    It is essential to improve the practice of community healthcare service for the resolution of the problem of inadequate and overly expensive medical services, to promote the harmonization of doctor-patient relationship.   • Chest Drainage Management after Pulmonary Lobectomy

    Proper management of chest drainage after pulmonary lobectomy is a topic that every thoracic surgeon must face up to. Reasonable chest drainage plays a critical role in postoperative normal physiological recovery. However, there are still controversies and discrepancies in many aspects of chest drainage management after pulmonary lobectomy. In this review,we focus on five aspects of chest drainage management after pulmonary lobectomy,including the choice of chest drainage system,single or double chest tubes,suction or not,treatment of persistent air leak,and removal of chest tube.

  • The Diagnosis and Management of Traumatic Tracheobronchial Ruptures

    Abstract: Objective To investigate the methods of diagnosis and treatment for early and late cheobronchial rupture, in order to improve the understanding of this disease. Methods We retrospectively analyzed the records of 19 patients treated for traumatic tracheobronchial rupture in our hospital between October 1988 and August 2010. There were 9 males and 10 females with a median age of 28 years (ranged from 8 to 48 years). We analyzed the clinical characteristics of the disease, including clinical presentation, and the results of chest Xrays, computed tomography(CT), and fibrobronchoscopy. There were 2 cases of tracheal repair, 1 case of tracheostomy, 8 cases of bronchial reconstruction, 7 cases of bronchial repair, and 1 case of pneumonectomy. Results Seventeen patients were cured by operation and 2 patients died of multiple organ failure. Blood oxygen saturation resumed normal in most patients after operation (9680%±159% vs. 8840%±390%,Plt;0.01). Postoperative followup time was ranged from 3 to 24 months, and no tracheobronchial stenosis was observed and no patients needed stent or dilatation for treatment. Conclusion The diagnosis of tracheobronchial rupture depends on history of trauma, chest Xray, CT and fibrobronchoscopy results. Surgical treatments should be based on the tracheobronchial reconstruction or repair, and the clinical outcome is satisfying.

  • A Study on the Minimized Randomization Network System in Multi-Center Clinical Trials

    In order to improve the efficiency and guarantee the quality of the research of cross-regional multi-center clinical trials, with the combination of WIN 2003 Server IIS, SQL Server 2005 database and ASP.Net programming techniques, the Pocock and Simon minimized randomization system model facing the network was put forward, and this system based on that model was developed. The two sides in this system can achieve cooperation process from screening subjects, random allocation and drug management to filling electronic case report form. Moreover, the customers of this system can also publish official documents, exchange information and take full use of other available assistant functions through the office automation platform.

  • A Study on Safety Culture Management of Patients Taking Radioexamination

    As the development of medical imageology, radioexamination has become one of the main approaches of disease diagnosis. However, the society and hospital have not taken the hidden danger and harmfulness of radiation seriously. Through discussing the following eight aspects of radiation safety culture management, this paper aims to reduce the risk of radiation and ensure the safety of patients and medical staff: a) Improving the awareness of safety culture and the understanding of patients on safety culture; b) Consummating the safety management system of the radiation; c) Attaching importance to implementing the relevant laws and regulations of radiation; d) Mastering the examination indications, and especially the contraindications of radiation; e) Strengthening the clinical cooperation and exchange; f) Improving the staff’s ability to distinguish hidden danger and identify patients in high risk; g) Strengthening the nursing behavior safety management of the radiation department; and h) Strengthening the biological security management of the radiation department.

  • International Comparative Analyses of Incidents Reporting Systems for Healthcare Risk Management△

    Objective To compare administration of incidence reporting systems for healthcare risk management in the United Kingdom, the United States, Canada, Australia, and Taiwan, and to provide evidence and recommendations for healthcare risk management policy in China. Methods We searched the official websites of the healthcare risk management agencies of the four countries and one district for laws, regulatory documents, research reports, reviews, and evaluation forms concerned with healthcare risk management and assessment. Descriptive comparative analysis was performed on relevant documents. Results (1) A total of 142 documents were included in this study. The United States had the most relevant documents (68). (2) The type of incidents from reporting systems has expanded from medication errors and hospital-acquired infections to near-misses, and now includes all patient safety incidents. (3) The incidence-reporting systems can be grouped into two models: government-led and legal/regulatory/NGO-collaborative. (4) In two cases, reporting systems were established for specific incident types: One for death or serious injury events (the sentinel events database in Britain, SIRL), and one for healthcare-associated infections (NHSN in America). (5) Compared to the four countries, Taiwan’s system put more emphasis on public welfare, confidentiality, and information sharing. The contents of reporting there covered every aspect of risk management to create a more secure environment. Conclusion (1) Britain’s national reporting and learning system was representative of a government-led model; (2) The United States was the earliest country to have a reporting system, which included a limited range of incident types. Management of incidents became more reliable with increased application of laws, regulations, and guidances; (3) Both the Canadian and the Australian systems drew from the American experience and are still developing; (4) The Taiwanese system was comprehensive and is an instructional case.

  • Management and Nursing Pattern for Ambulatory Surgery Ward

    Ambulatory surgery ward is a fast and effective way of treating, nursing and patients’ education. We report the practice of ambulatory surgery ward in West China Hospital of Sichuan University about patients’ reception, treatment and nursing, medical safety, health education and post-operative follow-up. Results show that ambulatory surgery ward would guarantee patients’ safety, shorten hospitalization days and decrease medical cost, with more than 98% of patients’ satisfaction.

  • Investigation on the Current Status of Management and Operation in Public Hospital Pharmacies

    Objective To investigate the current status of management and operation in the public hospital pharmacies, and to provide the evidence and suggestions for improving the performance of the public hospital pharmacies. Method According to the principles and methods of business diagnosis, we designed the questionnaire to investigate the ideas of management and operation among 306 managers and pharmacists working in 74 public hospital pharmacies. We used percentage and proportion for statistical description. Result (1) Over 70% participants understood the strategic positioning, brand and development of hospital pharmacy. They had very b senses of innovation and risk awareness, and the comprehensive understanding to the risk and competitive factors. (2) Over 60% considered that the public pharmacies lacked in the awareness of the market competition and crisis, clear management plans as well as the active adaptation to market changes. They were also short of professional dedication and innovation capacity. (3) 52% thought that there was promising future of the public hospital pharmacies. Conclusion The public hospital pharmacies urgently needs the improvement and innovation of the management idea and models.

  • Emergency Management and Security of First-line Medical Supplies in the 3A Hospital During the Wenchuan Earthquake——May 12-June 30,2008

    Objective We analyzed the emergency management of medical supplies of People’s Hospital of Deyang City in the first week after the Wenchuan earthquake o provide first-hand evidence concerning the management of medical supplies in a public emergency. Methods Retrospective statistical analysis was used to analyze the surgery related demand in the first week after earthquake, including the demand, the supply, donations and the inventory of the pre-3 demand. Results The demand during the first day after the earthquake was the largest, But some of the supplies were not enough because of the lack of supply in the first 1-2 days. During the first 3-4 days after the earthquake, emergency procurement and supplies donated just met the supply gap. One week after the earthquake because of a lag inthe supply chain and a reduction in surgeries, there was an excess of some supplies. Conclusions China should accelerate establish a medical consumables supply and demand instant information feedback system, as well as building a public emergency material platform and a guide for society to promply donate materials.
