  • Role of Apoptosis-Related Gene survivin, caspase-3 and cyclin-B1 in Gastric Carcinoma Tumorigenesis

    【Abstract】ObjectiveTo investigate the role of apoptosis-related gene survivin, caspase-3 and cyclin-B1 in gastric carcinoma by detecting the expressions of survivin, caspase-3 and cyclin-B1 in gastric carcinoma. Methods The expressions of survivin mRNA, caspase-3 mRNA and cyclin-B1 mRNA were determined by reverse transcription-polymerase chain reaction (RT-PCR) method in 30 gastric carcinoma specimens and 10 normal gastric tissue specimens. ResultsThe positive expression rate of survivin mRNA in 30 gastric carcinoma specimens was 66.7%(20/30). While 10 normal gastric tissue specimens did not express survivin mRNA. Although all of 30 gastric carcinoma tissues and 10 normal gastric tissues expressed caspase-3 mRNA and cyclin-B1 mRNA, the expressions of caspase-3 and cyclin-B1 in 20 survivin-positive gastric carcinoma tissues were significantly lower than those of 10 survivin-negative gastric carcinoma tissues (P<0.01) and 10 normal gastric tissues (P<0.01). The expressions of caspase-3 mRNA and cyclinB1 mRNA in 10 survivin-negative gastric carcinoma tissues were significantly lower than those of 10 normal gastric tissues (P<0.01). And in gastric carcinoma tissues the expression of survivin mRNA was negatively related with that of caspase-3 mRNA (r=-0.923,P<0.01) and cyclin-B1 mRNA (r=-0.886,P<0.01), the expression of caspase-3 mRNA was positively related with that of cyclin-B1 mRNA (r=0.892, P<0.01). Conclusionsurvivin enhances gastric tumorigenesis, caspase-3 and cyclin-B1 inhibit gastric tumorigenesis.

  • The Effect of Highly Selective Vagotomy Plus Mucosal Antrectomy and Highly Selective Vagotomy Alone on Motor Function of Gastric Antrum

    Objective To investigate the effects of highly selective vagotomy plus mucosal antrectomy (HSVMA) and highly selective vagotomy (HSV) alone on the functions of the gastric antrum. MethodsEighteen dogs and 48 patients with chronic duodenal ulcer were tested. The time of gastric emptying, antral myoelectric activity, antral pressure were measured 4-6 months after operation in 18 dogs which were randomized into the control, HSV and HSVMA groups. The gastric evacuation, frequency and amplitude of electrogastrography (EGG) were observed 4-6 months after operation in 48 patients who were randomly divided into HSV (20 patients) and HSVMA (28 patients) group, their preoperative values were considered as control group. ResultsIt was found by barium meal examinations that the shape of the stomach and duodenum was normal and gastric peristalsis was clearly visible in all the animals and patients of two postoperative groups. The beginning and ending time of gastric emptying were (5.0±0.06) min and (4.0±0.4) h respectively in the dogs of HSVMA group, which were similar to those of the dogs of control and HSV group (Pgt;0.05). The frequency of the antral myoelectric action potential was (3.11±0.65 ) cycles/min in the dogs of HSVMA group, the frequency of EGG was (3.25±0.75) cycles/min in the patients of HSVMA group, which were significantly lower than that of the dogs or patients of control and HSV group (P<0.05). Injection of pentagastrin in dogs or taking meal in patients significantly increased the antral pressure or amplitude of EGG. Conclusion The motor function of the reconstructed antrum in the HSVMA group approaches the normal level despite a lower antral myoelectric frequency. It is believed that HSVMA will be a choice for the treatment of duodenal ulcer.

  • THE EARLY AND LATE CLINIC INVESTIGATION AFTER HIGH SELECTIVE VAGOTOMY AND MUCOSAL ANTRECTOMY FOR DUODENAL ULCER

    Objective To study the earlier and later clinic results of 64 chronic duodenal ulcer patients treated with high selective vagotomy and mucosal antrectomy (HSV+MA). Methods The clinic results of the patients and the changes of gastrin, motilin and somatostatin in the blood were prospectively investigated. Results Fifty nine (92.2%) patients after 3-6 months of follow-up and 26 (92.9%) patients after 5-8 years of follow-up achieved Visick grates Ⅰ-Ⅱ. No patients died. Gastric acid secretion and infection rate of helicobacter pylori in antral mucosa were significantly reduced after operation. No significant difference was showed in bile acids and total bacterial counts of gastric juice before and after operation. No ulcer recurrence was found by barium meal and endoscopy. There was no significant difference in serum gastrin and plasma motilin before and after operation. The level of somatostatin in the blood of patients after 5-8 years of follow-up was decreased. Conclusion HSV+MA is the better operative treatment for duodenal ulcer, since it can not only effectively and lastingly decrease acid secretion and rates of ulcer recurrence, but also preserve the function of the antrum and pylorus and keep the gastric milieu interne relatively stable.

