  • Expression of Extracellular SignalRegulated Kinase and p38 MitogenActivated Protein Kinase in Autogenous Vein Grafts

    ObjectiveTo investigate the expression of extracellular signalregulated kinase (ERK) and p38 mitogenactivated protein kinase (p38 MAPK) in autogenous vein grafts during vascular remodeling.MethodsAn autogenous vein graft model was established by transplanting the right jugular vein to infrarenal abdominal aorta in 80 Wistar rats. Vein graft samples were harvested 6 hours, 24 hours, 3 days, 7 days, 2 weeks, 4 weeks, 6 weeks and 8 weeks after surgery. Gene expression of ERK and p38 MAPK was measured by reverse transcriptionPCR. Western blot was used to detect the expression of protein products and phosphorylation protein products of ERK and p38 MAPK. Apoptosis of vascular smooth muscle cells (VSMCs) was determined by TUNEL. Proliferating cell nuclear antigen(PCNA) of VSMCs also was studied.ResultsThe expression of ERK1 mRNA and p38 MAPK mRNA increased considerably after surgery. ERK1 mRNA reached the peak on the 7th day 〔(33.2±14.2)%, P<0.01〕, but p38 MAPK mRNA reached the peak on the second week after surgery 〔(58.8±26.2)%, P<0.01〕. The expression of ERK1/2 detected by western blot reached the peak during 1 to 2 weeks and decreased gradually to normal level 6 weeks after surgery. The expression of p38 MAPK reached the peak during 2 to 4 weeks and decreased to 1/4 to 1/2fold 8 weeks after surgery. There was a positive relationship between ERK1 and PCNA(r=0.759 6,P<0.01) and a positive relationship between p38 MAPK and apoptosis(r=0.892 2,P<0.01). ConclusionActivation of MAPK system exists in autogenous vein grafts and it may become a new target for the therapy of stenosis after vein grafts.

  • RECONSTRUCTION OF NASAL SUBUNITS WITH REVERSED VASCULARIZED PREAURICULAR AND EAR COMPOSITE FLAP

    Objective To explore the technique for reconstructing nasal subunits with the reversed vascularized preauricular and ear composite flap. Methods From June 2003 to August 2005 the preauricular and ear composite flap, which was pedicled by the reversed superficial temporal vessel, was harvested and transplanted to repair the defects of the nasal tip, soft triangle, and ala by the microsurgical technique. The flap ranged in area from 3.5 cm×2.5 cm to 2.0 cm×1.5 cm, with the pedicles 4 to 6 cm in length. The reversed superficial temporal vessel pedicles were directly anastomosed to the angular artery and vein adjacent to the nasal ala. Results Among the 12 patients who had been treated, 11 had their vessel pedicles successfully anastomosed to the angular vessels, while the remaining 1 patient had the stump of the superficial temporal vessel anastomosed after the flap harvest via the vessel graft because the angular vessels were not found in the recipient area. All the flaps survived with a satisfactory esthetic result after operation. Conclusion The technique for reconstructing nasal subunits with the reversed vascularized preauricular and ear composite flap, which is directly anastomosed to the angular vessel, can avoid the vessel grafting and therefore it can avoid an additional damage to the patient.

  • Analysis of Serum IL-6 And TNF-αLevels in Children With Obstructive Sleep Apnea Syndrome

    Objective To investigate the levels of IL-6 and TNF-α in children with obstructive sleep apnea syndrome (OSAS) and to determine their clinical significance. Methods One hundred children with OSAS in our department from August 2005 to February 2006, and 40 healthy children were enrolled in the study. The serum levels of IL-6 and TNF-α were measured. Results Serum levels of IL-6 and TNF-α were significantly higher in patients with OSAS than those in the control group (Plt;0.05). Both IL-6 and TNF-α were not correlated with AHI. Conclusion It is concluded that OSAS is a chronic inflammatory process. A close correlation was observed between high levels of IL-6 and TNF-α and OSAS. High levels of IL-6 and TNF-α account for the risk factors in the development of cardiovascular diseases in children with OSAS.

  • Relationship among Carotid Atherosclerotic Plaque and Its Related Factors and Cerebral Infarction

    【摘要】 目的 探讨颈动脉粥样硬化(CAS)斑块及血脂、血糖(BG)、纤维蛋白原(Fbg)水平与脑梗死的关系。方法 对2007年11月—2008年12月入院的91例脑梗死患者,应用彩色多普勒检测其颈动脉内中膜厚度(IMT)、斑块数和性状,同时检测血脂、血糖、纤维蛋白原水平,并与正常对照组比较。结果 ①与正常对照组比较,脑梗死组IMT明显增厚、CAS斑块检出率、软斑百分比明显增高(Plt;005)。②血清总胆固醇(TC)、低密度脂蛋白(LDL)、BG及Fbg水平脑梗死组明显高于正常对照组(Plt;005);脑梗死有斑块亚组明显高于无斑块亚组(Plt;005)。③脑梗死组IMT与TC、LDL、BG、Fbg水平(r分别为0.32、0.34、0.30、0.36,Plt;005)。结论 脑梗死患者IMT增厚,CAS斑块及软斑发生率高。BG、TC、LDL及Fbg水平增高是脑梗死及CAS斑块发生的危险因素。

