  • The Equipment and the Working Model of Pakistan International Medical Rescue Team in Wenchuan Earthquake

    Objective To analyze the equipment and the working model of Pakistan international medical rescue team in Wenchuan earthquake in order to provide the first hand references for our medical rescue. Methods We analyzed the aim, the staff composition, the medical equipment, the logistic material and the working model of the Pakistan international medical rescue team by working with the Pakistan team, and summarized its strengths and weaknesses. Result The main aim of the Pakistan international medical rescue team is to search, rescue and treat the wounded and sick in mass casualty incident. The team consisted of 28 staffs, including 2 leaders (the executive leader was an epidemic expert, and the vise leader was an administrative officer), 6 clinical doctors, 2 nurses, 3 medical technicians, 10 field search and rescue staffs and 5 logistics staffs. The medical equipment included a field ambulance, 7 field tents and 2 marine satellite phones. The medical equipment and instruments include an X-ray table, a B-ultrasound scanner, an ECG tester, a set of surgery operation equipment and 2 sets of ICU instruments. The main medicines included antibiotics, analgesics and for trauma treatment. Narcotic drugs and surgical supplies supported 200 patients; fluid infusion and drugs were enough for 500 patients; other laboratory reagents were enough for 500 patients; oral drugs, disinfection cotton and bandages were for 3 000 patients. Logistics materials contained a set of cooking appliances, 2 sets of toilet facilities and enough rice, flour and water for the whole team for two weeks. The field tent hospital was built in one day. In the field tent hospital, 1 256 patients were treated, 14 surgeries were performed, 236 X-ray and B-ultrasound examinations as well as 314 biochemical examinations were conducted within 10 days. Conclusion With complete disciplines and rational structure, the configuration of the Pakistan team is fitful for its aim, and it balances the function of search, rescue and public health. Apart from equipment and instruments enough for 300-500 patients rescue, the team also considers both its own survival and environmental protection. The outcome would be better if they could arrive at the spot in the first week after the earthquake happened.

  • Evidence-Based Analysis of Nosocomial Infection in Surgery Department of National Hospital

    Objective To investigate on the epidemiologic characteristics of nosocomial infection in surgery departments of general hospitals by analyzing the data collected from documents which were published in recent years, so as to provide references for the construction of precautionary system model. Methods Applying comprehensive search strategies, we searched various electronic databases as CBM (1978 to 2008), CNKI (1912 to 2008), VIP (2001 to 2009) and WanFang Data (2001 to 2009). MeSH terms and/or text words included: nosocomial infections, cross infection, hospital infection, prevent and control. Data from top and second grade hospital were included in this analysis. Results Sixty four articles and a total of 1 990 929 inpatients were included. Results showed: average nosocomial infection rate was 4.46%; the total rates of medicine department and surgery department were 23.28% and 17.33% respectively and no significant difference was found between the two departments; the infection rates of G– and G+ germ were 47.71% and 21.31% respectively; the rates of average antibiotics use was 60.59% and the rate of missing report was 12.42%. Noscomial infection was related to season change and the wave peak was from February to May. Conclusion Most of the included studies were retrospective studies and cross-sectional studies. The type of data was inconsistency and incomplete, causing weak strength of evidence. High missing rate of reports makes the precautionary model hard to build in future.

  • Analysis on Operation Date in Clinical Path Designing of Cleft Lip-palate in Gansu Province

    Objective To analyze the significance of operation date in clinical path designing of cleft lip-palate. Methods The case records of cleft lip-palate patients from 8 hospitals in Gansu province were collected from 2005 to 2008. By means of comprehensive analysis of case records and frequencies of hospitalization duration, analyzed the influence of operation date selection on hospitalization duration in clinical path designing. Result In Gansu province, the average hospitalization duration of cleft lip-palate was 11 days, and the operation was usually done at the sixth day. The main preparations for operation were kinds of examinations. Within five days after operation, most treatments were postoperative care, diet and antibiotic therapy. Conclusion The strategy of operation date selection is much important to ascertain the real hospitalization duration in clinical path designing.

  • Evidence-Based Evaluation of Medical Risk Management System in UK and Its Application to Medical Risk Management in China

    Objective To analyze experiences of medical risk management in the United Kingdom so as to explore the possible application for the construction of a Chinese medical risk monitoring and early warning system. Methods We searched Engineering Information, SCI and SSCI, EMBASE, SCOPUS with 100% MEDLINE, VIP, CNKI, and government or official websites. This search was conducted in Jan. 2006. We included articles about medical risk, patient safety and medical errors in the UK. Languages of articles were limited either in English or in Chinese. Results Eleven articles were included, of which 9 article are evidence of level B (about 80%) and the other 2 are evidence of level C (about 20%). The report of “An Organization with a Memory” revealed the severity of medical errors and adverse events in the UK in 2000, and subsequently Minister Blair announced a five-year reform program for NHS. Within 7 years of reform, NHS budget has been increased from £33 billion to £674 billion,(check numbers-doesn’t sound correct) the National Patient Safety Agency (NPSA) and the New National System for learning from adverse events and near misses have been established, a series of practicable measures aimed at ensuring patient safety, preventing medical risk and improving healthcare quality have been implemented, all of which have effectively resolved many problems that perplexed the government and public, such as patients waiting time, range of NHS service, the availability of medical facility and mortality induced by high-risk diseases. Conclusion There are both advantages and disadvantages in the present status of the UK medical risk management. Both of them will provide a guide to prevent medical risk, improve healthcare quality and to realize the ultimate goal that everybody could share healthcare sources fairly and safely in our country.
