  • Reoperation for Local Recurrence of Rectal Cancer(Analysis of 56 Cases)

    Objective To approach the surgical therapeutic efficacy of local recurrence of rectal cancer. Methods Fifty-six patients with local recurrence of rectal cancer suffered from reoperation from January 2003 to January2011 in this hospital were collected. Chi-square test was performed to analyze the complete excision rates of reoperation for different recurrent time after radical resection and different surgical procedures after primary radical resection of rectalcancer. The method of log-rank test was used for survival analysis of the Miles and Dixon procedure. Results ①The opera-tion time and intraoperative bleeding of reoperation for local recurrence of rectal cancer were more than those of the primary radical resection of rectal cancer (P<0.05). ②The complete resection rate of the local recurrence of rectal cancer in 5 years after primary radical resection of rectal cancer was higher than that of the local recurrence of rectal cancer within 2 years after primary radical resection of rectal cancer, and the difference was statistically significant (P<0.01). ③The complete resection rate of the local recurrence of rectal cancer of the technique of Dixon in the primary radical resection of rectal cancer was higher than that of Miles, and the difference was statistically significant (P<0.05). ④The median survival time and 2-year survival rate and 5-year survival rate of the technique of Dixon in the reoperation were longer or higher than those of Miles, and the differences were statistically significant (P<0.05). Conclusions Surgical procedure and postoperative recurrence time after primary operation can both influence complete excision rate of reoperation for local recurrence of rectal cancer. And reoperation for local recurrence of rectal cancer can prolong the survival time.

  • Application and progress of enhanced recovery after surgery in perioperative period of esophageal cancer

    The concept of enhanced recovery after surgery (ERAS) was firstly demonstrated in practice by the Danish scholar Henrik Kehlet in the early 2000s. At present, the ERAS concept has been widely used in a variety of surgical fields, but its application in esophageal cancer surgery is still limited. The new esophageal ERAS guidelines issued by ERAS Association bring new opportunities for the application and promotion of esophageal cancer surgery. Combined with the current situation of esophageal cancer surgery in China and related literature, in this paper we discuss the specific measures of ERAS concept in perioperative application of esophageal cancer in China.

  • Prognostic analysis of pT1b esophageal squamous cell carcinoma after open or minimally invasive esophagectomy

    ObjectiveTo compare the short-term and long-term effects of minimally invasive esophagectomy (MIE) and traditional open esophagectomy (OE) in patients with stage T1b esophageal squamous cell carcinoma (ESCC).MethodsWe retrospectively analyzed the clinical pathology data of 162 patients undergoing thoracic surgery at Northern Jiangsu People's Hospital from 2015 to 2018 whose pathological diagnosis was stage pT1b ESCC. According to the surgical approach, they were divided into MIE group and OE group. There were 55 males and 21 females in the OE group, with an average age of 63.3±5.6 years, and 60 males and 26 females in the MIE group, with an average age of 64.7±6.1 years. The preoperative, intraoperative and postoperative data of the two groups were compared and followed up. Survival data were compared using Kaplan-Meier and log-rank tests between the two groups, and Cox proportional hazard regression models were used to analyze prognostic factors.ResultsCompared with the OE group, the intraoperative bleeding volume of the MIE group was less (119.8±70.0 mL vs. 210.5±136.2 mL, P<0.001), and the lymph nodes dissected during the operation were more (19.1±7.4 vs. 13.8±5.9, P<0.001), the rate of postoperative pulmonary infections was lower (9.3% vs. 21.1%, P=0.036), but the operation time was longer (240.0±52.4 min vs. 179.5±35.7 min, P<0.001). Twenty-one patients had lymph node metastasis, and the lymph node metastasis rate was 13.0%. At the end of the follow-up, 19 patients died, and the overall survival (OS) at 1 year, 3 years, and 5 years after operation were 97.5%, 88.8% and 82.9%, respectively; 31 patients had recurrence and metastasis, and the disease-free survival (DFS) rate at 1 year, 3 years, and 5 years after operation was 95.1%, 80.9% and 75.6%. There was no significant difference in OS and DFS between the two groups. Multivariate Cox regression analysis of OS found that lymph node metastasis, anastomotic fistula and chylothorax were independent risk factors for OS. Multivariate Cox regression analysis of DFS found that lymph node metastasis, anastomotic fistula, chylothorax, and vascular cancer thrombus were independent risk factors for OS.ConclusionMIE can achieve the same long-term effects as OE, with less intraoperative bleeding, more lymph nodes dissected, and lower incidence of postoperative pulmonary infections, but it takes longer operation time.

