ObjectiveTo study the relationship between expression of p27KIP1 and progression of hepatocellular carcinoma(HCC).MethodsThe expression of p27KIP1 in 52 cases of HCC was detected by immunohistochemistry of strept avidinbiotin complex and mRNA in situ hybridization.ResultsThe positive cells of p27KIP1 protein were diffused in HCC.The positive signal was localized in nuclei.The labeling index (LI) of p27KIP1 protein was significantly higher in tumorsurrounding tissues than that in tumor tissues. p27KIP1 protein LI showed a positive correlation with the differentiation grade of HCC.The better differentiation of cancer cells, the higher LI of p27KIP1 protein (P<0.01).The positive cells of p27KIP1 mRNA were also diffused in HCC.The positive signal was localized in nuclei and cytoplasm. As to the expression of p27KIP1 at the mRNA level,there was no significant correlation with tumorsurrounding tissues and stages of HCC.Conclusionp27KIP1 protein is associated with progression and differentiation grade of hepatocellular carcinoma.
Objective To assess the affect of problem-based learning (PBL) versus literature based learning(LBL) in clinical medicine students. Methods Computer retrieval was conducted to search for controlled studies comparing PBL with LBL. The quality of included studies was critically evaluated and data was analyzed by using The Cochrane Collaboration’s RevMan 5.0 software. Results Finally 11 articles were included in the review, but most of them were of low quality.There are clinical heterogeneity between different studis which cause descriptive results. The PBL tended to be superior to LBL in the analysis-skill, self-directed learning skill, data-searching skill, and original skill.Conclusion PBL tends to be associated with better learning results among clinical medicine students compared withLBL methods. We need large-scale, randomized controlled trials of higher quality to confirm this.
【Abstract】ObjectiveTo generally analyze the current situations of clinical research and applications in early enteral nutrition (EEN) after abdominal surgery. MethodsThe published papers about the current situations of clinical research and applications in EEN after abdominal surgery were reviewed. ResultsEEN after abdominal surgery seems to be safe and effective, produces a positive nitrogen balance, keeps the integrality of structure and function of the apparatus, protects gut barrier, and reduces or prevents septic complications. ConclusionEEN may be of more benefits and will be one of the best methods of nutrition support after abdominal surgery.
Objective To observed the effect of IL-1β on expression of caudal-related homeobox gene 1 (CDX1) mRNA and mucoprotein 2 (MUC2) mRNA in cultured human gastric epithelial cells GES-1, and to investigate the underlying signal transduction pathways. Methods ①GES-1 cell was activated with IL-1β of different concentrations and time, the expression levels of CDX1 mRNA and MUC2 mRNA were detected by using real-time PCR. ②GES-1 cell was pretreated with PDTC, a NF-κB inhibitor, for 1 h prior to the addition of IL-1β, then the expressions of CDX1 mRNA and MUC2 mRNA were measured. Results Both CDX1 mRNA and MUC2 mRNA were not examined in GES-1 cell under normal culture conditions. But they could be induced by IL-1β with a dose-dependent manner in a concentration range (P<0.05); 8 h after treatment with IL-1β, the peak values of the expression levels of CDX1 mRNA and MUC2 mRNA were reached (P<0.05), then declined gradually. When pre-incubated with NF-κB inhibitor PDTC, the expression levels of CDX1 mRNA and MUC2 mRNA were significantly decreased (P<0.05). Conclusion IL-1β significantly induces the expressions of CDX1 mRNA and MUC2 mRNA in cultured human gastric epithelial cell GES-1 through the NF-κB signal pathway, which indicates that IL-1β plays a role in the process of intestinal metaplasia.
N-of-1 trial design offers a methodologically sound approach to determining optimum treatment for an individual patient and solves some limitations of randomized controlled trials. This design could offer an efficient method of reaching a personal treatment regime tailored to suit individual needs and preferences. The paper introduces practical application, objects and the implementation process of N-of-1 trial, to explore its design points and implementation.
Objective To evaluate the psychological trauma incurred by the hospitalization survivor of Zhouqu district after the Zhouqu debris flow so as to provide relevant information for psychological and medical interventions. Methods The psychological state of 67 hospitalized survivors of the disaster and other 47 inpatients with similar complaints but not coming from the disaster area was investigated through a mental health self-assessment questionnaire, self-rating anxiety scale (SAS), self-rating depression scale (SDS) and PTSD-SS scale. Results The post-disaster survivors had different levels of psychological problems and post-traumatic stress disorder, and there were significant differences compared to the control group (Plt;0.01, Plt;0.01). The SDS score and the SAS score of the survivor were 48.44+15.648 and 52.92+11.672, respectively, which were all much higher than those of the control group (Plt;0.01, Plt;0.05). Conclusion The debris flow disaster bring serious psychological trauma to the victims. It is necessary to pertinently carry out post-disaster psychological relief including psychological intervention and regulation for the hospitalized survivors, so as to alleviate and reduce their psychological suffering.
It is potential for N-of-1 trials to evaluate economics of health care, however, it is still in the exploratory stage. With the advantage of accurate estimation of costs and effects, it is beneficial to promote the application of N-of-1 trials for economic evaluation in the era of precision medicine. In this study, we introduce the necessity, feasibility, selection, calculation of indicators and influence factors of N-of-1 trials for economic evaluation, and in order to provide references for researchers to perform related studies.
The study appeared the comparison between CONSORT and CENT, and promoted the combination with GRADE and N-of-1 trial. Our objective is to further develop the method of N-of-1 trial and to widely use it in clinical researches of some diseases.
An N-of-1 trial was conducted in a single patient. Statistical analysis is one of the most important parts of N-of-1 trials. The methods of statistical analysis for N-of-1 trials were reported in some reviews. However, there was still a lack of comparative analysis of these methods. In this study, we introduced the characteristics of statistical methods commonly used as well as some statistical problems which should be paid attention in N-of-1 trials. It is useful to provide some reference for statistical methods in order to high quality N-of-1 trials.
Bayesian N-of-1 trials is increasingly popular in recent years. This study introduced the principle, statistical requirements, application status, advantages and disadvantages of Bayesian N-of-1 trials. Although the application of Bayesian N-of-1 trials is still limited in small scale and some problems remain to be solved, but it can provide more posterior information, and it can be the most important type of N-of 1 trial in future.
