  • Research on the Impact of Health Education on Health Knowledge Improvement and Healthy Lifestyle Establishment for Patients with Hypertension

    目的 探讨健康教育对提高高血压病患者健康知识及建立健康生活方式的影响。 方法 2011年8月-12月对社区中100例高血压病患者进行系统的健康教育,观察其健康知识和生活方式的改变情况。 结果 健康教育前后其健康知识得分由(10.48 ± 0.82)分提高到(17.10 ± 1.21)分,亚类健康责任、饮食和运动得分也明显提高,差异有统计学意义(P<0.05);健康生活方式得分由教育前的(18.65 ± 1.38)分提高到教育后的(25.91 ± 2.25)分,差异有统计学意义(P<0.05)。 结论 在社区对高血压病患者开展长期、系统的健康教育能有效提高他们的健康知识,促进其建立健康的生活方式。

  • Investigation on the Governance Model and Effect of Medical Schools Merged with Comprehensive Universities in China

    This investigation analyzes the management of medical schools merged with comprehensive universities through internet search and research review in order to reveal management model and effect of the merger. The conclusion is safely reached that governance models are divided into two different patterns: centralized management and decentralized management. Eight universities, representing the two models, were selected and evaluated comprehensively. Among them, the universities that carried out decentralized management have greater development after the merger based on a quality comparison concerning freshmen, faculty, teaching and research between the two patterns. In China, decentralized management in comprehensive universities is more beneficial to the development of medical schools

  • An Experimental Study of Restraining Bacterial Translocation of Acute Necrotizing Pancreatitis

    ObjectiveThe changes of intestinal permeability and relationship of intestinal mucosa and bacterial translocation were studied in rat acute necrotizing pancreatitis (ANP) models.MethodsThe ANP models were made by injection of 5% sodium taurocholate 1.0 ml into pancreatic subcapsula.Then wistar rats were divided into four groups,control group (n=20),ANP group(n=22),treatment model group fed with lactose (n=22) and treatment model group fed with MgSO4 and antibiotic (n=22).After 72 hours,the experimental models were sacrificed.Tissues of pancreas,mesenteric lymph node, ascites were collected for microbiological study.The intestinal permeability was observed by lanthanum tracer.The blood samples were obtained from portal vein and ascites in order to assay the amount of amylase in serum.The pathologic lesions were found in the intestinal villus of the model group, including acute necrosis of intestinal mucosa,necrotichaemorrhage as well as enteroparalysis and a mass of haemorrhagic ascites.ResultsBacterial translocation of model group were markedly elevated than that of control (P<0.05).There were statistically significant differences in bacterial translocation among three model groups (P<0.05).The pathologic lesions were found in the intestinal villus of the model group,including acute necrosis of intestinal mucosa,necrotichaemorrhage as well as enteroparalysis and a mass of haemorrhagic ascites.The lanthanum grain in clearance of intestinal cell of model group can be observed by eletron microscope.ConclusionThere is a severe gut barrier damage and injury in the intestinal mucosa,which lead to bacterial translocation from intestine as the source of pancreatic infection.Cleaning out enteric bacteria,improving intestinal movement and feeding with lactose could decrease bacterial translocation to treat and prevent acute necrotizing pancreatitis.

  • Identification of the influencing factors of admission priority decision in department of respiration in West China Hospital based on logistic regression

    ObjectivesBased on the historical data of inpatients, a logistic regression model was established. It aimed to identify the influencing factors of patient's admission scheduling decisions and compare them with the actual scheduling rules, so as to discover the differences and deficiencies.MethodsWe extracted data of outpatients and inpatients in Department of Respiration in West China Hospital of Sichuan University from January 1st, 2016 to December 31st, 2016, and standardized the original dataset. We established the binary multivariate logistic regression model through R software and ‘glm’ package.ResultsThe analysis of multi-factor logistic regression showed that the effect of the five variables (type of medical insurance, time of registration, waiting time, type of disease and admission priority) on patient schedule was statistically significant.ConclusionsThe logistic regression model constructed in this study has a good effect on patient planning, which is helpful to provide decision support for admission schedule through identification factors.

