Abstract: Objective To summarize the technical characteristics and experience on the surgical treatment of esophagobrochial fistula induced by esophageal carcinoma and explore the safe and effective operation procedures. Methods This report retrospective1y summarized 12 cases of esophagobronchial fistula induced by esophageal cancer between January 2007 and November 2010 in Tangdu Hospital, Fourth Military Medical University. There were 9 male patients and 3 female patients with their mean age of 51.24 years (ranging from 37 to 62 years). Four types of surgical procedures were performed to patients according to their respective conditions: (1) Esophagectomy +“tunnel”esophagogastrostomy + pulmonary lobectomy (2 patients); (2) Esophagectomy + stapled esophagogastrostomy + pulmonary lobectomy (5 patients); (3) Esophagectomy + colon interposition for esophagus + pulmonary lobectomy (4 patients); (4) Esophagectomy + esophagogastrostomy + left pneumonectomy (1 patient). Results Among those 12 cases presenting to our hospital, 2 patients died during the postoperative period and the overall morality was 16.67%(2/12). One patient died of acute congestive heart failure on the 4th postoperative day after esophagectomy, “tunnel”esophagogastrostomy and left lower lobectomy of the lung for esophageal carcinoma directly invading the left lower bronchus, and another patient died of severe infection and renal failure on the 11th postoperative day after esophagectomy, stapled esophagogastrostomy and left upper lobectomy of the lung for esophageal carcinoma directly invading the left upper bronchus. Four patients developed mild empyema and 1 patienthad bronchial fistula after surgery, who finally recovered and were discharged after treatment of antibiotics and drainage. The postoperative morbidity was 41.67%(5/12). All surviving patients were followed up from 1 month to 3 years. During follow-up, there was one death, and the other patients were alive without any clinical events. Conclusion Individualized surgical procedure is a safe and effective therapeutic choice for patients with esophagobronchial fistula induced by esophageal carcinoma.
ObjectiveTo investigate the occurrence of indwelling urinary catheter in patients receiving thoracoscopic lobectomy and relevant risk factors.MethodsWe retrospectively reviewed the clinical data of the 737 patients who received thoracoscopic lobectomy in our hospital and analyzed the risk factors of indwelling urinary catheter during postoperative hospitalization using univariate analysis and multiple-variate logistic regression analysis between December 2018 and May 2019. There were 253 males and 484 females at median age of 57 (50, 64) years.ResultsA percentage of 14.4% (106/737) of the patients adopted postoperative indwelling urinary catheter. Univariate regression analysis showed that gender and postoperative bedridden time were risk factors for indwelling urinary catheter in the patients after thoracoscopic lobectomy (P<0.05). Multiple-variate logistic regression analysis showed that male gender (OR=2.018, 95% CI 1.316-3.096, P<0.001) and postoperative bedridden time >18 hours (OR=2.298, 95%CI 1.502-3.516, P<0.001) were the independent risk factors for indwelling urinary catheter.ConclusionMale gender and those with longer postoperative bedridden time are high-risk population to indwell urinary catheter. Positive measures should be taken to reduce the chance of indwelling urinary catheter.
