  • Effect of Nimesulide on DMBAInduced Mammary Tumors in Rats

    【Abstract】ObjectiveTo observe the effect of nimesulide (NIM) on DMBA-induced mammary tumors and to investigate possible mechanisms of inhibiting tumors. MethodsThe Wistar rats were randomly divided into four groups: DMBA group, NIM+DMBA group, NIM+diet group and diet group. The incidence and mean latent phase of mammary tumors were observed. The number and volume of tumors in every rat were measured. The apoptosis index and proliferation index were evaluated by TUNEL assay and PCNA immunohistochemical staining respectively.Results The latent phases of mammary tumors in NIM+DMBA group were strikingly longer than those in DBMA group 〔(115±14.8) d vs (84±15.6) d,P<0.01〕. The incidence of mammary tumors was 69.2% in DMBA group, 46.4% in NIM+DMBA group. There was obviously decreased incidence in NIM+DMBA group (P<0.05). The number and the mean volume of mammary tumors in every rat in NIM+DMBA group were markedly less than DBMA group 〔(0.82±0.31) vs (1.52±0.43),(3.24±1.21) cm3 vs (6.03±1.84) cm3,P<0.05〕. The apoptosis index was obviously higher and the proliferation index was markedly lower in NIM+DMBA group compared with DMBA group〔(42.54±13.24)% vs (18.14±6.66)%, (20.25±4.97)% vs (36.36±5.02)%, P<0.05〕. ConclusionNIM can inhibit the development of DMBA-induced mammary tumors by inhibiting proliferation and inducing apoptosis.

  • Efficacy of ultrasound-guided percutaneous lavage for rotator cuff calcific tendinopathy: a meta-analysis

    ObjectivesTo systematically review the clinical efficacy of ultrasound-guided percutaneous lavage (UGPL) for calcifying tendinitis of rotator cuff.MethodsPubMed, The Cochrane Library, EMbase, WanFang Data, CBM, VIP and CNKI databases were electronically searched to collect the randomized controlled trials (RCTs) of ultrasound-guided percutaneous irrigation for calcifying tendinitis of rotator cuff from inception to September 2017. Two reviewers independently screened literature, extracted data and assessed the risk of bias of included studies. Meta-analysis was performed by using RevMan 5.3 software.ResultsA total of 7 RCTs involving 587 patients were included. The results of meta-analysis showed that: there were significant differences between the UGPL group and extracorporeal shock wave therapy (ESWT) group in 12-month follow-up visual analogue scale (VAS) (MD=–1.96, 95%CI –2.18 to –1.75, P<0.000 01), the average particle size of the calcium deposition (MD=–3.13, 95%CI –5.05 to –1.22,P<0.001), the calcification disappearance rate (RR=1.65, 95%CI 1.36 to 2.01,P<0.000 01). However, there were no significant differences between the two groups in 6-week follow-up VAS (MD=–0.85, 95%CI –2.84 to 1.14,P=0.40), and the complications (RR=1.20, 95%CI 0.03 to 49.69, P=0.93).ConclusionsCurrent evidence shows that UGPL has certain clinical effect for the calcifying tendinitis of rotator cuff. Compared with ESWT, UGPL may be superior in clinical pain relief and calcification clearance. Due to limited quality and quantity of the included studies, more high quality studies are required to verify above conclusions.

  • Analysis and Countermeasures of Work Satisfaction in Operating Room

    【摘要】 目的 调查手术室工作多层面满意度,分析影响其满意度的因素,提出干预措施。 方法 采用自行设计调查表对手术医生、手术患者或家属、病区护士、手术室护生进行手术室工作满意度调查分析。 结果 2009年4季度各层面满意度gt;95%,比1季度满意度提高了9.51%,与1季度相比有统计学意义(Plt;0.05)。且护理人员素质、手术室管理、人文服务等都有不同程度的提高。 结论 对手术室满意度进行多层面定期调查和不定期抽查,全方位地分析不同层面影响满意度的因素,采取有效的干预措施,达到持续质量改进。【Abstract】 Objective To investigate the multidimensional satisfaction in the operating room’s work and to analyse various factors and intervention measures. Methods The multidimensional satisfaction in the operating room’s work of surgeon, surgical patients, family members, ward nurses, and practical nurses were investigated and anlyzed by self-designed questionnaire. Results In 2009, various satisfaction rate of the fourth quarter is 95%, compare with the the first quarter, the satisfaction rate of the fourth quarter increased 9.51%, the difference was statistically significant(Plt;0.05). The paramedic’s quality, management of operating room, quality of nurse, human services have got different increase. Conclusion To conduct the multidimensional periodic surveys and occasional spot checks about satisfaction rate of the operating room, and to analyze roundly the different levels factors of affecting the satisfaction rate can take effective interventions to achieve continuous improvement of the nurse quality.

  • Therapeutic efficacy of GM-CSF inhalation in patients with recurrent pulmonary alveolar proteinosis

    Objective To evaluate therapeutic efficacy and safety of recombinant human granulocyte-macrophage colony-stimulating factor (GM-CSF) inhalation in patients with recurrent pulmonarv alveolar proteinosis (PAP). Methods Three cases of recurrent PAP were treated by GM-CSF inhalation after whole lung lavage. The clinical data of the pulmonary function and SpO 2, the clinical symptoms and pulmonary lesions were compared before and after treatment. Results The pulmonary function and manifestations were improved obviously after GM-CSF inhalation. Also the ground-glass opacity was improved in high-resolution CT. The pulmonary function and SpO 2 increased obviously after received GM-CSF inhalation. There were no any adverse reactions in 3 cases. Conclusion GM-CSF inhalation therapy is effective and safe in recurrent PAP, but the long-term effect remains to be seen.

  • The application of minimally invasive tricuspid valvuloplasty technique with patch augmentation in reoperative cardiac surgery

    Objective To evaluate the efficacy of a combination of beating-heart minimally invasive approach and leaflets augmentation technique treating severe tricuspid regurgitation (TR) after cardiac surgery. Methods From January 2015 to August 2017, patients undergoing reoperative tricuspid valve repair (TVP) with minimally invasive approach and leaflets augmentation were enrolled. Cardiopulmonary bypass (CPB) was established via femoral vessels and the procedures were performed on beating heart with normothermic CPB. A bovine pericardial patch was sutured to leaflets to augment the native anterior and posterior leaflets. Other repair techniques, such as ring implantation and leaflet mobilization, were also applied as needed. Results A total of 28 patients (mean age 55.6±10.1 years, 5 males, 23 females) were enrolled. One patient was converted to median sternotomy due to pleural cavity adhesion. Twenty-seven patients underwent totally endoscopic TVP with leaflets augmentation. No patients was transferred to tricuspid valve replacement. Two patients died in hospital. All patients were followed up for 7.4±5.0 months and there was no late death and reoperation. Regurgitation area was converted from 20.7±10.1 cm2 to 3.3±3.3 cm2 after TVP according to the latest echocardiography (P<0.001). Conclusion Minimally TVP with leaflets augmentation is effective in treating severe isolated TR after primary cardiac surgery. It can significantly increase success rate of tricuspid valvuloplasty and decrease the surgical trauma.
