  • Case series of alendronate in treatment of bone marrow edema syndrome of the hip

    ObjectiveTo explore the clinical outcome of alendronate in the treatment of bone marrow edema syndrome of the hip.MethodsA retrospective analysis of 8 cases (10 hips) of bone marrow edema syndrome of the hip treated with alendronate between December 2017 and January 2020 was conducted. Harris score and MRI exam of hip were used to assess the clinical and radiographical outcomes. Clinical cure rate and length of treatment period were used to evaluate whether alendronate was benefitial for the treatment of bone marrow edema syndrome of the hip.ResultsAll the cases were followed up for at least 3 months and got complete clinical data, the mean follow-up period was (5.2±1.2) months. Seven cases (9 hips) in 8 cases (10 hips) had clinical cure, with a clinical cure rate of 90%. The length of treatment period was 1 to 2 months with a mean value of (1.7±0.2) months. The Harris score increased from 57.8±6.3 before treatment to 98.6±1.0 at the last follow-up, and the difference was statistically significant (t=18.299, P<0.001). One case (1 hip) developed osteonecrosis of femoral head at one month after the initiation of alendronate therapy, who received continuous combination therapy of alendronate, calcium and aspirin, and no collapse or expansion of necrosis was found after 12 months follow-up.ConclusionsBone marrow edema syndrome of the hip is not a self-limiting disease completely, some patients maybe develop osteonecrosis of femoral head. Alendronate is benefitial for alleviating clinical symptom, accelerating bone edema disappearance, and shortening the course of disease.

  • A prospective randomized controlled trial on the short-term effectiveness of domestic robot-assisted total knee arthroplasty

    ObjectiveTo explore the short-term effectiveness of domestic robot-assisted total knee arthroplasty (RATKA) conducted by a prospective randomized controlled trial.MethodsPatients who were scheduled for primary unilateral TKA between October 2020 and December 2020 were eligible in this randomized controlled trial. According to the random number table method, they were allocated to the traditional TKA group and the RATKA group [application of the Yuanhua robotic-assisted TKA (YUANHUA-TKA) system during operation]. A total of 63 patients met the selection criteria were enrolled in the study, of which 3 cases voluntarily withdrew from the trial. And finally 60 cases were enrolled for analysis; of which 28 cases were in the RATKA group and 32 cases were in the traditional TKA group. There was no significant difference in gender, age, body mass index, American Society of Anesthesiologists (ASA) classification, duration of osteoarthritis, surgical side, and preoperative knee visual analogue scale (VAS) resting and motion scores, joint range of motion (ROM), Knee Society Score (KSS), Western Ontario and McMaster University Osteoarthritis Index (WOMAC) pain, stiffness, and function scores, hip-knee-ankle angle (HKA) deviation (P>0.05). The operation time and intraoperative blood loss of the two groups were recorded. Knee joint VAS resting and motion scores, ROM, KSS scores, and WOMAC pain, stiffness, and function scores were used to evaluate the knee joint function and pain. Gait analysis (flexion and extension angle) was conducted at 3 months after operation. The full length X-ray films of lower limbs and anteroposterior and lateral X-ray films of knee joint were taken. The HKA deviation, lateral tibia component (LTC), frontal femoral component (FFC), frontal tibia component (FTC), and lateral femoral component (LFC) measured on the X-ray films were used to evaluat the lower limb alignment and prosthesis position.ResultsThe operations of the two groups completed successfully; the incisions healed by first intention after operation, and no complications related to the operation occurred. The operation time of the RATKA group was significantly longer than that of the traditional TKA group (t=12.253, P=0.001), and there was no significant difference in intraoperative blood loss between the two groups (t=3.382, P=0.071). All patients were followed up 3 months. At 3 months after operation, the knee joint VAS resting and motion scores, ROM, KSS scores, and WOMAC pain, stiffness, and function scores improved significantly when compared with preoperatively in the two groups (P<0.05); there was no significant difference of pre- and post-operative indicators between the two groups (P>0.05). The gait analysis showed that the flexion and extension angle in the RATKA group was significantly bigger than that in the traditional TKA group (t=9.469, P=0.003). X-ray films reexamination at 3 months after operation showed that the prostheses in the two groups were in good positions, and there was no adverse events such as prosthesis loosening or sinking. There were significant differences in the HKA deviation between pre- and post-operation in the two groups (P<0.05), but the difference of pre- and post-operative HKA deviation between the two groups was not significant (t=1.254, P=0.267). There was no significant difference in FFC, FTC, and LFC between the two groups (P>0.05); the LTC was significantly smaller in the RATKA group than in the traditional TKA group (t=17.819, P=0.000), which was closer to the ideal value.ConclusionYUANHUA-TKA system can improve the accuracy of osteotomy and the prosthesis placement as well as the lower limb alignment. Its short-term effectiveness can be promised, but long-term effectiveness needs to be further studied.

