  • 碱性成纤维细胞生长因子和硫糖铝联合局部应用对扩张皮肤组织结构的影响

    OBJECTIVE: To investigate the effect of local application of basic fibroblast growth factor (bFGF) and sucralfate on the skin tissue structure after expansion. METHODS: Continuous tissue expansion (CTE) experimental animal model was made with constant pressure pump in nine white pigs. The bFGF and sucralfate were perfused between tissue and tissue expander. Both bFGF and sucralfate were injected in group I, both bFGF and normal saline in group II, only sucralfate in group III, and normal saline in group IV as control group. The samples were took from each pig for histomorphological assessment on the 3rd day and 6th week after expansion. RESULTS: Compared with control group, epidermal, granular spinous and basal cell thickness increased significantly (P lt; 0.05) in group I; in dermal layer, dermal thickened slightly and the collagen fibers became thicker and orderly; the elastic fiber regenerated significantly; the density of fibroblasts and capillaries significantly increased (P lt; 0.05) in group I. The collagen fibers were broken on the 3rd day and 6th week after expansion in control group. However, the thickness of the fibrous capsule layer was similar in each group. CONCLUSION: Local application of exogenous bFGF and sucralfate combined with CTE can greatly promote the skin growth. It can accelerate the tissue expansion.

  • THE EFFECT OF ANTI-EXUDATE OF WOUND WITH PERMANENT MAGNETIC FLAT DRESSINGS

    An experimental study was designed on the treatment of 8 superficial burn wounds and 5 wounds after removing the split-thickness skin grafts with 200GS permanent magnetic flat dressings, and some of the wounds were chosen at random which did not received any treatment and would be served as control. It was noted that the volume of exudate in the treatment group was 11.9 50.7% less than that of the control group, and the total protein, the albumin and globulin contents in the treatment cases were far less in exudate than those of the control cases. It was concluded that the magnetic therapy not only decresed the exudate, but also diminished the amount of total protein, albumin and globulin in the exudating fluid and it was of benefit to wound recovery.

  • Significance and Surgical Skill for Lymphadenectomy Around Common Hepatic Artery in Gastric Cancer

    Objective To investigate the significance and surgical skill for lymphadenectomy around common hepatic artery in gastric cancer. Methods Two hundred and fifty-seven cases with undergoing lymphadenectomy around common hepatic artery in gastric cancer between January 2001 and December 2006 were retrospectively reviewed. Experiences and understanding of dealing with this procedure in curative gastrectomy for gastric cancer were concluded. Results The number of dissected No.8 lymph node was 2.2±1.7, and the positive rate of No.8 lymph node was 30.35%. There were no lymphadenectomy related complications, such as anastomotic leakage, lymphatic fistula and postoperative hemorrhage in this series. Dissection around common hepatic artery lymph nodes along artery intrathecal space, division and ligation of left gastric vein at its root, and sufficient exposure of anatomic structures were important to dissection lymph nodes around the common hepatic artery. The exposure of anatomic structures included liberation of common hepatic artery by traction with the band, and the exposure of posterior side of pancreas by Kocher incision.Conclusion Being familiar with the anatomy around common hepatic artery, careful dissection, and attention to the surgical skill of lymphadenectomy are very important to improve the effectiveness of lymphadenectomy around common hepatic artery in gastric cancer.

  • EVALUATION OF HUMAN GROWTH HORMONE ON GASTRIC AND COLONIC CANCER PATIENTS AFTER SURGERY

    To study the effects of human growth hormone on protein catabolic state of gastric and colonic cancer patients after surgical intervention and whether it can improve the postoperative host immune function and reduce the postoperative fatigue syndrome (POF) by using rhGH. Thirtyeight gastric and colonic cancer patients (21 cases of gastric cancer; 17 cases of colonic cancer) were diveided into control group (n=18) and rhGHtreated group (n=20). All the patients were performed resection and treated by early postoperative intraperitoneal thermochemotherapy (EPIC) and total parenteral nutrition (TPN). Subcutaneous injections of 8 U rhGH at 9∶30 am was administered to the rhGHtreated group (six days) at the same time. Results: In the control group, a significant decrease in serum levels of albumin, prealbumin, transferri, IgG, IgA, IgM and CD+3, CD+4, CD+8 were observed after operation (P<0.01). In the rhGHtreated group, CD+3, CD+4 and CD+8 raised significantly and the other did not change significantly. The postoperative vigour state of the patient was better than that in the control group. In the control group, pronouced weight loss of 3-5 kg, was detected on the 10th pastoperative day, while the weight loss was 1-2 kg in the rhGHtreated group (P<0.01). Conclusion: The treatment with rhGH together with TPN and EPIC not only overcomes the protein catabolism of the cancer patient after operation by increasing protein synthesis, but also improves postoperative host immune function, reduces POF, and can raise the killing effect of chemotherapy on cancer cells, enhances the tolerance to chemotherapy.