ObjectiveTo investigate the incidence, severity and longitudinal trajectories of symptoms at various time points in the perioperative period of lung cancer patients, and to provide scientific basis for clinical staff to implement predictive nursing and dynamic management of symptom clusters. MethodsA prospective longitudinal investigation was conducted. The patients with lung cancer who underwent thoracoscopic lung surgery in four wards of the Department of Thoracic Surgery in our hospital were investigated by face-to-face and telephone follow-up before surgery, 1-2 days after surgery, on the day of discharge and 2 weeks after discharge. The investigation tool was the revised Chinese version of MD Anderson Symptom Inventory lung cancer specific module. Results A total of 192 patients with lung cancer were included in this study, including 59 males and 133 females, with an average age of 55.68±11.01 years. There were two symptom clusters (respiratory-gastrointestinal and emotional/psychological-disturbed sleep symptom clusters) before surgery, three symptom clusters (respiratory, gastrointestinal, and emotional/psychological-disturbed sleep symptom clusters) 1-2 days after surgery, three symptom clusters (pain-fatigue-emotional/psychological, respiratory, and gastrointestinal symptom clusters) on the day of discharge, and two symptom clusters (pain-fatigue-respiratory and respiratory symptom clusters) 2 weeks after discharge. The composition of symptoms was different in each time point during perioperative period. ConclusionThere are four symptom clusters in patients with lung cancer during perioperative period, which are pain-fatigue-disturbed sleep symptoms, gastrointestinal symptoms, respiratory symptoms and emotional/psychological symptoms. The symptom clusters of lung cancer patients at different time points are relatively stable, but the symptoms within the symptom clusters show dynamic changes. Medical staff should attach great importance to and continuously monitor the dynamic changes of perioperative symptom groups of lung cancer patients, do relevant education and nursing in advance, and timely adjust the management plan according to the symptom group evaluation results.
ObjectiveTo explore the independent risk factors for postoperative retention of urinary catheters in the ward of lung tumor patients due to urinary retention under the concept of enhanced recovery after surgery (ERAS).MethodsSeventy-five patients with lung tumors who had urinary catheters left in the postoperative ward between June 2019 and August 2019 were selected as a case group, and 75 patients with lung tumors who did not have urinary catheters in the perioperative period as a control group. Independent risk factors for indwelling urinary catheters in the postoperative ward were screened by univariate and multiple-variate logistic stepwise regression analysis.ResultsThere were 45 males and 30 females in the case group with an average age of 55.33±10.78 years, 28 males and 47 females in the control group with an average age of 57.12±10.06 years. Univariate analysis showed that gender, operative time>2 h, intraoperative fluid volume≥1 200 mL, and fluid volume within 6 h of returning to the ward after surgery>1 200 mL were associated with the occurrence of indwelling urinary catheters in patients with lung tumors in postoperative wards (P<0.05). Multiple-variate logistic regression showed that male (OR=2.311, 95%CI 1.173-4.552, P=0.015), infusion volume within 6 h of returning to the ward after surgery>1 200 mL (OR=2.491, 95%CI 1.149-5.401, P=0.021) and intraoperative infusion volume≥1 200 mL (OR=2.105, 95%CI 1.022-4.340, P=0.044) were independent risk factors for postoperative retention of urinary catheters in patients with lung tumors.ConclusionThe occurrence of indwelling urinary catheter in lung tumor patients under the ERAS concept is the result of a combination of factors, and patients who are male, have infusion volume>1 200 mL within 6 h of returning to the ward after surgery, and have intraoperative infusion volume≥1 200mL are the high-risk group for postoperative ward indwelling urinary catheter, and health care personnel should strengthen the assessment and observation, provide targeted health education, appropriately control the perioperative fluid volume, and take other measures to reduce the occurrence of indwelling urinary catheters due to urinary retention postoperatively in ward.
Objective To explore the incidence, age distribution and change tendency of female breast cancer in urban Shenyang from 2000 to 2009. Methods The incidence data of female breast cancer recorded in Shenyang Center for Disease Control and Prevention from 2000 to 2009 were collected, and the incidence and standardized incidence of breast cancer were analyzed by using SPSS 16.0 software. The incidence trend was analyzed by means of χ2 trend test and linear regression. Results The annual average crude incidence rate of female breast cancer in urban Shenyang from 2000 to 2009 was 46.03/100 000, while the annual average standardized incidence rate was 35.63/100 000, increased by 160.41% and 109.39% respectively during past 10 years, and the annual average increase was 11.22% and 8.56%, respectively. Both crude incidence rate and standardized incidence rate were in increasing trends with statistical significance (Plt;0.001). The crude incidence rate of the female at age of 50-64 showed a perpendicular rise trend, increased by 271.97%, and the annual average increase was 15.72%. The χ2 trend test and linear regression showed a significant difference (χ2=276.743, P=0.000; y= –22 355.560+11.205x, P=0.000). Conclusion The incidence of female breast cancer in urban Shenyang tends to rise with a younger-aged trend, and the female at age of 50-64 are commonly affected.
