Objective To detect the tissue factor (TF) mRNA expression in hepatocellular carcinoma and to elucidate its significance. Methods TF mRNA was detected by reverse transcription-polymerase chain reaction (RT-PCR) in 27 cases of human hepatocellular carcinoma tissue specimen with their adjacent tissues and in 27 non-tumorous process tissues. Then the relationship between mRNA expression and pathological data were analyzed. Results The expression and the relative expression intensity of TF in hepatocellular carcinoma tissues were 62.96%(17/27) and 0.567±0.268 respectively, which were significantly higher than those in their adjacent tissues 〔33.33%(9/27), 0.469±0.184〕 and in 27 non-tumorous process tissue 〔29.63%(8/27), 0.353±0.121〕, Plt;0.05. The relative expression intensity of TF were associated with tumor size, intrahepatic and extrahepatic metastasis and portal vein invasion, but unrelated to gender, AFP level, differentiation, HBsAg, cirrhosis, number of tumor lesions, and lymph node metastasis (Pgt;0.05). Conclusion Expression of TF mRNA were significantly higher in hepatocellular carcinoma and in the invasive and metastatic tissue, which indicated that TF may play an important role in carcinogenesis, invasion and metastasis of hepatocellular carcinoma.
Objective Engineer heart tissue (EHT) was constructed with mesenchymal stem cells (MSCs) and poly lacticacidCOglycolic acid (PLGA), and grafted onto the surface of myocardial infarction rats. We hypothesized that great omentum wrapping would increase EHT blood supply and ameliorate EHT microenvironment which is in favor of cardiac collagen remodling and heart function. We hope that omentun wrapped EHT could provide a valuable strategy for surgically myocardial infarction therapy. Methods MSCs were isolated from SD rats.Eight weeks after SD rats were subjected to left anterior descending (LAD) ligation, 18 rats were enrolled and divided into three groups, group A(n=6): great omentum wrapped MSCsPLGA EHT implantation; group B (n=6):MSCsPLGA EHT implantation; control group (n=6): the myocardial infarction; the sham group (n=6): only opened and closed chest, underwent LAD ligation, but no EHT implantation. Four weeks after transplantation, the following variables were evaluated: specimen stained with picrosirius red, left ventricle function evaluated by echocardiography, infarction ventricular wall motion by color kinesis (CK). Results Hearts of group A showed significantly less fibrosis than group B and control group (Plt;0.05). Infarction ventricular wall motion assessed by CK indicated significantly improvement in group A compared with group B and control group (Plt;0.05). Four weeks after transplantation, cardiac echocardiography showed left ventricle ejection fraction was lower in control group and group B compared with group A (Plt;0.05). Conclusion Transplantation of MSCsPLGA EHT with great omentum wrapping ameliorated infarction ventricular collagen remodeling, ameliorated infarction ventricular wall motion and preserved left ventricular function.
Objective To systematically review the value of ultrasound in the diagnosis of fracture of the distal radius and ulna for providing evidence for clinical diagnosis. MethodsWe searched PubMed, EMbase, The Cochrane Library (Issue 7, 2016), CNKI, CBM and WanFang Data databases for studies about ultrasound in the diagnosis of fracture of the distal radius and ulna from inception to August 1st, 2016. Two reviewers independently screened literatures according to inclusion and exclusion criteria, extracted data, and assessed the risk of bias of included studies by using the QUADAS-2 (Quality Assessment of Diagnostic Accuracy Studies 2) tool, and then meta-analysis was performed by using Meta-Disc 1.4 software. ResultsA total of 16 articles involving 1 182 patients and 631 fractures were included. The results of heterogeneity test revealed that there was statistical heterogeneity among the results of studies, thus, random effects model was used for meta-analysis. The results of meta-analysis showed that the pooled sensitivity, specificity, positive likelihood ratio, negative likelihood ratio, diagnosis odds ratio for ultrasound diagnosis fracture of the distal radius and ulna were 0.95 (95%CI 0.94 to 0.97), 0.92 (95%CI 0.89 to 0.94), 9.98 (95%CI 5.20 to 19.15), 0.07 (95%CI 0.04 to 0.13), 187.94 (95%CI 65.67 to 537.84), respectively. The area under the SROC curve was 0.976 4. ConclusionUltrasound is a useful non-invasive method as a routine examination for fracture of the distal radius and ulna. Due to the limited quality and quantity of included studies, the above conclusion is needed to be validated by more high-quality studies.
