  • Isolated IgG4-related mediastinal disease: A case report

    IgG4-related disease is an immune-mediated chronic inflammatory disease with fibrosis, which can affect almost all organs in the body. In a few cases, only a single organ is affected. The incidence of isolated IgG4-related mediastinal disease is even rarer. This article reports a rare case of isolated IgG4-related middle mediastinal disease and provides a detailed description of its diagnosis and surgical treatment. The patient had a good prognosis and did not receive any medication such as corticosteroids after surgery. Follow-up after 6 months showed no recurrence.

  • Risk factors and the predictive model for occult lymph node metastasis in cT1N0M0 stage squamous cell lung cancer

    Objective To investigate the risk factors for lymph node metastasis in cT1N0M0 stage squamous cell lung cancer and develop a logistic regression model to predict lymph node metastasis. Methods A retrospective study was conducted on patients with cT1N0M0 stage lung squamous cell carcinoma in our department from August 2017 to October 2022. The correlation between basic clinical data, imaging data, and pathological data and lymph node metastasis was analyzed. Univariate and multivariate logistic regression analyses were employed for risk factor analysis. Receiver operating characteristic curves and the Hosmer-Lemeshow test were utilized to evaluate the model’s discrimination and calibration. The Bootstrap method with 1 000 resamples was employed for internal validation of the model. Results Tumor location of central-type, tumor differentiation, cytokeratin 19 fragment (CYFRA21-1) levels, and tumor size were independent risk factors for lymph node metastasis in cT1N0M0 stage squamous cell lung cancer. The optimal cutoff values for tumor size and CYFRA21-1 levels were determined to be 2.05 cm and 4.20 ng/mL, respectively. The combination of tumor location, CYFRA21-1 levels, and tumor size demonstrates superior predictive capability compared to any individual factor. Conclusion Tumor location of central-type, poorly differentiated tumors, CYFRA21-1 levels, and tumor size are risk factors for lymph node metastasis in cT1N0M0 stage lung squamous cell carcinoma. The combined predictive model has certain guiding significance for intraoperative lymph node resection strategies in cT1N0M0 stage lung squamous cell carcinoma.

  • APPROPRIATE APPLICATION OF STATISTICAL ANALYSIS METHOD IN MEDICAL SCIENCE AND TECHNOLOGY ARTICLES

    Objective To introduce the appropriate application of statistical analysis method in medical science and technology articles. Methods Expatiated the correct application of statistical theories in statistical package, statistical analysis methods and test criterion which compose the basic statistical content in an article.Results If the distribution of numerical variable is normal, mean and standard deviance can be used to describe this variable. In the same way, t test and analysis of variance (ANOVA) can be used to test the difference of mean in each group. If it is not normal, median and range can be used to describe the variable and rank sum test can be used to test the difference of distribution in each group. Categorical variable can be described by rate, proportion and ratio. There are chi-square test, fisher’s exact test and ranksum test to test the difference of rates. Conclusion It is the key of choosing rational statistical methods to distinguish the type of design and variable. 

  • Expression of Livin and Caspase-3 in Colorectal Adenoma-Carcinoma Sequence and Their Correlation

