  • Therapeutic Effect of Breviscapine Combined with Enalapril on Early Diabetic Nephropathy

    【摘要】 目的 探讨灯盏花素注射液联合血管紧张素转换酶抑制剂(ACEI)依那普利治疗糖尿病早期肾病的临床效果。 方法 2006年10月-2009年12月,将59例临床确诊2型糖尿病早期肾病的患者随机分为治疗组(n=30)与对照组(n=29)。对照组在基本治疗的基础上应用依那普利,治疗组在基本治疗的基础上联合应用依那普利和灯盏花素注射液,疗程均为3周。观察治疗前后两组患者24 h尿微量白蛋白排泄率(UAER)、全血黏度、血浆黏度、甘油三酯(TG)、尿素氮(BUN)、血肌酐(SCr)的变化。 结果 两组患者治疗前后自身对比,24 h UAER均有明显下降(Plt;0.05);治疗组UAER下降较对照组更为明显(Plt;0.05)。 结论 ACEI联合灯盏花素,其降低24 h UAER疗效优于单纯ACEI疗效,还可有效降低全血黏度和血浆黏度,降低纤维蛋白原含量,改善患者血液流变性。【Abstract】 Objective To investigate the therapeutic effects of breviscapine combined with angiotensin converting enzyme inhibitor (enalapril) on early diabetic nephropathy.  Methods A total of 59 patients with early diabetic nephropathy diagnosed between October 2006 and December 2009 were randomly divided into treatment group (n=30) and control group (n=29). The patients in treatment group were treated by breviscapine combined with enalapril, while the patients were treated with only enalapril in the control group. All of the patients were treated for three weeks. Urinaryalbuminexcretion (UAE), whole blood viscosity, plasma viscosity, triglycercide, blood urea nitrogen and serum creatinine in the two groups were detected before and after the treatment. Results After the threatment, UAE decreased in both of the two groups compared with the value before the treatment (Plt;0.05); the decrease in treatment groups was more obvious than that in the control group (Plt;0.05). Conclusion The combination of breviscapine and enalapril is effective on early diabetic nephropathy.

  • Evaluation on the Effect Index of Diagnostic Test

    Based on the guidelines of the diagnostic test systematic review, this study elaborated the statistical processing of the pooling of data detailed in the systematic review of diagnostic test, discussed the methods for identification and handling of the heterogeneity, evaluated the advantages and disadvantages of the index of the accuracy in diagnostic tests, and proposed the identification method of the publication bias. It also took the data from the published article entitled “Diagnostic Value of ProGRP and NSE for Small Cell Lung Cancer: A Meta-Analysis” as an example for analysis and illustration, which presented clearly the data processing and interpretation of the systematic review of diagnostic test, in order to provide references for clinical researchers to study and conduct the systematic review of diagnostic test.

  • Inhibition effect of minocyclinye on pressureinduced apoptosis of retinal neuron cells cultured in vitro

    Objective To observe the effects of minocycline to the viability and apoptosis of ratprime;s retinal neuron cells (RNC) under pressure, and to investigate the neuroprotective mechanisms of minocycline against the RNC damage. Methods Establish a model of ratprime;s RNs under pressure cultured in vitro, the protective effect of minocycline is observed by different methods, including observing the morphology of the cells, evaluating the cellsprime; viability by methyl thiazolyl tetrazolium (MTT) colorimetry assay, and detecting the cellular apoptosis with acridine orange/ethidium bromide (AO/EB) double staining by fluorescence microscopy. Immunocytochemistry was used to detect the expression of iNOS and caspase-3 in the cells. Results Obvious morphology changes of RNC were found in cells under pressure compared with the control; the viability of RNC decreased and cellular apoptosis was found in 53.93% cells. The cellular morphology improved in the cells treated by 20 mu;mol/L minocycline, the cellular viability significantly increased, and the cellular apoptosis was found in 17.29% cells. In addition, the expression of iNOS and caspase3 in the treated cells decreased compared with which in the pressured group. Conclusion Minocycline with a certain concentration can effectively inhibit pressureinduced damage and apoptosis of RNC of rats, and the inhibitory effect on expression of iNOS and capases-3 may be the underlying mechanism.