The postpericardiotomy syndrome (PPS) is an inflammation of the pericardium or pleura following a variety of pericardial injuries. The potential pathogenic factors of the PPS are autoimmune, special virus and latent virus infection. PPS is self-limited, but may lead to prolonged hospital stay, readmissions, and need for invasive interventions. The therapy for PSS is mainly empiric anti-inflammatory therapy. The perioperative use of colchicine could reduce the incidence of PPS but is not effective for postoperative atrial fibrillation or postoperative pericardial/pleural effusion. This article mainly analyzes the incidence, risk factors, clinical features, diagnosis and treatment standards, preventive measures and prognosis of PPS.
Objective To investigate the genotype and phenotype in patients with leber congenital amaurosis (LCA), and offer accurate genetic counseling and prenatal diagnosis for those families. Methods Three LCA patients and their parents were recruited for this study and received detailed collection of medical history and family history from March to August 2016. The three patients received fundus fluorescein angiography examination and their parents received slit-lamp microscope and indirect ophthalmoscopy examinations. DNA was extracted from the patients and their family members. Whole-exome sequencing method was used for genetic diagnosis and typing of the three LCA patients and their parents. Results The three patients with different clinical features had a definite clinical diagnosis of LCA. Patient 1 showed pale disc, attenuated vessels aroud the optic disc and the salt-and-pepper appearance of the retina, had the homozygous c.744.745insT (p.249, L>Ffs4) mutation inSPATA7. Patient 2 showed optic disc pallor and attenuated retinal vessels, had the heterozygous c.535G>A, p.A179T mutation inWFS1. Patient 3 showed pale disc, atrophic macular and retinal and choroidal degeneration, had the heterozygous mutation in CRB1, RPGRIP1, SPATA7. Conclusion LCA has characteristics of genetic heterogeneity and clinical and phenotypic diversity.
ObjectiveTo explore the significance of ultrasound combined with magnetic resonance imaging (MRI) in the diagnosis of pernicious placenta previa (PPP).MethodsA total of 101 patients with suspected PPP admitted to Chengdu Women’s and Children’s Central Hospital from January 2016 to February 2019 were retrospectively analyzed. The surgical results were used as the gold standard, and the diagnostic value of ultrasound, MRI, ultrasound combined with MRI was compared.ResultsSurgery results showed that among the 101 patients with suspected PPP, 79 patients had positive PPP diagnosis and 22 patients had negative PPP diagnosis. There was no significant difference in the diagnostic sensitivity, negative predictive value, or diagnostic coincidence rate of PPP among ultrasound, MRI, and ultrasound combined with MRI (P>0.05). The diagnostic specificity of the series combination of ultrasound and MRI (86.36%) was higher than that of ultrasound (59.10%), MRI (59.10%), and the parallel combination of ultrasound and MRI (31.32%), and the differences were statistically significant (P<0.05). The diagnostic coincidence rates of abnormal placental attachment of MRI (96.20%) and the parallel combination of ultrasound and MRI (96.20%) were higher than that of ultrasound (87.34%) and the series combination of ultrasound and MRI (87.34%) (P<0.05).ConclusionsUltrasound combined with MRI has a high diagnostic value for PPP. If necessary, ultrasound combined with MRI can be considered to improve the diagnostic efficiency.
ObjectiveTo study the expression of p27 in hepatocellular carcinoma (HCC) and its clinical significance.MethodsFortyfive specimens of HCC and 30 specimens of adjacent noncancerous lesions obtained from 45 patients who underwent surgery were examined for p27 expression by immunohistochemistry SABC method. The diameter of tumor ranged from 1 cm to 19 cm (d ≤5 cm, 9 samples; 5 cmlt;d≤10 cm, 19 samples; d>10 cm, 17 samples). These tumors were graded according to the criteria described by EdmondsonSteiner: highdifferentiated HCC group (Grade Ⅰ+Ⅱ), 26 samples; lowdifferentiated HCC group (Grade Ⅲ+Ⅳ), 19 samples. According to the clinicopathological features: 19 samples were poorly encapsulated, 15 samples had portal invasion, 11 samples had extrahepatic metastasis, 12 samples had intrahepatic metastasis; all of the above were classified as the invasive and metastatic group, while the others were classified as the noninvasive and nonmetastatic group. ResultsThe average labeling index (LI) of p27 in HCC was significantly higher than that of adjacent noncancerous lesions (45.87±14.21 vs 33.77±12.92, Plt;0.001). The LI of p27 in lowdifferentiated HCC group (stage Ⅲ+Ⅳ) was significantly lower than that of highdifferentiated group (stage Ⅰ+Ⅱ), 34.46±12.29 vs 52.80±11.36 (Plt;0.001). The LI of p27 had significant difference between the large ones (d>10 cm, 37.59±13.12) and the small ones (d≤5 cm, 53.28±15.17 or 5 cmlt;d≤10 cm, 49.50±10.96) (Plt;0.05). The LI of p27 in the invasive and metastatic group was significantly lower than that in the noninvasive and nonmetastatic group (41.42±12.86 vs 51.44±14.10, Plt;0.05).ConclusionThe expression of p27 is more frequently detected in HCC than in adjacent noncancerous lesions. It indicates that p27 might be a compensatory factor during HCC carcinogenesis. The LI of p27 significantly decreases in poor differentiation group, invasive and metastatic group. It indicates that p27 might be related with the differentiation, invasion and metastasis of HCC.