    【摘要】 目的 探讨凋亡抑制蛋白Livin与凋亡蛋白Caspase-3在结直肠腺瘤-癌序列中的表达变化及其相关性。 方法 2006年7月—2009年12月,采用免疫组织化学染色链霉菌抗生物素蛋白-过氧化物酶链接法streptavidin-peroxidese,SP)法检测18例正常黏膜、84例结直肠腺瘤、72例结直肠癌中Livin及Caspase-3的表达情况。 结果 结直肠腺瘤组织中Livin蛋白的阳性表达率明显高于正常黏膜组织(Plt;0.05),而低于腺癌组(Plt;0.05);腺瘤组内绒毛状腺瘤与管状腺瘤相比较,Livin蛋白表达率差异有统计学意义(Plt;0.05)。结直肠腺瘤组织中Caspase-3的阳性表达率明显高于正常黏膜组织(Plt;0.05);而腺瘤组织与癌组织之间Caspase-3阳性表达率差异(Plt;0.05);腺瘤组内绒毛状腺瘤与管状腺瘤相比较,Caspase-3蛋白阳性表达率差异无统计学意义(Pgt;0.05)。Livin表达与Caspase-3表达呈负相关(Plt;0.05)。 结论 凋亡抑制蛋白Livin参与了大肠肿瘤的发生,且在大肠腺瘤-腺癌阶段起到了重要作用;凋亡抑制蛋白Livin与Caspase-3表达呈负相关,抑制Caspase-3蛋白的活性可能是Livin促进结肠癌发生的途径之一。【Abstract】 Objective To investigate the expression of Livin and Caspase-3 among colorectal adenoma-carcinoma sequence, and to identify the relationship between Livin and Caspase-3 expression in colorectal adenoma-carcinoma sequence. Methods Formalin-fixed paraffin embedded colorectal tissues from 174 patients, including 84 adenomas, 72 carcinomas, and 18 normal mucosa, were examined for expression of Livin and Caspase-3 by streptavidin-peroxidase (SP) immunohistochemistry between July 2006 and December 2009. Results The positive rates of Livin protein expression in colorectal adenoma was significantly higher than that in normal mucosa (Plt;0.05), but lower than that in adenocarcinoma (Plt;0.05); the expression of Livin in tubular adenoma was significantly higher than that in villous adenoma (Plt;0.05). The positive rates of Caspase-3 protein expression in colorectal adenoma were significantly higher than that in normal mucosa and carcinoma (Plt;0.05), and the difference in positive rate of Caspase-3 expression was not significant between the villous adenoma and tubular adenoma (Pgt;0.05). Livin expression had negative correlation with the Caspase-3 expression (Pgt;0.05). Conclusion The difference in expression of Livin between adenoma and adenocarcinoma indicates the potential value of it in carcinogenesis of colorectal cancer, which suggestes that suppressing Caspase-3 protein activity is one of the channels by which livin promotes colorectal carcinogenesis.

  • Da Vinci robot system in the surgical treatment of mediastinal lesions: A case control study

    Objective To summarize the clinical data about mediastinal lesions, then to analyze the treatment effect of da Vinci robot system in the surgical treatment of mediastinal lesions. Methods We retrospectively analyzed the clinical data of 49 patients with mediastinal lesions in our hospital between January 2016 and October 2017. These patients were divided into two groups including a da Vinci robot group and a video-assisted thoracoscopic surgery (VATS) group according to the selection of the treatments. There were 25 patients with 14 males and 11 females at age of 56.5±17.9 years in the da Vinci group and 24 patient with 15 males and 11 females at age of 53.0±17.8 years in the VATS group. Results There was no statistical difference in surgery time between the two groups (t=–0.365, P=0.681). Less intraoperative blood loss (t=–2.569, P<0.001), less postoperative drainage amount within three days after surgery (t=–6.325, P=0.045), shorter period of bearing drainage tubes after surgery (t=–1.687, P=0.024), shorter hospital stays (t=–3.689, P=0.021), lower visual analogue scale (VAS) scores of postoperative 48 hours (t=–7.214, P=0.014) with a statistical difference in the da Vinci robot group compared with the VATS group. Conclusion The da Vinci robot system is safe and efficient in the treatment of mediastinal lesions compared with video-assisted thoracoscopic approach.

