  • Short-term Outcomes of Thoracoscopic and Laparoscopic-assisted Minimally Invasive McKeown Procedure for EsophagealCancer

    Objective To explore clinical application values of thoracoscopic and laparoscopic-assisted minimally invasive McKeown procedure for esophageal cancer. Methods Clinical data of 196 patients with esophageal cancer (EC) who underwent thoracoscopic and laparoscopic-assisted minimally invasive McKeown procedure in West China Hospital of Sichuan University from February 2008 to August 2012 were analyzed retrospectively. There were 145 male and 51 female patients with their age of 40-76 (58.8±6.6) years. There were 43 patients with EC in the upper segment of the esophagus, 115 patients with EC in the middle segment of the esophagus and 38 patients with EC in the lower segment of the esophagus.Results Total operation time was 215-780 (305.0±40.7) minutes,including thoracoscopic operation time of 50-580 (105.0±38.4) minutes and laparoscopic operation time of 28-105 (54.0±8.6) minutes. Intraoperative blood loss was 20-440 (285.4±38.5) ml. The number of dissected lymph nodes was 6-39 (20.4±1.6) for each patient. Postoperative hospital stay was 7-93 (12.8±5.2) days. Sixty patients (30.6%) had postoperative complications,including 28 patients (14.3%) with pulmonary comp-lications,5 patients (2.5%) with chylothorax,5 patients (2.5%) with arrhythmias,15 patients (7.6%) with anastomotic fistula,10 patients (5.0%) with recurrent laryngeal nerve injury,and other complications in 2 patients. Conclusion Thoracoscopic and laparoscopic-assisted minimally invasive McKeown procedure is a safe and feasible operation for esophageal cancer.

  • Advances in diagnosis and surgical treatment of infective endocarditis

    Infective endocarditis (IE) is a disease with severe complications and high mortality. It is heterogeneous in etiology, clinical manifestations, and course. At the same time, there are many disputes on the clinical practice of antibiotic treatment, surgical indications and timing. In this review, we discuss the epidemiology, diagnosis, treatment, and prevention of IE, especially the latest advances in surgical treatment after the release of European Society of Cardiology and American Heart Association guidelines in 2015.

  • Analysis of Helicobacter pylori infection and its risk factors among health examination participants in a general hospital

    ObjectiveTo investigate and analyze the prevalence and influencing factors of Helicobacter pylori (Hp), to provide scientific basis for the development of Hp infection prevention and control program.MethodsThe Hp infection of healthy population who received 13C-urea breath test in Sichuan Science City Hospital from January to December 2018 were retrospectively analyzed. Medical examination reports were collected and sorted out. We compared the gender and age differences of Hp infection, and binary logistic regression analysis was performed to analyze the risk factors of Hp infection.ResultsA total of 8 093 healthy participants were included, including 5 530 males (68.33%) and 2 563 females (31.67%). The infection rate of Hp was 37.80% (3 059/8 093) in all subjects. The infection rate of males [39.48% (2 183/5 530)] was significantly higher than that of females [34.18% (876/2 563)] (χ2=20.899, P<0.001). The infection rate of 50-59 years old group was the highest (43.87%), and that of <30 years old group was the lowest (30.93%). The difference of Hp infection rate among different age groups was statistically significant (χ2=64.577, P<0.001). Logistic regression analysis showed that male [odds ratio (OR) =1.257, P<0.001], 40-49 years old (OR=1.446, P<0.001), 50-59 years old (OR=1.756, P<0.001), 60-69 years old (OR=1.512, P<0.001), high total cholesterol level (OR=1.221, P=0.003) and obesity (OR=1.403, P<0.001) were risk factors for Hp infection.ConclusionsThe prevalence of Hp infection in the general hospital is lower than the national average level, and male, 40-69 years old, high total cholesterol level and obesity are predictors of Hp infection. Effective measures should be taken to prevent and control the infection of Hp.

  • A design and evaluation of wearable p300 brain-computer interface system based on Hololens2

    Patients with amyotrophic lateral sclerosis ( ALS ) often have difficulty in expressing their intentions through language and behavior, which prevents them from communicating properly with the outside world and seriously affects their quality of life. The brain-computer interface (BCI) has received much attention as an aid for ALS patients to communicate with the outside world, but the heavy device causes inconvenience to patients in the application process. To improve the portability of the BCI system, this paper proposed a wearable P300-speller brain-computer interface system based on the augmented reality (MR-BCI). This system used Hololens2 augmented reality device to present the paradigm, an OpenBCI device to capture EEG signals, and Jetson Nano embedded computer to process the data. Meanwhile, to optimize the system’s performance for character recognition, this paper proposed a convolutional neural network classification method with low computational complexity applied to the embedded system for real-time classification. The results showed that compared with the P300-speller brain-computer interface system based on the computer screen (CS-BCI), MR-BCI induced an increase in the amplitude of the P300 component, an increase in accuracy of 1.7% and 1.4% in offline and online experiments, respectively, and an increase in the information transfer rate of 0.7 bit/min. The MR-BCI proposed in this paper achieves a wearable BCI system based on guaranteed system performance. It has a positive effect on the realization of the clinical application of BCI.

  • BIOCOMPATIBILITY EVALUATION OF NANO TCP/ GELATIN/ VELVET ANTLER POLYPEPTIDE MATERIAL

    Objective To evaluate the biocompatibil ity of a new nano TCP/ gelatin / velvet antler polypeptide material. Methods The nano TCP/ gelatin / velvet antler polypeptide material was prepared, and the morphous was observed by scanning electron microscope. L929 and NIH/3T3 cell l ines were cultured conventionally. Acute toxicity test, hemolysis test, cell prol iferation and cytotoxicity test were used to evaluate the biocompatibil ity of the material. Results The compositemicrosphere material was about 10 μm in diamerter and had good spherical geometry, high monodispersity with nanometer size holes on the surface. Toxic symptoms such as hyperspasmia, palsy and death did not appear during the observing stage in acute toxicity test. Maximum hemolysis rate of the material was less than 5% which met the requirement of hemolysis test standard as a medical material. Different concentrations of the materials leaching l iquor could enhance the prol iferation of NIH/3T3 cells, which showed the good biologic activity. Toxicity grade was 0, and the material was no cytotoxic. Conclusion Nano TCP/ gelatin / velvet antler polypeptide material has good biocompatibil ity.
