  • Postoperative Pain in Patients with Da Vinci Surgical System versus Video-assisted Mini-thoractomy: A Case Controlled Study

    ObjectiveTo compare the effect of da Vinci Surgical System and video-assisted mini-thoractomy (VAMT) on postoperative pain of patients with lungs or mediastinal tumor. MethodsWe retrospectively analyzed the clinical data of 88 patients with lung or mediastinal tumor who underwent surgical treatment in our hospital from January 2015 through April 2015. The patients were divided into two groups including a robot group and a VAMT group. There were 49 patients in the robot group with 23 males and 26 females at age of 55.14±13.03 years and 39 patients with 23 males and 16 females at age of 56.92±8.98 years in the VAMT group. ResultsCompared with the VAMT group, shorter operation time (t=-2.298, P=0.024) and shorter time of drainage (t=-2.421, P=0.018) were found in the robot group with statistical differences. There was a statistical difference in visual analogue scale (VAS) scores of postoperative 24 hours between the robot group and the VAMT group (1.00±0.74 vs. 2.33±1.64, t=-4.704, P=0.000). While no statistical difference was found in VAS scores of postoperative 48 hours (t=-0.244, P=0.808) between the two groups. ConclusionCompared with VAMT, da Vinci Surgical System can be used in the treatment of lung and mediastinal tumor with shorter operation time, shorter time of drainage, less pain, and less invasiveness.

  • Application of Endoscopic Surgery Combined with Intraoperative Color Doppler Ultrasound in Removal of Injected Breast Augmentation Agents (Report of 16 Cases)

    Objective To evaluate the effect of endoscopic surgery combined with intraoperative color Doppler ultrasound on removing the injected breast augmentation agents and share our experiences. Methods Sixteen female who accepted the bilateral removal of injected breast augmentation agents through endoscopic surgery combined with intraoperative color Doppler ultrasound between 2008 and 2010 were enrolled in this study. The results, techniques, and advantages of management were analyzed retrospectively. Results One incision was made in 18 breasts, 2 in 4 breasts, 3 in 10 breasts. The length of incision was 0.5 to 1 cm. The mean operative time was 128.70 min per person. The average amount of bleeding was 52.67 ml per person. Complications such as postoperative bleeding, infection, poor drainage, or breast augmentation agents remain did not happened in all cases. No case was turned into normal operation. Female who accepted this operation were all satisfied with the appearance of incisions. During 1-3 months follow up, neither clinically palpable mass nor sensory disturbance in nipple or areola of breast was observed. Color Doppler ultrasound or magnetic resonance showed 16 cases had been cleared free of breast augmentation agents. Conclusion With the advantages of beauty, safe, minimal invasion, and partial resection of lesions at the same time, endoscopic surgery combined with intraoperative color Doppler ultrasound was an effective approach in the removal of injected breast augmentation agents.

  • Clinical Application of Precise Hepatectomy Techniques in Hepatolithus

    ObjectiveTo explore the curative effect of precise hepatectomy techniques in hepatolithus. MethodsTotally 132 patients underwent precise hepatectomy and 52 patients underwent irregular hepatectomy were retrospectively analyzed, and the intraoperative and postoperative indexes such as operation time, blood loss, postoperative complications, hospitalization time, clearance rate of calculus, and cost of hospitalization were analyzed. ResultsCompared with the patients in irregular hepatectomy group, although the operative time was longer in precise hepatectomy group 〔(364.6±57.8) min vs. (292.9±44.7) min, Plt;0.001〕, but the patients in precise hepatectomy group had less blood loss 〔(558.3±90.6) ml vs. (726.7±88.7) ml, Plt;0.001〕, less postoperative complications (11.4% vs. 23.1%,P=0.004 3), and higher clearance rate of calculus (89.4% vs. 73.1%, P=0.005 5). Thus, the patients in precise hepatectomy group had shorter hospital stay 〔(22.9±4.4) d vs. (28.8±3.5) d, Plt;0.001〕 and less cost of hospitalization 〔(1.8±0.7)×104 yuan vs. (2.1±0.9)×104 yuan, P=0.016 5〕. Conclusion Precise hepatectomy is better than irregular hepatectomy in treatment for hepatolithus.