  • da Vinci robot system for radical surgery of lung cancer in elderly patients

    ObjectiveTo evaluate the feasibility and safety of da Vinci robotic surgery for elderly patients with radical surgery of lung cancer.MethodsWe retrospectively analyzed the clinical data of 59 patients aged over 70 years who underwent radical surgery of lung cancer in our hospital between 2016 and 2019. These patients were divided into two groups including a da Vinci robot group and a single-utility port video-assisted thoracoscopic surgery (VATS) group according to the patients’ selection of the treatments. There were 32 patients with 20 males and 12 females aged 73.1±2.3 years in the da Vinci robot group and 27 patients with 16 males and 11 females aged 71.2±1.3 years in the VATS group. The clinical data of the two groups were compared.ResultsThere was no statistical difference in surgery time between the two groups (t=–0.341, P=0.484). Compared with the VATS group, the da Vinci robot group had more number of lymph nodes dissected (t=1.635, P=0.015), less intraoperative blood loss (t=–2.569, P <0.001), less postoperative drainage amount within 3 days after surgery (t=–6.325, P=0.045), lower visual analogue scale (VAS) scores at postoperative 3rd day (t=–7.214, P=0.021).ConclusionThe da Vinci robot system is safe and efficient in the treatment for elderly patients with radical surgery of lung cancer with less trauma.

  • The effectiveness and safety of Da Vinci robotic-assisted thoracic surgery versus video assisted thoracic surgery for non-small cell lung cancer: a meta-analysis

    ObjectiveTo systematically review the efficacy and safety of robotic-assisted thoracic surgery (RATS) and video assisted thoracic surgery (VATS) for patients with non-small cell lung cancer (NSCLC). MethodsWe searched PubMed, EMbase, The Cochrane Library (Issue 9, 2016), Web of Science, CNKI, VIP, WanFang Data and CBM databases to collect clinical studies about RATS vs. VATS for patients with NSCLC from inception to October 2016. Two reviewers independently screened literature, extracted data and assessed the risk of bias of included studies, then meta-analysis was performed by using RevMan 5.3 software.ResultsA total of 14 cohort studies involving 19 921 patients were included; among them, 4 322 cases were in the RATS group, and 15 599 were in the VATS group. The results of meta-analysis showed that the operation time (MD=22.90, 95%CI 9.97 to 35.84, P<0.000 5) was longer in the RATS group than the VATS group. However, the conversion rate (OR=0.72, 95%CI 0.44 to 1.18, P=0.20), the incidence of postoperative complications (OR=1.06, 95%CI 0.96 to 1.17, P=0.28), intraoperative blood loss (MD=2.75, 95%CI –8.39 to 13.89, P=0.63), postoperative hospitalization time (MD=–0.00, 95%CI –0.02 to 0.02, P=0.99) and in-hospital mortality rate (OR=0.60, 95%CI 0.35 to 1.05, P=0.07) were not significant differences between both groups.ConclusionThe current meta-analysis indicates that the efficacy and safety of RATS and VATS for NSCLC is equivalence, however the operation time for RATS is longer. Due to the limited quantity and quality of inclued studies, the above conclusions still need to be verified by more high quality studies.

  • Analysis on Effect of Liver Transplantation in Treatment of Upper Gastrointestinal Hemorrhage in Patients with Portal Hypertension

    Objective To explore the feasibility and safety of liver transplantation (LT) in treatment of upper gastrointestinal hemorrhage in patients with portal hypertension, and to compare the therapeutic effects with conventional operation (CO). Methods The clinical data of 303 patients with bleeding portal hypertension from Feb. 2009 to Feb. 2012 in the department of hepatobiliary and pancreatic surgery of First Affiliated Hospital of Zhejiang University were retrospectively analyzed. One hundred and one patients received LT procedure (LT group), whereas the other 202 patients received CO procedure (CO group). Postoperative follow-up period was 8-44 months (average 26 months). Results Liver function before operation in CO group was significantly better than that in LT group(P<0.01). The mortality of CO group and LT group were 7.4%(14/189) and 3.0%(3/101, P=1.00), respectively. The rebleeding rate of patients underwent LT was 2.0%(2/101), significantly lower than that of CO group 〔9.5%(18/189), P<0.05〕. The vanish rate of esophagogastric varice in patients underwent LT was 86.1%(87/101), significantly lower than that of CO group 〔54.5%(86/189), P<0.01〕. Conclusions LT treatment for bleeding portal hypertension is feasible and safe. Patients with good liver function despite hemorrhage history may be managed satisfactorily with conventional surgery. LT is the only curative treatment for patients with portal hypertension in end-stage liver disease.
