  • Effects of Nitroglycerine, Verapamil and Papaverine on Relaxing Function of Human Radial Arteries

    Abstract: Objective To compare the effects of nitroglycerine (NTG), Verapamil(VP), papaverine(PA) and the mixed solution of Verapamil and nitroglycerine (VG) on relaxing function of human radial arteries. Methods The radial arteries of thirty patients were used during the operation of coronary artery bypass grafting (CABG). A short segment (1.0-1.5cm) of radial arteris were taken from the distal end of radial arteries of each patient and were cut into vascular rings, which were mounted in the organ bath chamber and then subject to a series of tests for vascular smooth muscle viability and endothelial integrity. The effects of five storage solutions on the relaxing function were evaluated by “OrganBath” technique. The five solutions included: (1) Ringer’ s solution (control group); (2) VP solution (VP group); (3) NTG solution (NTG group); (4) PA solution (PA group); (5) VG solution (VG group). First, challenged with phenylephrine (10-5mol/L), vasorelaxant effect of these drugs (effect onset and efficacy) was observed at different time point and resting tension was recorded. Second, after 30min preincubation with either verapamil, papaverine, phenoxybenzamine or VG mixture, potassium chloride (final concentration of 60mmol/L) was added in the organ bath chamber and then vasoconstriction was observed subsequently. Finally, after 30min pretreatment of different antispasmodic agent in the same way as described above, the vascular rings were mounted in organ bath chamber and challenged with phenylephrine(10-5mol/L). Vascular spasticity and vosospasm duration were observed at different time point which might provide guidance for optimal timing of clinical application. Results The radial arteries in VG, VP, NTG and PA solutions were relaxed in 11 min after vasospasm and there was no difference between them (Pgt;0.05). But during the initial three minutes,the relaxation effect of VG and NTG was significantly better than other two groups. Relaxation curve showed that the ability of vasodilatation of VG, NTG, VP and PA decreased in order. In the experiment about antivasospasm pretreatment of radial arteries, there was no difference between VG and VP group (Pgt;0.05 ), whose effects were better than NTG and PA group(Plt;0.05 ). After cold storage for 24h, VG and VP still could prevent vasospasm. But NTG and PA hardly had any function and there was no difference compared with the control group (Pgt;0.05 ). Conclusion Although in the final all these drugs could prevent and relieve vasospasm of radial arteries in the different level, it appeared that a combination of verapamil and nitroglycenn is more fit for treating radial artery during CABG operation than other drugs.

  • Feasibility and clinical significance of monitoring diabetic macular edema by Heidelberg retina tomograph Ⅱ

    Objective To investigate the feasibility and clinical significance of monitoring diabetic macular edema by Heidelberg retina tomograph Ⅱ (HRT). Methods The diabetic macular edema (DME) was diagnosed by slit-lamp microscopy combined with three-mirror contact lens examination and fundus fluorescein angiography (FFA). The exponential of macular edema (e value) of healthy people and patients with DME or without DME (NDME) (the total number is 77 individuals and 120 eyes) were detected by HRT Ⅱ. All of the 77 people were divided into three groups. In DME group, there were 23 patients (40 eyes), including 13 males (23 eyes) and 10 females (17 eyes), at the age of 44-68 (average of 55.17plusmn;8.26). In NDME group, there were 32 patients (40 eyes), including 18 males (22 eyes) and 14 females (18 eyes), at the age of 44-68 (average of 55.17plusmn;6.5). In normal control group, there were 22 patients (40 eyes), including 10 males (19 eyes) and 12 females (21 eyes), at the age of 42-65 (average of 53.32plusmn;6.04). According to the results of FFA, the 40 eyes in DME group were divided into: grade 1 of FFA in 9 eyes, with macular suspicious leakage or the area of leakage of lt;25%; grade 2 of FFA in 10 eyes, with the area of leakage between 25% and 66%; grade 3 of FFA in 21 eyes, with the area of leakage of gt;66%. The differences of sex and age among the 3 groups were not significant (Pgt;0.05). The relationship among e value, leakage area, and visual acuity was observed. Results There was a significant difference of e value (the macular diameter was 1, 2, and 3 mm) among the 3 groups(Plt;0.05). The e value in normal control group didnrsquo;t differ much from which in NDME group (Pgt;0.05), but was statistically different from which in DME group (Plt;0.05). Significant difference of e value was also found between NDME group and DME group (Plt;0.05). There was a correlation between visual acuity and e value in DME group (Plt;0.05). In DME group, the difference of e value among FFA grade 1, 2, and 3 groups was found according to the variance analysis; the macular leakage area in FFA grade 3 group differed much from which in grade 1 (Plt;0.05) and grade 2 group (Plt;0.05), while no significant difference was found between grade 1 and grade 2 group. The result was not correlated with the macular diameter. Conclusion E value in the macular module of HRT Ⅱ can detect and evaluate the degree of DME. (Chin J Ocul Fundus Dis,2007,23:252-255)

