  • Relationship between Fatigue and Quality of Life in Patients with Obstructive Sleep Apnea

    ObjectiveTo assess the fatigue in patients with obstructive sleep apnea hypopnea syndrome (OSAHS), and analyze the factors caused fatigue and the relationship between quality of life (QOL) and fatigue. MethodsOne hundred and sixty-nine patients with OSAHS and 78 subjects without OSAHS diagnosed by polysomnography (PSG) between December 2010 and March 2011 in West China Hospital were recruited in the study. Fatigue was assessed by using multidimensional fatigue inventory (MFI), excessive daytime sleepiness by Epworth sleepiness scale(ESS), QOL by functional outcomes of sleep questionnaire (FOSQ). ResultsFatigue in the patients with OSAHS was more severe than that of the controls (51.06±13.39 vs. 44.82±9.81, P < 0.001), but no difference was revealed in the patients with different degree of OSAHS. Fatigue was positively correlated with ESS score(r=0.210), total sleep time intervals(r=0.156), and the ratio of time of SpO2 below 90% in total sleep time(r=0.153)(P < 0.05), and was negatively correlated with the average oxygen saturation(r=-0.171, P < 0.05) and all subscales of FOSQ(P < 0.01). ConclusionsFatigue in patients with OSAHS is more severe than that of controls. Fatigue can significantly reduce QOL, and the impact is greater than that of excessive daytime sleepiness.

  • The Value of Three Brief Scales to Assess the Severity of Acute Exacerbation in Patients with COPD Complicated by Hypercapnic Respiratory Failure

    ObjectiveTo explore the value of three brief scales (BAP-65 class, DECAF score, and CAPS) on assessing the severity of acute exacerbation in patients with chronic obstructive pulmonary disease(COPD) complicated by hypercapnic respiratory failure. MethodsTwo hundred and forty-four cases with acute exacerbation of COPD complicated by hypercapnic respiratory failure, admitted in West China Hospital from August 2012 to December 2013, were analyzed retrospectively.The scores of each scale were calculated.The areas under the receiver operating characteristic curves (AUROC) of each scale for hospital mortality, mechanical ventilation use, mortality of patients requiring mechanical ventilation, invasive mechanical use were analyzed and compared. ResultsThe AUROCs of BAP-65 class, DECAF score and CAPS for hospital mortality were 0.731, 0.765, and 0.711; for mechanical ventilation were 0.638, 0.702, and 0.617; for mortality of patients requiring mechanical ventilation were 0.672, 0.707, and 0.677; for invasive mechanical ventilation use were 0.745, 0.732, and 0.627(BAP-65 vs.CAPS, P < 0.05).Mortality and mechanical ventilation use increased as the three scales escalated.In the patients whose BAP-65 or DECAF score were more than 4 points, the hospital mortality was nearly 50%, and about 95% of the patients underwent mechanical ventilation. ConclusionsThe BAP-65 class, DECAF score, and CAPS of patients on admission have predictive values on assessing the severity of acute exacerbation in patients with COPD complicated by hypercapnic respiratory failure, especially the simple and practical BAP-65 class and DECAF score.

  • The Relevance of the Ratio of Pulmonary Arterial Diameter to Aortic Diameter Exceeding One with Brain Natriuretic Peptide and Inflammatory Factors in Patients with Acute Exacerbation of COPD

    ObjectiveTo explore the relevance of the ratio of pulmonary arterial diameter to aortic diameter exceeding one (PA:A>1) with brain natriuretic peptide (BNP) and inflammatory factor levels in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD). MethodsFrom August 2013 to December 2013,95 inpatients with AECOPD in West China Hospital were divided into two groups according to the ratio of pulmonary arterial diameter to aortic diameter. The clinical data of the patients were collected. Meanwhile,arterial blood gas,plasma levels of BNP,C-reactive protein (CRP),and interleukin-6 (IL-6) within 24 hours were measured. ResultsThe plasma BNP level was 2005(483-4582)ng/L in the group with PA:A>1,and 404(137-1224)ng/L in the group with PA:A<1. There was significant difference in plasma BNP level between two groups (P<0.01). There was no significant difference in CRP or IL-6 level between two groups (P>0.05). ConclusionThe ratio of pulmonary arterial diameter to aortic diameter is correlated with BNP level in patients with AECOPD,but is not correlated with CRP or IL-6.

