ObjectiveTo compare the effects of povidone iodine handwashing with brush and brush-free handwashing on the hand skin condition of nurses in operation room. MethodA random sampling method was used to choose 150 nurses from the operation room of a grade-3 class-A hospital as our study subjects from June 2013 to December 2014. They were randomly divided into control group and study group according to the random number table with 75 in each. The control group used the traditional povidone iodine handwashing with brush, while the study group applied brush-free handwashing method. Then, we compared the hand skin condition and disinfection effect of these two kinds of handwashing methods. ResultsThe control group had dry skin in 34 nurses (45.3%), dry desquamation in 9 (12.0%), tight feeling in 51 (68.0%), and allergy in 5 (6.7%). The study group had dry skin in 19 nurses (25.3%), dry desquamation in 0 (0.0%), tension in 21 (28.0%), and allergy in 0 (0.0%). The differences between the two groups were statistically significant (P<0.05). ConclusionsThe brush-free handwashing method is able to achieve the requirements of surgical hand disinfection, and can protect the skin of nurses in operation room.
ObjectiveTo investigate the risk factors associated with failure of weaning from invasive mechanical ventilation in gerontal patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD). MethodsA retrospective study was conducted on 76 patients aged 65 years and older with AECOPD who received invasive mechanical ventilation and met the weaning criteria from July 2012 to June 2014. The subjects who passed the spontaneously breathing trial (SBT) and did not need mechanical ventilation within 48 h were enrolled into a weaning success group. The subjects who did not pass the SBT or needed mechanical ventilation again within 48 h were enrolled into a weaning failure group. The risk factors associated with failure of weaning were studied by univariate and multivariate Logistic regression analysis. ResultsThere were 53 subjects in the weaning success group and 23 in the weaning failure group. The incidences of sepsis, multiple organ dysfunction syndrome (MODS), fungal infection, hypoproteinemia, duration for mechanical ventilation > 14 d, the prevalences of aeropleura, cardiac failure, diabetes, coronary heart disease and hepatic insufficiency were higher in the weaning failure group than those in the weaning success group (P < 0.05). Logistic analysis revealed that MODS (OR=8.070), duration for mechanical ventilation > 14 d (OR=17.760), cardiac failure (OR=4.597) and diabetes (OR=13.937) were risk factors of weaning failure (P < 0.05). ConclusionMODS, duration for mechanical ventilation > 14 d, cardiac failure and diabetes were associated with the failure of weaning from invasive mechanical ventilation in gerontal patients with AECOPD.
ObjectiveTo explore the clinical significance of quantitative CT measurement of pulmonary vascular indexes in chronic obstructive pulmonary disease (COPD). MethodsFifty-three stable COPD patients who were diagnosed in our hospital between May 2013 and February 2015 were recruited in the study. The HRCT results were analyzed, and the pulmonary vascular indexes were measured including the main pulmonary artery diameter (mPAD), axial diagonal mPAD, sagittal mPAD, right pulmonary artery diameter (RPAD), left pulmonary artery diameter (LPAD) and etc. The % CSA<5 (the percent of total cross-sectional area of the vascular with cross-sectional area less than 5 mm2 to total area of the lung) was calculated with Image J 1.48. The % LAA[volume percentage of low attenuation areas (<-950 HU)] representing degree of emphysema was calculated with GE ADW 4.5. Lung function test and questionnaires including CAT, mMRC, SGRQ and frequency of acute exacerbation of COPD (AECOPD) were completed and collected. ResultsIn COPD patients, the mPAD,axial diagonal mPAD, sagittal mPAD, RPAD and LPAD were positively correlated with % LAA respectively (r=0.36, 0.33, 0.43, 0.45, 0.33) and % CSA<5 was negatively correlated with % LAA (r=-0.37). mPAD was positively correlated with CAT, SGRQ and frequency of AECOPD respectively (r=0.52, 0.29, 0.35), and negatively correlated with FEV1% pred (r=-0.30).% CSA<5 was negatively correlated with CAT, SGRQ and frequency of AECOPD, respectively (r=-0.29,-0.30,-0.29), and positively correlated with FEV1% pred (r=0.28). The multivariate linear regression analysis revealed a linear relationship of mPAD with body mass index,% LAA and CAT. There was also linear relationships between axial diagonal mPAD, sagittal mPAD, RPAD and % LAA, CAT, respectively, and linear relationships between % CSA<5 and FEV1% pred,% LAA. The P value were all less than 0.05. ConclusionQuantitative CT measurement of cross-sectional area of small pulmonary vessels and pulmonary artery diameter are associated with clinical indexes in COPD patients, which will provide a new appraoche for the disease assessment of COPD patients.