  • Comparison of Milrinone, Phenoxybenzamine, and Mixture of Nitroglycerin and Verapamil for the Prevention and Treatment of Human Radial Artery Spasm

    Objective To compare milrinone (MIN), Phenoxybenzamine (PHE),and mixture of nitroglycerin and verapamil(NVC) for the prevention and treatment of human radial artery spasm. Methods Residuary radial arteries from 30 patients undergoing coronary artery bypass grafting (CABG) using autologous radial artery from March to September of 2012 in General Hospital of Shenyang Military District were collected. All the artery specimens were cut into 60 vessel rings of 3 mm in width and hanged in the Organ-Bath. Using a random number table,all the vessel rings were divided into 4 groups:PHE group,NVC group,MIN group and blank group (control group). Firstly,20 vessel rings were used for spasm relaxation experiment with 5 rings in each group. Phenylephrine was added into the Organ-Bath to reach final concentration of 10-3 mol/L in order to cause vessel rings spasm. PHE of 1 mmol/L,NVC of both 30 μmol/L,MIN of 30 μmol/L and Krebs-Ringer’s solution were added respectively into above groups to compare the relaxant effect of these vasodilators on radial artery spasm. Secondly,the other 40 vessel rings were used for spasm prevention experiment with 10 rings in each group. All the 40 vessel rings were dipped in above 4 groups. After 30 minutes,phenylephrine was added to the Organ-Bath to reach final concentration of 10-3 mol/L. Time-dependent constriction of the vessel rings were compared to examine the prevention effect of these vasodilators on radial artery spasm. Results In the spasm relaxation experiment, vessel ring spasm relaxed most quickly in NVC group (P< 0.01). Vessel rings in MIN group relaxed more slowly but steadily (P< 0.05). Vessel rings in PHE group relaxed most slowly (P<0.01). And there was no relaxation in the control group. In the spasm prevention experiment,vessel rings in the control group contracted (spasm) 100% immediately after phenylephrine administration. Vessel rings in MIN group contracted immediately after phenylephrine administration too,but slower than the control group(P<0.01). Vessel rings in NVC group did not contract right after phenylephrine administration,but the sedentary tension of these vessel rings gradually increased 120 minutes after phenylephrine administration,and vessel ring contraction reached 46.89% 240 minutes after phenylephrine administration. There was no vessel ring contraction in PHE group. Conclusions All the vasodilators in our experiment are effective for the prevention and treatment of radial artery spasm to different degree,but each medication has its own characteristics,vasodilatation result and time-dependent effect. NVC is most effective to dilate radial artery which has been in spasm state, while radial artery pretreated by PHE is least to become spasm after being stimulated.