  • Treatment of Unresectable Late Pancreatic Cancer with Laparoscopic Choledochojejunostomy and Gastric Bypass (Report of 15 Cases)

    ObjectiveTo assess the feasibility of the treatment of unresectable late pancreatic cancer with laparoscopic choledochojejunostomy and gastric bypass. MethodsFrom June 2000 to December 2003, laparoscopic choledochojejunostomy and gastric bypass were successfully performed in 15 patients with unresectable late pancreatic cancer. Endoscopic nosobiliary drainage (ENBD) was performed before the operation. ResultsAll procedures were completed laparoscopically. Jaundice and hepatic function of the patients were obviously improved after the bypass. Oral nutrition was recovered after operation. The mean operative time was (100±26) min (range 70-200 min); the mean operative blood loss was (60±15) ml (range 30-120 ml); the bowel function recovery was on the 3rd-5th postoperative day; the average hospital stay was (8.1±0.7) days (range 6-13 days). Incision infection ocurred in one patient. No operative complications occurred in other patients. ConclusionTreatment of unresectable late carcinoma of the pancreas with laparoscopic choledochojejunostomy and gastric bypass aided by ENBD is a minimally invasive technique with less postoperative pain,shorter hospital stay, lower procedurerelated morbidity,and better oral nutrition. The life quality of patients with late pancreatic cancer can be obviously improved.

  • Total Mesorectal Excision and Low,Ultralow,Colo-Anal Anastomoses Laparoscopically with Harmonic Scalpel in the Treatment of Rectal Cancer

    Objective To assess the feasibility and adequacy of Harmonic Scalpel in a totally laparoscopic total mesorectal excision (TME) and low,ultralow,colo-anal anastomoses for rectal cancer. Methods Excision of the mesorectum and low,ultralow site anastomoses were performed laparoscopically on 30 patients with low rectal cancer based on the concept of TME. Results All 30 TME were successfully completed by laparoscopic approach, and no one was converted to open procedures. A cholecystectomy and/or an ovariotomy were meanwhile performed laparoscopically for 3 patients with rectal cancer,and 1 patient with chronic cholesyctitis, gallstone,ovarian cyst and torsion of the ovary. The operation time was 155 min (115-320 min). Operative blood loss was 20 ml (5-80 ml).The time of bowel function returned and the time to resume postoperative diet was 1-2 days after the operation. Fourteen patients had postoperative analgesic requirement. Average hospital stay was 8 days (5-14 days) and there were no intraoperative and postoperative complications in all 30 patients.Conclusion Laparoscopic excision of the mesorectum and low,ultralow,coloanal anastomoses with Harmonic Scalpel for low rectal cancer is a perspective minimally invasive technique, which is feasible, safe, effective and has dramatic high rates of sphincter preservation with decreased postoperative pain, rapid recovery.

  • ADVANCES OF MINIMALLY INVASIVE SURGERY FOR BREAST CANCER

    【Abstract】Objective To study the advances of minimally invasive surgery for breast cancer. Methods The latest references on the minimally invasive surgery of breast cancer were reviewed. Results Electrochemotherapy,endoscopy, percutaneous microwave coagulation therapy, high intensity focused ultrasound, and targeting therapy were widely applied to treat breast cancer with the characteristics of minimal wound, little hemorrhage and fast rehabilitation. Conclusion The minimally invasive surgery for breast cancer will be extensively carried out in the future.

  • Minimally Invasive and Routine Mitral Valve Repair or Replacement for Patients with Single Mitral Valve Disease:A Case Control Study

