  • EFFECT OF FEMORAL VEIN-OCCLUSION ON REPLANTED LIMB SURVIVAL AT DIFFERENT STAGES

    Objective To study the effect of vein-occlusion on the replanted limb survival in SD rats at different stages. Methods Twenty-five adultSD rats were randomly divided into 5 groups according to the time of the femoral vein occlusion after the replanted limbs:2- ,3- ,4 -,6-,and 8- day groups. The limbs were observed through naked eye, measurement of dermal temperature and angiography. Results No formation of collateral veinlet was found, and necrosis wasseen in the replanted limbs of 2- , 3- day groups. Reflux-vein was gradually increased in the replanted limbs of 4,6,and 8 day groups. Angiographic score of capillary density and dermal temperaturein the thigh muscles were greater in groups 4-,6-,and 8- day than in groups 2 and 3 day. Conclusion Within 2 and 3 days,the replanted limbs of SD rats will necrose because of vein-occlusion; and 4 days later the replanted limbs can survive depending on the reflux-vein of new collateral veinlet.

  • EXPRESSION OF VASCULAR ENDOTHELIAL GROWTH FACTOR IN REPAIRING BONE DEFECT WITH VASCULARIZED BONE GRAFT-RECONSTITUTED BONE XENOGRAFT

    Objective To study efficiency of vascularized bone graft combining with reconstituted bone xenograft (RBX) in repairing bone defect and the expression of the vascular endothelial growth factor (VEGF) in serum. Methods From January 1998 to December 2002, 27 cases of bones defects were treated and randomly divided into 3 groups according to different repair materials: group A (the vascularized bone graft-RBX group, n=9), group B (the vascularized bone graft group, n=10)and group C(the RBX group, n=8). The bone defect repair, the bone healing time and the bone graft resorption were observed by radiograph after 3 months, 6 months and 12 months of operation, and the expression of VEGF in serum was assayed with lumino-enzyme immunoassay before operation and after operative 2 weeks, 4 weeks, 6 weeks and 8 weeks respectively. Results The X-ray films showed that the bonehealing was achieved in 8 cases of group A, in 6 cases of group B and in 3 cases of group C after 3 months; in 1 case of group A, respectively in 3 cases of both group B and group C after 6 months. The bone graft resorption was observed in1 case of group B and in 2 cases of group C after 12 months. The serum VEGF values after operative 2 weeks and 4 weeks were higher than those before operation in all of 3 groups(Plt;0.05), and the VEGF values of groups A and B were higher than that group C(Plt;0.05) after 4 weeks. There were no significant differences (Pgt;0.05) in serum VEGF level between postoperative 6, 8 weeks and preoperation in 3 groups. Conclusion The expression of serum VEGF obviously increase in the early period of bone transplanting, it is value of clinical evaluation of reparative efficiency of bone defect.

  • 成年山羊皮肤干细胞体外克隆与诱导分化

    Objective To establish a good method and culture system to isolate skin stem cell and expand it in vitro so as to lay a foundation for exploring the proliferation and differentiation mechanism of skin stem cell. Methods Skin stem cells were obtained by explant culture and identified by using alkaline phosphatase(ALP) staining and differentiating experiment in vitro. Stem cell was induced by the cocktail of conditional medium with cell growth factor (insulin like growth factor and epidermal growth factor). Results Skin stem cell colonies were derived from ear skin tissues of adult dairy goats. The colonies had some characteristics of embryonic stem cells, such as the ability to be continously passaged (Passage 5) and the morphology nest-like. They continued to be ALP positive and had the capacity of forming embryoid bodies. These cells were pluripotent and stem-like cells. In vitro these stem cell can be induced to be Follicle-like structure, Astrocyte-like cells, osteoblast-like cell. Conclusion Explant culture is a good method to isolate skin stem cell, which can be induced to be committed differentiation and trans-differentiation.

  • TREATMENT OF COMPLEX ACETABULAR FRACTURES BY A MODIFIED EXTENDED ILIOFEMORAL APPROACH

    Objective To evaluate the effect of the modified extended iliofemoral approach on treatingcomplex acetabular fractures. Methods Thirty-six cases of complex acetabular fractures were treated by the open reposition and internal fixation by a modified extended iliofemoral approach. Results Thirty-sixcases were followed up for 7-46 months, with an average of 23.8 months. According to the Matta standard, anatomical reposition was performed in 24 cases, perfect reposition in 8 cases, and unsatisfactory reposition in 4 cases. By the modified d’Aubignepostel score, among the 36 cases, 22 had an excellent result, 9 had a good result, and 5 had a poor result. Conclusion Themodified extended iliofemoral approach facilitates the operative exposure of the anterior and posterior walls and both columns of the acetabulum in the surgically-treated acetabular fractures by the open reposition and internal fixation.Because of the reconstruction, the functions of the abductor muscle mass managed by the lagscrew-fixed osteotomies of the iliac crest, and greater trochanter, the patients can achieve a rapid rehabilitation of the joint.

