  • Clinical Study of Laparoscope Combined with Choledochoscope in Treatment of Patients with Cholecystolithiasis and Choledocholithiasis

    Objective To study the clinical effects of laparoscope combined with choledochoscope in patients with cholecystolithiasis and choledocholithiasis. Methods Clinical data of 74 patients with cholecystolithiasis and choledocholithiasis between Mar. 2009 and Feb. 2011 in our hospital were retrospectively analyzed. Among them, 37 cases underwent the laparoscope combined with choledochoscope operation (referred to as the double mirror operation group) and 37 cases underwent the conventional open operation (open operation group). The intraoperative conditions, therapeutic effect, and complications of patients in 2 groups were compared. Results The stone clearance rates of the two groups were 100%. The recurrence rate of 2 groups in the follow-up process was no significantly difference (P>0.05). In the double mirror operation group, the operative time was significantly longer than that open operation group (P<0.01);the intraoperative bleeding was significantly less than that open operation group (P<0.01);the anal exhaust time after operation, get out of bed time after operation, and total duration of hospitalization were significantly shorter than that open operation group (P<0.01);and the incidence of postoperative complications was significantly lower than that open oper-ation group(P<0.01). Conclusion Laparoscope combined with choledochoscope in treatment of cholecystolithiasis and choledocholithiasis patients has exact effects, with minor trauma, quicker recovery, and fewer complications, and it’s worthy of promotion.

  • High-frequency ultrasounic observation of localization and operation accuracy of traditional blind acupotomy lysis in treatment of scapulohumeral periarthritis

    ObjectiveTo observe the accuracy of location and operation of traditional blind acupotomy lysis in the treatment of scapulohumeral periarthritis by using the high-frequency ultrasound.MethodsForty-two patients with scapulohumeral periarthritis diagnosed in the First Affiliated Hospital of Xinjiang Medical University and Urumqi Hospital of Traditional Chinese Medicine from February to April 2018 were selected. Four common sites of needle knife in the treatment of scapulohumeral periarthritis were operated blindly, and the process of the needle insertion points location and needle perform were both observed by high-frequency ultrasound.ResultsUsing high-frequency ultrasound to observe and confirm the bare-handed positioning point and needle-knife operating point, we found that the accuracy rate of bare-handed positioning needle-point was 100.0% (42/42). In the process of needling, the accuracies of needle insertion at the point of small tubercle of humerus and the point of bursa of deltoid muscle were high, which was 95.2% (40/42) and 100.0% (42/42), respectively. However, because of the deviation of the needle depth and direction, the accuracies of needle insertion at the coracoid point and the sulcus point between the humeral tubercles were low, which was 45.2% (19/42) and 4.8% (2/42), respectively.ConclusionsTraditional acupotomy lysis is a commonly used method of needle knife treatment. Using high-frequency ultrasound, it is found that even by experienced needle knife doctors, there may still be positioning deviation when using blind method to insert needles. Because the visualization of clinical needle knife is difficult to be carried out universally due to the limitations of time and technology, it is suggested that high-frequency ultrasound could be used as a visualization teaching tool in the training of needle knife operation to assist the training of blind needle knife operation technology, which may improve the accuracy of blind needle knife operation.

  • Predictive analysis on discharged patients based on curve estimation and trend-season model

    Objective To explore the predicted precision of discharged patients number using curve estimation combined with trend-season model. Methods Curve estimation and trend-season model were both applied, and the quarterly number of discharged patients of 363 hospital from 2009 to 2015 was collected and analyzed in order to predict discharged patients in 2016. Relative error between predicted value and actual number was also calculated. Results An optimal quadratic regression equation Yt=3 006.050 1+202.350 8×t–3.544 4×t2 was established (Coefficient of determination R2=0.927, P<0.001), and a total of 23 462 discharged patients were predicted based on this equation combined with trend-season model, with a relative error of 1.79% compared to the actual number. Conclusion The curve estimation combined with trend-season model is a convenient and visual tool for predicting analysis. It has a high predicted accuracy in predicting the number of hospital discharged patients or outpatients, which can provide a reference basis for hospital operation and management.

  • Research on formulating an adaptive index criterion of hospital pharmaceutical management under new medical reform: estimating method and process

    To achieve policy goals of new medical reform, a control line of drug proportion was delimited for hospitals by the Department of National Administration. However, to formulate criteria of drug proportion in a scientific and rational approach has been a challenge, which plagued numerous medical workers. This study aims to analyze the baseline data of drug proportion and its impact factors in clinical departments, and quality control charts are applied to explore an estimating method and process for rationally formulating an index criterion of pharmaceutical management, so as to provide a reference for hospital management practice.

  • Atypical Adrenal Phaeochromocytoma: A Retrospective Clinical Study

    Objective To retrospectively analyze the clinical information of a series of patients with atypical adrenal phaeochromocytoma in order to improve the diagnosis of atypical phaeochromocytoma. Methods Ninety patients diagnosed pathologically as adrenal phaeochromocytoma from January 1998 to December 2004 in the Affiliated Hospital of Inner Mongolia Medical College were included in the study. Patients with phaeochromocytoma were classified into the typical group and atypical group based on their clinical manifestations. The differences were analyzed between the two groups in terms of patient age, tumor location and maximal diameter, incidence of hypertension, and plasma catecholamine level. Results About 24.4% (22/90) of the patients with phaeochromocytoma were classified as atypical. There was no significant difference in the maximal tumor diameter between the two groups. The plasma norepinephrine level of the atypical group was significantly lower than that of the typical group (P=0.001), and the positive rate of plasma norepinephrine of the atypical group was also lower than that of the typical group (P=0.003). Conclusion  Compared with typical phaeochromocytoma, atypical phaeochromocytoma is associated with a lower plasma norepinephrine level. The size of adrenal tumor cannot contribute to the diagnosis of atypical phaeochromocytoma. Based on atypical manifestations, CT examination results, and plasma catecholamine level, most atypical phaeochromocytoma should be diagnosed correctly.

  • Expression of MIZ1 in peripheral blood mononuclear cells of patients with community-acquired pneumonia and its clinical significance

    Objective To measure the expression level of Myc-interacting zinc finger protein-1 (MIZ1) in peripheral blood mononuclear cells (PBMC) of patients with severe and non-severe community-acquired pneumonia (CAP) and its relationship with inflammatory factors. Methods Thirty-six CAP patients from Beijing Chaoyang Hospital from April 2018 to June 2019 were enrolled in this study. MIZ1 mRNA level in PBMC were measured by reverse transcriptase-quantitative polymerase chain reaction. The levels of interleukin (IL)-6, IL-8, IL-10, and interferon-α in the serum of patients were measured by enzyme-linked immunosorbent assay. The levels of MIZ1 mRNA and inflammatory factors were compared between the severe CAP patients and the non-severe CAP patients. Results Compared with non-severe CAP patients, the MIZ1 mRNA level in the PBMC of severe CAP patients was lower (P<0.05) than non-severe group. Receiver operating characteristic (ROC) curve of the expression level of MIZ1 in PBMC was calculated according to whether CAP was severe or non-severe, and the area under ROC curve was 0.731 (P=0.018). Spearman correlation analysis showed that MIZ1 mRNA was negatively correlated with IL-10 level in the severe CAP patients (Spearman correlation co-efficient was –0.620, P<0.05). Conclusions MIZ1 may indicate the severity of CAP. MIZ1 may affect IL-10 so as to play a role in inflammation regulation.
