  • Anti-hypertensive Medication Analysis in A Medical Examination Center

ObjectiveTo investigate the use of anti-hypertensive drugs among hypertension patients in examination medical center. MethodThis was a cross-sectional survey. Data of 3 000 cases of medical examination center health records from December 2012 to December 2013 were sampled, and 1 310 patients with hypertension were screened for analysis of medication. ResultsIn the 1 310 hypertension patients, 959 used antihypertensive drugs before, among whom 423 (44.11%) accepted only one anti-hypertensive drug and 536 (55.89%) accepted compound anti-hypertensive drugs or combined medicine. In patients of the previous kind, calcium channel blocker classes of antihypertensive drugs were used the most (n=244, 25.44%). Among the second kind of patients, 359 (37.43%) were treated with a two-compound drug or a combination of two drugs, and most of them used the combination of calcium channel blocker with angiotensin receptor blocker or calcium channel blocker with angiotensin converting enzyme inhibitor. ConclusionsThe current using of anti-hypertensive drug is in accordance with the recommendation of anti-hypertensive treatment guidelines in some way, but there are still some gaps between real clinical practice and guidelines.

  • Improvement of the Use Value of Medical Equipment in the Health Management Center Based on PDCA Cycle

ObjectiveTo strengthen the management of medical equipment and improve the use value of medical equipment by PDCA cycle. MethodsBetween March and June 2014, we introduced PDCA cycle into the management and use of newly-bought equipment. The use of newly-bought arteriosclerosis detector, TCD and ultrasound bone densitometer between March and June 2014 was regarded as the control group (before the implementation of PDCA cycle), and the use of these three kinds of equipment between March and June of 2015(after the implementation of PDCA cycle) was seen as the observation group.By analyzing the status quo through collected information, we drew a fishbone diagram to find out the factors that could influence the use efficiency of medical equipment. Then, corresponding measures were taken for continuous improvement, including adopting revised regulations, training technicians, quantifying work indexes, informatization, changing service mode, and examining and assessing the effect of implementation. Finally, feedback was summarized for the next PDCA cycle. ResultsAfter the implementation of PDCA cycle, the daily check numbers of hospital medical equipment including arteriosclerosis detector, TCD, and ultrasound bone densitometer all improved significantly (P < 0.05).The daily check numbers of those three kinds of equipment after the implementation of PDCA cycle were respectively 48.52±19.72, 32.14±10.14 and 55.59±25.12, while the numbers before were respectively 32.46±20.69, 15.46±10.05 and 29.83±20.20.Moreover, working days, maximum working hours daily, and total working time were also increased after the implementation of PDCA cycle. ConclusionThe application of PDCA cycle can effectively improve the use value of medical equipment, and improve the social benefit and economic benefit.

  • Construction of medical examination control mechanism

With the increasing application of medical examination in clinical diagnosis and treatment, the contradiction between the diversified demands of medical examination and the shortage of resources has gradually become prominent, and it is extremely urgent to establish the control mechanism of medical examination. This paper summarizes the present situation of medical examination and its control mechanism, sorts out the basic conditions for establishing a medical examination control mechanism from the aspect of establishing the medical examination standards of rationality, perfecting the supervision system and promoting the reform of supporting systems, and puts forward main obstacles to establishing a medical examination control mechanism. It is expected to provide a reference policy basis for the establishment of the medical examination control mechanism, improving the rational use of medical resources, and promoting the development of medical examination.

  • Evaluation criteria for rationality of medical examination

The purpose of this paper is to discuss the evaluation standard of rationality of medical examination from different perspectives, so as to provide theoretical evidence for medical department to establish the evaluation standard of the rationality of medical examination. By researching the relevant literature in the field of rational medical examination and combing the existing research results, this paper discusses the evaluation criteria of the rationality of medical examination from four dimensions: technology, ethics, law, and health economy. The following four suggestions are proposed for the current status of medical examination: construct clinical pathway management to avoid excessive medical examination; establish the internal supervision and evaluation mechanism to improve the professional quality of medical staff; improve medical examination-related policies, laws, and regulations, carry out specialized legislation; apply diagnosis-related group payment method, control the cost of medical examination.

  • Research on the dilemma and breakthrough path of medical examination control

After the reform of drugs and medical consumables, the problem of unreasonable medical examination has gradually become prominent, which has become the potential threat of doctor-patient disputes and rising medical costs. At present, the reform of the medical and health system has entered the deep water zone. In the process of standardized management of medical examination, the difficulties in doctor-patient trust, medical examination supervision, medical examination resource allocation, the implementation of the mutual recognition system of medical results and the guarantee of information security have gradually emerged. In view of the dilemma of medical examination control, this paper proposes the following three breakthrough paths: based on the perspective of doctors and patients, improve doctor-patient trust; coordinating institutional supervision and administrative supervision to create transparent medical examinations; technical support to promote the standardized operation of medical examination, in order to provide a theoretical basis for the standardized operation of medical examination.

  • Analysis of current situation and key elements of medical examination control mechanism

With the continuous promotion of China’s medical and health system reform, the problem of unreasonable medical examination has gradually become prominent after the elimination of “raising healthcare by medicine” and “raising healthcare by medical device”, which has become the core of aggravating the economic burden of patients’ diseases. Based on the current situation of medical examination control mechanism in China, this study explores the medical examination control mechanism including institutional restraint mechanism, supervision and management mechanism, quality control mechanism from external regulation and internal control, so as to regulate the medical examination behavior, realize the normalization of supervision and management, promote the continuous improvement of medical examination quality, in order to enhance the rationality of medical examination. It is expected to provide a theoretical basis for health departments to explore and perfect the control mechanism of the medical examination.
