  • Orthotopic Implantation Model for Human Anaplastic Thyroid Cancer Cell Line

    ObjectiveTo establish an animal model of anaplastic thyroid cancer with high metastatic activity as in human body. MethodsHuman anaplastic thyroid cancer cell line TAK was injected into one of the lateral lobes of the thyroid gland, as well as in the subcuitis in a series of nude mice. Mice were sacrificed when found moribund, and autopsy and histology were performed subsequently.ResultsThe implantation of human anaplastic thyroid cancer cells in an ectopic enviroment did not permit expression of metastasis potential. In contrast, intrathyroid implantation did. Lymph node (5/10), lung (3/10) and one metastasis (1/10) were noted upon histological examination. ConclusionAn animal model with high metastatic activity is established when human anaplastic thyroid cancer cell line TAK is implanted orthotopically into nude mice.

  • Investigation of Succession Model for Goiter in Iodine Deficiency Area

    ObjectiveTo investigate the succession model for hyperthyroidism and thyroid carcinoma secondary to nodular goiter in iodine deficiency area. MethodsA total of 216 specimens of goiter patients from iodine deficiency area were collected in the former 3rd hospital of Norman Bethune Medical College from January 1980 to December 1994. Twentyfour heteroploid samples were selected by the method of Hedley with Flow cytometry (FCM) analysis. Paraffin-embedded tissues from the same position were used to perform immunohistochemical staining for proliferating cell nuclear antigen (PCNA), laminin (LN), factor Ⅷ related antigen (FⅧ-RAg), and p53. The proliferative activity, stroma change, and angiogenesis were observed. ResultsPCNA label index (PCNA-LI) and proliferation index (PI) consistent in 24 heteroploid samples with PCNA staining were significantly higher value. PCNA positive cells were mainly distributed over nonfollicular parenchymatous structures, small follicles, and multilayered structures with large bubbly follicles. Destroyed basement membrane and necrosis were found by LN staining in PCNA positive position with vigorous reproductive capacity. Combining FⅧ-RAg staining with LN staining, interstitial proliferation and angiogenesis were obvious in follicular epithelial cells with vigorous reproductive capacity, providing nutrition and superior environment for them. ConclusionsThe reproduction of thyroid follicular epithelial cells, interstitial proliferation, and angiogenesis are all involved in tuberosis and hyperthyroidism, forming precancerous lesion, which suggest the succession model of goiter in iodine deficiency area.

  • The advances of ultrasound-guided thermal ablation for the treatment of papillary thyroid microcarcinoma

    Objective To summarize the progress of the application of ultrasound-guided thermal ablation for treatment of papillary thyroid microcarcinoma (PTMC). Methods The relevant literatures of thyroid nodules treated by ultrasound-guided thermal ablation were reviewed by adopting the methods of literature review. Results In conditions of grasping the therapeutic indication strictly and evaluating preoperative various aspects sufficiently, it reveals a certain feasibility and validity applying ultrasound-guided thermal ablation in the treatment of PTMC classified in the low-risk group. Conclutions Possessing the advantages of minimal invasive techniques, low-risks, beauty and rapidness, ultrasound-guided thermal ablation might be recommended as an alternative to a low-risk PTMC patient who is at high risk in general anaesthesia operation or intolerant to open operation.

  • Effects of DNA Electrotransfer in Muscle on the Implanted Tumor Growth in Nude Mice

