Objective To introduce the technique of surgical patch angioplasty for the treatment of patients with isolated left main coronary artery stenosis. Methods Retrospective investigation of results of surgical patch angioplasty in patients with isolated left main coronary artery disease. Results All 8 patients who underwent left main coronary artery patch angioplasty survived with no major perioperative complications. One patient had recurrent angina and required coronary artery bypass grafting 6 months after patch angioplasty. All other patients were symptom free and had normal activity, at a mean follow-up of 5. 3 years. Conclusions Patch angioplasty can be used as an alternative surgical technique in cases of isolated left main coronary artery stenosis with no distal coronary artery disease. However, it may not be suitable for patients with significant left main coronary artery calcification.
Objective To summarize the surgical technique and the effectiveness of CK Parastomal patch in laparoscopic repair of parastomal hernia. Methods The cl inical data were retrospectively analysed from 24 patients who received laparoscopic repair of parastomal hernia using CK Parastomal patch between June 2006 and March 2010. There were 15 males and 9 females with a median age of 55 years (range, 47-80 years). Among them, 19 patients were with colon parastomalhernia in the left lower quadrant and 5 patients with ileum parastomal hernia in the right lower quadrant. The parastomal hernia duration was 1 to 4 years (mean, 2.4 years). The maximal diameter of the hernia ring was 3 to 7 cm (mean, 5.2 cm). All patients did not receive hernia repair. Results Laparoscopic repair of parastomal hernia was successfully performed in 18 cases, and open repair was used in 6 cases because of extensive adhesion. The average operating time was 121 minutes (range, 78 to 178 minutes). All wounds healed by first intention. Wound seroma occurred in 8 cases at 3 to 7 days after operation and disappeared by aspiration. Eleven patients complained of pain in the operative area within 1 month after operation and it disappeared without intervention. All patients were followed up 6 to 39 months (mean, 27 months). One recurrence occurred at 3 months after operation, and no recurrence occurred in the other patients. Conclusion Laparoscopic repair of parastomal hernia using CK Parastomal patch is a safe and feasible procedure with a satisfactory short-term effectiveness, but the long-term effectiveness should be further observed.
Objective To observe the electrophysiological and morphological features of retinal ganglion cells (RGCs) in rats, and investigate its effect on the visual signal conduction. Methods Whole cell recordings were obtained from 112 RGCs of 30 rats at the age of 7-30 days. Resting membrane potential (RMP) was recorded, and input impedance was noted after given 2 mV hyperpolarizing current by voltage clamp. The action potential (AP) was induced by deplorizing current at different densities. The histological staining was actualized by injecting with biotin into the RGCs, and the diameter of the cells was measured. Results Three different discharge patterns of RGCs in response to maintained depolarizing currents were recorded: single spike (25 RGCs), transient firing (40 RGCs), and sustained firing (47 RGCs).The dia meter was 14-16μm in 57.14% transient firing RGCs, and 10-12 μm in 62.50% sustained firing RGCs. The maximum frequency of AP of sustained firing RGCs was significantly higher than that of transient firing RGCs (P<0.05). Conclusion The single firing of RGCs was an immature electrophysiological feature. The electrophysiological features of transient firing and sustained firing RGCs may be important to make the visual information code in spatial and temporal pathway. The electrophysiological and morphological features of RGCs in rats may be correlated with each other. (Chin J Ocul Fundus Dis,2004,20:160-164)
Objective To investigate the relationship between electrophysiological and morphological properties of neurons in visual cortex of developing rat, speculate the coincided degree between electrophysiological and morphological change and realize the mechanism of normal visual development. Methods Whole cell patch-clamp recording and intracellular staining were used to acquire cellular microelectrode recording in visual cortex from Sprague-Dawley rats (4~28 days old). The histological process was made. Results The differences of electrical feature between pyramidal cells and non-pyramidal cells were significant. The morphological maturity degree is different in developing visual cortex. Conclusion The different function of pyramidal and non-pyramidal cells in local integrition is reflected by their electrical feature in the process of visual development. In critical period of visual development, the coincision degree of the electrophysiological and morphological change in visual cortex is larger than that in the subcortex constructure. Objective To study the clinical effect of laparoscopic repair of abdominal incision hernia. Methods The clinical data of 41 abdominal incision hernia patients undergone laparoscopic repair were retrospectively analyzed. Results 〗The operation was successfully performed for 38 cases, and 3 cases were conversed to open. Operative time ranged from 78 to 186 minutes, with an average of 95 minutes. Pain was minor after operation. The first flatus and defecation ranged from 25 to 41 hours, with an average of 32 hours. Food intake started on day 2 after operation. The average length of hospitalization was 6 days (range 5-7 days). After a mean follow-up of 9 (6-16) months, no incision hernia occurred. Conclusion 〗Laparoscopic abdominal incision repair with composite patch is a safe and effective method, which is worthy of clinical application.
