  • Research status of valvular thrombosis after transcatheter heart valve replacement

    In recent years, the field of transcatheter heart valve interventional therapy has developed rapidly. Valvular thrombosis is a rare postoperative complication, which can affect valvular function early or lead to clinical embolic events, and is gradually being valued by surgeons. The clinical manifestations of thrombosis after different types of interventional valve replacement are different. Although anticoagulant therapy is believed to be effective for valve thrombosis, the selection of anticoagulant drugs and the duration of anticoagulation are still controversial. This article reviews the definition, clinical features, prevention and treatment of valve thrombosis after several types of transcatheter heart valve replacement, mainly related to transcatheter aortic valve replacement and transcatheter mitral valve replacement, and aims to provide a reference for the diagnosis and treatment of valve thrombosis after transcatheter heart valve replacement.

  • Type Ⅱ endoleak originating from inferior mesenteric artery after endovascular abdominal aortic aneurysm repair: A retrospective analysis in a single center

    Objective To investigate the management experience of type Ⅱ endoleak originating from inferior mesenteric artery (IMA) after endovascular abdominal aortic aneurysm repair (EVAR). Methods The clinical data of patients with type Ⅱ endoleak originating from IMA after EVAR were treated in the Department of Vascular Surgery, Shandong Provincial Hospital Affiliated to Shandong First Medical University from October 2016 to November 2021 were collected and analyzed. Results There were 12 males and 3 females at age of 57-89 (68.00±7.84) years. Eleven patients received embolization of the abdominal aortic aneurysm lumen and initial segment of the IMA via the superior mesenteric artery-middle colic artery-Riolan arch-left colic artery-IMA route. Three patients received embolization of the initial segment of the IMA by the above route. One patient underwent open dissection of the abdominal aortic aneurysm, and orifice of IMA was sutured in the aneurysm cavity while stents were retained. All 15 patients were successfully treated by surgery. The symptoms of back pain, abdominal pain and abdominal distension disappeared in 6 patients after surgery. Neither perioperative deaths nor complications happened during the treatment and follow-up period. The median follow-up time was 11.00 (9.00, 18.00) months. Two patients with typeⅡendoleak recurred during the follow-up period and were admitted to hospital for secondary embolization. No recurrence was observed at 12 months postoperative follow-up. Conclusion Type Ⅱ endoleak is one of the most common complications after EVAR. IMA is the most common criminal origin of typeⅡendoleak. TypeⅡendoleak that lead to persistent expansion of the aneurysm cavity requires aggressive intervention.

  • Using MCM2 as A Novel Cell Replication Marker in Diagnosing Colorectal Carcinoma

    【Abstract】ObjectiveTo study the current research status of minichromosome maintenance protein 2 (MCM2), the cell cycle proliferation marker, in the diagnosis of colorectal carcinoma. MethodsLiteratures about the application of MCM2 in the study of colorectal carcinoma were collected and reviewed.ResultsMCM2, as a marker of cell dysplasia and malignancy, was usually used in the study of carcinoma. The study on expression of MCM2 in the cell of colorectum in different proliferational stage might help to screen colorectal carcinoma as early as possible. ConclusionAs a relatively specific and sensitive marker of cell proliferation, MCM2 might become a promising mark for diagnosing colorectal carcinoma in the early stage.

  • Efficacy of Noninvasive Positive Pressure Ventilation Plus Plateau Exhalation Valve Combined with Respiratory Stimulant on Pulmonary Encephalopathy in Patients with Acute Exacerbation of COPD

    Objective To explore the efficacy of bi-level positive airway pressure ( BiPAP)ventilation plus plateau exhalation valve ( PEV) combined with respiratory stimulant for the treatment of pulmonary encephalopathy in patients with acute exacerbation of chronic obstructive pulmonary disease( AECOPD) . Methods 70 AECOPD patients with pulmonary encephalopathy were randomly divided into a control group and a treatment group. All patients received BiPAP ventilation and conventional therapyincluding antimicrobial, bronchodilation, and expectorant treatment. In the treatment group, the BiPAP ventilator was connected to PEV additionally, and naloxone and nikethamide were administered for 3 days.Clinical symptoms, blood gas analysis, vital signs, gas leakage conditions, and adverse reactions were recorded. Results Heart rate, respiratory rate, PaCO2 , and APACHEⅡ score were more significantly lower,the time to recover consciousness was shoter, meanwhile PaO2 , SaO2 , pH, and glasgow coma scale were significantly higher in the treatment group compared with the control group( all P lt; 0. 01) . Two cases in the treatment group and 5 cases in the control group received tracheal intubation and invasive ventilation due to treatment failure. Two elderly patients in the control group died in hospital. Conclusion Noninvasive positive pressure ventilation plus PEV combined with respiratory stimulant can significantly improve symptoms, shorten the time to recover consciousness, reduce the rate of endotracheal intubation, and improve hypoxemia and hypercapnia rapidly in AECOPD patients with pulmonary encephalopathy.

