  • Clinical features of bacterial liver abscess with diabetes

    ObjectiveTo compare the clinical features of bacterial liver abscess (BLA) with or without diabetes and provide reference for clinical diagnosis and treatment.MethodsThe clinical data of 312 patients with BLA admitted to Huaian First People’s Hospital of Nanjing Medical University from January 2016 to January 2020 were collected. The differences in the clinical symptoms and signs, results of laboratory, imaging, etiological tests, treatment methods, and outcomes of the patients between with and without diabetes were compared.ResultsA total of 312 patients with BLA were collected, 128 BLA patients with diabetes and 184 BLA patients without diabetes. ① There were no significant differences in gender and age between patients with and without diabetes, but the boby mass index of BLA patients with diabetes was higher than that of the patients without diabetes [(25.54±4.99) kg/m2 versus (23.75±3.92) kg/m2, t=3.546, P<0.001]. ② In terms of etiology, the main cause of BLA was the biliary tract infection [37.18% (116/312)]. The incidence of cryptogenic infections in the BLA patients with diabetes was significantly higher than that of the BLA patients without diabetes [39.06% versus 26.63%, χ2=5.386, P=0.020]. ③ In terms of clinical manifestations, the fever was the main symptom of BLA [97.76% (305/312)]. The incidences of abdominal pain and percussion pain in the liver area of BLA patients with diabetes were significantly lower than those in the BLA patients without diabetes [32.03% versus 51.63%, χ2=11.793, P=0.001; 15.63% versus 30.98%, χ2=9.572, P=0.002]. ④ In terms of laboratory tests, compared with the patients without diabetes, the albumin level was lower [(30.88±5.25) g/L versus (33.67±4.33) g/L, t=–5.139, P<0.001], the procalcitonin level and neutral cell ratio were higher [(44.22±39.56) μg/L versus (36.03±22.73) μg/L, t=2.312, P=0.021; (86.68±7.05)% versus (80.73±8.12)%, t=6.710, P<0.001] in the patients with diabetes. ⑤ In terms of imaging findings, the BLA was mainly single abscesses [77.56% (242/312)] and mainly in the right lobe [66.34% (207/312)]. ⑥ In terms of microbiological examination, the bacterial positive detection rate of all patients was 71.15% (222/312). The main pathogen was Klebsiella pneumoniae [71.62% (159/222)]. The Klebsiella pneumoniae infection rate in the BLA patients with diabetes was higher than that in the BLA patients without diabetes [79.30% versus 66.67%, χ2=4.161, P=0.041]. ⑦ The septic shock occurred in 20 (6.41%) patients with BLA. After treatment of all patients, 4 cases of BLA with diabetes and 2 cases of BLA without diabetes died. The patients who died all came from septic shock.ConclusionsClinical manifestations of BLA patients with diabetes are atypical and main infection is Klebsiella pneumoniae. When BLA combined with septic shock, individual treatment strategy should be chosen basing on actual situation of patient.

  • Clinical Epidemiological Characteristics of Gastric Cancer in Recent 10 Years

    Objective To explore the clinical epidemiologic characteristics and tendency of gastric cancer during recently ten years in northern Henan province. Methods The clinical data of 1 090 patients with gastric cancer in our department were collected from January 1998 to May 2008. The ten-year period was divided into two groups: previous 5 years group (n=433) and post 5 years group (n=657). The age, gender, pathologic characteristics and the relationship between age and pathologic features were analyzed retrospectively. Results ①The patient’s age was 15-83 years old 〔mean (57.60±10.84) years old〕 and men-momen ratio was 3.71∶1 in previous 5-year group. The patient’s age was 18-82 years old 〔mean (58.95±10.81)years old〕 and men-momen ratio was 2.84∶1 in post 5-year group. There was no significant difference in age or gender between two groups (Pgt;0.05). ②Gastric cancer arised in cardia and gastric fundus more common and for the incidence of the site no change was found in two groups. The incidence of gastric antrum cancer descended but that of gastric body arised. ③The mean age of patients with gastrocardiac cancer was significantly different (Plt;0.05) between two groups. No significant difference occurred on the mean age of patients with gastric antrum (Pgt;0.05) or gastric body cancer (Pgt;0.05). The mean age of patients with highly malignant tumor (signet ring cell cancer, poorly differentiated adenocarcinoma and undifferentiated carcinoma) was significantly different compared with that of patients with lowly malignant tumor (well differentiated and moderately differentiated adenocarcinoma) (Plt;0.05). ④The proportion of patients with highly and lowly malignant tumor presented significantly different between the previous 5 years and the post 5 years (Plt;0.05). ⑤The proportion of early gastric cancer was very low and no correlation was found between infiltrating depth and age in all patients (Pgt;0.05). Conclusion Cardia and gastric fundus is the most common site of gastric cancer in northern Henan province. The patients with gastrocardiac cancer are often older but gastric antrum and gastric body cancer patients are younger. Highly malignant gastric cancer is often found in youths, however, lowly malignant gastric cancer occurred predominantly in elderly. The proportion of gastric cancer found early is very low in northern Henan province.

  • RECONSTRUCTION OF MANDIBULAR BONE DEFECTS USING THREE-DIMENSIONAL SKULL MODEL AND INDIVIDUALIZED TITANIUM PROSTHETICS FROM COMPUTER ASSISTED DESIGN

    【Abstract】 Objective To evaluate the feasibility and effectiveness of reconstruction of mandibular bone defects using three-dimensional skull model and individualized titanium prosthetics from computer assisted design. Methods Between July 2002 and November 2009, 9 patients with mandibular defects accepted restorative operation using individualized bone prosthetics. Among 9 cases, 4 were male and 5 were female, aged 19-55 years. The causes of mandibulectomy were benign lesions in 8 patients and carcinoma of gingival in 1 patient. Mandibular defects exceeded midline in 2 cases, involved condylar in 4 cases, and was limited in one side without involvement of temporo-mandibular joint in 3 cases. The range of bone defects was 9.0 cm × 2.5 cm-17.0 cm × 2.5 cm. The preoperative spiral CT scan was performed and three-diamensional skull model was obtained. Titanium prosthetics of mandibular defects were designed and fabricated through multi-step procedure of reverse engineering and rapid prototyping. Titanium prosthetics were used for one-stage repair of mandibular bone defects, then two-stage implant denture was performed after 6 months. Results The individualized titanium prosthetics were inserted smoothly with one-stage operative time of 10-23 minutes. All the cases achieved incision healing by first intention and the oblique mandibular movement was corrected. They all got satisfactory face, had satisfactory contour and good occlusion. In two-stage operation, no loosening of the implants was observed and the abutments were in good position with corresponding teeth which were designed ideally before operation. All cases got satisfactory results after 1-9 years of follow-up. At last follow-up, X-ray examinations showed no loosening of implants with symmetry contour. Conclusion Computer assisted design and three-dimensional skull model techniques could accomplish the design and manufacture of individualized prosthetic for the repair of mandibular bone defects.
