Objective To summarize the experience of the surgical treatment of complex congenital heart diseases in 29 newborns. Methods Twenty-nine newborns were operated on for various congenital heart diseases, ages were from 3 to 28 d. There were ventricular septal defect 3 cases, D-transposition of the great arteries (D-TGA) 10 cases, pulmonary atresia (PA) 1 case,tricuspid atresia 3 cases, single ventricle 1 case, tetralogy of Fallot 6 cases, endocardial cushion defect 4 cases and truncus arteriosus 1 case. All patients were combined with atrial septal defect and patent ductus arteriosus. All operations were performed under hypothermic cardiopulmonary bypass. Results The operative mortality was 13.8%(4/29). One case with D-TGA and 1 case with PA succumbed due to low cardiac output syndrome, and 2 cases due to acute respiratory distress syndrome and low blood oxygen saturation. Twenty-five cases were discharged. Follow-up was completed in 19 patients,with a duration of 1-31 months, all patients have been normal. Conclusion Anesthesia and cardiopulmonary bypass should be performed smoothly and satisfactorily in newborn babies for cardiac surgery. Surgical manipulation should be accurate with less trauma and well protection of myocardium and lung is important.
Objective To observe the treatment efficacy and safety of glucosamine hydrochloride tablets on uremia patients with knee osteoarthritis (OA). Methods A total of 118 uremia patients with knee OA were selected and randomly divided into the glucosamine hydrochloride tablets treatment group (treatment group) and the coated aldehyde oxystarch capsules group (control group) with 59 cases in each group. The course was 8 weeks. The Lequesne Index was assessed for curative effect evaluation, and the change of blood indexes was observed to evaluate drug safety. Results The total effective rate of Lequesne Index in the treatment group was 72.9%, while that in the control group was 13.6%; the difference was statistically significant (χ2=42.303, P<0.001). There was no significant change in the two groups before and after treatment in terms of the patients’ dialysis adequacy, routine blood, blood electrolytes, liver and kidney function (P>0.05). Conclusion Glucosamine hydrochloride tablets is curative and safe in the treatment of uremia patients with OA.
Objective The Chengdu initiative essential medicine policy is part of the Special Healthcare Program of Comprehensive Reform for Coordinated and Balanced Urban-rural Development. We aimed to investigate the current situation of medicine use in rural hospitals and community health service centers, so as to provide evidence for policy-makers to select essential medicines and facilitate rational use of medicines. Method We selected 7 township/community health institutions from which to collect medicine use information, including medicine category, number of medicine categories, cost and consumption. Descriptive analysis and the ABC classification method were applied for statistical analysis. Results The number of medicine categories used in the community health institutions was four times greater than that in the township health institutions. Traditional Chinese medicine preparations accounted for 40% of the total medicine cost. Polypharmacy, overuse of injections, and improper use of antibiotics were major manifestations of the irrational use of medicines. Conclusion The selection and use of essential medicines should be base on high quality evidence as well disease burden, the economic situation and specific demands in different areas. Drug and therapeutics committees should be set up to perform dynamic monitoring, education, evaluation and continual improvement of an essential medicines list.