    Objective To compare the clinical outcomes and safety of minimally invasive and routine mitral valve repair or replacement for patients with single mitral valve disease. Methods We retrospectively analyzed the clinical data of 67 patients with single mitral valve disease (without aortic valve and tricuspid valve lesion or other heart diseases including atrial septal defect) who underwent mitral valve repair or replacement in the First Affiliated Hospital of China Medical University between January and July 2011. The patients were divided into two groups according to different surgical approaches:the minimally invasive surgery group (n=29,8 males and 21 females,age 51.4±9.4 years) underwent minimally invasive mitral valve repair or replacement via right mini-thoractomy;and the routine surgery group (n=38,11 males and 27 females,age 53.6±11.9 years) underwent mitral valve repair or replacement via middle sternotomy. In the minimally invasive surgery group,9 patients underwent mitral valve repair while the other 20 patients underwent mitral valve replacement. And no patient underwent transition to routine operation. In the routine surgery group,15 patients underwent mitral valve repair and 23 patients underwent mitral valve replacement. Clinical outcomes and safety of the operations were compared between the two groups. Results There was no statistical difference in operation time between the two groups (207.9±18.1 min versus 198.4±27.5 min,P=0.076). The amount of postoperative drainage (126.7±34.5 ml versus 435.6±87.2 ml,P=0.000) and blood transfusion (red blood cell 1.4±0.8 U versus 2.3±1.1 U,P=0.000;blood plasma 164.3±50.4 ml versus 405.6±68.9 ml,P=0.000) of the minimally invasive surgery group were significantly lower than those of the routine surgery group. The cardiopulmonary bypass time (81.7±23.9 min versus 58.7±13.6 min,P=0.000) and aortic-clamping time (51.6±12.7 min versus 38.4±11.7 min,P=0.000) of the minimally invasive surgery group were significantly longer than those of the routine surgery group. The length of ICU stay (22.5±3.6 h versus 31.7±8.5 h,P=0.000),mechanical ventilation (7.4±3.2 h versus 11.2±5.1 h,P=0.000) and postoperative hospitalization (7.1±1.6 d versus 13.5±2.4 d,P=0.000) of the minimally invasive surgery group were significantly shorter than those of the routine surgery group. There was no statistical difference in postoperative complications between the two groups. Minimally invasive surgery group patients were followed up for 5.3±2.4 months with a follow-up rate of 72.4%(21/29). Routine surgery group patients were followed up for 5.5±3.8 months with a follow-up rate of 71.0%(27/38). There was no significant complication during follow-up in both two groups. Conclusion Minimally invasive mitral valve operation via right mini-thoracotomy is effective and safe with a good cosmetic result. Compared with routine operation,patients undergoing minimally invasive operation recover better and faster.

  • Robot-assisted Lobectomy for Non-Small Cell Lung Cancer

    Objective To summarize our initial experience in robot-assisted lobectomy for the treatment of non-small cell lung cancer (NSCLC). Methods A total of 20 NSCLC patients underwent robot-assisted pulmonary lobectomy in General Hospital of Shenyang Military Command from March to September 2012. There were 13 males and 7 females, and their age was 43-80 (60.40±8.07) years. Single-direction thoracoscopic lobectomy technique was used,and systemic mediastinal and hilar lymph node dissection was routinely performed during the operation. There were 4 right upper lobectomies,7 right lower lobectomies,1 right middle lobectomy,7 left lower lobectomies,and 1 left upper lobectomy. Results Postoperative pathological examination showed adenocarcinoma in 12 patients,squamous cell carcinoma in 5 patients,adenosquamous carcinoma in 2 patients,and mucoepidermoid carcinoma in 1 patient. One patient undergoing left upper lobectomy had intraoperative pulmonary artery bleeding of 500 ml,who was healed by pulmonary artery repair via an accessory small incision and blood transfusion of 400 ml. All the other 19 patients successfully underwent robot-assisted lobectomy with their mean intraoperative blood loss of 60.00±42.95 (10-200) ml, and no blood transfusion was needed for them. All the patients were successfully extubated after operation, and none of the patients had severe postoperative complication. The mean thoracic drainage time was 9.35±3.48 (3-15) days. All the patients were discharged uneventfully and followed up for 2-9 (6.01±2.09) months without recurrence or metastasis. Conclusions Robot-assisted pulmonary lobectomy using Da Vinci S Surgical System is safe and feasible,and especially advantageous for lymph node dissection. It can be used for the treatment of early stage NSCLC.

