  • TREATMENT OF OLD SCAPHOID FRACTURE WITH ABSORBABLE SCREW

    To explore a new approach to treat old scaphoid fracture by grafting bones from distal radius and fixing absorbable screws. Methods From January 2002 to May 2007, 18 patients with old scaphoid fractures were treated by grafting bones from distal radius and fixing absorbable screws. All of the 18 patients (13 males and 5 females, aged from 17 to 41 years old) had the injury history of the palm of hand touching the ground. They felt tenderness on the anatomical snuff-box and tubercle of scaphoid bone. The grasping force decreased. The range of motion of the wrist was (36 ± 2)° extension/(30 ± 3)° flexion, (8 ± 3)° radial is/(13 ± 2)° ulnaris. Results The average postoperative follow-up period was 15.7 months.A total of 18 cases achieved bone union. The range of motion of the wrist in the 16 healed cases was (68 ± 2)° extension/(65 ± 3)° flexion, (15 ± 3)° radial is/(28 ± 5)° ulnaris. There was no pain of motion and symptom of powerlessness. As to the other 2 cases, the range of motion of the wrist was (40 ± 8)° extension/(35 ± 6)° flexion, (8 ± 5)° radial is/(12 ± 5)° ulnaris; no obvious improvement was found; the patients occasionally felt painful in daily l ife; the extension force decreased. No infection and break of internal fixation were seen. Conclusion The absorbable screw as a new biomaterial for internal fixation has obvious advantages of good biocompatibil ity, promoting union of the fracture and no need of a second operation for removal.

  • EFFECTIVENESS OF POSTERIOR APPROACHES FOR TREATMENT OF POSTERIOR CORONAL FRACTURE OF TIBIAL PLATEAU

    Objective To observe the effectiveness of posterior approaches for the treatment of posterior coronal fractures of tibial plateau, and to analyze the fracture morphology, radiographic features, and the recognition of Schatzker classification. Methods Between June 2003 and June 2009, 23 patients with posterior coronal fractures of tibial plateau were treated surgically by posterior approaches. There were 15 males and 8 females with an average age of 38 years (range, 32-56 years). All patients had closed fractures. Fracture was caused by traffic accident in 15 cases, by sports in 3 cases, and by falling from height in 5 cases. According to Moore classification, there were 10 cases of type I, 9 cases of type II, and 4 cases of type IV. The X-ray films, CT scanning, and three-dimensional reconstruction were performed. The time from injury to operation was 3-14 days (mean, 6 days). Results After operation, 17 cases had anatomical reduction and 6 had normal reduction. Incisions healed by first intention. All cases were followed up 12 to 36 months (mean, 24 months). The average fracture healing time was 7.6 months (range, 6-9 months). No related complication occurred, such as nerve and vessel injuries, failure in internal fixation, ankylosis, traumatic osteoarthritis, and malunion. According to Rasmussen’s criteria for the function of the knee, the results were excellent in 14 cases, good in 7 cases, and fair in 2 cases with an excellent and good rate of 91.3%. Conclusion Posterior coronal fracture of tibial plateau is rare, which has distinctive morphological features, and Schatzker classification can not contain it totally. The advantages of posterior approach include reduction of articular surface under visualization, firm fixation, less complications, and earlier functional exercise, so it is an ideal surgical treatment plan.

  • EVALUATION OF PERCUTANEOUS ILIOSACRAL SCREWS FOR UNSTABLE PELVIC FRACTURES

    【Abstract】 Objective To evaluate the effectiveness of percutaneous fixation with il iosacral screw in the management of unstable pelvic fractures. Methods From March 2003 to January 2007, 15 patients with unstable pelvic fractures were treated by percutaneous fixation with il iosacral screws. There were 6 males and 9 females, aged 21-56 years. Fractures were caused by traffic accident in 8 cases, high fall ing in 6 cases and crushing injury in 1 case. The disease course ranged from 4 hours to 3 days. Of 15 patients, 7 had fractures of pubium and sacrum; 2 had fractures of il ium and sacrum; 4 had dislocation of sacroil iac joint with fractures of pubic rami; and 2 had dislocation sacroil iac joint with fracture of sacrum of pubium. Anterior fixation was performed by means of plating in 3 cases, by external fixation in 7 cases, and by both in 5 cases. Results The blood loss was from 1 000 to 1 500 mL in 4 patients with acetabular fracture and about 50 mL in the others. The average operative time was 153.6 minutes (range, 60-305 minutes). Fifteen patients were followed up 6 months to 3 years with an average of 18months. According to Matta criterion for fracture reduction, the results were excellent in 14 cases and good in 1 case. After a follow-up of 28.8 months, fracture union was achieved in all patients. Screw loosening occurred in 1 case, pain of lumbosacral region in 3 cases after tiredness and mild claudication in 2 cases. Nine patients returned to their occupation, three patients changed occupation, and 3 patients could not work at last follow-up. According to Majeed functional scoring, the results were excellent in 11 cases and good in 4 cases. Conclusion Percutaneous il iosacral screw fixation is a useful method in treatment of unstable pelvic fracture.
