  • COMPARISON OF DIFFERENT CHEMICAL METHODS FOR PREPARATION OF ACELLULAR NERVE SCAFFOLD

    Compare the effect of different chemical methods for preparation of acellular nerve scaffold and to provide an effective nerve scaffold for tissue engineering. Methods Fifteen male SD rats of 2 months old, weighing 200-250 g were selected; the bilateral sciatic nerves were harvested and divided into 3 groups according to preparation methods: group A (normal nerve), group B (Sondell method) and group C (optimal method by Triton X-200, SB-10 and SB-16). The morphology was compared by HE, immunohistochemistry and SEM after dispose; the degrees of decellularization, degrees of demyel ination and integrity of the nerve fiber tube were assessed by scoring system. Results HE staining: In group A, thecross section of nerve was roundness, the cell nuclei was dark blue and the myel in sheath was reticular structure. In group B, the axon and cell nuclei disappeared and the structure of endoneurium was destroyed. In group C, the axon and cell nuclei disappeared and the endoneurium become anomal istic round cavum. The immunohistochemistry staining of Laminin: In group A, the myel in sheath was surrounded by basement membrane with dark blue SC nuclei inside. In group B, the myel in sheath and SC nuclei disappeared and the structure of basement membrane destroyed. In group C, the myel in sheath and SC nuclei disappeared and basement membrane become anomal istic round cavum. The immunohistochemistry staining of S-100: In group A, the myel in sheath and SC were brown. In groups B and C, there were no apparent stained myel in sheath. SEM: In group A, the myel in sheath and axon were clear. In group B, the axon and myel in sheath disappeared and basement membrane became anomal istic. In group C, the basement membrane was more regular than that of group B. The degrees of acellularization and demyel ination of groups B and C were superior to that of group A (P lt; 0.05), and the degrees of demyel ination of group C were superior to that of group B (P lt; 0.05). The integrity of nerve fiber tube of group C was superior to that of group B (P lt; 0.05) and similar to that of group A (P gt; 0.05). The total score was the lowest in group C but the qual ity was the best. Conclusion The effect of decellularization of optimal method was similar to that of traditional Sondell method, but the effect of demyel ination and integrity of nerve fiber tube were better than that of traditional Sondell method. And this acellular nerve can be used as a new kind of nerve scaffold material.

  • Clinical Study on Critically Ill PatientsSuffering from Obstructive Sleep Apnea-Hypopnea Syndrome

    Objective To explore the diagnosis and treatment of critically ill patients suffering from obstructive sleep apnea-hypopnea syndrome ( OSAHS) . Methods Critically ill patients with OSAHS admitted in intensive care unit from January 2003 to December 2007 were retrospectively analyzed. Results Seventy-nine critically ill patients were diagnosed as OSAHS. The initial diagnosis of OSAHS was made by history requiring, physical examination, and Epworth sleepiness score evaluation. The final diagnosis was comfirmed by polysomnography thereafter. Base on the treatment of primary critical diseases, the patients were given respiratory support either with continuous positive airway pressure ( CPAP) or with bi-level positive airway pressure ventilation ( BiPAP) . Two cases died and the remaining 77 patients were cured anddischarged. Conclusions Timely diagnosis of OSAHS is important to rescue the critically ill patients. Respiratory support combined with treatment of primary critical diseases can improve the outcomes of these patients.