ObjectiveTo evaluate the advantages and disadvantages of patch aortoplasty and extended side-to-end anastomosis for the treatment of coarctation of the aorta (CoA) and hypoplastic aortic arch, and provide a more reasonable surgical choice. MethodsClinical data of 45 patients who underwent surgical correction for CoA and hypoplastic aortic arch in Beijing Anzhen Hospital from June 2008 to June 2013 were retrospectively analyzed. According to different surgical strategies for aortic arch hypoplasia, all the 45 patients were divided into 2 groups. In group I, there were 26 patients including 15 males and 11 females with their age of 0.5-6.8 (0.9±2.5) years and body weight of 5.0-20.3 (9.5±7.3) kg, who received patch aortoplasty and whose preoperative pressure gradient between right upper and lower limbs was 38.3±15.6 mm Hg. In groupⅡ, there were 19 patients including 14 males and 5 females with their age of 0.6-7.5 (1.0±2.7) years and body weight of 5.5-21.5 (10.2±6.6) kg, who received extended side-to-end anastomosis and whose preoperative pressure gradient between right upper and lower limbs was 40.7±16.1 mm Hg. Postoperative changes of pressure gradient between right upper and lower limbs of the 2 groups were examined and compared with preoperative values. ResultsTwo patients died postoperatively (4.4%) including 1 patient with low cardiac output syndrome and the other patient with severe lung infection. None of the patients in either group had renal failure or neurological complications. Postoperatively, there were 28 patients whose systolic blood pressure (SBP) of lower extremities was 10-20 mm Hg higher than that of upper extremities, 13 patients whose SBP gradient between upper and limbs was less than 10 mm Hg, and 4 patients whose upper limb SBP was 20 mm Hg higher than lower limb SBP. Postoperative average pressure gradient of right upper and lower extremities was 3.2±13.5 mm Hg and significantly lower than preoperative value (P < 0.05). Postoperative pressure gradient of upper and lower extremities was significantly lower than preoperative value in both groups (P < 0.05). There was no statistical difference in preoperative and postoperative changes of pressure gradient of upper and lower extremities between the 2 groups (P > 0.05). Thirty-eighty patients (88.4%) were followed up from 3 months to 5 years. During follow-up, there was 1 patient whose blood flow velocity of the descending aorta was increasingly accelerated. Pressure gradient across the aortic arch was larger than 40 mm Hg. Computer tomography showed aortic arch restenosis. This patient received reoperation 8 months after the first discharge. Three patients whose aortic pressure gradient was larger than 20 mm Hg were still followed up. Aortic arch pressure gradient was less than 20 mm Hg in all the other patients. ConclusionBoth patch aortoplasty and extended sideto-end anastomosis are ideal surgical methods for the treatment of CoA and hypoplastic aortic arch. Appropriate surgical method should be chosen according to individual conditions of pediatric patients.
ObjectiveTo study the external biocompatibility bewteen the mouse induced pluripotent stem cells (miPSCs) and poly-3-hydroxybutyrate-co-3-hydroxyhexanoate (PHBHHx). MethodsAfter we recovered and subcultured miPSCs, we divided them into two groups. There was one group cultured with material of PHBHHx films outside the body. We observed the adhesive pattern of miPSCs on film by fluorescence of 4, 6-diamidino-2-phenylindole (DAPI) staining. The cell vitality was detected by cell counting kit-8 (CCK-8). The morphology of miPSCs attached on the film was visualized under scanning electron microscope (SEM). We used the traditional petri dish to culture miPSCs and detect the cell activity by CCK-8. ResultsMiPSCs can adhere and proliferate on PHBHHx films. The result of cell vitality which detected by CCK-8 showed that there was a statistical difference in OD value between culturing on PHBHHx films and traditional cultivation (0.617±0.019 vs. 0.312±0.004, P < 0.05). ConclusionThere are adhesion and proliferation on the surface of cells patch made by miPSCs co-culturing with PHBHHx film. Compared with traditional culturing in the cell culture dish, culturing in PHBHHx films have great advantages in the process of adhesion and proliferation. PHBHHx can be used as one of the scaffold for stem cells treating various disease.
ObjectiveTo observe the changes in open probability and protein expression of large conductance Ca2+-activated K+ (BK) channel in retinal vascular smooth muscle cells (RVSMCs) of diabetic rats. MethodsStreptozotocin (STZ)-induced rat diabetic animal model was established by STZ injection intraperitoneally.RVSMCs were isolated by enzyme digestion. The BK currents in control and diabetic groups were recorded by patch clamp technique in single channel configuration. BK channel protein expression in control and diabetic group were measured by Western blot. ResultsCompared with control group, the NP0 of BK channels in diabetic group were significantly increased (t=4.260, P < 0.05). Compared with control group, there was no significant difference inα-subunit protein expression in diabetic group in RVSMCs (t=10.126, P > 0.05); however, β1-subunit protein expression was remarkably increased in diabetic group (t=5.146, P < 0.05). ConclusionThe NP0 of BK channels andβ1-subunit protein expression are increased in RVSMCs of diabetic rats.
Objective To investigate the effects of docosahexenoic acid (DHA) on large conductance Ca2+-activated K+ (BK) channels in normal retinal artery smooth muscle cells (RASMCs). Methods Cultured human RASMCs (6 th-8 th generations) were used to patch clamp experiment. The open probabihties (NP0) in BK channels with different concentrations (0.0, 1.0, 3.0, 5.0, 7.5, 10.0 μmol/L) of DHA were recorded by patch clamp technique in single channel configuration. RASMCs were intervened by different concentrations (0.0, 1.0, 5.0 μmol/L) of DHA as control group, low and high doses of DHA groups, respectively. The protein expressions of β subunit of BK channels in RASMCs from three groups were measured by Western blot. Results The NP0 of BK channels were 0.044 4±0.001 2, 0.081 2±0.004 2, 0.209 0±0.006 1, 0.310 5±0.005 3, 0.465 0±0.007 8 and 0.497 7±0.014 5 with perfusate of 0.0, 1.0, 3.0, 5.0, 7.5, 10.0 μmol/L DHA. DHA activated BK channels in a dose-dependent manner (F=2.621,P<0.05). There was no significant difference in the protein expression of control group, low and high doses of DHA groups (F=11.657,P>0.05). Conclusion DHA can directly activate BK channels, no increasing in subunit expression of BK channels.
