  • MUSCLE FLAP FORMATION OF POPITEAL VEIN IN THE TREATMENT OF SECONDARY ULCER AFTER STRIPPING OF GRAET SAPHENOUS VEIN (REPORT OF 23 CASES)

    Twenty-three patients (28 extremities) suffering secondary ulcer after high ligation and stripping of great saphenous vein were chosen to receive muscle flap formation of potiteal vein. Of which, 21 patients (25 extremities) ulcer scabbed within one week and healed in 2 weeks after operation. The other s were cured in 5 weeks. All patients were followed up 6-12 months with no recurrence and satisfactory results.

  • Application of Gore-Tex Patch in Chest Wall Reconstruction

    Abstract: Objective To summarize the application experience of Gore-Tex patch in clinical chest wall reconstruction. Methods A retrospective analysis was performed in 33 patients who underwent chest wall reconstruction using Gore-Tex patch from January 2001 to December 2010 in Shanghai Changhai Hospital, Second Military Medical University. There were 19 men and 14 women, ranging in age from 20 to 73 years with a median age of 45.7 years. The surgical strategies included choosing different incisions according to tumor location and size, and preserving normal chest wall soft tissue as much as possible during surgery. Gore-Tex patch was used to reconstruct the huge chest wall defect, and covered by transferred muscle flaps. Results All the 33 patients underwent surgical reconstruction successfully and there was no perioperative death. Complete tumor resection was performed in all the patients, including 25 patients with malignant tumor and 8 patients with benign tumor. The diameter of the resected tumors ranged from 8 to 20 cm. All the patients were followed up from 5 to 60 months, except that 3 patients (9.09%) were lost during follow-up. There was no rejection response, abnormal breathing and foreign body sensation during follow-up. The infection incidence was 3%(1/33). Conclusion Gore-Tex patch is a safe and effective material for chest wall reconstruction due to its excellent biocompatibility. Appropriate selection of muscle flap for covering Gore-Tex patch can reduce postoperative complications.

  • REPAIRING POSTOPERATIVE SOFT TISSUE DEFECTS OF TIBIA AND ANKLE OPEN FRACTURES WITH MUSCLE FLAP PEDICLED WITH MEDIAL HALF OF SOLEUS

    Objective To investigate the therapeutic effect of repairing postoperative soft tissue defects of tibia and ankle open fractures with muscle flap pedicled with medial half of soleus. Methods From February 1998 to January 2009, 15male patients with postoperative soft tissue defects of internal fixation for tibia and ankle open fractures were treated. Their age was 18-54 years old (average 32 years old). The injury was caused by traffic accident in 13 cases and hit of heavy objects in 2 cases. The injury was in the left side in 9 cases and the right side in 6 cases. The soft tissue was necrotic and combined with purulent secretion. All patients presented with exposure of bone and steel plate. The soft tissue defect was located on the upper-segment of tibia in 2 cases, the middle and lower-segments of tibia in 9 cases, and the ankle in 4 cases. The size of the defect was 5 cm × 4 cm- 13 cm × 6 cm. The time from the internal fixation to the operation was 3-6 months (average 4 months). The method of anterograde transposition of muscle flap pedicled with medial half of soleus was used to repair the defects in 2 cases, and the method of retrograde transposition was appl ied to repair the defects in 13 cases. The muscle flap harvested during operation was 5 cm × 4 cm- 13 cm × 5 cm in size. The muscle flap was covered with spl it thickness skin graft (2.5 cm × 1.5 cm-10.0 cm × 5.0 cm) of femoribusinternus in 14 cases, and island flap with nutritional vessel pedicle of sural nerve (7 cm × 6 cm) in 1 case. Results One case had skin graft necrosis 5 days after operation and healed after re-debridement, vacuum seal ing drainage, and dermatoplasty. For the rest 14 patients, the incision all healed by first intention, and the skin graft, skin flaps, and muscle flaps were all survived. All wounds of the donor sites healed by first intention. Thirteen patients were followed up for 6 months to 8 years (average 3 years). The grafted skin presented with good wearabil ity and without ulceration and overstaffed appearance. At the final follow-up, the activity range of ankle was 5-10° in extension and 10-15° in flexion, and the gait was abnormal. Conclusion Muscle flap pedicled with medial half of soleus transposition is easy to be operated with a big rotating arc, can fill the narrow cavity and repair the soft tissue defect simultaneously, and provide flat and non-bloated postoperative incision with minor donor-site injury. It is one of the effective methods of repairing the postoperative soft tissue defect after internal fixation of tibia and ankle open fractures.

  • DEVELOPMENT OF THE TISSUE-AUTOGRAFTING

    Objective To introduce the current situation and prospect of the tissue-autografting, such as the flaps, muscle flaps, and bone(periosteum) flaps, andits application in reparative and reconstructive surgery. Methods Based on our own experiences and combined with the review of the literature at home and abroad, the latest development of the tissue autografting was analyzed. Results The femoral anterolateral flap, latissimus dorsi muscle flap, upper arm lateral flap, scap flap, temporal fascial flap and perforator flap are the frequently used in clinic. Of all the perforator flap had such advantages as better repair of the recipient sites and less damage of the donor site. Beacause of more advantages of the free myocutaneous flap transplantation, it substituted thefree muscle transplantation. The atissimus dorsi muscle myocutaneous flap was the most frequently used in the transplant of the vessels, with preserved function of the thoracodorsal nerve or with repair of the defected tissues by the bridge. The most common donor sites of the bone were ribs,iliac bone,fibula andscapula, so the severe bone defects or the bone nonunion, femoral head ischemic necrosis, and the bone graft from the tumor removal could be managed with the bones from those sites. Conclusion The autografting in repairing the tissue defect has become one of the most important surgical techniques in reparative and reconstructive surgery.