  • Effect of Antibiotic Treatment on Ventilator-Associated Tracheobronchitis: A Meta-analysis

    Objective To evaluate systematically the effect of antibiotic treatment on ventilator-associated tracheobronchitis (VAT). Methods Pubmed,Web of Science,OVID SP (ACP Journal Club,Cochrane Central Register of Contralled Trials,Embase,Medline),as well as China National Knowledge Infrastructure,China Science and Technology Journal Database,and Wanfang Data were searched for literatures about antibiotic treatment on VAT.The search deadline was March 2016.Meta-analysis was conducted with RevMan 5.3 software. Results A total of 6 studies with 769 patients were included.Among 769 patients,432 patients were treated by antibiotics,and 337 patients in control group were treated without antibiotics.Meta-analysis showed antibiotics treatment significantly reduced incidence of ventilator-associated pneumonia compared with control group [OR=0.27,95%CI (0.17,0.43),P<0.05],and shortened length of ICU stay [MD -1.51,95%CI(-2.04,-0.98),P<0.00001] .There were no significant difference in duration of mechanical ventilation [MD -2.52,95%CI (-6.85,1.81),P=0.25],mortality [OR=0.41,95%CI(0.15,1.14),P=0.09],or drug-resistant bacteria production [OR=0.62,95%CI(0.17,2.19),P>0.05]. Conclusions Antibiotic treatment can reduce incidence of ventilator-associated pneumonia in patients with VAT.Further more high quality randomized controlled trials are needed to assess the effect of antibiotic treatment on VAT.

  • A Study on Epidemiology and Risk Factors of Tuberculosis in Mianyang City of Sichuan Province

    ObjectiveTo investigate the epidemiology of tuberculosis in Mianyang city, and investigate its risk factors to provide scientific evidence for tuberculosis prevention and management. MethodsA stratified cluster random sampling method was applied and 184094 residents were recruited in the study. The prevalence of tuberculosis was analyzed by questionnaires, physical examinations, X-ray, and laboratory tests. The risk factors of tuberculosis were analyzed by binary logistic regression method. ResultsOne hundred and seventy-five subjects were diagnosed with active tuberculosis, and the incidence of tuberculosis was 95.06 per thousand. There were 32 smear-positive pulmonary tuberculosis patients, and the incidence was 17.38/100 per thousand. Males, farmers and workers, diabetes, family history of tuberculosis, smoking, drinking were risk factors for tuberculosis. ConclusionsThe prevalence of tuberculosis in Mianyang city is similar with the whole country. Male, farmers and workers, diabetes, family history of tuberculosis, smoking, and drinking maybe the risk factors of tuberculosis.

  • A Study on Tuberculosis Characteristics and Risk Factors of Case-finding Methods for Tuberculosis in Mianyang City

    ObjectiveTo understand basic characteristics and clinical features of tuberculosis in Mianyang, Sichuan, and to investigate the risk factors of two case-finding methods for tuberculosis:"passive finding" and "active screening". MethodsOne-hundred and seventy-five active tuberculosis patients screened by physical examination in 21 villages of Fucheng and Jiangyou from June 2013 to March 2014 were included. Information about basic characteristics and clinical features of tuberculosis was collected using questionnaire, chest X-ray and laboratory examinations. We also applied binary logistic regression to explore the risk factors of finding for tuberculosis. ResultsIn all 175 tuberculosis patients, 41.71% were over 60 years old and 73.71% were farmers. Productive cough over two weeks was the most common symptom, accountting for 58.29%. 19.63% were sputum smear-positive while 70.87% were PPD-positive. Binary logistic regression analysis demonstrated that tuberculosis history significantly influenced the findings for tuberculosis (OR=15.809,95%CI: 6.708~37.258). ConclusionsActive tuberculosis patients screened by physical examination in Mianyang, Sichuan were mainly elderly male over 60 years old and farmers, with single pulmonary symptom as the primary clinical feature. The patients with tuberculosis history may be found by "passive finding".
