Numerical Rating Scale score=5) 6 h after surgery. The average hospitalization expenses was (10021.55±1142.10) yuan. Three patients complained the poor wound healing. All patients replied a normal life and work with a good satisfaction and without any severe complications by telephone follow up on the 30th day postoperatively. Conclusion Surgical treatment of mild to moderate gluteal muscle contracture is safe, effective and feasible in the day surgery mode.

  • Preliminary exploration of day surgery management model in West China Hospital of Sichuan University

    Since its establishment in 2009, adhering to the concept of patient-centered service, the Day Surgery Center of West China Hospital of Sichuan University has been improving the management model constantly, and perfecting the service process gradually, to ensure the medical quality and safety of the hospital, and optimize the utilization of medical resources. In view of the management of day surgery, the hospital has formulated a sound patient access system, surgeon access principle, standardized appointment process and contingency plans, and made changes in patient admission process, responsibilities of surgeons, and nursing care comparing with the traditional hospital operation. The purpose of this paper is to introduce the management model of day surgery in West China Hospital of Sichuan University, discuss the clinical application of centralized and decentralized management in combination with the actual situation, and explore the model of day surgery with high applicability and more consistent with our national conditions.
