  • Contrast of Ultrasonography in Diagnosis for Hepatorrhexis with Operative Exploration (Report of 120 Cases)

    Objective To discuss the diagnosis value of ultrasonography in hepatorrhexis. Methods The contrast of ultrasonography for 120 cases of hepatorrhexis with operative exploration was made. Results All of the 120 patients were examined with ultrasonography, fragmentation of liver parenchyma in 55 cases, dehiscence of liver parenchyma in 38 cases, liver subcapsular hematoma in 12 cases, and spontaneous rupture of liver cancer in 7 cases. All of the 120 patients were confirmed hepatorrhexis with laparotomy. The positive diagnostic rate of ultrasonography in the hepatorrhexis was 93.3%. Conclusion Ultrasonography is valuable for diagnosing hepatorrhexis and it is one of the important diagnostic approaches.

  • PREVENTION OF COLONIC NECROSIS IN TRANSABDOMINO-PERINEAL SAVING SPHINCTER RESECTION IN TREATMENT OF RECTAL CANCER (REPORT OF 46 CASES)

    Objective To investigate the prevention of gangrene of exteriorized colon following transabdomino-perineal saving sphincter resection of rectal cancer. Methods From Aug. 1988 to Feb. 2000, 46 cases of cancer of the rectum were treated by transabdominoperineal saving sphincter with severing the anal sphincters and anorectal ring. During this procedure the anal sphincters and anorectal ring were severed to prevent gangrene of the exteriorized colon. Results In these cases, the exteriorized sigmoid colon had good blood supply and no gangrene was found. Conclusion This method can effectively prevent the gangrene of exteriorized sigmoid colon stump and gives no permanent fecal incontinence.

  • Effects of Angiogenesis Inhibitor SU5416 on Microvessel Density in Rat Model of Pancreatic CancerDepartment of General Surgery, The People’s Hospital of Songgang, Shenzhen 518105, China

    ObjectiveTo study the effects of angiogenesis inhibitor SU5416 on the microvessel density(MVD) of pancreatic cancer and to evaluate its influence on the growth and metastasis of pancreatic cancer. Methods A rat model of pancreatic cancer was established with dimethylbenzanthracine(DMBA). 60 rats with pancreatic cancer were randomly divided into 4 groups: saline group, 5-Fu group, SU5416 group, 5-Fu and SU5416 group. Thirteen weeks after injection, the microvascular density (MVD) of pancreatic cancer was detected.Results The microvascular densities (MVD) were (12.3±3.2)%, (11.4±3.8)%, (2.1±1.5)% and (1.8±1.1)% in the saline group, 5-Fu group, SU5416 group and 5-Fu+SU5416 group respectively. The MVDs in the SU5416 group and 5Fu+SU5416 group were statistically lower than those in the saline group and 5-Fu group(P<0.05). There was no significant difference between the 5-Fu group and saline group(Pgt;0.05). ConclusionSU5416 can inhibit the microvascular growth in pancreatic cancer. And the inhibition can be enhanced when combined with chemotheraputic drugs.

  • Clinical Research on Nonoperative Treatment for Incomplete Adhesive Small Bowel Obstruction after Laparotomy

    ObjectiveTo study the application value of mixed formulations consisting of paraffin oil, dimethyl silicone oil, and senna preparations in treatment for incomplete adhesive intestinal obstruction after laparotomy. MethodsOne hundred and twentyeight patients diagnosed incomplete adhesive intestinal obstruction admitted to this hospital from March 2005 to May 2008 were randomly divided into trial group and control group. For the control group, the tradition therapy including fasting, gastrointestinal decompression, fluid replacement therapy, and enema with soap and water were used for treatment. For the trial group, the mixed formulations consisting of paraffin oil, dimethyl silicone oil, and senna preparations were injected into stomach by the nasogastric tube on the basis of traditional treatment used for the control group. Some indicators including the successful rate of nonoperative treatment, the time that obstructive symptoms resolved and returned to normal exhaust and defecation and normal diet, and recurrence rate were compared between two groups. ResultsThe successful rate of nonoperative treatmentin in the trial group were significantly higher than that in the control group 〔92.1% (70/76) versus 69.2% (36/52), Plt;0.01〕. The average time that recovered to normal exhaust and defecation in the trial group and the control group was 32.5 d and 47.8 d, respectively. The average time that recovered to normal diet in the trial group and the control group was 3.2 d and 5.3 d, respectively. The time that recovered to normal exhaust and defecation, and diet in the trial group were significantly shorter than those in the control group (Plt;0.01). The recurrence rate had no significant difference between two groups (Pgt;0.05). ConclusionThe mixed formulations consisting of paraffin oil, dimethyl silicone oil, and senna preparations improve recovery of intestinal function and reduce surgical intervention rate.