  • Adhesion and Proliferation of Osteoblasts on Pure Titanium Surface Treated by Microarc Oxidation and Collagen Ⅰ

    【摘要】 目的 探讨微弧氧化(microarc oxidation,MAO)结合应用于纯钛种植体表面处理的可行性。 方法 根据对纯钛钛片处理的不同将实验分为对照组(A组,不作处理)、MAO组(B组,纯钛片上进行MAO处理)及MAO加Ⅰ型胶原组(C组,纯钛片上MAO处理后吸附Ⅰ型胶原)。将成骨细胞培养于各组钛片上,通过扫描电镜、MTT法检测不同时间点各组钛片表面的细胞生长及增殖情况,并检测碱性磷酸酶(alkaline phosphatase,ALP)活性。 结果 扫描电镜显示成骨细胞在C组钛片上细胞黏附情况优于A、B组;MTT法及ALP活性检测示培养3、6 d,成骨细胞在C组钛片上的增殖及ALP活性与A、B组比较差异均有统计学意义(Plt;0.05)。 结论 MAO结合Ⅰ型胶原处理的钛片可更有效提高成骨细胞表面附着、增殖,且具有较高的ALP活性。【Abstract】 Objective To study the feasibility of applying microarc oxidation (MAO) with collagen Ⅰ in surface modification of pure titanium. Methods According to different processing methods, the pure titanium was divided into three groups: the control group (without surface modification, group A), MAO group (with microarc oxidation applied in pure titanium surface modification, group B), MAO+Ⅰ group (with microarc oxidation and collagen Ⅰ applied in pure titanium treatment, group C). Osteoblasts were cultured on the surface of titanium in each group, and the cell proliferation in each group was detected at different time points by scanning electron microscopy and MTT method. Moreover, the activity of alkaline phosphatase (ALP) was also detected. Results Scanning electron microscopy showed that adhesion of osteoblasts for group C was better than group A and group B. MTT method and ALP activity detection indicated that there was a significant difference between group C and group A, B in cell proliferation and ALP activity on the third and sixth day of cultivation (Plt;0.05). Conclusion MAO with collagen Ⅰ applied in surface modification of pure titanium may increase osteoblast attachment, and promote its proliferation and ALP activity.

  • Spermatic Cord Paraganglioma:A Case Report and Literature Review

    目的:提高对精索副神经节瘤的诊断和治疗认识。方法:报告我院2009年2月收治1例精索副神经节瘤患者,并复习文献。患者,男,66岁,因发现右侧阴囊上部包块20年入院。查体发现双侧精索静脉曲张,右侧阴囊上部占位,B超提示双侧精索静脉曲张,右侧阴囊上部34 mm×28 mm不均质包块,血供丰富。结果:行双侧精索静脉高位结扎及右侧精索包块切除术。术中见包块程球形、质中等、边界清楚。术后病检:副神经节瘤。1周后行扩切,未发现有肿瘤浸润。术后随访2月至今无复发及转移。

  • Effect of Ghrelin on Insulin Secretion and Expression of Glut-2 Protein in Isolated Pancreas of rat