  • Medical consumable access management practice based on evidence-based principles

    As a methodological tool, evidence-based management can assess the technological advantages and economical value of medical consumables scientifically, and select the most cost-effective products into the hospital access directory with guarantees of the development of clinical disciplines. Now this method has been widely used for consumable access management in our hospital. Policies and processes have been established to normalize the assess, allocation, and usage, and the management mode is transformed from experience-based management into evidence-based management. With clinical nominalism being changed, the method of evidence-based management can be used for every link of comsumble management, to reduce the costs and ensure the safety and quality of medical consumables at the same time, so as to promote the rational cost of medical consumables.

  • Research progress of minimally invasive techniques in the management of sacrococcygeal pilonidal disease

    ObjectiveTo summarize the clinical progress of minimally invasive techniques in treatment of pilonidal disease.MethodThe relevant literatures about minimally invasive techniques in the treatment of pilonidal disease in recent years were reviewed.ResultsAbout hair removal and carbolic acid injection, they had less damage, fewer complications, higher recurrence rate compared to other surgery, and did not affect the second treatment. The fibrin injection could not clarify its role in the treatment of pilonidal disease due to some defects in the clinical design. Bascom Ⅰhad the advantages of fast recovery and no need for hospitalization, with a recurrence rate of 8%–16%. According to the results of some current clinical researches, it was a promising operation. Sinus resection required further clinical evaluation due to the limited results of current researches. A clinical study of more than 1 000 cases over 10 years showed that the recurrence rate in 10 years was 16%. Compared with frequently used pilonidal operations, the trephine technique was associated with a lower recurrence rate and a lower incidence of postoperative complication. Some short-term clinical researches showed that endoscopic pilonidal sinus treatment was a safe, minimally invasive, and less complication treatment.ConclusionsCompared with frequently used pilonidal operations, minimally invasive technique has the advantages of shortening the hospital stay, shortening the healing time, and reducing complications. It is worth of application.

  • Clinical Analysis of Sixty-Two Cases of Diffuse Parenchymal Lung Disease

    Objective To analyze the clinical symptoms, imaging, laboratory tests, efficacy and other indicators of patients diagnosed as diffuse parenchymal lung disease ( DPLD) , in order to provide a reference for differential diagnosis of secondary DPLD and idiopathic interstitial pneumonia ( IIP) .Methods Sixtytwo patients diagnosed as DPLD were retrospectively analyzed. Results In 62 patients with DPLD, 19 patients ( 30. 6% ) were diagnosed as secondary DPLD, 42 cases ( 67. 7% ) as IIP, 1 case ( 1. 6% ) as Langerhans cell histiocytosis. The smoking rate of the DPLD patient was 33. 8% , which was higher than general population ( 29. 7% ) . 94. 7% of the secondary DPLD patients had cough, which was higher than the IIP patients ( 71. 4%) . The average age of onset of the secondary DPLD and IIP was ( 45. 9 ±16. 8) years and ( 60. 5 ±7. 7) years respectively, without significant difference ( P gt; 0. 05) . Etiological factors of secondary DPLD were dust, pets, drugs, pesticides, decoration material, etc. The secondary DPLD patients had higher response rate to steroid therapy, but had no statistical difference compared with the IIP patients ( 46. 2% vs. 37. 5% , P gt;0. 05) . Conclusions As a group of diseases of known causes, history taking is very important for DPLD diagnosis and differential diagnosis. Clinical symptoms, imaging, and laboratory tests may provide reference for differential diagnosis of secondary DPLD and IIP.

  • A review of studies on priority evaluation of patient admission

    Patient priority evaluation has been studied and applied abroad for a long time, which is a mature theory and widely used in practice now. This article uses the priority, patients, waiting list and criteria as keywords to search Wiley Inter Science, Web of Science, Scopus Pub Med, The Cochrane Library, Science Direct, Springer, and Jstor database (searching time is up to December 2017), to collect relevant indicators for patient admission priority evaluation. In addition, relevant citations and grey literature were searched, and experts from relevant fields in China were consulted to obtain more comprehensive research literature. On this basis, this article describes the concept of patient admission priority evaluation, and describes the meanings of the indicators and the countries of application from the three dimensions of clinical indicators, expected results, and social factors. It is considered that the research and implementation of the evaluation of the priority of patient admission has been relatively many. However, there are only a few related researches in the country and without unity. There is no systematic patient-related priority evaluation. It is necessary to use foreign mature theory research to establish a hospital admission priority evaluation system suitable for China’s national conditions.

