  • Correlation Between Expression of Vascular Endothelial Growth Factor of Serum and Tumor Tissues and Clinical Prognosis in Patients with Breast Cancer

    bjective To investigate the correlation between expression of vascular endothelial growth factor(VEGF) of serum and tumor tissues and the clinical prognosis in patients with breast cancer. Methods The expressions of VEGF level of serum and tumor tissues in 44 patients with invasive duct breast cancer, 13 with benign breast diseases and 40 healthy controls. Serum VEGF level was measured by ELISA method. The protein expression of tissue VEGF, ER and C-erbB-2 were evaluated by immunohistochemistry LSAB method. Results Serum VEGF level and tissue VEGF expression in breast cancer were higher than those in benign breast diseases (P<0.001), and there was no significance in benign breast diseases and healthy controls (Pgt;0.05). VEGF expression was correlated with lymph node metastasis (P<0.01), ER and C-erbB-2 expression (P<0.05, P<0.01) and clinical stage (P<0.01). There were no statistical correlation between VEGF expression and age, tumor size (Pgt;0.05). Conclusion There is positively correlation between serum VEGF level and tissue VEGF expression, and between VEGF expression and clinic prognosis. Serum VEGF level may be one of important index of prognosis estimation in patients with breast cancer.

  • Current Status of Postmastectomy Radiation and Breast Reconstruction

    Objective To explore the interaction of postmastectomy radiotherapy (PMRT) and breast reconstruction, and elucidate how to choose the type and timing of breast reconstruction. Method Literatures about PMRT and breast reconstruction were reviewed. Results PMRT might increase the incidence of complications and impair the cosmetic satisfaction of breast reconstruction. Breast reconstruction might also compromise the effect of PMRT. Conclusions In patients who will receive or have already received PMRT, the optimal approach is delayed autologous tissue reconstruction after PMRT. If PMRT appears likely but may not be required at the time of mastectomy,delayed-immediate reconstruction may be considered, or immediate autologous tissue reconstruction may be considered in case of patients awareness of the increased complications and impaired cosmetic outcomes from PMRT.

  • The association between cancer location and lymph node metastasis in early stage of breast cancer

    Objective To summarize the relation between tumor location and lymph node metastasis in early stage of breast cancer, which is aimed at providing a more individualized treatment for breast cancer patients. Method The literatures about breast cancer location and lymph node metastasis in recent years were extracted, through the literatures study we made a thematic review of the relation between them. Results There were two main classification methods for the location of breast tumors at present: tumor in the different quadrants and tumor to skin distance. In the quadrant classification method, the tumor in the upper inner quadrant (UIQ) had the lowest lymph node metastasis rate, while the lower inner quadrant (LIQ) tumor recurrence-free survival rate and overall survival rate were significantly lower than other quadrants. When measuring tumor to skin distance, the closer the tumor was to the skin, the more likely lymph node metastasis occurred. In combination with the distribution, histology, and anatomical differences of lymphatic and lymphatic networks, our study group proposed to classify tumors according to different anatomical levels of the breast, thus the anatomic location of the tumor was divided into four types: constricted in the gland, break the anterior gland, break the posterior gland, and break both anterior and posterior gland. Conclusions Regardless of the way the location is classified, the location of breast tumors is closely related to lymphatic and lymph node metastasis. The new classification according to the distribution of tumors at different anatomical levels of the breast accords with the law of lymphatic metastasis is scientific and reasonable. Therefore, during clinical practices, we recommend to use the new method to classify tumor location, and we should consider the differences in the location of the patients’ tumor to assess the status of axillary lymph node, which may provide a more individualized treatment for breast cancer patients.

  • Research progress on the relationship between estrogen metabolism and breast cancer

    ObjectiveTo investigate the relationship between female estrogen metabolism and breast cancer.MethodBy searching the contents of Chinese Knowledge Network, Wanfang Database, PubMed and other databases, the relationship between breast cancer hormone receptor status, menstrual status and estrogen metabolism and breast cancer, estrogen metabolism pathway and signal in vivo a review of the transduction aspects.ResultsEstrogen and its metabolites was closely related to the occurrence and development of breast cancer. Among the main metabolic pathways of estrogen, 2-methoxyestradiol in the 2-hydroxy metabolic pathway had a potential protective effect on breast cancer; 4-hydroxyestradiol, 16α-hydroxyestrone might induce breast cancer; the ratio of 4-hydroxyestradiol to 2-hydroxyestradiol might be used as a biomarker for predicting malignant breast tumors. However, the estrogen metabolism patterns before and after menopause were different, and the effects on hormone receptor status were also different. Different hormone receptor status might affect the treatment and prognosis of breast cancer.ConclusionsThe relationship between estrogen and its metabolites and breast cancer in different receptor states is not completely clear. More large-scale prospective studies are needed to help us find out the rules to promote early diagnosis and early prevention of breast cancer.

