  • SURGICAL MANAGEMENT OF LIMB SALVAGE FOR OSTEOGENIC MALIGNANT TUMORS AROUND KNEES

    Objective To probe a satisfactory surgical management of the limbsalvage for osteogenic malignant tumors around the knees. Methods From January1989 to December 2001, 42 patients (19 males and 18 females, aged 12-46) with osteogenic malignant tumors around the knees underwent surgical management of the limb salvage, including prosthesis replacement, allogenous bone grafting, and bone cement with adriamycin filled. Based on the pathological examination, osteosarcoma was found in 11 patients, synoviosarcoma in 4 patients, malignant fibrous histiocytoma in 3 patients, and giant cell tumor of the bone in 19 patients. All the patients underwent neoadjuvant chemotherapy for 1-2 courses before operatioexcept the patients with giant cell tumor of the bone. The patients underwent prosthesis replacement, allogenous bone grafting, bone cement with adriamycin filled, and postoperative chemotherapy. By the Enneking evaluating system, the patients were assessed on their reconstructed limb functions after the reconstructive operation for the musculoskeleta malignant tumors. Of the patients, 37 were followed up after operation. Results According to the follow-up for 3-11 years (mean, 5.6 years) in the 37 patients, 2 patients had recurrence and metastasis of the tumor, and died 3 and 4 years after operation, respectively. One patient underwent amputation of the limb for local recurrence of the tumor. One patient had amputation of the limb for the preoperative radiotherapy and the infection and necrosis of the operative wound after the limb-salvage surgery. Two patients had amputation of the limb for the rejection of the allogenous bone graft and theformation of the fistula.Thirty-one patients had good wound healing andgood functions of the limb. The results were evaluated by the Enneking evaluating system as follows: excellent in 7 patients, good in 14, fair in 10, and poor in 6. Conclusion Making an early diagnosis, recognizing the operative indication, choosing the operative method, and performing the preoperative and postoperative chemotherapy and/or radiotherapy are the keys to achieving an ideal limb-salvage surgery for osteogenic malignant tumors around the knees. 

  • Application of anterior subcutaneous internal fixator combined with posterior plate in treatment of unstable pelvic fractures

    ObjectiveTo investigate the effectiveness of anterior subcutaneous internal fixator combined with posterior plate in the treatment of unstable pelvic fractures.MethodsBetween January 2015 and January 2019, 26 cases of unstable pelvic fractures were treated with anterior subcutaneous internal fixator combined with posterior plate. There were 16 males and 10 females, with an average age of 42.8 years (range, 25-66 years). According to the Tile classification, 9 of them belonged to type B2, 6 to type B3, 7 to type C1, 3 to type C2, 1 to type C3. The injury severity score (ISS) was 6-43 (mean, 18.3). Four cases combined with brain injury, 7 with limb fractures, 3 with hemopneumothorax, 1 with sciatic nerve injury. The time from injury to operation was 4-12 days (mean, 6.4 days). The intraoperative blood loss, operation time, and the complications were recorded. The fracture reduction and the postoperative function of patients were evaluated.ResultsAll patients were followed up 12-26 months (mean, 16.8 months). The operation time was 65-142 minutes (mean, 72.5 minutes) and the intraoperative blood loss was 42-124 mL (mean, 64.2 mL). There were 2 cases of unilateral lateral femoral cutaneous nerve stimulation, 1 case of femoral nerve paralysis, and 1 case of superficial infection of incision, which were cured after corresponding treatment. X-ray films showed that all fractures healed at 3 months after operation. At last follow-up, according to Matta criteria for fracture reduction, the results were excellent in 8 cases, good in 15 cases, fair in 2 cases, and poor in 1 case, with an excellent and good rate of 88.5%. According to Majeed scoring system for pelvic function, the results were excellent in 10 cases, good in 12 cases, and fair in 4 cases, with an excellent and good rate of 84.6%.ConclusionFor unstable pelvic fractures, the anterior subcutaneous internal fixator combined with posterior plate has fewer operative complications, high security, and achieve good effectiveness.

  • Effect of “noncycloplegic retinoscopy for screening myopia + subsequent wearing orthokeratology lens” process for primary and secondary school students based on physical examination center

    ObjectiveTo investigate the effect of “noncycloplegic retinoscopy for screening myopia + subsequent wearing orthokeratology lens” process for primary and secondary school students based on physical examination center.MethodsA total of 172 primary and secondary school students undergoing vision examination in the Health Management Department (i.e. physical examination center) of the Second Affiliated Hospital of Xi’an Jiaotong University between January 2017 and December 2018 were selected as the research objects. After examination by noncycloplegic retinoscopy in the health management department, they were examined by mydriatic retinoscopy in the ophthalmology department, and then the consistency of the results of the two methods was analyzed. Then 93 students with myopia diagnosed by both methods were randomly divided into control group (n=46) and trial group (n=47), wearing frame glasses and orthokeratology lenses respectively. The diopter, eye axial length, corneal curvature and vitreous cavity depth before wearing glasses, as well as the increment of the above indicators at 3, 6 and 12 months after wearing glasses/lenses were compared between the two groups, and the incidence of complications of the two groups were compared.ResultsCompared with mydriatic retinoscopy, the positive predictive value of noncycloplegic retinoscopy was 88.6%, the sensitivity was 96.9% and the specificity was 84.2%, and the consistency kappa coefficient was 0.821 (P<0.001). Before wearing glasses/lenses, there was no significant difference in diopter, eye axial length, corneal curvature or vitreous cavity depth of both eyes between the two groups (P>0.05); at 3, 6 and 12 months after wearing glasses/lenses, the diopter increment and eye axial increment of both eyes of the trial group were less than those of the control group [left eye diopter increment: (0.48±0.07) vs. (0.73±0.08) D, (0.69±0.13) vs. (1.04±0.11) D, (0.88±0.11) vs. (1.13±0.11) D; left eye axial increment: (0.18±0.05) vs. (0.26±0.04)mm, (0.22±0.04) vs. (0.36±0.04) mm, (0.27±0.05) vs. (0.40±0.05) mm; right eye diopter increment: (0.46±0.10) vs. (0.73±0.09) D, (0.71±0.12) vs. (1.04±0.10) D, (0.90±0.10) vs. (1.17±0.11) D; right eye axial increment: (0.17±0.04) vs. (0.24±0.04) mm, (0.23±0.04) vs. (0.37±0.04) mm, (0.26±0.05) vs. (0.42±0.05) mm] (P<0.05). At 3, 6 and 12 months after wearing glasses/lenses, the changing trends of corneal curvature and vitreous cavity depth in both eyes of the trial group were different from those of the control group (PInteraction<0.05), and the corneal curvature of both eyes at each time point was lower than that before wearing lenses and that of the control group (P<0.05), while the vitreous cavity depth of both eyes was not statistically different from that before wearing lenses (P>0.05) but lower than that of the control group (P<0.05). There was no statistical difference in the incidence of common complications between the two groups (P>0.05).ConclusionIn the physical examination center, the accuracy of noncycloplegic retinoscopy for general survey of myopia in primary and secondary school students is high, and then after the diagnosis by mydriatic retinoscopy in the special department, the use of orthokeratology lens can effectively control the progress of myopia, so the process of “noncycloplegic retinoscopy for screening myopia in physical examination center + subsequent wearing orthokeratology lens in specialty” is feasible.