    【Abstract】ObjectiveTo study the antitumor effects of DNA electrotransfer in muscle (ETM) by using established animal model for human anaplastic thyroid cancer cell line TA-K.MethodsNude mice with implanted TA-K were divided into five groups including: control group, pcDNA-3 plasmid electrotransfered into muscle (pcDNA ETM group), TIMP-3 plasmid injected into muscle (TIMP-3 IM group), TIMP-3 plasmid electrotransfered into muscle (TIMP-3 ETM group), TIMP-3 plasmid electrotransfered into implanted tumor (TIMP-3 ETT group). Electrical parameters used in electrotransfer were: 200 V/cm, 20 ms/pulse; 8 pulses, 1 Hz in muscle and 600 V/cm, 20 ms/pulse; 1 pulses, 1 Hz in implanted tumor respectively. ResultsThe growth of TA-K was inhibited more significantly in the groups of TIMP-3 plasmid electrotransfered into muscle and TIMP-3 plasmid electrotransfered into implanted tumor than the other groups (P<0.05), and the content of TIMP-3 protein in tumor tissues was higher in these two groups . ConclusionAnti-oncogene has the antitumor effects by DNA electrotransfer in muscle.

  • Construction of financial toxicity early warning model for breast cancer patients undergoing daytime chemotherapy under diagnosis intervention packet

    ObjectiveTo explore the current situation of financial toxicity (FT) of breast cancer patients undergoing daytime chemotherapy under the background of diagnosis intervention packet (DIP) and its influencing factors, and to build a risk early warning model.Methods Convenient sampling method was used to select breast cancer patients undergoing chemotherapy in the daytime ward of Tianjin Medical University Cancer Institute & Hospital between April and May 2022. The general data questionnaire and FT comprehensive score scale were used to investigate them, and the influencing factors of patients’ FT were discussed through single factor analysis and logistic regression analysis, and the risk early warning model was established. Hosmer-Lemeshow fitting effect test was used to evaluate the prediction effect of the model.Results A total of 278 patients were included. The median (lower quartile, upper quartile) of FT score was 14.00 (8.75, 23.00), of which 195 patients (70.14%) had FT score≤22; 83 patients (29.86%) had FT scores>22. Logistic regression analysis showed that age, per capita monthly income of families, commercial health insurance, chemotherapy cycle, tumor stage, neoadjuvant chemotherapy were the influencing factors for high-risk FT of breast cancer patients undergoing daytime chemotherapy. The results of Hosmer-Lemeshow goodness of fit test showed that the model-predicted FT of breast cancer patients undergoing daytime chemotherapy was in good agreement with the actual observation value (χ2=10.685, P=0.220). The area under the curve of the model was 0.931 [95% confidence interval (0.900, 0.962)], the sensitivity was 0.807, and the specificity was 0.913.Conclusions The FT of breast cancer patients undergoing daytime chemotherapy is at a high level. Older age, purchase of commercial health insurance, and high per capita monthly income of families are protective factors for high-risk FT. The wind with chemotherapy cycle≤4 weeks, tumor stage Ⅱ, neoadjuvant chemotherapy are high-risk FT risk factors. The final warning model has been tested to have a good prediction effect, which can provide a reference for clinical medical staff to identify high-risk FT patients early and make preventive strategies as soon as possible.

  • Clinical trial of perindopril influence for the complication in hypertensive patients with stroke history by lowering blood pressure

    Objective To investigate perindopiI influence for the complication in hypertensive patients with stroke history by lowering blood pressure. Methods One-hundred and fifty-five essential hypertensive patients with stroke history were randomly assigned to either perindopril group or placebo group, receiving a randomized, double-blind, placebo-controlled trial as long as 3 years. Their blood pressure, the morbidity and mortality of cardiocerebral complications were monitored. At the end of trial, three days Ambulatory Blood Pressure Monitoring were performed. Result It was shown that in perindopril group blood pressure was controlled satisfactorily, and the morbidity and mortality of cardiocerebral incidences (6.85 % and 2.74 %repectively) were lower than those of placebo group (17.10% and 9.21% respectively). Conclusions Perindopril lowers blood pressure effectively and persistently. It has important clinical effect for prevention the cardiocerebral complications in hypertensive patients with stroke history by lowering blood pressure.