  • Weighted gene co-expression network analysis for excavation of Hub genes related to the development of breast cancer

    ObjectiveTo explore the key modules and Hub genes related to the development of breast cancer from the level of gene network, and to verify whether these Hub genes have breast cancer specificity.MethodsThe key modules for the development of breast cancer were screened by weighted gene co-expression network analysis (WGCNA). The gene annotation database Gene Ontology and Kyoto Encyclopedia of Genes and Genomes (KEGG) were used to enrich the function of the modules, exploring the Hub genes having the highest correlation between the development of breast cancer. Simultaneously, we analyzed the relationship between Hub genes and tumor collection unit.ResultsWGCNA defined 10 co-expression modules, of which the blue module was the key module related to the development of breast cancer and other malignant tumors. The genes included in this module were significantly enriched in pathways such as the Cell Cycle pathway (KEGG ID: cfa04110), the Viral Oncogenic pathway (KEGG ID: hsa05203), Cancer pathway (KEGG ID: hsa05200), and Systemic Lupus Erythematosus (KEGG ID: hsa05322). The top eight Hub genes were finally extracted from the blue module including NUSAP1, FOXM1, KIF20A, BIRC5, TOP2A, RRM2, CEP55, and ASPM. Among them, NUSAP1, KIF20A, TOP2A, CEP55 and ASPM were also closely related to the occurrence and development of tumor collection unit.ConclusionWGCNA can screen for key modules and Hub genes which are biologically relevant to the clinical features of our interest, and the Hub genes have no breast cancer specificity participating in breast cancer development .

  • Application progress of vacuum assisted rotary cutting technique in benign and malignant breast diseases

    ObjectiveTo understand the application of vacuum assisted rotary cutting technique in benign and malignant breast diseases.MethodThe related literatures about the application of vacuum assisted rotary cutting technique in the breast benign and malignant diseases were reviewed.ResultsThe technique of vacuum assisted rotary cutting had been applied to the treatment of intraductal and phyllodes tumors. The non-lactation mastitis lesions could all be removed by this technique, and as a minimally invasive treatment for gynecomastia. It could guided by the color Doppler ultrasound, mammography and MRI, which could obtain enough tissue specimens for the screening and biopsy of early breast cancer. The indication of vacuum assisted rotary cutting technique should be further discussed in the breast conserving treatment of breast cancer.ConclusionsVacuum assisted rotary cutting technique has been widely used in treatment of various benign diseases of the breast and diagnosis and treatment of breast cancer. The best indication for operation should be determined based on clinical manifestations and auxiliary examination results.

  • Clinical application of immediate prepectoral implant-based breast reconstruction with TiLoop Bra after skin-sparing or nipple-areola-complex-sparing mastectomy for breast cancer patients

    ObjectiveTo explore the surgical technique and preliminary safety and aesthetics results of immediate prepectoral implant-based breast reconstruction (BR) with titanium-coated polypropylene mesh (TiLoop Bra) after skin-sparing mastectomy (SSM) or nipple-areola-complex-sparing mastectomy (NSM) for breast cancer patients. MethodsThe clinical data of consecutive patients who underwent immediate prepectoral implant-based BR with TiLoop Bra after SSM or NSM in West China Hospital from January to July 2022 were retrospectively analyzed. The operation time, intraoperative blood loss, early complication were collected. The preliminary aesthetics results were assessed with the Ueda score and Harris score. Results All the patients were female with a mean age of 39.0±6.8 years. One patient had bilateral breast malignant tumors, and the others had unilateral malignant tumors. Six patients received neoadjuvant chemotherapy before surgery. The mean diameter of the tumors was 24.4±11.9 mm under the color ultrasound before the neoadjuvant chemotherapy. The mean operation time was 153.9±49.4 min. The mean intraoperative blood loss was 29.2±18.3 mL. There were 3 patients with tumor stage 0, 10 patients with stage Ⅰ, 6 patients with stage Ⅱ, 3 patients with stage Ⅲ and 1 patient was found no residual cancer after neoadjuvant chemotherapy. All the patients were successfully followed up with a median follow-up time of 4.8 (3.0-9.2) months. There were 2 (8.3%) patients with major complications, including 1 wound dehiscence and 1 hematoma, and 4 (16.7%) minor complications, including 2 wound dehiscence and 2 infection. The patients with excellent and good Ueda score and Harris score accounted for 82.6% and 87.0%, respectively. None of the patients had animation deformity, capsular contracture, nipple-areola or skin flaps necrosis, or implant loss. During the follow-up period, no local/regional recurrence or distant metastasis was found. Conclusion For selected reliable patients, immediate prepectoral implant-based BR with TiLoop Bra after SSM or NSM for breast cancer patients is safe and has good aesthetics results in the early postoperative period. It has broad application space in patients with suitable indications, and can be promoted as a routine operation.

