  • Long-term outcomes of surgical closure of atrial septal defects and concomitant surgical radiofrequency ablation for atrial fibrillation

Objective To evaluate long-term outcomes of surgical closure of atrial septal defect (ASD) and combined surgical radiofrequency ablation for atrial fibrillation (AF). Methods A total of consecutive 15 patients with ASD undergoing surgical closure of ASD and combined surgical radiofrequency ablation in our department between March 2003 and April 2015. There were 7 males and 8 females at an average age of 47.1±10.8 years ranging from 16 to 62 years. Retrospective analysis and follow-up were performed to evaluate long-term success rate freedom from AF after surgery. Results All patients recovered and discharged, and no patient suffered death or stroke. The duration of follow-up was from 3 to 136 months for all patients. Success rate freedom from AF at 1, 3, 5 and 10 years was 81.3%, 75.0%, 68.8% and 61.1%, respectively. During follow-up, there was no death or stroke. One patient required permanent pacemaker implantation. Conclusion Concomitant surgical closure of ASD and biatrial radiofrequency ablation is safe and effetive with better long-term outcomes. It is necessary to perform the two procedures together for ASD patients.

  • Establishment of Duodenal-Jejunal Bypass Animal Model in Goto-Kakizaki Rat

Objective To explore the feasibility and operation points of establishing duodenal-jejunal bypass (DJB)surgery animal model in Goto-Kakizaki (GK) rats. Methods Sixteen GK rats were randomly divided into experimental group (n=8) and control group (n=8). In a standardized preoperative, intraoperative, and postoperative operation, the rats of experimental group and control group received DJB and sham surgery respectively. The fasting plasma glucose and body mass were observed before operation, and 1, 2, 3, and 4 weeks after operation in order to evaluate whether the models were established successfully. Survival situation of rats were observed too. Results All experimental rats survived at 4 weeks after the operation. Compared with the levels before operation, the fasting plasma glucose levels of experimental group decreased significantly (P<0.05) at 1 week after operation, and remained stable at 2, 3, and 4 weeks after operation. The fasting plasma glucose levels of control group did not change statistically at all time points after operation (P>0.05). Compared with control group at the same time point, the fasting plasma glucose level of experimental group was lower (P<0.05), indicating that DJB models were established successfully. After 4 weeks, the value of body mass added in experimental group was significantly lower than those of control group (P<0.05). Conclusions DJB is a feasible, safe, and effective hypoglycemic surgery. The application of this set of experimental operating procedures can reduce the risk of intraoperative and postoperative mortality, and can develop a stable DJB model in Goto-Kakizaki rats.

  • A fusion network model based on limited training samples for the automatic segmentation of pelvic endangered organs

When applying deep learning to the automatic segmentation of organs at risk in medical images, we combine two network models of Dense Net and V-Net to develop a Dense V-network for automatic segmentation of three-dimensional computed tomography (CT) images, in order to solve the problems of degradation and gradient disappearance of three-dimensional convolutional neural networks optimization as training samples are insufficient. This algorithm is applied to the delineation of pelvic endangered organs and we take three representative evaluation parameters to quantitatively evaluate the segmentation effect. The clinical result showed that the Dice similarity coefficient values of the bladder, small intestine, rectum, femoral head and spinal cord were all above 0.87 (average was 0.9); Jaccard distance of these were within 2.3 (average was 0.18). Except for the small intestine, the Hausdorff distance of other organs were less than 0.9 cm (average was 0.62 cm). The Dense V-Network has been proven to achieve the accurate segmentation of pelvic endangered organs.