  • OBSERVATION OF RECONSTRUCTION OF WRIST JOINT BY REPLACEMENT OF DISTAL END OF RADIUS WITH VASCULARIZED FIBULAR HEAD

    OBJECTIVE This paper is aimed to observe the long-term result of reconstruction of wrist joint by replacing distal end of radius with vascularized fibular head in patient of giant tumor. METHODS From July 1978 to November 1993, 12 cases of giant tumor of distal end of radius were treated by this procedure and followed up for 6 months to 15 years, and the movement range of wrist, grip strength, and pain were evaluated. RESULTS The painful wrist released in 11 cases, only one case still existed mild pain and felt discomfortable. The average range of movement of wrist joint was 34.05 degree in volar flexion, 52.80 degree in dorsal extension, 26.25 degree in ulnar deviation, and 19.75 degree in radial deviation, and average grip strength was 44.6 kg. Compared with the contralateral side, there were accounted for 46.2%, 72.7%, 76.1%, 80.6%, and 76.7% respectively. CONCLUSION Replacement of distal end of radius with fibular head to reconstruct wrist joint can restore function of carpal joint, which is proved to be a safe and effective method.

  • Research Progress of Cerebral Protection Strategy in Aortic Arch Surgery for Adults——Moderate Hypothermic Circulatory Arrest with Selective Antegrade Cerebral Perfusion

    Increasing evidences show that a gradual trend away from deep hypothermia toward moderate hypothermic circulatory arrest, which has been proved to be safe and effective in clinic. By summarizing and analyzing the research progress and applying status of the moderate hypothermia circulatory arrest with selective antegrade cerebral perfusion, the article aims at promoting the application of this tenique as a cerebral protection strategy in aortic arch surgery for adults in China.

  • Clinical Analysis of Peutz-Jeghers Syndrome and Its Canceration

    Objective To analyse the clinical features of Peutz-Jeghers syndrome and its canceration, and to summarize the management and the follow-up strategy for patients with Peutz-Jeghers syndrome. Methods Clinical data of 30 patients with Peutz-Jeghers syndrome dating from October 1985 to September 2005 in West China Hospital of Sichuan University were analyzed retrospectively. Results Fourteen (46.67%) definite family histories of Peutz-Jeghers syndrome were found. Pigmentation of skin and mucosa, abdominalgia and hematochezia were major clinical manifestations of the syndrome. There were 18 patients (60.00%) complicated with intussusception and acute intestinal obstructions, 16 patients (53.33%) with gastrointestinal bleeding, and 6 patients (20.00%) developed malignancy. The frequent types of malignancy were carcinoma of small intestine (3 cases), colon carcinoma (2 cases) and gastric carcinoma (1 case) in order. The mean age of the canceration-diagnosed patients was 32 years old. The type of pathohistology of all the malignancy was poorly differentiated mucus adenocarcinoma. High-frequency endoscopic electroresection, orthdox polypectomy and enterectomy were the major means of treatment. Conclusion Patients with Peutz-Jeghers syndrome are at high risk of canceration at relatively early ages and usually the differentiation of the tumor is poor. Endoscopy should be performed regularly and the high-frequency electroresection is an effective therapy in disposing intestinal polyp. Screening can also improve the efficacy of Peutz-Jeghers syndrome.