  • BREAST RECONSTRUCTION AFTER SKIN-SPARING MASTECTOMY OR NIPPLE-SPARING MASTECTOMY FOR BREAST CANCER

    Objective To evaluate the feasibility, oncological safety, and aesthetic result of skin-spring mastectomy (SSM) or nipple-spring mastectomy (NSM) in breast reconstruction of implant (permanent gel or expander) for breast cancer patients who were not fit for the breast conserving surgery (BCS). Methods Between October 2005 and July 2011, 89 women with breast caner underwent SSM or NSM, with an average age of 42.4 years (range, 19-55 years) and an average disease duration of 5.7 months (range, 1-24 months). The pathological examination revealed invasive ductal carcinoma in 55 cases, ductal carcinoma in situ (DCIS) in 15 cases, invasive ductal carcinoma + DCIS in 8 cases, DCIS with infiltration in 10 cases, and occult breast cancer in 1 case. According to tumor staging criterion of American Joint Committee on Cancer (AJCC), 15 cases were rated as stage 0, 51 cases as stage I, 22 cases as stage II, and 1 case as unclear. Finally, 33 patients underwent SSM and 56 patients underwent NSM according to the location and diameter of tumor and the infiltration of tumor to nipple. Secondary breast reconstruction was performed with permanent gel replacement after axillary lymph node dissection in 9 patients with positive sentinel lymph node and 1 patient with occult breast cancer; immediate breast reconstruction was performed with permanent gel in the other patients. All the patients received the chemotherapy or/and radiotherapy according to the National Comprehensive Cancer Network (NCCN) guideline. Results Complications occurred in 5 patients undergoing breast reconstruction of permanent gel after NSM, including 1 case of haemorrhage, 2 cases of infection, and 2 cases of local skin necrosis. Primary healing of incision was obtained in the others. No nipple necrosis was observed in patients undergoing NSM. All the patients were followed up 14-88 months (median, 40 months). At 10 months after operation, the aesthetic results were excellent in 40 cases, good in 33 cases, fair in 14 cases, and poor in 2 cases, with an excellent and good rate of 82%. No recurrence or metastasis was found during follow-up. Conclusion The SSM or NSM is feasible and oncological safe for patients who are not fit for BCS, with satisfactory aesthetic result.

  • A New Technique of Minimally Invasive Excision and(or) Biopsy of Breast Masses——Mammotome Operation

    【Abstract】ObjectiveTo introduce the minimally invasive excision and biopsy of breast neoplasm with Mammotome vacuum device guided by ultrasound. MethodsFiftytwo breast masses were detected in 30 patients through color Doppler. The ages of these patients range from 18 years to 49 years. Forty-six of those masses (88.5%) were clinically impalpable. Complete excision and biopsy of 52 breast masses were performed using Mammotome vacuum device guided by ultrasound. The patients were followed up and their postoperative condition were recorded. ResultsFiftytwo breast masses of 30 cases were excised completely, which was verified by color Doppler.The tissue excised by Mammotome were enough for pathological examination. Fortysix masses were proved to be fibroadenoma and the other 6 were proved to be adenosis. Hematoma was found in four patients after operation. The incisions were small and hidden, and the appearance of breasts remained well. Twenty patients were followed up for a shot period of time and no residual nidus or recurrence of the mass was found.ConclusionMinimally invasive excision and biopsy of breast masses with Mammotome vacuum device is an accurate and safe method. It is an ideal minimally invasive operation in treating benign breast neoplasm with few complications. It can be applied to biopsy of breast masses and complete excision of benign breast masses which are less than 2 cm in the same time.

  • CT texture analysis in gastric cancers

    CT texture analysis (CTTA) can objectively evaluate the heterogeneity of tissues and their lesions beyond the ability of subjective visual interpretation by extracting the texture features of CT images, then performing analysis and quantitative and objective evaluation, reflecting the tissue micro environmental information. This article reviews the recent studies on the applications of CTTA in gastric cancers, in the aspects of identification of gastric tumors, prediction of stage, correlation with Lauren classification, prediction of occult peritoneal carcinomatosis, evaluation of efficacy and prognosis, and prediction of biomarkers. It is regarded that CTTA has a good application prospect in gastric cancers.