  • Arthroscopic Reduction and Fixation of Comminuted Fracture of Posterior Cruciate Ligament from Tibia Using Suture Anchors through an Posterolateral Portal via Popliteal Fossa

    【摘要】 目的 探讨关节镜辅助下经腘窝小切口应用锚钉固定治疗后交叉韧带胫骨止点粉碎骨折的临床效果。 方法 2007年2月-2008年12月对22例急性后交叉韧带胫骨止点骨折患者,采用关节镜辅助下经腘窝小切口切开复位,利用缝合锚钉重建止点,固定治疗后测试交叉韧带的张力和稳定,采用Lysholm膝关节功能评分评定膝关节功能。 结果 骨折平均愈合时间为6周,平均屈膝活动度(120±3)°。Lysholm膝关节功能评分为(92±2)分。 结论 关节镜辅助经腘窝小切口锚钉固定治疗后交叉韧带胫骨止点粉碎骨折,可协助诊断关节内韧带及骨折损伤情况,对韧带止点进行重建,操作简便,早期功能锻炼有助于关节功能恢复。【Abstract】 Objective To explore surgical technique and the results of arthroscopic reduction and fixation for the treatment of comminuted fracture of the posterior cruciate ligament (PCL) from the tibia using suture anchor through an posterolateral portal via popliteal fossa. Methods Twenty-two patients who were operated through an posterolateral portal via popliteal fossa, the fragment was fixed by using suture anchor to reestablish the insertion, test the tension and stabilization of PCL. Lysholm score was used to evaluate the knee function. Results The bone union was confirmed by X-ray films at the 6 weeks postoperatively. The ROM of knee joint relaxation was 120±3°. The Lysholm score was 92±2. Conclusion The displaced fragment of the comminuted fracture of the posterior cruciate ligament from the tibia can be reduced and fixed with the suture anchor arthroscopicly. Using suture anchors demonstrate a reliable and easy to use technique. Operation under arthroscopy helps diagnose and treat other complications inside knee joint. In addition, early functional exercise contributes torapid recovery of knee joint’s function.

  • Respiratory Support of Pressure Regulated Volume Control Ventilation after Liver Transplantation

    Objective To study the application of pressure regulated volume control ventilation in respiratory support after liver transplantation. MethodsTwenty patients underwent liver transplantation were randomly averagely divided into two groups: pressure regulated vlume control ventilation (PRVCV) group and volume control (VC) group. The parameters of respiratory mechanics, hemodynamics and blood gas analysis of patients in two groups were compared, such as oxygen delivery (DO2), oxygen consumption (VO2), oxygen incepation ratio (O2ER), arteriovenous oxygen content difference (C(a-v)O2), cardiac output (CO), mean arterial pressure (mABP), mean pulmonary arterial pressure (mPAP), alveolar-arterial PO2 difference 〔P(A-a)O2〕, gas exchange index (PaO2/FiO2), ratio of shunted blood to total perfusion (Qs/Qt), peak inspiratory pressure (PIP) and mean airway pressure (mAP). Results The P(A-a)O2 and Qs/Qt were significantly decreased in PRVCV group than those in VC group 〔P(A-a)O2: (101.42±28.07) mm Hg vs. (136.76±39.13) mm Hg; Qs/Qt: (1.78±0.86)% vs. (3.28±0.99)%〕, P<0.05, P<0.05, while the C(a-v)O2 and O2ER were significantly increased 〔C(a-v)O2: (20.70±10.41) mm Hg vs. (12.83±2.49) mm Hg; O2ER: (16.34±9.79)% vs. (9.37±1.83)%〕, P<0.05, P<0.01. There was no difference in the hemodynamics and airway pressure parameters between PRVCV group and VC group. Conclusion PRVCV mode could be a more suitable mechanical ventilation pattern to patients after liver transplantation.

  • Acinetobacter baumannii peritoneal dialysis-related peritonitis: a review of 10 cases

    ObjectiveTo investigate the clinical characteristics, treatment and outcomes of patients with Acinetobacter baumannii peritoneal dialysis-related peritonitis.MethodsWe retrospectively analyzed the clinical data of patients with Acinetobacter baumannii peritoneal dialysis-related peritonitis in the First Affiliated Hospital of Airforce Military University from January 2011 to December 2018. The clinical baseline data, treatment process, microbiological data, antibiotic susceptibility test of the bacterial isolates and outcomes were analyzed.ResultsA total of 10 patients were enrolled, including 4 males and 6 females. The average age of all patients was (44.90±17.03) years, the average age of peritoneal dialysis was (21.70±17.06) months. Seven cases were infected for the first time, and 3 cases were reinfected. The infections were mainly caused by mechanical failure of catheter connection system (3 cases) or enterogenous infection (3 cases). The main symptoms were abdominal pain (10 cases), fever (7 cases) and diarrhea (3 cases). Empirical anti-infective treatment was given after admission, only 1 case was effective, and the treatment of the other 9 cases were adjusted according to the results of drug sensitivity. Acinetobacter baumannii was sensitive to cefoperazone, carbapenem (meropenem, imipenem), quinolones (ciprofloxacin, levofloxacin), aminoglycosides (gentamicin) and polymyxin. Only one case was resistant to ceftazidime. Among the 10 patients, 8 cases were cured (continued peritoneal dialysis), 1 case died, and 1 case dropped out from peritoneal dialysis to hemodialysis.ConclusionsAcinetobacter baumannii peritoneal dialysis-related peritonitis in this hospital is mainly caused by mechanical disturbance of catheter connection system or enterogenic infection. Appropriate measures, including aseptic standard operation, follow-up and effective anti-infective treatment, should be taken to decrease the incidence and mortality of Acinetobacter baumannii peritoneal dialysis-related peritonitis.