【摘要】 目的 探讨在基层医院建立脑卒中单元模式的临床意义。 方法 将2006年1-3月和2007年同期共381例脑卒中患者分为脑卒中单元组和普通病房组。脑卒中单元组按照脑卒中单元模式进行治疗,普通病房组按照传统方法治疗,分别于治疗前、治疗后7、15、30 d时,对比分析两组患者神经功能缺损及疗效。 结果 脑卒中单元组和普通病房组治疗后,随时间两组的神经功能缺损评分(NIHSS)均逐渐降低,两组在治疗后15、30 d时差异有统计学意义(P=0.007),脑卒中单元组家属满意度明显高于普通病房组(P=0.002)。 结论 在基层医院脑卒中单元模式对脑卒中患者的治疗效果明显优于传统治疗。【Abstract】 Objective To investigate the clinical significance of setting up stroke unit model in basic hospital. Methods Three hundreds and eighty-one stroke patients were randomly divided into stroke unit group (n=186) and general ward group (n=189). The stroke unit group patients were treated with the stroke unit model designed by ourselves, while the control group patients were treated with traditional method, The clinical efficacy was compared before treatment, seven days, 15 days and 30 days after treatment. Results Improvement in neurological score was significantly better among patients treated in the SU than in the GW. NIHSS scores gradually reduce in the both groups after treatment, which were significantly lower than those in the control group on 15 days and 30 days after treatment(P=0.007,0.004). The satisfactory of relatives in the stroke unit group was better than that in the general ward group(P=0.002). Conclusion The efficacy of treatment with stroke unit model was better than that of treatment with traditional method in the stroke patients.
Objective To investigate the application and clinical efficacy of orthotopic autologous renal transplantation combined with inferior vena cava (IVC) resection and reconstruction in retroperitoneal tumor. Methods The clinical data of a patient with complex retroperitoneal tumor was analyzed retrospectively. Abdominal CT examination showed that the tumor originated from IVC and invaded the retrohepatic IVC and bilateral renal vein trunks. Intraoperative ultrasound exploration revealed mechanized thrombosis in the IVC and bilateral renal vein trunks. After blocking the left renal vein, no significant hemodynamic changes were seen before and after intraoperative ultrasound exploration and contrast-enhanced ultrasound. Results After complete removal of the left renal vein and suture of the severed end, the right renal vein was successfully reconstructed with an orthotopic autologous right kidney graft combined with IVC resection after removal of the mechanized thrombus. The patient recovered well after surgery, and a repeat CT showed that the reconstructed artificial IVC was patent, and the color Doppler ultrasonography of both kidneys showed good perfusion and no obstruction of return. The patient was given oral rivaroxaban anticoagulant therapy after operation, and discharged at 19 days after operation. The postoperative pathological findings suggested inferior vena cava smooth muscle sarcoma. Conclusion Orthotopic autologous renal transplantation combined with IVC resection and reconstruction for complex retroperitoneal tumor is safe and feasible, and the left renal vein can be ligated and dissected intraoperatively, but a comprehensive evaluation with intraoperative ultrasound (imaging) is required.