  • Protective Strategies for Organ Function of Fulminant Acute Pancreatitis in Early Stage

    ObjectiveTo explore protective strategies for organ function of fulminant acute pancreatitis (FAP) in early stage. MethodsThe clinical data of 37 patients with FAP admitted to this hospital within 72 h after onset of symptoms between January 2000 and September 2010 were analyzed retrospectively. FAP was defined as presence of multiple organ dysfunction syndrome (MODS) within 72 h after onset of symptoms. A total of 37 patients with a confirmed diagnosis of FAP were divided into two groups based on whether adopting the protective strategies for organ function in early stage or not. Patients treated between January 2000 and May 2004 did not adopt the protective strategies for organ function (Named nonprotection group, n=21); The other patients treated between June 2004 and September 2010 adopted protective strategies for organ function (Named protection group, n=16). With the exception of the protecting strategies for organ function, the patients with severe acute pancreatitisin received standard treatment in two groups. The acute physiology and chronic health evaluation (APACHE) Ⅱ score and multiple organ dysfunction score (Marshall), rate of pancreas infection, and case fatality were compared between two groups. ResultsCompared with the nonprotection group on day 3 after admission, APACHEⅡ score and Marshall score decreased in the protection group (APACHEⅡ score : 15.71±2.95 versus 17.72±3.77, P=0.137; Marshall score: 6.93±2.73 versus 8.06±2.36, P=0.206, respectively). In addition, the case fatality on day 7 and in hospital case fatality in the protection group decreased as compared with the nonprotection group (case fatality on day 7: 18.75% versus 33.33%, P=0.461; in hospital case fatality: 43.75% versus 57.14%, P=0.515, respectively). There was no significant difference of the rate of pancreas infection between two groups (P=1.000). ConclusionsIt from a case of a particular disease perspective, though the difference is not significant, but the observed improvement in prognosis is attributed by protective strategies for organ function of FAP in early stage to a certain degree, however it is verified by needing to more cases.

  • Surgical Treatment for Early Left Ventricular Rupture after Mitral Valve Replacement

    Abstract: Objective To investigate the cause, treatment, and prevention strategy of early left ventricular rupture after mitral valve replacement (MVR). Methods We retrospectively analyzed clinical data of 7 patients who had early left ventricular rupture after MVR, among a total of 2 638 MVR patients, between May 1981 and November 2010 in General Hospital of Shenyang Military District. There were 2 male patients and 5 female patients with their age ranging from 28 to 71 years old. One patient was New York Heart Association (NYHA) functional classⅡand 6 patients were NYHA functional classⅢ. Preoperative echocardiography showed that their left ventricular ejection fraction (LVEF) was 49%-60% and their left ventricular end diastolic volume (LVEDV) was 29-42 ml. All the patients underwent prothetic valve replacement under moderate hypothermia, general anesthesia and extracorporeal circulation. Results All the 7patients underwent surgical repair for their early left ventricular rupture. There were 2 patients with TypeⅠ, 4 patients with TypeⅡ, and 1 patient with TypeⅢleft ventricular rupture. Four patients died of hemorrhagic shock or low cardiac output syndrome. Three patients were successfully healed and discharged from hospital 7-15 days after the operation. The 3 surviving patients were followed up for 4.5-18.0 years, and they all had good general condition and satisfactory quality of life. No late pseudo-aneurysm was found during follow-up. Conclusions Early left ventricular rupture is difficult to repair with a high mortality. Effective prevention strategy should be applied to avoid it as much as possible. Once left ventricular rupture occurs during the surgery, extracorporeal circulation should be quickly established, and it’s possible to save patients’ life by reliable intracardiac and epicardial repair according to patients’ individual condition.

  • Evaluation of Fluid Resuscitation Strategy for Severe Acute Pancreatitis in Early Stage