  • EXPERIMENTAL STUDY ON EFFECT OF MONOCYTE CHEMOATTRACTANT PROTEIN 1 ON MIGRATION OF INDUCED AND DIFFERENTIATED MOUSE BONE MARROW MESENCHYMAL STEM CELLS IN VITRO

    Objective To investigate the effect of monocyte chemoattractant protein 1 (MCP-1) on the migration of the induced and differentiated mouse bone marrow mesenchymal stem cells (BMSCs) for raising the efficacy of intravenous transplantation of BMSCs. Methods The BMSCs were cultured with the method of differential adhesion and density gradient centrifugation of C57/BL10 mice, and were identified by alkal ine phosphatase Gomori modified staining after osteogenic inducing. At the 3rd passage, the BMSCs were induced to the myoblasts with 5-azacytidine (5-Aza). The chemotaxis of MCP-1 in the induced and differentiated BMSCs in vitro at concentrations of 25, 50, 100, 200, and 400 ng/mL was observed through the migration test, by counting the number of the migrated cells. The expression of the chemokine receptor 2 (CKR-2) in the induced and differentiated BMSCs was detected with the flow cytometry. Results The cells could be cultured with the methods of differential adhesion and density gradient centrifugation and still had higher prol iferative and differentiative potency; the induced cells at the 3rd passage could differenciate to the osteoblasts, confirming that the cells were BMSCs; the myogenic induced BMSCs possesed the sarcotubule structure. The number of the migrating BMSCs at MCP-1 concentrations of 25-400 ng/ mL were respectively 35.066 7 ± 6.584 2, 43.200 0 ± 6.460 8, 44.466 7 ± 4.823 5, 45.600 0 ± 8.650 3, and 50.733 3 ± 7.582 5; showing significant difference when compared with control group (28.333 3 ± 8.917 6, P lt; 0.05), and presenting significant difference among 25, 50, 400 ng/mL groups compared with each other (P lt; 0.05). The expression of CKR-2 in the mouse BMSCs (48.0%) was significantly higher (P lt; 0.001) than those of blank control (0.6%) and negative control (17.0%). Conclusion The results indicate that the MCP-1 can induce the migration of mouse BMSCs by MCP-1/CKR-2 pathway.

  • Salpingectomy before IVF-ET for Hydrosalpinx among Chinese Women: A Systematic Review

    Objective To assess the necessity and effectiveness of salpingectomy before IVF-ET for hydrosalpinx among Chinese infertile patients. Methods The relevant papers published from 2000 to December of 2010 in China were electronically searched in CBM, VIP and CNKI to collect randomized controlled trials (RCTs) involving Chinese hydrosalpinx women with or without salpingectomy before IVF-ET. The quality of the included trials was independently assessed by two reviewers, and the data were extracted and analyzed by RevMan 5.0 software. Results Nine RCTs involving 687 patients and 730 IVF-ET cycles were identified. The results of meta-analyses showed that: a) There were significant differences between the two groups in Gn dosage (WMD=1.23, 95%CI 0.17 to 2.30, P=0.02), fertilization rate (RR=1.07, 95%CI 1.02 to 1.13, P=0.006), cleavage rate (RR=1.05, 95%CI 1.00 to 1.09, P=0.03), clinical pregnancy rate (RR=1.92, 95%CI 1.41 to 2.61, Plt;0.000 1), and abortion rate (RR=0.34, 95%CI 0.13 to 0.86, P=0.002); and b) There were no significant differences between the two groups in days of Gn (WMD= –0.27, 95%CI –0.59 to 0.06, P=0.11), E2 in HCG day (WMD=59.15, 95%CI –9.61 to 127.91, P=0.09), number of eggs (WMD= –0.27, 95%CI –0.44 to 0.99, P=0.46), quality embryonic rate (RR=1.02, 95%CI 0.91 to 1.14, P=0.79), and ectopic pregnancy rate (RR=0.22, 95%CI 0.03 to 1.82, P=0.16). Conclusion The current evidence shows that salpingectomy before IVF-ET for hydrosalpinx Chinese hydrosalpinx patients is necessary and effective. For the low quality of methodology of the included studies, more reasonably-designed and double-blind RCTs with large sample are required to provide more high-quality proof.