  • SELECTIVE TREATMENT OF AGED OSTEOPOROSIS THORACOLUMBAR VERTEBRAE BURST FRACTURE WITH BALLOON KYPHOPLASTY

    Objective To evaluate the effectiveness and the value of balloon kyphoplasty in treating aged osteoporosis thoracolumbar vertebrae burst fracture. Methods Between January 2003 and January 2008, 36 thoracolumbar vertebrae burst fracture patients were treated. There were 15 males and 21 females with an average age of 65.4 years (range, 59-72 years). Fourteen cases had no obvious history of trauma, 19 had a history of sl ight trauma, and 3 had a history of severe trauma. Forty vertebral bodies were involved, including 1 T7, 3 T9, 8 T10, 10 T12, 9 L1, 7 L2, and 2 L4. All patients displayed local pain and osteoporosis by bone density measurement with no neurological symptom of both lower limbs. Balloon kyphoplasty through unilateral (31 cases) or bilateral (5 cases) vertebral pedicles with polymethylmethacrylate was performed at the injection volume of (3.46 ± 0.86) mL per vertebral body. Before and after operation, the anterior height and posterior height of fractured vertebral body and the sagittal displacement were measured. Results Two cases had intraoperative cerebrospinal fluid leakage; 1 case had no remission of waist-back pain and pain was released after symptomatic treatment. All 36 patients were followed up 2.3 years on average (range, 1.5-4.0 years). No cement leakage was found with good diffusion of cement on X-ray film. The restoration of the height of vertebral bodies was satisfactory without nerve compression symptoms and other compl ications. The Visual Analogue Score at last follow-up (2.34 ± 1.03) was significantly lower than that of preoperation (6.78 ± 1.21), (P lt; 0.05). The compressive percentage of anterior height (19.80% ± 1.03%) of fractured vertebral body after operation was significantly lower than that before operation(25.30% ± 2.50%), (P lt; 0.05). There was no significant difference in posterior compressive percentage and sagittal displacement between pre- and post- operation (P gt; 0.05). Conclusion Based on roentgenographic scores, balloon kyphoplasty is selectively used to treat aged osteoporosis thoracolumbar vertebrae burst fracture, and the radiographic and cl inical results were satisfactory.

  • Clinical Application of Pocket Esophagogastric Anastomosis after Esophagectomy

    Objective To evaluate preventive effectiveness of pocket esophagogastric anastomosis for postoperativeanastomotic leak,stricture and gastroesophageal reflux disease (GERD),and investigate clinical significance of Montreal definition and classification of GERD after esophageal reconstruction. Methods Clinical data of 1 078 patients whoreceived 2 different surgical procedures for resection of esophageal or cardiac carcinoma from June 2007 to June 2011 in our hospital were retrospectively analyzed. In the experimental group,there were 582 patients who received pocketesophagogastric anastomosis,including 403 male and 179 female patients with their age of 60.4±12.6 years. There were 399 patients with esophageal carcinoma and 183 patients with cardiac carcinoma,392 patients receiving esophagogastrostomyabove the aortic arch and 190 patients receiving esophagogastrostomy below the aortic arch respectively. In the control group,there were 496 patients who received conventional end-to-side esophagogastric anastomosis,including 343 male and 153 female patients with their age of 59.2±12.8 years. There were 322 patients with esophageal carcinoma and 174 patients with cardiac carcinoma,317 patients receiving esophagogastrostomy above the aortic arch and 179 patients receivingesophagogastrostomy below the aortic arch respectively. A survey questionnaire was made on the basis of relevant diagnosticstandards to investigate the incidence of postoperative anastomotic stricture and GERD of the 2 groups during follow-up.Results The incidence of postoperative anastomotic leak of the experimental group was significantly lower than that of the control group [0% (0/582)versus 1.0% (5/496),χ2=5.835,P=0.016]. Patients in the experimental group had less severeGERD symptoms,and the percentage of patients who needed antacid therapy for extraesophageal symptoms of GERD ofthe experimental group was significantly lower than that of the control group [1.6% (33/541) versus 12.6% (57/453),χ2=23.564,P=0.000]. The incidence of anastomotic stricture of the experimental group was significantly lower than that of thecontrol group [0.9% (5/539) versus 7.3% (34/465),χ2=25.124,P=0.000],and especially,the incidence of severe anastomoticstricture of the experimental group was significantly lower than that of the control group [0% (0/539) versus 4.7% (22/465),χ2=24.883,P=0.000]. There was no statistical difference in five-year survival rate. Conclusion Pocket esophagogastric anastomosis is better than conventional end-to-side esophagogastric anastomosis for the prevention of postoperative anastomoticleak,stricture and GERD. Montreal definition and classification of GERD is suitable for the diagnosis of postoperativeGERD after esophageal reconstruction.