  • Association between obstructive sleep apnea syndrome and carotid atherosclerotic disease: a systematic review

    Objective To systematically review the correlation between obstructive sleep apnea syndrome (OSAS) and the incidence of carotid atherosclerosis. Methods PubMed, EMbase, CNKI, WanFang Data, CBM, and VIP databases were electronically searched to collect studies on the correlation between OSAS and carotid atherosclerosis and carotid intima-media thickness (CIMT) from inception to August 2021. Two reviewers independently screened literature, extracted data, and assessed the risk of bias of the included studies. Meta-analysis was then performed using Stata 16.0 software and RevMan 5.3 software. Results A total of 32 studies, including 2 915 patients were included. The results of the meta-analysis showed a higher incidence of carotid atherosclerotic plaque in OSAS patients than in the control group (OR=5.56, 95%CI 0.27 to 8.38, P<0.000 01); subgroup analysis revealed that, compared with the control group, patients who were male (OR=5.38, 95%CI 2.79 to 10.38, P<0.000 01) or had mild-to-moderate OSAS (OR=3.9, 95%CI 1.66 to 9.15, P=0.002) or severe OSAS (OR=19.86, 95%CI 6.49 to 60.82, P<0.000 01) had a higher risk of carotid atherosclerosis. The CIMT of the OSAS group was significantly higher than that of the control group (SMD=1.24, 95%CI 0.97 to 1.51, P<0.000 01). There was a positive correlation between the apnea hypopnea index (AHI) and CIMT in OSAS patients (r=0.52, 95%CI 0.44 to 0.60, <0.000 1), and the CIMT increased with OSAS severity. Conclusion OSAS is associated with a high incidence of carotid atherosclerotic plaque that is highly correlated with CIMT, which increases with an increase in the AHI. These findings are required to be verified in prospective high-quality studies to overcome the limitations of quantity and quality of studies included in this systematic review.

  • The surgical effects of total endoscopy and right thoracic small-incision for atrial septal defect repair: A retrospective cohort study

    ObjectiveTo compare the surgical effects of total endoscopy and right thoracic small-incision for atrial septal defect repair.MethodsThe clinical data of 60 patients undergoing atrial septal defect repair in our hospital in 2019 under cardiopulmonary bypass (CPB) were collected. The patients were divided into two groups according to different surgical methods: a right thoracic small-incision group (n=31), including 11 males and 20 females, aged 44.5±11.5 years; a thoracoscopic surgery group (n=29), including 12 males and 17 females, aged 46.5±12.7 years. The clinical data were compared between the two groups.ResultsThe baseline data of the patients were not statistically different (P>0.05). The surgeries were successfully completed in the two groups of patients. The volume of chest drainage in 24 h after the surgery (59.1±43.9 mL vs. 91.0±72.9 mL, P=0.046), red blood cell input (78.0±63.9 mL vs. 121.0±88.7 mL, P=0.036), length of postoperative hospital stay (5.5±2.1 d vs. 7.2±2.1 d, P=0.003), postoperative complications rate (6.9% vs. 22.6%, P=0.029) in the thoracoscopic surgery group were significantly better than those in the right thoracic small-incision group. There was no significant difference in the CPB time, aorta blocking time, operation time, mechanical ventilation time, ICU retention time or postoperative pain score between the two groups (P>0.05).ConclusionThe two techniques are safe and effective. Patients undergoing thoracoscopic repair of atrial septal defect have small trauma, short postoperative hospital stay, mild pain, beautiful incision, and no bone damage, which is worthy of clinical promotion.