    Objective To explore the effect of ghrelin on insulin secretion and expression of glucose transporter protein-2 (Glut-2) in isolated pancreas of rats. Methods Twenty five Wistar rats were randomly devided into normal control group (NC group), high concentration of glucose group (HCG group), high concentration of glucose with high concentration of ghrelin group (10-8mol/L, HCG+HCGh group), medium concentration of ghrelin group(10-9mol/L, HCG+MCGh group), and low concentration of ghrelin group (10-10mol/L, HCG+LCGh group) with 5 rats in each group. The rat isolated pancreas perfusion models were established firstly, then from the distal end of abdominal aortas, the models were perfused with low concentration of glucose (5.5mmol/L), high concentration of glucose (33.3mmol/L) or high concentration of glucose added with different concentrations of ghrelin. Levels of insulin outflowed from portal vein were tested by ELISA method, expression levels of Glut-2 protein were tested by immunohistochemical method,and ultrastructure changes of islet β cell were observed under the transmission electron microscope. Results There were no significant difference on levels of fasting blood glucose (FBG), fasting insulins (FINS), homeostasis model of assess-ment for insulin resistence index (HOMA-IR), and homeostasis model of assessment for pancreatic β cell function (HOMA-β),(P>0.05). There were no significant difference on insulin levels of effluent from portal vein of 5 groups (P>0.05) when isolated pancreas perfused with 5.5mmol/L glucose, while had 2 secretion peaks in 3min and 10-12min after 33.3 mmol/L glucose perfusion, where HCG+HCGh group at the top. The mean density value of Glut-2 protein in NC group was higher than that of other 4 groups (P<0.05). The results of transmission electron microscopy showed that apoptosis was lighter in NC group than that of other 4 groups, and apoptosis of HCG+HCGh group was lighter than that of HCG+MCGh group and HCG+LCGh group. Conclusions In isolated pancreas of rats, ghrelin promotes high concentration of glucose-stimulated insulin secretion, decreases expression of Glut-2 protein, and protects the islet β cell.

  • Advances in Research of MicroRNA in The Pathogenesis of Type 2 Diabetes

    Objective To summarize the relationship of diabetes and its complications with microRNA. Methods Domestic and international researches were collected by searching to summarize the role of microRNA in diabetes and its complications. Results MicroRNA could affect the secretion of insulin and interfer metabolism of gulcose in fat cells, muscle cells, and liver cells, which resulting in insulin resistance. At the same time, the microRNA also played an role in damage of vascular endothelial cells and myocardial cell in diabetes. Conclusion MicroRNA acts an important role in the process of diabetes and its complications.

  • Investigation on Perioperative Renal Aquaporin 2 Expression in Experimental Obstructive Jaundice

    Objective To investigate the changes of renal medulla aquaporin 2 expression and morphological changes of epithelia of collecting tube after bile duct recanalizaiton operation. Methods Thirty rats were divided into two groups randomly. Common bile duct ligation was performed on 20 experimental rats with silicon tubes 2 mm in extre-diameter, and sham operation on the other 10 rats. Seven days later, bile duct recanalizaiton was performed on obstructive jaundice group and sham operation on contrast group. Experimental rats were divided into two subgroups randomly. Half of them were killed immediately and the others would be killed 24 hours later. Serum of each rat was collected to detect hepatic function and renal function. Renal medulla was fixed for microscopic examination and was kept in the -80 ℃ refrigerator for aquaporin 2 expression measurement by Western blot technique. Results All of the animals accomplished the experiment smoothly. Golden ascites were found in the rats of obstructive jaundice group. Twenty-four hours after recanalization, serum bilirubin levels decreased 〔(45.95±8.39) μmol/L〕, P<0.01, and there was no significant change in blood urine and creatine level. Compared with sham operation group (21 966.20±1 544.70), expression of aquaporin 2 decreased significantly after common bile duct ligation in obstructive jaundice group (15 665.30±1 181.85), P<0.01. After recanalizaion, the expression of aquaporin 2 in obstructive jaundice group increased (19 490.80±4 239.32), P<0.01. Conclusion Common bile duct obstruction would lead to epithelium injury of renal collecting tube, and down regulate the aquaporin 2 expression.

  • Diagnosis and treatment progress in the femoral insertion injury of medial collateral ligament of knee

    ObjectiveTo summarize the diagnosis and treatment progress in the femoral insertion injury of the medial collateral ligament (MCL) of knee, and to provide a clinical reference for diagnosis and treatment. MethodsThe literature on the femoral insertion injury of the MCL of knee was widely reviewed. The incidence, mechanisms of injury and anatomy, the diagnosis and classification, and status of treatment were summarized. ResultsThe mechanism of the femoral insertion injury of the MCL of knee is related to its anatomical and histological characteristics, as well as the abnormal valgus of the knee joint, excessive external rotation of the tibial platform and it is classified according to the injury characteristics to guide the refined and individualized clinical treatment. ConclusionDue to different understanding of femoral insertion injury of MCL of knee, the treatment methods are different, and thus is the healing effect. Additional studies are still needed to promote the healing of insertion injuries.
