  • Clinical Efficacy of CoughAssist for Cleaning Airway Secretions in Neuromuscular Disease Patients with Respiratory Insufficiency

    ObjectiveTo investigate the clinical efficacy of CoughAssist for cleaning airway secretions in neuromuscular disease patients with respiratory insufficiency. MethodsForty-six cases of neuromuscular disease with respiratory insufficiency were recruited in the study,with Guillain-Barre syndrome in 24 cases,myasthenia gravis in 18 cases,and multiple myositis in 4 patients.Thirty-four patients underwent intubation and mechanical ventilation,and 12 patients underwent tracheotomy.They were randomly divided into group A using CoughAssist and group B using suction tube to clear airway secretions after mechanical vibration.The frequency of suction within 24 hours,oxygenation index,pulmonary static compliance,incidence of lung infections,lung auscultation and chest radiograph were recorded and compared between two groups. ResultsCoughAssist could more effectively clean respiratory secretions with higher oxygenation index and pulmonary static compliance in group A.Lung auscultation and chest radiograph significantly improved,and the incidence of lung infection significantly decreased in group A compared with group B.Furthermore,CoughAssist reduced nursing workload with lower frequency of suction within 24 hours. ConclusionCoughAssist can effectively clean up airway secretions,improve oxygenation,while reducing pulmonary infection and nursing workload for neuromuscular disease patients with respiratory insufficiency,so it is aworthy tool in clincal practice.

  • Research Progress of Esophageal Cancer Stem Cells

    Esophageal carcinoma is a kind of common malignant tumor in the digestive tract. Although a lot of basic researches and clinical trials have been carried out all over the world, neither the diagnostic level nor the therapeutic effects have been obviously improved. The 5-year survival rate of esophageal cancer patient is still lower than that of other malignant tumors. Up to now, some frontier researches consider that the reason of the esophageal carcinoma being difficult to cure is related to the stem cells in it. Elimination or suppression of these stem cells may bring new hope for the treatment of esophageal cancer. This article generally introduces the specific markers, separation and indentification methods about the esophageal cancer stem cell. The targeted therapy is also mentioned.

  • Gonadotropin Releasing Hormone Pump in the Treatment of Hypogonadotropic Hypogonadism: A Report of One Case and the Literature Review

    ObjectiveTo research on the clinical efficacy of gonadotropin releasing hormone (GnRH) pump in the treatment of hypogonadotropic hypogonadism (HH) by using micropulse pump subcutaneous pump Gonadorelin to treat patients with HH. MethodsThrough the report of one case of GnRH pump treatment patient and related literature review, we studied the effect of GnRH pump therapy and the traditional treatment on HH patients' sex hormone level, secondary sexual characteristics, spermatogenesis and pregnancy rate. ResultsThrough treatment, GnRH pump effectively improved the patient's sex hormone level, secondary sexual characteristics development and sperm production ability, and eventually the patient attained the goal of fertility. The patient had a change in his voice tone; Beard and armpit hair grew out; The patient had a good mood with sexual drive; luteotropic hormone, follicle-stimulating hormone and testosterone became normal. The patient's sperm density was 20×109/L, and the average sperm motility was higher than 30%. ConclusionPulse pump GnRH conforms to the physiological rhythm of GnRH secretion with a good clinical efficacy and tolerance. So we recommend GnRH pump as the preferential choice for treatment of hypogonadotropic hypogonadism.