ObjectiveTo systematically review the efficacy and safety of simvastatin and its different doses in the adjunct therapy of chronic obstructive pulmonary disease (COPD).MethodsPubMed, EMbase, Web of Science, The Cochrane Library, CNKI, WanFang Data, CBM and VIP databases were electronically searched to collect randomized controlled trials (RCTs) on adjunct therapy of simvastatin in patients with COPD from inception to May 15th, 2020. Two reviewers independently screened literature, extracted data and assessed risk bias of included studies; then, meta-analysis was performed by using Stata 14.0 software.ResultsA total of 22 RCTs involving 2 377 patients were included. The results of meta-analysis showed that treatment with 20 mg simvastatin could improve FEV1%pred, FEV1/FVC, and reduce inflammatory indexes such as CRP, hs-CRP, IL-8 and TNF-α, while 40 mg failed to improve. Simvastatin could reduce COPD score (CAT), but failed to increase the 6-minute walking distance or alleviate acute exacerbation.ConclusionsCurrent evidence shows that treatment with 20 mg simvastatin can improve pulmonary function, reduce inflammatory index and optimize CAT score in COPD patients, but it cannot increase the 6-minute walking distance and reduce the number of acute exacerbations of COPD. Due to the limited quantity and quality of included studies, the above conclusions are needed to be verified by more high-quality studies.
Atrial septal defect (ASD) is a congenital heart disease that causes blood communication between the left and right ventricles due to partial atrial septal tissue defects, accounting for about 13% of all heart malformations. Secondary ASD is the most common type of ASD and can generally be treated with minimally invasive closure. At present, the commonly used minimally invasive methods in clinical practice mainly include X-ray-guided percutaneous occlusion, transesophageal ultrasound-guided transthoracic occlusion and ultrasound-guided percutaneous occlusion. This review focuses on the basic research process of occluder materials, and advantages and disadvantages of three different surgical methods.
Objective To investigate the cl inical efficacy, compl ications and necessity of removing internal fixation in treatment of fresh Tossy type III acromioclavicular joint dislocations and Neer type II distal clavicle fractures with clavicularhook plate. Methods From June 2005 to June 2008, 24 patients with fresh Tossy type III acromioclavicular joint dislocations and 20 patients with fresh Neer type II distal clavicle fractures were treated. There were 32 males and 12 females with an agerange of 18-66 years (38.5 years on average), involving 18 left shoulders and 26 right shoulders. The injury was caused bytraffic accident in 31 cases and by fall ing in 13 cases. The mean time from injury to operation was 4 days (range, 2-8 days). All patients were treated by reduction with clavicular hook plate fixation. The coracoclavicular l igaments were not sutured. The shoulder functions were evaluated according to University of Cal ifornia-Los Angeles (UCLA) score system and analysed before and after removing internal fixation. Results Wound infection occurred in 2 cases 1 week after operation and healed after symptomatic managment, the other incisions healed by first intention. One case accepted hook plate fixation again because of loosening hooking-up 1 week after operation. One case accepted hook plate removal and Kirschner wire fixation because of severe shoulder’s pain on the postoperative third day. Thirty-eight patients were followed up for 8-32 months (18 months on average), there was no plate breakage. Clavicle fractures got bony union after 3-6 months (4.2 months on average). At last followup (before plate removal), according to UCLA shoulder function score system, the results were excellent in 11 cases, good in 22 cases, and fair in 5 cases; the excellent and good rate was 86.8%. Because of shoulder’s pain, plates were removed in 20 patients 3-16 months (10 months on average) after operation. The cases were followed up 3-8 months (5 months on average) after removing plate. No dislocation and fracture occurred again. There was statistically significant difference (P lt; 0.01) in the functional scores of shoulder between before (30.55 ± 4.00) and after removing internal fixation (33.85 ± 1.95). Conclusion Clavicular hook plate fixation is an effective treatment for fresh Tossy type III acromioclavicular joint dislocations and Neer type II distal claviclefractures. Normative operating, correct plate moulding, functional rehabil itation after operation are key factors in preventingcompl ications and reaching good cl inical efficacy. For the patients with postoperative symptoms, the plate should be removed to improve the shoulder’s function.