  • The Expression and Significances of Cyclind1,P16 in Cutaneous Squamous Carcinoma

    目的:观察CyclinD1,P16在皮肤鳞状细胞癌组织中的表达,探讨CyclinD1,P16在皮肤鳞状细胞癌发生发展中的作用。方法:用免疫组织化学技术检测40例皮肤鳞癌石蜡包埋组织和10例正常皮肤组织的CyclinD1,P16表达。结果:CyclinD1在皮肤鳞癌组织中表达高于皮肤正常对照组,两者差异有显著统计学意义(Plt;0.01); P16在皮肤鳞癌组织中表达低于皮肤正常对照组,但两者差异无显著统计学意义(Pgt;0.05);CyclinD1,P16在皮肤鳞癌中表达呈正相关性(r=0.782,Plt;0.01)。结论:CyclinD1在皮肤鳞状细胞癌组织中阳性表达上调,P16在皮肤鳞状细胞癌组织中阳性表达下调,CyclinD1,P16在皮肤鳞癌中表达呈正相关。

  • Assessment and therapy progress in Parkinson’s disease rehabilitation

    People with Parkinson’s disease (PD) exhibit multi-system damaged. Medication mainly targets impairments related to dopaminergic lesions. Moreover, in later stages of the disease, medication becomes less effective. Rehabilitation therapy is believed that it can improve multiple functional disorders, including myotonia, bradykinesia, and postural gait abnormalities. It not only reduces the severity of non-motor symptoms and improves the quality of life in PD patients, but also delays the development of PD and improves the activity of daily life of patients. This article summarizes the progress of rehabilitation assessment and the therapy of PD.

  • BERT-based automated risk of bias assessment

    ObjectiveTo realize automatic risk bias assessment for the randomized controlled trial (RCT) literature using BERT (Bidirectional Encoder Representations from Transformers) as an approach for feature representation and text classification.MethodsWe first searched The Cochrane Library to obtain risk bias assessment data and detailed information on RCTs, and constructed data sets for text classification. We assigned 80% of the data set as the training set, 10% as the test set, and 10% as the validation set. Then, we used BERT to extract features, construct text classification model, and evaluate the seven types of risk bias values (high and low). The results were compared with those from traditional machine learning methods using a combination of n-gram and TF-IDF as well as the Linear SVM classifier. The accuracy rate (P value), recall rate (R value) and F1 value were used to evaluate the performance of the models.ResultsOur BERT-based model achieved F1 values of 78.5% to 95.2% for the seven types of risk bias assessment tasks, which was 14.7% higher than the traditional machine learning method. F1 values of 85.7% to 92.8% were obtained in the extraction task of the other six types of biased descriptors except "other sources of bias", which was 18.2% higher than the traditional machine learning method.ConclusionsThe BERT-based automatic risk bias assessment model can realize higher accuracy in risk of bias assessment for RCT literature, and improve the efficiency of assessment.

  • The Relationship between Serum Uric Acid and Insulin Release and Sensitivity

    目的 探讨血尿酸与胰岛素分泌和敏感性的关系。 方法 对成都市龙泉驿区2007年6月-7月间20~75岁常住自然人群565人进行人口学、腰围、臀围、血压、75 g葡萄糖耐量及胰岛素测定(0、30、120 min),血脂、血肌酐及血尿酸检测。以早期胰岛素分泌功能指数(△I30/△G30)和葡萄糖处置指数(DIo)代表胰岛素分泌功能,以Matsuda 指数代表胰岛素敏感性,分别与血尿酸进行相关性研究。 结果 男性的血尿酸与年龄呈负相关,与舒张压、葡萄糖负荷后120 min胰岛素和甘油三酯呈正相关。女性的血尿酸与甘油三酯、低密度脂蛋白、血肌酐呈正相关,与高密度脂蛋白、Matsuda指数呈负相关。无论男性和女性,均未发现血尿酸与△I30/△G30、DIo相关。 结论 女性的胰岛素敏感性下降与血尿酸升高有关。未发现血尿酸与胰岛素分泌有相关关系。

