Abstract: Objective To evaluate clinical outcomes of painless flexible fiberoptic bronchoscopy in the treatment for pulmonary complications in postoperative pediatric patients with congenital heart diseases. Methods We retrospectively analyzed clinical data of 58 patients who received fiberoptic bronchoscopy for pulmonary complications after surgical repair for atrial septal defect, ventricular septal defect, tetralogy of Fallot, double outlet right ventricle or transposition of the great arteries in First Affiliated Hospital of Harbin Medical University From August 2009 to February 2012. There were 26 male patients and 32 female patients with their age ranging from 20 days to 2 years. Olympus xp-60 fiberoptic bronchoscopy was used for removal of airway secretions and bronchial alveolar lavage under anesthesia with propofol, fentanyl and lidocaine. Pulse oximetry, respiratory sound and chest X-ray changes were observed. Results The examination time of painless fiberoptic bronchoscopy was 10-25 minutes in the 58 patients. After removal of airway secretions by fiberoptic bronchoscopy, their respiratory status improved significantly, pulse oxygen saturation increased by 5%-12%, and their pulmonary crackles were significantly reduced. In 29 patients with atelectasis, their pulmonary lobes demonstrated significant reexpansion in chest X-ray reviews on the next day after fiberoptic bronchoscopy, and 8 patients received another fiberoptic bronchoscopy treatment on the next day to achieve complete pulmonary reexpansion. One patient with severe pulmonary hypertension (PH) stayed in intensive care unit (ICU) for 6 days, 3 patients with transposition of the great arteries stayed in ICU for 5 days, 3 patients with double outlet of right ventricle and moderately high PH stayed in ICU for 4 days, and all the other patients were discharged from ICU within 48 hours after admission. There was no severe complication related to fiberoptic bronchoscopy, except 2 patients with minor nasal mucosa bleeding who were cured with hemostatic drugs and local compression. Conclusion Painless flexible fiberoptic bronchoscopy can significantly enhance clinical outcomes and shorten ICU stay for postoperative pediatric patients with congenital heart diseases.
ObjectiveTo study clinical value of perioperative multimodal analgesia for laparoscopic cholecystectomy based on concept of fast track surgery (FTS). MethodsThe clinical data of 268 patients were analyzed retrospectively, who underwent laparoscopic cholecystectomy in the Department of General Surgery, the 2nd Hospital of Baiyin City from July 2013 to July 2015. All these patients were divided into FTS group and traditional group according to the chronological order, 133 patients in the traditional group were performed traditional perioperative analgesia before August 31, 2014, and 135 patients in the FTS group were performed perioperative multimodal analgesia method based on the concept of FTS after September 1, 2014. The data of both groups were collected and analyzed, including point of numerical rating scale, intestinal function recovery time, intake food time, ambulation time, drainage tube duration, postoperative hospital stay, postoperative sleep time at 72 h, and complications. Results①The points of numerical rating scale at 1 h, 4 h, 8 h, 12 h, 24 h, 48 h, and 72 h in the FTS group were significantly lower than those in the traditional group (P < 0.01).②Compared with the traditional group, the intestinal function recovery time, intake food time, ambulation time, drainage tube duration, and postoperative hospital stay were obviously shorter, the postoperative sleep time at 72 h was obviously longer in the FTS group.③The incidence of nausea and vomiting in the FTS group was significantly lower than that in the traditional group (P < 0.01), the incidences of the other complications such as acid reflux, urinary retention, and breathing difficulty had no significant differences between these two groups (P > 0.05). The satisfaction rate of postoperative analgesic effect in the FTS group was significantly higher than that in the traditional group (P < 0.01). ConclusionsPerioperative multimodal analgesia for laparoscopic cholecystectomy based on concept of FTS is safe and effective. It could achieve painless effect of laparoscopic cholecystectomy.
Objective To evaluate the advantages of perioperative painless indwelling urethral catheters in lobectomy of lung cancer. Methods We recruited 133 patients who were scheduled for lung cancer lobectomy under general anesthesia in Department of Thoracic Surgery in West China Hospital from April through December 2014. These patients were divided into two groups including a control group (68 patients) and a trial group (65 patients). The trial group was painless indwelled urethral catheter, and the control group was indwelled urethral catheter routinely. The clinical effectiveness between the two groups was compared. Results The rates of emergence agitation (EA) occurrence and urinary tract infection in the trial group (10.77%, 9.23%) were reduced than those in the control group (26.47%, 26.47%) with statistical differences (P=0.022, P=0.047). And the rate of comfort level (0 degree) of the patients in the trial group (87.69%) was significantly increased than that in the control group (48.53%, P=0.001). And postoperative hospitalization duration in the trial group (5.00±1.60 d) was shorter than that in the control group (6.48±3.14 d, P=0.004). Conclusion Perioperative painless indwelling urethral catheters in lobectomy of lung cancer has benefit of improving the comfort level of the patients and promoting fast-track rehabilitation in the patients with lung cancer.