ObjectiveTo summarize the key operative points and efficacy of ex-vivo ex-vivo liver resection and autologous liver transplantation (ELRA) using various vascular materials for hepatic vein reconstruction in the treatment of end-stage hepatic alveolar echinococcosis (HAE). MethodThe clinicopathologic data of a patient with end-stage HAE who underwent ELRA combined with complex hepatic vein reconstruction were retrospectively analyzed. ResultsThe patient was a 60-year-old male who was admitted to the Sichuan Provincial People’s Hospital due to giant alveolar hydatid in the liver, with a body weight of 60 kg and a standard liver volume of 1 024.5 mL. The imaging showed that the hydatid invaded the first and second hepatic portals, middle hepatic vein, left hepatic vein, and retrohepatic inferior vena cava. The three-dimensional reconstruction of CT showed that the residual liver volume was 1 270.6 mL. The patient received supportive treatment after admission and underwent ELRA following strict evaluation. Intraoperatively, it was found that the multiple hepatic veins and retrohepatic inferior vena cava were widely invaded. The liver was split in vivo and the mass was excised ex vivo by “in vivo first” principle. The hepatic vein was repaired and reconstructed into a wide mouth outflow tract using allogeneic veins, autologous inferior mesenteric vein, and hepatic round ligaments, then performed the autotransplantation by wide mouth outflow-artificial inferior vena cava anastomosis (end to side). The operative time was 16 h, and the intraoperative blood loss was approximately 2 000 mL. FK506 was orally administered after operation, and low-molecular-weight heparin sodium was administered 24 h later for anticoagulation. The patient was returned to the general ward on the 6th day after the operation, and the enhanced CT scan showed that the hepatic outflow tract was unobstructed, without stenosis and thrombosis, and the patient was discharged on day 18 after the operation. The patient was pathologically diagnosed with alveolar echinococcosis. ConclusionsFrom the results of this case, combination of multiple vascular materials to reconstruct the hepatic outflow tract is an optional procedure for ELRA in treatment of end-stage HAE. Strict preoperative evaluation, skillful vascular anastomosis technique, and postoperative anticoagulation are important measures to maintain patency of postoperative reconstruction vessel.
ObjectiveTo investigate the value of laparoscopic liver venous deprivation (LLVD) in promoting the growth of contralateral future liver remnant (FLR) during two-step hepatectomy. MethodThe clinicopathologic data of a 45-year-old female patient with pancreatic neuroendocrine tumor with multiple liver metastases (grade G2) treated by two-step hepatectomy based on LLVD in January 2022 in the Sichuan Provincial People’s Hospital were analyzed retrospectively. ResultsThe liver function returned to normal within 10 d after LLVD, and the relative increase ratio of FLR reached to 98.35% on postoperative day 10. The laparoscopic right hemi-hepatectomy and distal pancreatectomy plus splenectomy was performed without any postoperative complications, and the patient was discharged from hospital on postoperative day 8. No tumor recurrence or metastasis occurred during the follow-up period. ConclusionsFrom the analysis results of this case, the LLVD could promote the growth of FLR safely and effectively. LLVD provides an alternative surgical method of two-step hepatectomy for treatment of benign and malignant liver tumors.
ObjectiveTo construct a structural equation model of the mechanism of the role of medical humanities literacy in job competency and to conduct empirical analysis to verify the supporting role of medical humanities literacy in cultivating job competency. MethodsLiterature research was conducted to select the initial indicator system for medical humanities literacy and job competency, and then a conceptual model was constructed. Questionnaire data from four hospitals in Chengdu were collected. Through exploratory factor analysis, an indicator system for medical humanities literacy and job competency more suitable for the research population was obtained and the conceptual model was adjusted. Finally, the positive mechanism of medical humanities literacy on job competency was verified using confirmatory factor analysis (structural equation model). ResultsThe Cronbach's alpha coefficient of the overall questionnaire involved in the exploratory and confirmatory factor analyses was greater than 0.9, and the KMO values of the questionnaire were greater than 0.8, with a Bartlett's sphericity test P value less than 0.01, indicating good reliability and validity of the questionnaire. The model fit of the structural equation model met the requirements, with a chi-square degree of freedom ratio (CMIN/df) of 2.768, root mean square error of approximation (RMSEA) of 0.077, comparative fit index (CFI) of 0.891, normalized fit index (NFI) of 0.840, and incremental fit index (IFI) of 0.891. ConclusionThe improvement of individual characteristics in job competency is mainly influenced by medical humanities knowledge and spirit. The improvement of cognitive characteristics in job competency is mainly influenced by medical humanities knowledge and ability. The improvement of achievement characteristics in job competency is mainly influenced by medical humanities ability and spirit. The improvement of management characteristics in job competency is mainly influenced by medical humanities spirit.