  • The Expression and Clinical Significance of P53, Epidermal Growth Factor Receptor and Ki-67 in Different Kinds of Breast Infiltrative Ductal Cancer

    目的 探讨不同分子分型乳腺浸润性导管癌手术病例标本中P53、表皮生长因子受体(EGFR)和Ki-67的表达及临床意义。 方法 采用免疫组织化学链霉菌抗生物素蛋白-过氧化物酶连接法法对2010年1月-2011年7月446例乳腺浸润性导管癌患者标本进行分子分型,并同时检测其P53、EGFR、Ki-67等的表达。 结果 P53和Ki-67在人类表皮生长因子受体2(HER2)过表达型、基底细胞样型、未分类型中的表达明显强于管腔A型及管腔B型(P<0.05);HER2过表达型和未分类型中的EGFR表达明显强于管腔A型及管腔B型(P<0.05)。 结论 在使用雌激素受体、c-erbB-2等指标对浸润性导管癌进行分子分型时同时检测P53、EGFR及Ki-67等标记物,有助于更加精准的评估肿瘤的生物学行为及预后 ,对靶向药物的个体化治疗提供参考和疗效预测有重要意义。

  • The burden of disease of Chinese females cervical cancer from 1990 to 2019

    ObjectiveTo analyze the trends of incidence, mortality, and burden of disease of cervical cancer in Chinese females from 1990 to 2019.MethodsThe global burden of disease database (GBD) and China health statistics yearbook data was used to analyze the incidence, standardized incidence, mortality, standardized mortality, urban and rural mortality, and burden of cervical cancer among Chinese females using Excel, SPSS 21.0 and Joinpoint Regression Program 4.8.0.1.ResultsThe standardized incidence of cervical cancer among Chinese females increased from 9.21/100 000 in 1990 to 12.06/100 000 in 2019, and the standardized mortality decreased from 8.40/100 000 to 7.36/100 000. The standardized mortality of cervical cancer in 2018 decreased when compared with 2015 in both urban and rural areas. Changes in age-group incidence and mortality indicated that there was a younger trend in cervical cancer. The disease burden indicators (DALY, YLL, and YLD) were increased from 86.49, 84.01, and 1.52 ten thousand person/years to 162.22, 157.40, and 4.83 ten thousand person/years, in which the YLD increased the most (217.76%). The APC of DALY, YLL and YLD were 2.39%, 2.56% and 4.25%, respectively. The proportion of cervical cancer disease burden in female cancer increased in 2019 compared with 1990. And DALY, YLL and YLD increased in the age group of 40 or over, in which DALY of the age group 50-54 increased 167.15%.ConclusionsThe situation of cervical cancer is not optimistic in China. Although the mortality of cervical cancer has decreased in recent years, the number of cases and mortalities is still increasing. Not only the burden of disease is continuously increasing, there is also a younger trend in cervical cancer. Active preventive measures should be taken to reduce the burden of cervical cancer.

    The COSMIN community updated the COSMIN-RoB checklist on reliability and measurement error in 2021. The updated checklist can be applied to the assessment of all types of outcome measurement studies, including clinician-reported outcome measures (ClinPOMs), performance-basd outcome measurement instruments (PerFOMs), and laboratory values. In order to help readers better understand and apply the updated COSMIN-RoB checklist and provide methodological references for conducting systematic reviews of ClinPOMs, PerFOMs and laboratory values, this paper aimed to interpret the updated COSMIN-RoB checklist on reliability and measurement error studies.