  • Pulmonary sclerosing pneumocytoma: a clinicopathologic analysis of 13 cases

    Objective To investigate the clinicopathologic features, diagnosis and differential diagnosis of pulmonary selerosing pneumocytoma (PSP). Methods A total of 13 cases of PSP were enrolled, and the clinical and imaging findings, pathologic features, and immunophenotype were collected and analyzed, with review of the literatures. Results Thirteen patients were all female, aged from 27 to 69 years old by first discovered, the average age was 53 years old. The maximum diameter ranged from 0.8 - 6 cm. It was mainly discovered accidentally by physical examination. According to the CT findings of 13 cases, all lesions were round or round-like, with a well-circumscribed mass. The “welt vessel sign” was suggestive to the diagnosis of PSP. Microscopically, the tumor was composed of two types of cells (surface epithelial cells and round mesenchymal cells), and four histological patterns (papillary, solid, hemorrhagic and sclerotic zone). Immuophenotype: thyroid transcription factor-1 (TTF-1) and epithelial membrane antigen were expressed on both epithelial cells and mesenchymal cells. Pan cytokeratin (PCK) and cytokeratin 7 were only expressed on epithelial cells, and vimentin on mesenchymal cells. Conclusion PSP is a rare lung benign tumor, preoperative and intraoperative freezing diagnosis are difficult, the diagnosis depends on the morphologic characteristics of paraffin-embedded tissue sections and immunohistochemical staining. The identification of mesenchymal cells with TTF-1 positive and PCK negative is the key to PSP diagnosis.

  • Study on the effect of small alcohol on lipid hydration and liposome formation

    Liposomes with precisely controlled composition are usually used as membrane model systems to investigate the fundamental interactions of membrane components under well-defined conditions. Hydration method is the most common method for liposome formation which is found to be influenced by composition of the medium. In this paper, the effects of small alcohol (ethanol) on the hydration of lipid molecules and the formation of liposomes were investigated, as well as its coexistence with sodium chloride. It was found that ethanol showed the opposite effect to that of sodium chloride on the hydration of lipid molecules and the formation of liposomes. The presence of ethanol promoted the formation of liposomes within a certain range of ethanol content, but that of sodium chloride suppressed the liposome formation. By investigating the fluorescence intensity and continuity of the swelled membranes as a function of contents of ethanol and sodium chloride, it was found that sodium chloride and ethanol showed the additive effect on the hydration of lipid molecules when they coexisted in the medium. The results may provide some reference for the efficient preparation of liposomes.

  • Effect of Local Transfection of Survivin Antisense Oligodeoxyribonucleotides on Intimal Hyperplasia in Vein Graft

    Objective To investigate the effect of survivin antisense oligodeoxyribonucleotides (survivin ASODNs) on intimal hyperplasia (IH) in vein graft in rats. Methods  Autogenous vein graft models were established in 60 Wistar rats by transplanting the interior jugular vein to the common jugular artery using microsurgical technique. The rats were divided into 5 groups according to random digits table, including survivin ASODNs 50 μg group and 200 μg group, scramble ODNs 200 μg group (ODNs group), Lipofectin+pluronic group and control group. Vein graft samples were collected on 7 d and 14 d after transplantation, respectively. The degrees of hyperplasia were determined and then compared by histomorphology between different groups. The expression of survivin mRNA was measured by RT-PCR and immunohistochemistry. The relevant protein products were detected by Western blot and immunohistochemistry was also used to detect the expression of PCNA. Apoptosis of VSMC was measured by TUNEL.Results Day 7 and 14 were the days that intimal hyperplasied most in control group, ODNs group and Lipofectin+pluronic group, there was no significant difference among these groups yet (Pgt;0.05). The IH could be suppressed by locally transfecting 50 μg of survivin ASODNs (P<0.05), and it showed a better inhibiting effect in 200 μg of survivin ASODNs group (P<0.05). The expression of survivin mRNA increased significantly in control group. The expressions of both survivin and PCNA in VSMC significantly decreased in survivin ASODNs group (P<0.05), whereas the positive cells of TUNEL increased significantly (P<0.05). Conclusion Transfection of survivin ASODNs may inhibit the IH after vein graft through suppressing the hyperplasia and stimulating the apoptosis of VSMC, and inhibiting the expression of survivin.