    Objective To evaluate the effectiveness of fluid therapy on an early goal-directed approach to resuscitation basis and the standard fluid therapy of adequate resuscitation on the prognosis of severe acute pancreatitis (SAP)in early stage. Methods The clinical data of SAP patients admitted to surgical intensive care unit within 72 h after onset of symptoms from January 2000 to November 2011 were analyzed retrospectively. A total of 97 patients with a confirmed diagnosis of SAP were divided into two groups based on whether adopting the early goal-directed fluid resuscitation strategies or not. Patients admitted from January 2000 to December 2004 did not adopt the early goal-directed fluid resuscitation strategies,therefore,being allocated into standard fluid therapy group (n=34); patients admitted from January 2005 to November 2011 adopted the early goal-directed fluid resuscitation strategies and who were allocated into early goal-directed fluid therapy group (n=63). With the exception of the fluid therapy strategies,the two group patients received standard treatment for SAP. The acute physiology and chronic health evaluation Ⅱ score (APACHEⅡ) and multiple organ dysfunction score (Marshall), rate of multiple organ dysfunction syndrome (MODS) within the first week after admission,pancreatic infection rate,and in-hospital mortality were compared between the two groups. Results Compared with the standard fluid therapy group on day 3 after admission,APACHEⅡ score and Marshall score decreased significantly in the early goal-directed fluid therapy group (APACHEⅡ score:7.38±4.01 versus 11.35±4.27, P=0.011;Marshall score:4.13±2.06 versus 6.82±3.15, P=0.016). In addition,patients in the early goal-directed fluid therapy group experienced a trend toward lower rates of MODS with the first week after admission,pancreatic infection,and in-hospital mortality (rate of MODS:46.0% versus 61.8%,P=0.139;rate of pancreatic infection:31.7% versus 44.1%, P=0.226;in-hospital mortality:15.8% versus 23.5%,P=0.355) that did not reach statistical significance. Conclusion Although early goal-directed fluid therapy for SAP in acute response stage may have ameliorated the patient’s condition and improved outcome to a certain degree,carefully designed human clinical trials should be performed in a randomized controlled manner to assess the role of such fluid resuscitation in SAP.

  • Clinical characteristics, diagnosis and treatment of thoracic inflammatory myofibroblastic tumor

    ObjectiveTo explore the clinical characteristics, diagnosis and treatment of thoracic inflammatory myofibroblastic tumor.MethodsThe clinical data, pathological features, treatment and prognosis from 10 patients with thoracic inflammatory myofibroblastic tumor confirmed by pathology were analysed retrospectively from April 2012 to April 2019 at Jiangsu Province Hospital (The First Affiliated Hospital of Nanjing Medical University).ResultsTen participants including six males and four females with a mean age of 37.9 years old. Lesions were detected by physical examination in five patients, cough, chest pain and hemoptysis were the common symptoms. A total of 10 lesions including six in the right lung, three in the left lung and one in the mediastinum. Nine patients were treated with surgery, and one patient received high-frequency electrocautery though rigid bronchoscopy under general anesthesia. All the patients were confirmed by immunohistochemistry, positive rate of smooth muscle actin was 70%, positive rate of anaplastic lymphoma kinase was 70%. The mean follow-up time was 35.9 months, and one patient relapsed the other nine patients were cured.ConclusionsInflammatory myofibroblastic tumor is potentially malignant or low malignant, the clinical manifestations and imaging findings are not specific, once confirmed by pathology, radical surgery is the first choice. For the lesion limited to the airway, interventional therapy could be the choice, but close follow up is needed.

  • Influence of balloon post-dilatation on cardiac conduction in patients undergoing transcatheter aortic valve replacement: A retrospective cohort study

    ObjectiveTo analyze the impact of balloon post-dilation on cardiac conduction in patients undergoing transcatheter aortic valve replacement (TAVR). MethodsFrom June 2021 to December 2022, patients with severe aortic valve stenosis or regurgitation who underwent TAVR surgery using domestically produced valves at Xijing Hospital, Air Force Military Medical University were selected. The occurrence of intraoperative and postoperative cardiac conduction block was recorded. According to whether balloon post-dilation was performed during the surgery, patients were divided into the post-dilation group and the non-post-dilation group. The baseline data, postoperative cardiac conduction block occurrence, and cardiac function of the two groups were analyzed. ResultsA total of 126 patients were included, including 83 males and 43 females, with an average age of 66.6±7.6 years. There were 30 patients in the post-dilation group and 96 patients in the non-post-dilation group. On the first day after TAVR, the average QRS intervals in the post-dilation group and the non-post-dilation group were 105.6±13.8 ms and 125.9±28.2 ms, respectively (P=0.017). At discharge, the average PR intervals in the two groups were 168.7±36.8 ms and 192.1±44.2 ms, respectively (P=0.024). After TAVR, 9 (7.1%) patients developed new atrioventricular block, 5 (4.0%) patients developed complete right bundle branch block, and 33 (26.2%) patients developed complete left bundle branch block. During hospitalization, 2 (1.6%) patients received permanent cardiac pacemakers, both of whom were in the non-post-dilation group. There was no statistical difference in postoperative left ventricular structure and function between the two groups (P>0.05). ConclusionPostoperative expansion using domestically produced interventional valves for TAVR do not increase the incidence of early atrioventricular block and permanent cardiac pacemaker implantation after valve implantation, and there are no significant changes in cardiac structure and function in patients with conduction block in the short term after surgery.