  • Distribution and Expression of Estrogen Receptor β Protein in Sigmoid Colon of Female Patients with Slow Transit Constipation

    Objective To investigate the express of ERβ protein in female slow transit constipation (STC) patients. Methods Immunohistochemistry and Western blot technique were used to detect the distribution and expression of estrogen receptor β (ERβ) protein of 20 patients with STC and 20 aged-matched controls. Results ERβ expressions were detected in mucous layer, myenteric nerve plexus and submucous nerve plexus in two groups. In comparison with the control group, the expression of ERβ protein of STC group was much lower (Plt;0.01). The expression of ERβ protein of sigmoid colon in STC group was significantly lower than that in control group (Plt;0.05). Conclusion The expression of ERβ protein decreased in myenteric and submucous nerve plexus of sigmoid colon tissues may involve in the pathogenesis of STC.

  • Application of Damage Control Theory in the Treatment of Cerebral Arteriovenous Malformations

    【摘要】 目的 探讨损伤控制理论(damage control theory,DCT)在脑动静脉畸形(arteriovenous malformations,AVM)诊断与治疗中的应用意义,以及指导治疗脑AVM的可行性。 方法 依据DCT原则,对2007年3月-2009年3月脑AVM患者进行积极治疗,并分析其治疗结果。 结果 根据DCT原则制订脑AVM患者的治疗方案,控制脑AVM的危险因素,降低其疾病发展风险,患者治疗结果比较满意。 结论 应用DCT原则指导脑AVM患者急性期处理及介入等治疗措施,可以提高患者的生存率,有效控制疾病的危险因素,提高治疗总有效率,具有临床指导意义。【Abstract】 Objective To investigate the significance and feasibility of damage control theory (DCT) in diagnosing and treating cerebral arteriovenous malformations (AVM). Methods According to the disease treatment principle of DCT, 76 patients with AVM diagnosed in our hospital from March 2007 to March 2009 were treated actively, and the treatment results were analyzed. Results The risk factors of AVM was controlled, the development risk of AVM slowed down, and the therapeutic effect was satisfactory. Conclusion According to the DCT principle, we could improve the survival rate of the patients, control the risk factors effectively, and improve the prognosis of the disease. DCT principle is clinically significant to guide the treatment of AVM.

  • Isolation and Culture of Adult Rat Liver and Identification of The Markers for Hepatic Oval Cells

    Objective To study a simple and practical method of isolation, culture and identification of hepatic oval cells from adult rat. Methods Wistar adult rats were fed by 2-acetaminofluorere (AAF) and were stimulated by partial hepatectomy to activate the proliferation of hepatic oval cells. After operation 12 days, the livers were resected for isolating oval cells. Hepatic tissue was digested by 0.10% collagenase Ⅳ and the obtained heterogeneous liver cells were then isolated and purified by density gradient centrifugation. The expressions of albumin and CK19 mRNA in hepatic oval cells were analyzed by immuno-fluorescence and RT-PCR. Results The survival rate of the newly isolated oval cells was more than 90%. The hepatic stem cells were shown by immuno-fluorescence of stem cell’s antigen c-kit. The expressions of mRNA CK19 and albumin of the oval cell were also detected by PCR. The proliferation activity of the newly isolated oval cells was significantly high and they could be induced to differentiate into both hepatic and bile ductal cells by some growth factors. Conclusion The successful development of the simple and feasible isolation and purification procedure as well as the identification method for hepatic oval cells may provide a fundamental for further studies about bionomics of the hepatic stem cell and the relation between stem cells and hepatic carcinoma.

  • Evaluation of surgical effectiveness for slow transit constipation in functional outcomes and quality of life

    ObjectiveTo evaluate the functional outcomes and quality of life in patients with surgery for slow transit constipation (STC).MethodsFrom March 2013 to July 2017, 29 patients undergoing total or subtotal colectomy for STC in our department were analyzed prospectively. Their preoperative and postoperative 1-year follow-up details were analyzed. Evacuation function of all patients was assessed by bowel movements, abdominal pain, bloating, straining, laxative, enema use and the Wexner constipation scales. Quality of life was evaluated by the Gastrointestinal Quality of Life Index (GIQLI) and the short-form (SF)-36 survey.ResultsA high number of patients (93.1%, 27/29) in STC stated that surgery received benefits to their health. Compared with that before operation, the number of bowel movements per week during a 1-year follow-up increased significantly (31.6±19.9 vs. 1.21±0.6, P<0.05). Significant trends toward improvement of bloating, straining, laxative and enema use were noted 1-year following surgery (P<0.05). The Wexner constipation scales scores during a 1-year follow-up decreased significantly (5.69±3.4 vs. 20.34±0.6, P<0.05). The GIQLI scores during a 1-year follow-up increased significantly (120.7±20.4 vs. 78.6±17.3, P<0.05). Moreover, results of SF-36 showed significant improvements in 7 spheres (role physical, role emotional, physical pain, vitality, mental health, social function and general health) during a 1-year follow-up compared with those before operation (P<0.05).ConclusionTotal or subtotal colectomy for STC did not only alleviate constipation symptoms dramatically, but also received significant improvements in the patients’ quality of life.