  • Significance of UltrasoundGuided Core Needle Biopsy in Diagnosis of Thyroid Nodules

    ObjectiveTo explore the value of ultrasound-guided core needle biopsy (CNB) in diagnosis of thyroid nodules. MethodsThe clinical data of 347 patients with thyroid nodules who underwent ultrasoundguided CNB were retrospectively analyzed, and the results of CNB pathology were compared with postoperative wax pathology results. ResultsAll patients completed CNB successfully and satisfaction rate for tissue samples was 100%. After CNB, local hematoma occurred in two cases and relieved by conservative therapy. The CNB pathology results of 347 cases of patients were as follows: 117 cases were malignancy, including papillary thyroid cancer in 115 cases, undifferentiated adenocarcinoma in 1 case, and squamous cell carcinoma in 1 case; 230 cases were benign, including thyroiditis in 53 cases, and nodular goiter in 141 cases, adenoma in 16 cases, and nodular goitre coexisting thyroiditis in 20 cases. In 132 cases of patients underwent surgery including 113 cases of malignancy and 19 cases of benign disease, the CNB pathology results in 127 cases were consistent with postoperative wax pathology results and false negative occurred in 5 cases. The diameter of thyroid nodules were not more than 0.5 cm in 4 cases, 0.5-1.0 cm in 59 cases, 1-2 cm in 46 cases, and more than 2 cm in 23 cases, and the accuracy rate of CNB pathology results was 75.0%(3/4), 98.3%(58/59), 97.8%(45/46), and 91.3%(21/23), respectively, which was the highest in 0.5-2.0 cm. The accuracy, sensibility, specificity, positive predictive value, negative predictive value, failure rate, and misdiagnosis rate of ultrasound-guided CNB for differential diagnosis of thyroid malignant nodules from benign nodules were 96.21% (127/132), 95.76% (113/118), 100% (14/14), 1 (113/113), 0.74 (14/19), 4.24%(5/118), and 0 (0/14), respectively. ConclusionUltrasound-guided CNB has important value on differential diagnosis of thyroid nodules, and important guiding significance on treatment of thyroid diseases.

  • Study on indicator system of quality and safety of day surgery based on structure-process-outcome theory

    Since 2015, when the day surgery mode was officially reckoned and supported by the National Health Commission, the day surgery model has entered the new period of rapid development. But at the same time, challenges in the vacancy of the evaluation indexes of the medical quality and safety of day surgery still pose obstacles to its growth. At present, there is no nationally unified evaluating index or appraising system for the day surgery-related medical quality and safety. In this paper, based on structure-process-outcome theory, the day surgery practice and involved researching literature were retrieved, reviewed, and analyzed. Also, the practice model at West China Hospital of Sichuan University (a pioneer day surgery hospital in China) was comprehensively compared with the National Clinical Improvement System. From the aspect of day surgery medical quality and safety evaluation indicators, it is hoped to provide an evidence-based method and evaluation of day surgery, and a theoretical basis for establishing policies and data reference.

  • Long-Term Primary Culture of Human Papillary Thyroid Carcinoma Cells

    ObjectiveTo investigate the primary culture method of human papillary thyroid carcinoma (PTC) cells for a long term and establish a monitoring and verification measures. MethodsPTC cells were isolated following routine procedures and cultured in the DMEM supplemented with 10% fetal bovine serum, glutamine, and 20 ng/ml epidermal growth factor (EGF). Thyroglobulin (Tg) and thyroperoxidase (TPO) in nutrient solution and specific antigen Tg expression of PTC cells cultured for different days were observed. ResultsThe PTC cells grew satisfactorily up to 45 days of incubation. Tg content in nutrient solution expressed the training period of a linear singular parabolic, achieved peak value (985.2 μg/L) at about 14 d. TPO had not been detected in nutrient solution. The Tg expressed positively by immunization fluorescent dyeing. ConclusionsPTC cells cultured in the present method can survive to over 45 days. A brief monitoring and evaluation systems of PTC cells has been established. This report prompts that cultured cells within 14 days maybe more suitable to gene research and provide alternative to the basic research of PTC events and features.