  • Minimally Invasive Sternal Lowering Operation for the Correction of Pectus Carinatum

    Objective To investigate surgical indications,techniques,safety and clinical outcomes of minimallyinvasive sternal lowering operation for the treatment of pectus carinatum (PC). Methods Clinical data of 40 PC patientswho underwent minimal invasive sternal lowering operation in Xinhua Hospital,School of Medicine,Shanghai Jiao Tong University from July 2009 to August 2012 were retrospectively analyzed. There were 39 male patients and 1 female patientwith their average age of 14.5 (12-22)years. All the patients underwent their surgical correction for the first time,and their chest deformity were characterized by a significant protrusion of the sternum and ribs including 21 symmetric and 19 asymmetric protrusions. Preoperative evaluation included electrocardiogram,CT scan of the chest,echocardiogram and pulmonary function test. Preoperative mean Haller index was 1.91±0.23. Sixteen patients had mild restrictive ventilatory disorder. Allthe patients received minimally invasive sternal lowering operation with a curved Nuss steel bar. Aggravating activities wererestricted within 3 months postoperatively,and the steel bar was removed 2 years later. Results All the operations were completed successfully,and thoracic appearance was significantly improved after correction. All the patients and their relativeswere very satisfied with the corrective outcomes. The operation time was 65-115 (82.0±15.6)minutes and average intraop-erative blood loss was less than 10 ml. Postoperative hospital stay was 3-5 (3.5±0.8) days. Postoperative Haller index was2.39±0.17,which was significantly higher than preoperative Haller index (P<0.01) . Postoperative complications included wound infection in 2 patients,pneumothorax in 1 patient (cured by closed thoracostomy),subcutaneous effusion in 3 patients,and persistent pain (longer than 14 days) in 2 patients. There was no other serious postoperative complication. All the 40 patients were followed up for 3-36 months after discharge. A steel bars was removed ahead of schedule because of wound infection in one patient. Other steel bars were in normal position in 39 patients and there was no displacement of the steel bars or the stabilizers. Eight patients received removal of the steel bars without PC recurrence. Conclusion Juveniles with PC who have good chest wall compliance are the best candidates for minimally invasive sternal lowering operation which is an easy,safe,reliable,minimally invasive and esthetic procedure with satisfactory corrective outcomes.

  • Diagnosis and Treatment of Congenital Coronary Artery Fistulas

    Objective To summarize diagnosis and treatment outcomes of coronary arterial fistulas (CAFs) so as to improve surgical results. Methods Clinical data of 23 patients with CAFs who underwent surgical repair in Xinhua Hospital,School of Medicine,Shanghai Jiaotong University from July 2006 to January 2013 were retrospectively analyzed. There were 15 male and 8 female patients with their age of 35.4±8.7 years. CAFs originated from left anterior descendingartery in 7 patients,right coronary artery in 13 patients,dual coronary arteries in 1 patient,left main coronary artery in 1 patient,and obtuse marginal artery in 1 patient. CAFs drained into pulmonary artery in 16 patients,right ventricle in 5patients,and right atrium in 2 patients. Twelve patients received surgical repair under off-pump condition with beating heart,and the other 11 patients underwent surgery under cardiopulmonary bypass (CPB). Surgical techniques included extracardiacgasket interrupted suture ligation,intracardiac mattress suture ligation,intracardiac gasket interrupted suture for fistula repair,and a combination of intracardiac suture ligation and extracardiac repair. Results There was no in-hospital death or seriouspostoperative complication in this group. For the 11 patients undergoing surgical repair under CPB,average CPB time was 78.6±7.4 minutes,and average aortic cross-clamping time was 39.0±5.9 minutes. Postoperatively,patients’ heart functionrecovered well,their symptoms disappeared,and there was no myocardial ischemia or residual fistula. Twelve patients werefollowed up at the outpatient department and via telephone for 3 months to 6 years after discharge. During follow-up,theirheart function all recovered well,and echocardiogram detected no abnormality. There was no late death,myocardial ischemiaor residual fistula. Conclusion All CAFs patients need early surgical repair once their diagnosis is clear. Appropriate surgical strategies should be chosen according to the characteristics of pathological anatomy and associated intracardiaclesions. Surgical outcomes are satisfactory.