  • The West China Hospital program for the perioperative management of domestic robot-assisted total knee arthroplasty

    In recent years, reports of domestic robot-assisted total knee arthroplasty have gradually increased. In response to problems that may arise during the perioperative period, West China Hospital of Sichuan University has adopted measures such as the establishment of multidisciplinary team, adequate preoperative evaluation, precise intraoperative operations, and whole process optimization management (incision management, pain management, functional exercise and follow-up management). Combined with the actual clinical situation, a domestic robot-assisted total knee arthroplasty management program during the perioperative period was explored and formulated to promote the recovery of patients. This article introduces the above-mentioned program, and aims to provide a reference for the management optimization of domestic robot-assisted total knee arthroplasty during the perioperative period.

  • The West China Hospital program of whole process management of patients undergoing artificial hip and knee arthroplasty

    In addition to implementing a series of measures in the hospital, enhanced recovery after surgery also needs to balance pre-hospital rehabilitation and post-hospital continuation management for patients. In order to optimize the patient management process of hip and knee arthroplasty, the orthopedic team of West China Hospital of Sichuan University has developed a comprehensive management plan for patients undergoing artificial hip and knee arthroplasty based on the latest domestic and foreign literature and previous practice. This article introduces the program from the definition of whole process management, as well as the pre-hospital, in-hospital, and post-hospital management of patients undergoing hip and knee arthroplasty, and aims to provide experience and reference for future clinical practice.

  • Application of augmented reality in the whole health management of hip replacement patients

    Objective To integrate augmented reality (AR) into the whole process of health management and observe its application effect in hip replacement patients. Methods Patients undergoing hip replacement in the Department of Orthopedic Surgery of West China Hospital, Sichuan University between April and September 2022 were selected. According to the random number table method, patients were divided into a trial group and a control group. The trial group adopted the whole process AR health management mode, and the control group adopted the conventional health education mode. The joint function score, functional exercise compliance, coping difficulties after discharge, Huaxi Emotional-distress Index and satisfaction of the two groups at different time points were compared. Results A total of 80 patients were included, with 40 patients in each group. At each follow-up time point after surgery, the scores of Harris Hip Score and Post-Discharge Coping Difficulty Scale among trial group patients were better than those of the control group patients (P<0.05). There was no statistically significant difference in the Huaxi Emotional-distress Index scores between the two groups (P>0.05). The compliance rate of functional exercise in the trial group (P=0.025) and the patient satisfaction were higher than those in the control group (Z=−4.918, P<0.05). Conclusions The AR-based whole process health management can make it easier for patients to grasp functional exercise (preoperative pre-exercise, postoperative rehabilitation), post-hospital health guidance and other educational knowledge. This new health management is conducive to enhancing patients’ exercise compliance, strengthening joint function recovery, daily living ability and patient satisfaction.It can be promoted and applied in clinical practice.