  • EXPERIMENTAL AND CLINICAL STUDY ON LUMBRICUS ACCELERATING POSTOPERATIVE WOUND HEALING AFTER HEMORROIDECTOMY

    Objective To probe the effects of lumbricus on wound healing after hemorroidectomy.Methods After the solution made from artificial grown lumbricus was sprung to the wound of animal and patient after hemorroidectomy, the wound inflammation, wound healing and changes of laboratory determinations were observed and compared to those of the control group. Results In animal study, lumbricus could inhibit the growth of staphylococci, bacillus coli and bacillus aeruginosus. The time of wound healing in experimental group was 4 days shorter than that in control group. At 4d and 7 day the numbers of the capillary, blood vessel endodermis and desmohemoblast desmocyte and splitting epithelium of trial group were much more than those of control group. At 4d the trial group′s numbers of splitting mesenchymal cell were much more than that of control group. From 3d on the wound healing and granulation filling of experimental group were much quicker than those of control group. In clinical study, the time of wound healing of trial group ( a mean of 16.5 days) was shorter than that of control group (21.2 days). From 3d on, the epidermis′ growth speed of the trial patients was much quicker than that of control group and was without wound infection and granulation overproliferation. Conclusion Lumbricus can inhibit wound inflammation response and accelerate wound healing. Lumbricus is inexpensive and easily preserved, and could be used on the wound after hemorroidectomy to accelerate wound healing.

  • Analysis of the Surgical Treatment for Apical Muscular Ventricular Septal Defects

    Objective To discuss the methods and results of the surgical treatment of apical muscular ventricular septal defects (VSD) , in order to improve the surgical outcome of this disease. Methods The clinical data of 29 patients with apical muscular VSD of Fuwai hospital through Jan. 1999 to July 2004 were analyzed retrospectively. The apical VSDwere repaired via 4 different approaches: right atrium, apical right ventriculotomy, apical left ventriculotomy and the incision of the outflow tract of right ventricle combined with apical left ventriculotomy. The operative outcomes between different approaches were analyzed and compared. Results There were 2 perioperative deaths (6. 9%). One patient repaired via apical left ventriculotomy died of low cardiac output syndrome, the other died intraoperatively from endocardial fibroelastosis, which was confirmed by myocardial biopsy. There were residual VSD in 4 (13. 8%) patients, who were repaired via the right atrium, which did not necessitate reoperation. Other patients were discharged uneventfully. Follow up was completed {n 11 patients and extended 2 to 34 months, none of the 6 survivors had apical ventricular aneurysms. Conclusions The operative outcome of patients with apical VSD is satisfactory although the incidence of residual VSD is higher. The intraoperative transesophageal echocardiography (TEE) should be performed routinely. Surgical approach should be chosen on the actual site of the VSD at operation. Try to repair the VSD via the right atrium as possible, via the apical right ventrieulotomy if needed and try to avoid left ventriculotomy.

  • Surgery Treatment of Chronic Moderate Ischemic Mitral Regurgitation in Coronary Artery Disease

    Objective To investigate the treatment and prognosis of moderate ischemic mitral regurgitation (IMR) in coronary artery disease(CAD). Methods From January 1998 to May 2006, 28 patients of CAD with moderate IMR underwent coronary artery bypass grafting (CABG) and mitral valve plasty(MVP, 24) or mitral valve replacement (MVR,4). The Reed method were used in 9 cases, the annuloplasty ring were used in 15 cases. Mechanical valve were implanted in 1 case and biological valve in 3 cases. Results There was no operative or hospital death. Twentysix patients were followed up to a mean period of 41 months. There were two late death(one was MVP, the other was MVR). In MVP cases, nineteen patients were in New York Heart Association (NYHA) functional class Ⅰ and Ⅱ, 3 in class Ⅲ, which was better than that of preoperative one. Ultrasonic cardiography (UCG) examination showed no mitral regurgitation in 5 cases, mild in 7, light in 6, moderate in 3, severe in 1. Left atrial volume (LAV) and left ventricular enddiastolic volume (LVEDV) were 54.1±12.7ml and 60.9±14.8 ml, decreased more significantly than that preoperatively (Plt;0.05). In MVR cases, 2 cases were survival and followed. One patient was in NYHA functional class Ⅰ, 1 in class Ⅱ, which was better than that of preoperative one. Conclusion Moderate IMR with CAD should be treated carefully. MVP with annuloplasty ring have better early results. For patients with bad heart function and abnormal left ventricular wall motion, the late results need more studies.
