  • The relationship of depression status with asthma control and quality of life in asthma patients before and after guided treatment including health care education

    Objective To investigate the depression status,score of asthma control test (ACT) and quality of life in asthma patients before and after health care education according to Global Initiative for Asthma guidelines.Methods 59 enrolled outpatients with asthma were asked to self-administer the CES-D (center for epidemiologic studies-depression) scale,ACT scale and the quality of life (QOL) questionnaire respectively.All the patients were educated and treated by health care professionals under the guidance of GINA 2004.After average of 2.5 months ± 15 days,they were asked to self-administer all the scales and questionnaire mentioned above once again on return visit.The data was collected and analyzed statistically based on whether or not the patient had depression according to the CES-D score and the data before and after the education was compared statistically.Results (A)40.7% (24/59) of the patients had depression emotion before guided treatment,and after that the percentage significantly decreased to 13.6% (8/59) (Plt;0.05).(B)Comparing the depressive and non-depressive groups,there was significant difference in 3 of 5 domains in QOL excluding limitation of activity and self concern about health (Plt;0.05) before guided treatment.While on return visit 3 domains in QOL excluding limitation of activity and psychologic status had shown a significant difference (Plt;0.05).There was no significant difference in ACT score before guided treatment,while wise versa after that (Plt;0.05).(C)Before guided treatment the depression status was inversely correlated with 3 in 5 domains of QOL (symptoms of asthma,psychologic status and response to irritant),as well as QOL as a whole (Plt;0.05),but not with the other two domains of QOL scale and ACT score.After guided treatment,the depression status had inverse correlation with QOL and 3 in 5 domains of QOL scale (excluding limitation of activity and psychologic status) (Plt;0.05),as well as ACT score (Plt;0.05).Conclusions The symptoms of asthma and response to irritants are common factors that influence the depressive emotion in asthma population.While psychologic status and self concern about health are both important factors that can not be overlooked.Health care education is important for asthma patients in view of appropriate treatment,symptom control and relief of depression emotion.

  • PSYCHOLOGY DURING THE PROCESS OF HUMAN HAND ALLOGRAFT

    OBJECTIVE: To study the psychology and its management during the process of hand allograft. METHODS: One psychologist participated through the whole process of the present hand allograft. 12 potential candidates of hand transplant were interviewed during the selection of patients to evaluate the state of psychiatry and their abilities to manage stressors like cooperation with medical workers and medical interventions, waiting for donors, adaptation to a new hand and post operation depression. The psychological state of 11 patients were believed to be able to receive hand transplant, and they are further prepared psychologically by the psychologist while waiting for a donor. Two lucky candidates were decided by tissue typing and received hand allograft simultaneously. After the operation, the two patients psychotherapy assisted with effective analgesia, supporting from family and environmental improvement. RESULTS: One out of 12 patients was found not suitable for the transplantation because of psychiatric problem. One week postoperation, the 2 patients were anxious, lack of patience, and horrified at seeing the long-expected grafted hand. After 1 week of treatments and adapation the patients managed to settle with the new hand, and accepted the hand as a whole 1 month postoperation. With the recovery of the hand sensation and motion 4 to 5 months postoperation, the patients held the hand as his own. CONCLUSION: Psychologists are required in the hand transplantation team during the pre-transplant selection of patients and post-transplant rehabilitation.

  • Investigation Report of Psychological and Behavioral Status of West China Hospital Outpatients after the Wenchuan Earthquake

    Objective To investigate the changes in terms of knowledge, attitudes, behavior, and mental status in hospital outpatients after the Wenchuan earthquake so as to provide more convenient and efficient outpatient services for patients from the disaster areas. Method A self- designed questionnaire was used to survey the outpatients in West China Hospital. Data analysis was performed with SPSS 13.0 software. Result The duration of earthquake feeling was correlated with age, gender, family address, disaster experience, and casualties of relatives and friends. Sleep quality after the earthquake was also significantly correlated with age, gender, family address, disaster experience, casualties of relatives and friends, and the duration of eartqhauke feeling. Seeking medical treatment after the earthquake was associated with the availability of medical insurance. Knowledge about post-quake epidemics was correlated with the responders’ educational background. Conclusion Stress response is related to age and disaster experience. Many factors may effect the development, extent, and duration of people’s stress response. People became more aware of risk after the quake and had a b demand for post-disaster psychological counseling.

  • A Comparative Study of Cognitive-Behavior Therapy and Paroxetine for Minor Depression

    Objective To compare the effects of cognitive-behavior therapy (CBT) and domestic paroxetine in the treatment of minor depression. Methods A therapeutic technique model for the treatment of minor depression was established. Sixty-four patients with minor depression meeting DSM-IV criteria were divided into CBT and paroxetine groups with 32 patients respectively, and were treated for six weeks. The Hamilton Depression Scale (HAMD) was used to evaluate the clinical efficacy and relapse/recrudescence rate of the two groups. Side effects of paroxetine were also recorded. Results At the end of 2 weeks, no significant difference was noted in the reduction of the HAMD score between the two groups. At the end of 6 weeks, there was little difference in thHAMD score reduction between the two groups (F=8.3,P= 0). No significant difference was found in the curative effects between the two groups (u=316.5,P=0.06).In the paroxetine group, cure rate was 20.7%, significant improvement rate was 48.3%; In the CBT group, cure rate was 10.0%, significant improvement rate was 36.7%.The relapse/recrudescence rate after six in the paroxetine group was higher than in cognitive-behavior therapy group at 6 months’ follow-up (u=106.5,P=0.04).Conclusion The clinical efficacy of cognitive-behavior therapy for minor depression seems to be similar to that of paroxetine. The established model of CBT can be used in clinical practice.

  • Research on the Psychological Change in Adult Women at the Initial Wearing of Fixed Orthodontic Appliance

    ObjectiveTo discuss whether there are some psychological changes in adult women patients when they first wear fixed orthodontic appliances. MethodsEighty adult women patients aged between 18 and 50 years old averaging 35, receiving fixed orthodontic treatment between January 2011 and August 2012 were selected as our study subjects. They completed a questionnaire regarding anxiety and depression at the first day when they came to the hospital and 7 days, one month, and three months after fixed appliance placement respectively. The scales of anxiety and depression of subjects were assessed according to the symptom checklist-90 (SCL-90). ResultsCompared with the norms, the SCL-90 scores of anxiety, depression and interpersonal relationship sensitivity were higher in these patients. The symptoms were more severe a week later, and were even more severe one month after the appliance placement. Three months later, the scores of all factors began to fall, but were still higher than those before the treatment. ConclusionThere is a certain extent of negative psychological influence on adult women patients during fixed orthodontic treatment. At the first week after the placement of fixed appliance, adult women patients may suffer from anxiety and depression.