  • Meta-analysis and systematic review of risk factors for dysphagia in lung transplant recipients

    ObjectiveTo investigate the risk factors of dysphagia in lung transplant recipients through meta-analysis. MethodCohort studies and case-control studies on risk factors for dysphagia after lung transplantation in Science, Scopus and ProQuest databases were searched from the establishment of the database to July 2023. After literature screening, data extraction and quality evaluation, Meta-analysis was performed using Rev Man 5.4 and Stata17.0 software. Results12 literatures were included, and the results of meta-analysis showed: The incidence of dysphagia in lung transplant recipients was 67% (95%CI 0.59~0.75, I2 =92.44%, P<0.001). There were three risk factors for dysphagia in lung transplant recipients, namely, reintubation (OR=5.34, 95%CI 3.07~9.28, I2 =0%, P<0.001), mechanical ventilation (OR= 4.25, 95%CI 2.95~6.12, I2 =0%, P<0.001), extracorporeal life support (OR=2.13, 95%CI 1.69~2.67, I2 =0%, P<0.001).ConclusionsLung transplant recipients with re-intubation, mechanical ventilation and extracorporeal life support are more likely to have dysphagia after surgery. Nursing staff can combine risk factors and formulate targeted nursing measures to reduce the incidence of dysphagia after lung transplantation.

  • Effect of left atrial enlargement on expression of the angiotensinⅡ, signal transducers and activators of transcription 3 and Rac GTPase activating protein 1 signaling pathways in patients with persistent atrial fibrillation and rheumatic heart disease

    Objective To evaluate the effect of left atrial enlargement on atrial myocardial fibrosis degree and levels of the angiotensinⅡ (AngⅡ)/Rac GTPase activating protein 1 (Rac1)/signal transducersand activators of transcription 3 (STAT3) signaling pathways expressing in patients with persistent atrial fibrillation and rheumatic heart disease (RHD). Methods From March to December 2011, 30 patients with RHD who underwent prosthetic valve replacement in our hospital were enrolled, including 16 males and 14 females, aged 42-70 (56.9±6.8) years. Twenty RHD patients with persistent atrial fibrillation as a research group and ten RHD patients with sinus rhythm as a control group (group A) underwent transthoracic echocardiography and right atrial appendage (RAA) tissue samples were obtained from these patients during mitral/aortic valve replacement operation. The research group according to left atrial diameter (LAD) was divided into two groups, ten patients in each group: a group B with LAD of 50–65 mm and a group C with LAD of LAD>65 mm. For each sample, histological examination was performed by hematoxylin-eosin and Masson’s trichrome staining. Light-microscopic pictures of atrial tissues samples were stained and tissue fibrosis degree in each group was analyzed. AngⅡ concentration was measured by enzyme linked immunosorbent assay. Rac1 and STAT3 were measured by western blotting. Results LAD was significantly greater in AF patients with RHD than in the control group. Hematoxylin-eosin staining demonstrated highly organized arrangement of atrial muscles in the control group and significant derangement in both group B and group C with reduced cell density and increased cell size. Moreover, Masson’s trichrome staining showed that atrial myocytes were surrounded by large trunks of collagen fibers in both group B and group C, but not in the group A. There was a positive correlation between atrial tissue fibrosis and LAD. AngⅡ content was positively correlated with LAD. Similarly, Rac1 and STAT3 protein levels were found considerably higher in the group C and group B than in the group A with excellent correlation to LAD. Conclusion In patients with RHD complicated with persistent atrial fibrillation, the degree of atrial fibrosis and the expression level of AngⅡ/Rac1/STAT3 signaling pathways significantly increase with the left atrialenlargement.