  • RECONSTRUCTION OF INFECTED MEDIAN STERNOTOMY WOUND DEHISCENCE AFTER CORONARY ARTERY REVASCULARIZATION

    Objective To introduce the experience about thereconstruction of median sternotomy wound dehiscence. Methods From February 2002 to October 2004, 10 patients with median sternotomy wound dehiscence due to coronary artery revascularization were treated. There were 7 males and 3 females, aging from 68 to 76 years. The sizes of defects ranged from3 cm×5 cm to 5 cm×15 cm. After debridement of necrotic soft tissue, sternum and rib, infected median sternotomy wound was reconstructed with rectus abdominis myocutanous flap, pectoralis major myocutanous flap and latissimus dorsi flap or single muscle flap. The sizes of flaps ranged from 3 cm×5 cm to 5 cm×16 cm.Results Allpatients were followed up from 3 to 11 months with anaverage of 6 months. All the patients achieved healing by first intention with normal respiration and normal function of upper limbs. The wound of donor site healed well.No abdominal hernia and other complications occurred. The wound of donor site healed well.The results were satisfactory.Conclusion According to different stages of the disease and different conditions of an operation, the surgical management should vary with each individual.

  • REPAIR OF DEFECTS IN CHEST WALL WITH THORACICO-ABDOMINAL SKIN FLAP AND MUSCLE FLAP OF MUSCULUS RECTUS ABDOMINIS

    Objective To evaluate the application value of repairing the defects of the chest wall with the thoracico-abdominal skin flap and the muscle flap of the musculus rectus abdominis. Methods From January 2002 to June 2005, five patients with defects in the chest wall underwent the prothesis with the thoracico-abdominal skin flap and the muscle flap of the musculus rectus abdominis under general anesthesia. Focal cleaning was performed first; then, the skin flap was designed and taken (15 cm ×10 cm); and finally, the defects of the chest wall were repaired with the muscle flap of the musculus rectus abdominis. Results Of the 5patients, 4 had the flap healing by the first intention, and 1 had the delayed healing, with no complication. The skin flap had a good appearance, without edema orpigmentation. The X-ray examination showed that the shadow of the sternal sequestrum disappeared. There was no recurrence or complication during the follow-upfor 1-3 years (average, 18 mon). Conclusion The repairing of the defects in the chest wall with the thoracico-abdominal skin flap and the muscle flap of the musculus rectus abdominis is a simple and effective surgical treatment for defects of the chest wall around the sternum, and this kind of treatment is worth applying extensively in clinical practice. 

  • V-Y ADVANCEMENT OF MEDIAL GASTROCNEMIUS MUSCLE FLAP FOR REPAIRING SOFT TISSUE DEFECTS IN MIDDLE AND LOWER SEGMENTS OF ANTERIOR TIBIA

    ObjectiveTo investigate the effectiveness of V-Y advanced medial gastrocnemius muscle flap to repair soft tissue defects in the middle and lower segments of the anterior tibia. MethodsBetween March 2008 and March 2014, 8 patients with skin and soft tissue defects of the anterior tibia were treated, including 6 males and 2 females with an average age of 36.2 years (range, 28-47 years). The soft tissue defects located at the left leg in 5 cases and at the right leg in 3 cases. The causes included traffic accident injury in 6 cases, and heavy pound injury in 2 cases. Three cases had simple soft tissue defects, and the disease course was 5 hours, 6 days, and 14 days, respectively. Five cases had soft tissue defects and fractures, including 1 case of Pilon fracture, and 4 cases of middle and distal tibial fracture; open reduction and internal fixation were performed in 3 cases, the implementation of external fixation in 2 cases; 1 case had chronic osteomyelitis at 11 months after operation, and 4 cases had skin necrosis and wound infection at 1 to 2 weeks after operation; the duration was 1-12 months (mean, 3.4 months). The skin and soft tissue defect area was 5.2 cm×2.5 cm to 13.0 cm×5.5 cm. Debridement was given, and vacuum sealing drainage was used in 6 cases, and then V-Y advancement of medial gastrocnemius muscle flap was used to cover the wound. Because of light wound contamination, the wound was repaired by the flap after emergency debridement in 1 case; 1 patient with osteomyelitis underwent flap repair at immediate after sensitive antibiotics use and debridement. The size of medial gastrocnemius muscle flaps ranged from 15 cm×6 cm to 26 cm×15 cm. The donor site was sutured in 3 cases or repaired with skin graft in 5 cases. ResultsOne case had tension blisters in the distal flap, which was cured after symptomatic treatment. The flap and skin graft survived, and primary healing was obtained in the other cases. Seven patients were followed up 6-18 months (mean, 9 months). The texture and appearance of the flaps were satisfactory. At 6 months after operation, two-point discrimination ranged from 12 to 18 mm (mean, 16 mm). The plantar flexion was weaker than that of normal side, but the patients could normally walk and had normal gait. ConclusionThe V-Y advancement of medial gastrocnemius muscle flap is recommendable to repair soft tissue defects in the middle and lower segments of the anterior tibia for the advantages of reliable blood supply, simple operation, high survival rate of the flap, and satisfactory appearance.