  • A NEW METHOD OF BONE DEFECT REPAIRING AFTER BONE CYST CURETTAGE

    OBJECTIVE To introduce a new method of bone defect repairing after bone cyst curettage. METHODS Eight cases with bone cyst were treated with this new method. The pieces of autogenous periosteum were implanted into the hematoma within the enveloped bone defect created after the bone cyst curettage. Among these patients, there were 5 males and 3 females, aged from 14 to 36 years old. All the lesions located in the upper of femur except one being located in humerus. The results were evaluated through the postoperative radiological findings with the preoperative ones and analysis of clinical functions. RESULTS All the patients were followed up for 2 to 11 years. X-ray films showed that osteogenesis developed well and that the enveloped bone defects had been repaired. No recurrence was found and the function of the affected limbs were maintained. CONCLUSION Autogenous periosteum grafting is effective in the treatment of solitary bone cyst.

  • Experimental study on targeting transfer of reporter gene in proliferative vitreoretinopathy of rabbit

    Purpose To study the possibility of prevention of proliferative vitreoretinopathy(PVR) by transduction of exogenous gene in vivo. Methods PVR model of rabbits was induced by intravitreal injection of fibroblasts.beta;-galactosidase (lacZ) gene as a reporter gene was transfered into the vitreous of PVR model eyes mediated by retroviral vector, and the expression of the gene in eye tissues was determined . Gene transfection was done on the 6th day after fibroblasts injection,and the dosage of intravitreal injection of reporter gene was 0.1ml PLXSN/lacZ serum-free supernatant (1.1times;106 cfu/ml). Results lacZ gene expression was seen in proliferative membranes after gene transfection, and the expression was located maily at the surface of PVR membrane.The reporter gene expression lasted at least more than 30 days.No expression was found in retinal tissues. Conclusions Retrovirus mediated gene can be directionally transducted in PVR membrane,and might possess the feasibility of gene therapy for PVR. (Chin J Ocul Fundus Dis, 2001,17:224-226)

  • REPEATED USE OF SCROTAL SEPTUM FLAP TO REPAIR FAILED HYPOSPADIAS

    Objective To introduce an effective method to repair failed hypospadias. Methods From October 1998 to October 2002, 8 cases of failed penoscrotal junction hypospadias were repaired by repeated use of scrotal septum vesicular pedicle island flap to reconstruct urethra. The patientsranged in ages from 4 to 8 years. Results All flaps in 8 patients survived, the reconstructed urethra was big and smooth. After a follow up of 6-12 months, the satisfactory appearance and normal urinary stream were obtained. No complications were found. Conclusion This method is an effective operative procedure to repair failed hypospadias and is a simple and convenient operation.

  • COMBINED SCAPULAR/PARASCAPULAR BILOBAR FLAPS FOR RECONSTRUCTION OF SEVERE NECK CONTRACTURE

    Objective To discuss the reconstruction of severe neck contracture by transplanting combined scapular/parascapular bilobar flaps, and the probability to reestablish three-dimensional movement of the neck. Methods From January 2003 to November 2004, 9 cases of sustained severeneck contractures were treated (aged 9-32 years). The combined scapular/parascapular bilobar flaps, pedicled on the circumflex scapular vascular bundle, were microsurgically used to cover the soft tissue defect after excision of hypertrophic scar and release of contracture. The maximum size of the combined bilobar flap was 20 cm×8 cm to 20 cm×11 cm,while the minimum one was 15 cm×4 cm to 15 cm×6 cm. Results The combined scapular/parascapular flapswere successfully used to treat 9 cases of severe neck contracture. All patients were satisfied with the final functional and aesthetic results. There was no recurrence during 3-9 months follow-up for 8 patients. The cervicomental angle was 90-105°.Conclusion The combined bilobar scapular/parscapular flap, providing a large area of tissue for coverage in three dimensions with a reliable blood supply by only one pedicle anastomosis during operation, is agood option for reconstruction of the severe neck contracture.

  • REPAIR OF FLEXOR CONTRACTURE OF THE HAND

    Objective To investigate the etiology of the flexor contracture of the hand, to diagnose and to explore its surgical treatment. Methods From May 1997 to June 2004, 212 cases of flexor contraction of the hand were treated with scar excision, thorough loosening the contracture, covering the secondary skin defects with free skin grafting, “Z”plasty, double “V-Y” plasty, transposition of the palmar dorsum flap of the hand and iliac-inguinal flap. There were 163 males and 49 females, whose ages ranged from 3 to 61. There were 85 cases of left hands, 54 cases of right hands, and 73 cases of both hands. Contracture sites were as follow: 117 cases were in fingers, 32 cases located in palms and 63 cases were in both. Causes of deformity were as follow: 29 cases derived from burn and explosion, 127 cases came from contracture of palmar aponeurosis, 31 cases were because of traffic accident and machine extrusion, 5 cases for getting an electric shock, 14 cases for improper postoperative immobility, and 6cases for other reasons. Course of diseases lasted for 2 to 24 months.Results 149 cases were followed up for 4 to 30 months. One fingertip was in necrosis and ended in nub plasty because of inappropriate time to leave hospital. Flexion contracture recurred in 7 cases, skin necrosis occurred to 3 cases whose scars were healed in the end, poor restoration of function was observed in 2 children patients for lack of exercise, and 2 skins contracted after free cut skin grafting. Others got satisfactory results. Conclusion Once the pathogenesis and contrature factors are clearly known, timing and correct surgical measures are chosen, thorough contracture is loosened, and early postoperative exercises are performed, good effect will be achieved.
